1.Obstetrical ultrasound data-base management system by using personal computer.

A computer progrem which performs obstetric calculation on Clipper Language using the datas from ultrasonography was developed from personal computer. It was designed for fast assessment of fetal development, prediction of gestagional age, and weight from ultrasonographic measurements which included biparietal diameter, femur length, gestational sac, occipito-frontal diameter, abdominal diameter, and etc. The Obstetricel-Ultrasound data-Base Managemant System was tested for its performance. The Obstetrical-Ultrasound Data-Base Management System was very useful in patient management with its convenient data filing, easy retrieval of previous report, prompt but accurate estimation of fetal growth and skeletal anomaly and production of equation and growth curve for pregnant women.

2.The role of domestic tap water in Acanthamoeba contamination in contact lens storage cases in Korea.

A survey was carried out from August to December 2004 in Pusan, Korea to document the presence of free-living amoeba (FLA), including the genus Acanthamoeba, in both contact lens storage cases and domestic tap water. Acanthamoeba was isolated from 5 (4.2%) in 120 contact lens storage cases. Four house tap water samples from residents, whose contact lens storage cases had been contaminated by Acanthamoeba, were also found to be contaminated with Acanthamoeba. Therefore, the contamination rate of FLA and Acanthamoeba in domestic tap water was investigated in order to examine the role of domestic tap water in Acanthamoeba contamination of contact lens storage cases. FLA and Acanthamoeba were identified in 97 (46.8%) and 16 (7.7%) of the 207 domestic tap water samples, respectively. There were no significant differences between the contamination rates of FLA in tap water according to the filtration plant of origin. No FLA was detected in the tap water directly supplied by the water purification plants. Water storage tanks appear to promote FLA colonization, including Acanthamoeba, in domestic tap water. This increases the risk of Acanthamoeba contamination in contact lens storage cases as well as increasing the risk of Acanthamoeba keratitis.

3.The Study of Talar Tilt Angle of Ankles in Korean Adult

An appreciation of the normal variation of the Talar Tilt angle under stress roentgenogram on the ankle joint may throw some light on the diagnosis and treatment in an ankle joint injuries. The purpose of this study is to show the normal range of the Talar Tilt angleof the ankle joint under various circumstances and has been to compare the results of each angle. The author have undertaken the neutral ankle mortise view and inversion and eversion stress roentgenogram in all ankles and measured its Talar Tilt angle. The data were based on study of free chosen 50 normal Korean adult male 25 and female 25 respectively. The results were summerized as follows; 1. Talar Tilt angle was O degree in neutral ankle mortise view. 2. In inversion stress roentgenogram, Talar Tilt angle ranged from 0 to 15 degrees, 97% of cases were less than 10 degrees. 3. In eversion stress roentgenogram, Talar Tilt angle ranged from 0 to 7 degrees. 4. In inversion stress roentgenoram, the difference between right and left of same male and female ranged from 0 to 7 degrees. 5. It is very important to check accurately Talar Tilt angle under the same method and same conditions.

4.Radiologic analysis & diagnostic value of lateral tomography on ossification of posterior longitudinal ligamentof c-spine

The ossification of posterior longitudinal ligament of the spine(OPLL) is newly recongnized clinical entity,although compression of the spinal cord by an OPLL was reported by key as early as in 1839 in Guy's HospitalReport. OPLL was noticeable in lateral tomography as an abnormal dense radiopacity along the posterior margins ofthe vertebral body. Authors retrospectively analysed the diagnostic values and findings of lateral tomography ofthe cervical spine in 11 cases at Kang Nam General Hospital Public Corporation during 1 yr from July 1984 to June1985. The results were as follows; 1. Among suspected 11 cases of OPLL, 9 cases were confiremd as OPLL on lateraltomogram. 2. Age range was 25 years old to 55 years old and more prevalent age was over 5th decades & male wasmore involved than female. 3. Frequent involvement was C2-C5 level and number of vertebral bodies involved was 3.6in average. 4. This ossification developed 4 modes, a continuous type 11%, segmental 33%, mixed type 33%,circumscribed type in 22%. 5. OPLL thickness were from 2mm to 4.5mm and spnal canal narrowing ratio were form 25%to 44% and there were norational relationships between clinical symptom and thicknness of OPLL. 6. On diagnosis ofOPLL, lateral tomography is accurate and recommendable screening study due to easy, noninvasive, indisipensable and less harmful technique, compared to those of myelography or computed tomography.

