3.Submucosal Gastric Actinomycosis in a Hematemesis Patient.

We experienced a case of submucosal gastric actinomycosis, presenting as bleeding. The 65-year-old woman had a symptom of epigastric pain, without any other intra-abdominal disease entity. A gastrofiberscopic study demonstrated a submucosal mass lesion with bleeding at the fundus. Because of the bleeding, laparotomy was undertaken, and a abscess containing a large mass was found at the gastric fundus, and total gastrectomy undertaken. Histological examination revealed a giant acute ulcer with a submucosal abscess due to actinomycosis. Because of its rarity, submucosal gastric actinomycosis is an entity overlooked by most surgeons. We report upon this case of submucosal gastric actinomycosis and include a review of the literature.

4.Lichtenstein's Tension-Free Repair of Groin Hernias: A Single-Surgeon Experience with 321 Cases.

PURPOSE: Tension-free hernioplasty has become the most popular procedure for the repair of groin hernias in the United States and United Kingdom. The purpose of this study is to describe a 7-year personal experience with Lichtenstein's tension-free groin hernia repair under local anesthesia. METHODS: We retrospectively studied the clinical outcome of 321 cases of Lichtenstein repairs, performed consecutively by an experienced surgeon between Jan. 1994 and Dec. 2000. RESULTS: Of the 321 cases, 242 (75.4%) were indirect, 34 (10.6%) were direct, 8 (2.5%) were femoral, 7 (2.2%) were pantaloon, and 30 (9.3%) were recurred hernias. The mean age was 55 years; 91% were male. The mean number of injections of analgesics required in the postoperative period was 3.2. The mean hospital stay following repair was 2.7 days. Complications occurred in 23 cases (7.1%). Most of these were minor, consisting of five cases of bruising or hematomas (1.6%), four superficial infections (1.3%), three seromas (0.9%), two hydroceles (0.6%), six patients with persisting groin pain for more than a month (1.8%), one foreign body granuloma, one urinary retention, and one testicular atrophy. There were no recurrences or operative deaths. CONCLUSION: Lichtenstein's tension-free hernioplasty is an easy and simple technique with less pain, minor complications and only rare instances of recurrence. This procedure can be performed on a same-day basis under local anesthesia. Lichtenstein repair may be the most promising technique for the repair of groin hernias.

5.Effects of 'Happy Plus Program' for Family Caregivers of Patients with Demenia on Sleep, Depression, Burden, and Quality of Life of Caregivers.

PURPOSE: This study was conducted to verify how a program for family caregivers can affect the negative effects of loss of sleep, depression, burden of caring and poor quality of life in people who care for a family member suffering from dementia. METHODS: This was a quasi-experimental study using a single-group pre-test-post-test design. This study was conducted between March 28, and May 30, 2013 in A city. The program was provided for 9 weeks. Eleven caregivers with a family member suffering from dementia participated in this study. RESULTS: The second hypothesis on lowering the depression index (Z=-2.93, p=.003) and fourth hypothesis on improving quality of life (Z=2.02, p=.044) were supported. CONCLUSION: The results of the study indicate that the program for family caregivers is effective in lowering the depression index and improving quality of life of people who care for a family member who is a patient with dementia. More programs for caregivers are needed and should be developed and provided for family caregivers in the near future.

6.Two Cases of Hemolytic Uremic Syndrome Associated with Pneumococcal Infection.

Liver is generally known as an organ which is most commonly involved by the metastic tumors. According to the tendency of using fine needle aspiration in the diagnosis of hepatic tumors, the differentital diagnosis between hepatocellular carcinoma and metastatic carcinoma frequently has been a main issue in the poorly differentitated cases, especially to the pathologists of Korea, an endemic area of hepatocellular carcinoma. Until now the problem has been usually solved by the comparison of cytologic characteristics of their tumor cells but not by background cytologic features which rarely have been studied. We observed the background cytologic features helpful for the differential diagnosis through the analysis of 20 cases who had confirmed primary cancer and were diagnosed as metastatic carcinomas in the liver by fine needle aspiration cytology. Twenty cases included 9 adenocarcinomas, 7 spuamous cell carcinomas, 1 small cell carcinoma, 1 carcinoid, 1 adenoid cystic carcinoma, and 1 renal cell cacinoma. Analysis of background cytologic features revealed that 77% of adenocacinoma cases showed benign mesenchymal components and hepatocytes and spuamous cell carcinoma cases disclosed benign mesenchymal tissue (71%) and necrosis (57%). Remaining cases showed variable combinations of benign mesenchymal component, necrosis, hepatocytes, and bile duct epithelial cells. No case revealed atypical hepatocytic naked nuclei, a useful cytologic finding of hepatocellular carcinoma. In summary, the background cytologic features more commonly observed in metastatic carcinomas than in the hepatocellular carcinoma were benign mesenchymal components, hepatocytes, necrosis, and bile duct epithelium. The endothelial cells and hepatocytic naked nuclei, two relatively specific findings of hepatocellular carcinoma were not observed except for renal cell carcinoma. Above background cytologic features are thought to be helpful for the differential diagnosis between the hepatocellular carcinoma and various metastatic carcinomas in the poorly differentiated cases.