6.Characterization of Principal Component Cell of DMBA induced Rat Malignant Fibrous Histiocytoma With Cell Culture and Cloning.

This experiment was performed to elucidate the cytologic origin of chemically induced MFH in Wistar rats. The tumor was produced by injections of DMBA(9,10-dimethyl-1,2-benzanthracene). With the produced MFH, cell culture and cloning were performed, followed by establishment of a cell strain, which was investigated by immunohistochemical and electron microscopic studies. The results were as follows. A) By immunohistochemistry of the tumor tissue, fibroblastic cells were positive for MEP-1(specific antibody for fibroblastlike cell of MFH, Takeya, 1993) and Anti-hPH(beta)(Anti-prolyl 4-hydroxylase beta), but negative for TRPM-3 and F4/80. Histiocytelike cells were positive for TRPM-3 and F4/80, but negative for MEP-1 and Anti-hPH(beta). In immunoelectron microscopy, normal spleen macrophage showed linear reactivity in cell membrane for TRPM-3, whereas histiocytelike cells of the tumor disclosed negative reaction. B) At 5 weeks of the primary tumor cell culture, the cells exhibited typical storiform pattern of MFH. C) The established cell strain revealed immunoreactivity for MEP-1 and Anti-hPH(beta), but negative for TRPM-3. The cloned tumor cells showed morphologic characteristics of undifferentiated fibroblastic cell. Latex particle (0.80 micrometer size) phagocytosis was negative in the cloned cell strain. The results of the current study support the concept that principal component cells of MFH is of fibroblastic cell origin.

7.Leiomyoma of the Ovary A report of two cases.

Ovarian leiomyoma is a rare form of the ovarian mesenchymal neoplasm and about 50 cases have been reported in the literature. It is believed that many cases may go unnoticed because they are usually small in size and frequently mistaken for the more common fibroma or fibrothecoma. Its origin is still controversial and many possibilities are considered including the smooth muscle in the blood vessel wall of the hilum or the multipotential ovarian stromal cell. Herein we describe two cases of ovarian leiomyoma with its characteristic histologic finding.

8.The Biochemical and Bacteriological Study of the Umbilical Venous Blood for Autologous Trnasfusion in Neonates.

This study was conducted to investigate the safety of the umbilical venous blood for antologous transfusion in the premature newborn babies. Umbilical venous blood was collected with aseptic technique immediately after delivery from 270 healthy mothers admitted to the Yeungnam University Hospital between March 1,1992 and August 31,1992 . The volume of the blood and the time taken for collecting the blood were measured. Also the hematological and biochemical changes of the blood during storage in the refrigerator of blood band and the incidence of bacterial contamination were studied. It took 67.6+/-25.9 seconds on the average for collecting the blood from an umbilical vein and the average volume of the blood was 77.1+/-23.2ml which would be sufficient volume for one time transfusion of 10ml/kg of packed red blood cells for the neonates. The hemoglobin and hematocrit values did not change significantly during storage for three weeks. Thus significant hemolysis did not seem to occur during storage. However, platelet counts decreased significantly from 248+/-59x10E3/mm3 to 193+/-47x10E3/mm3(p<0.01). Although plasma calcium level was not changed (8.4+/-0.9mg/dL), sodium was decreased significantly from 184.3+/-11.1mEq/L to 170.9+/-13.9mEq/L(p<0.01) and potassium was incresed from 5.0+/-1.0mEq/L to 26.7+/-4.8mEp/L.(p<0.01). Plasma glucose level was decreased from 521.7+/-106.7mg/dL to 294.3+/-120.8mg/dL (p<0.01), blood pH and bicarbonate concentrations were decreased significantly from 6.825+/-0.110 to 6.378+/-.087 and from 13.5+/-1.6mEq/L to 2.1+/-1.0mEq/L respectively (p<0.01). The smear of the umbilical venous blood and peripheral blood of the neonates stained by KleihauerBetke method revealed no significant differences in the percentages of adult red blood cells. Also there were no differences in the percentages of adult ted blood calls in the umbilical venous blood samples according to time for collection. The degree of decrement of hemoglobins (1.2gm/dL) after delivery in the mothers who had sampling of the umbilical venous blood was not different from that in the mothers who didn't hava sampling. The bacterial contamination rate of the umbilical venous blood was 3.5% (95% confidence interval was from 0.7 to 6.3). It seems to be safe to use the umbilical venous blood in the neonates for autologous transfusion but confirmation of bacterial contamination by culture is necessary.

9.A Case of Acute Generalized Exanthematous Pustulosis.

Acute generalized exanthematous pustulosis (AGEP) has symptoms of abrupt onset of a widespread pustular eruption on an erythematous. base. Most cases appear to be related to drug reactions, mainly antibiotics, but viral infections and hypersensitivity to mercury may cause AGEP. The essential features of AGEP include. (1) numerous (several dazen) small((5mm), mostly non follicular pustules arising on a widespread erythema:purpura and target-like lesions may be associated; (2) histology showing intraepidermal or subcorneal pustules associated with one or more of the following.clermal edema, vasculitis, perivascular eosinophils, or focal necrosis of keratinocytes; (3) fever (over 38C); (4) neutrophilia, and (5) acute evolution with spontaneous resulotion of pustules within 15 days. We report a case of AGEP which presented with widespread tiny pustules on the whole body except the face, palms and soles. There were petechia, purpura, and vesiculobullous lesions on the axilla, popliteal fossa and upper abdomen. A biopsy specimen from a pustule showed subcorneal pustules with perivascular polymorphous cellular infiltration, marked dermal edema and necrotic keratinocytes. There was complete resolution of the lesions within 10 days.

10.Factors Affecting Burnout of Staff in Emergency Medical Service (Focusing on 119 rescuers in Busan and Gyeongnam).

PURPOSE: This study aims to investigate the degree of burnout of 119 rescuers, to determine factors influencing their burnout, and to provide basic data for development of intervention programs to prevent the burnout. METHODS: Subjects of this study were all 119 rescuers working at fire stations located in Busan and Gyeongnam. The data were collected from May 1, 2010 to June 30, 2010. The collected data were analyzed with SPSS/WIN 17.0. RESULTS: When the difference in the degree of burnout by general characteristics was investigated, the means of burnout were significantly different depending on age, period of service, position, license, annual salary, desire to work continuously and types of working hours. Job stress, job satisfaction, social support, self efficacy and self esteem showed a statistically significant correlation with burnout. The degree of burnout became higher with more job stress and lower job satisfaction, social support, self efficacy and self esteem. As a significant factor affecting the degree of burnout of 119 rescuers, job stress showed 47.3% of explanatory power. The explanatory power of all of job stress, job satisfaction, self efficacy, types of working hours, annual salary and license was 62% and the power of job stress was the highest. CONCLUSION: From these results, job stress, job satisfaction and self-efficacy were found to be factors affecting the burnout of 119 rescuers. Therefore, intervention programs to reduce job stress and to improve job satisfaction, social support, self efficacy and self esteem should be developed to lower the degree of burnout of 119 rescuers. In addition, further researches to analyze works of 119 rescuers and legal and institutional strategies to improve their treatment are necessary and supplementary training in various practices by different circumstances based on standardized protocols should be conducted.