7.Influence of Knowledge about Lung Cancer, Attitude and Preventive Health Behavior about Cancer on Nicotine Dependency in Smoking Male College Students.

PURPOSE: The aim of this study was to identify the influence of knowledge, attitude and preventive health behavior of cancer on nicotine dependency in male college students who have a habit of smoking. METHODS: In this study, a cross-sectional survey design was adopted for the 254 male college students who smoke in D and S city. The data was analyzed for descriptive statistics, t-test, Pearson's correlation coefficient and multiple regression analysis using the SPSS 20.0 program. RESULTS: Knowledge about lung cancer (β=-.407, p<.001), preventive health behaviors about cancer (β=-.223, p<.001) and the smoking period (5years≤) (β=.198, p=.001) were identified as the factors influencing nicotine dependency of the subjects. The model explained 33% of variance in nicotine dependency (F=20.49, p<.001). CONCLUSION: The results suggest that anti-smoking educators should include the strategies to increase knowledge about lung cancer and preventive health behaviors of cancer, and to reduce the smoking period in their smoking cessation program.

8.Pulmonary Lymphangioleiomyomatosis: A case report.

Lymphangioleiomyomatosis(LAM) is a rare disease of women of child-bearing age in which there is progressive hyperplasia of atypical smooth mucle along lymphatics in the lung, and/or axial lymphatics in the thorax and abdomen, resulting in honeycombing of lung. Interestingly there has been a speculation that it represents a forme furste or incomplete expression of tuberous sclerosis complex. This is based on the observation that patients with tuberous sclerosis can manifest pulmonary lesions indistinguishable from LAM. We report a case of LAM occuring in a 39-year-old female, who complained of recurrent pneumothorax, chest pain and shortness of breath. Three years ago, the patient had right nephrectomy under the diagnosis of ruptured angiomyolipoma. A X-ray film of the chest showed honeycombing with a diffusely reticulonodular pattern and cyst-like spaces. She had a characteristic facial appearance of adenoma sebaceum, which her father and uncle had. Microscopically, the lung showed a marked smooth muscle proliferation around the slit-like lymphatic spaces and also some respiratory bronchioles.

9.Anatomical Danger Zone in Open Inguinal Herniorrhaphy.

PURPOSE: Because of complex inguinal structures, we cannot overcome the neurogenic complications in open inguinal herniorrhaphy. Nowadays, there are few studies of anatomical danger zones in open herniorrhaphy. So, in this study we would cover the danger zones in hernia surgery with a cadaver. METHODS: We studied the anatomic structures of three male cadavers. There was no anatomical variation among them. We can ascertain the running of the ilioinguinal nerve, iliohypogastric nerve, and genitofemoral nerve. RESULTS: We can see that the ilioinguinal nerve runs anterior to the spermatic cord in the inguinal canal and lies beneath the external oblique aponeurosis. The iliohypogastirc nerve runs on the anterior surface of the internal oblique muscle, and aponeurosis medial and superior to the internal ring. The genital branch of the genitofemoral nerve lies on the iliopubic tract and accompanies the cremaster vessels to form a neurovascular bundle. In these areas, we often suture the anchor in open herniorrhaphy, increasing probability of these nerves suffering injury. CONCLUSION: In open inguinal herniorrhaphy, surgeons must have a comprehensive understanding of the neural anatomy of the groin to reduce post-operative complications. Moreover, we must consider not only anatomical understanding but also surgical procedures reducing neurogenic complications. So, we suggest certain procedures to reduce nerve injury such as three dimensional mesh or the use of fibrin glue.
