We experienced a case of submucosal gastric actinomycosis, presenting as bleeding. The 65-year-old woman had a symptom of epigastric pain, without any other intra-abdominal disease entity. A gastrofiberscopic study demonstrated a submucosal mass lesion with bleeding at the fundus. Because of the bleeding, laparotomy was undertaken, and a abscess containing a large mass was found at the gastric fundus, and total gastrectomy undertaken. Histological examination revealed a giant acute ulcer with a submucosal abscess due to actinomycosis. Because of its rarity, submucosal gastric actinomycosis is an entity overlooked by most surgeons. We report upon this case of submucosal gastric actinomycosis and include a review of the literature.

PURPOSE: Tension-free hernioplasty has become the most popular procedure for the repair of groin hernias in the United States and United Kingdom. The purpose of this study is to describe a 7-year personal experience with Lichtenstein's tension-free groin hernia repair under local anesthesia. METHODS: We retrospectively studied the clinical outcome of 321 cases of Lichtenstein repairs, performed consecutively by an experienced surgeon between Jan. 1994 and Dec. 2000. RESULTS: Of the 321 cases, 242 (75.4%) were indirect, 34 (10.6%) were direct, 8 (2.5%) were femoral, 7 (2.2%) were pantaloon, and 30 (9.3%) were recurred hernias. The mean age was 55 years; 91% were male. The mean number of injections of analgesics required in the postoperative period was 3.2. The mean hospital stay following repair was 2.7 days. Complications occurred in 23 cases (7.1%). Most of these were minor, consisting of five cases of bruising or hematomas (1.6%), four superficial infections (1.3%), three seromas (0.9%), two hydroceles (0.6%), six patients with persisting groin pain for more than a month (1.8%), one foreign body granuloma, one urinary retention, and one testicular atrophy. There were no recurrences or operative deaths. CONCLUSION: Lichtenstein's tension-free hernioplasty is an easy and simple technique with less pain, minor complications and only rare instances of recurrence. This procedure can be performed on a same-day basis under local anesthesia. Lichtenstein repair may be the most promising technique for the repair of groin hernias.

PURPOSE: The purpose of this study was to explore the levels of stress, self-efficacy for smoking cessation, smoking temptation, and nicotine dependency, and to identify factors influencing nicotine dependency among male college students who smoke. METHODS: In this study, a cross-sectional survey design was adopted for 283 male college students who smoke in D city, Korea. Data was analyzed for descriptive statistics, Pearson's correlation, and multiple regression using the SPSS 20.0 program. RESULTS: The mean score of stress was 27.00+/-6.28, self-efficacy for smoking cessation was 25.61+/-6.71, smoking temptation was 53.87+/-11.02, and nicotine dependency was 3.63+/-1.96. There was a significant positive correlation between stress and nicotine dependency (r=.58, p<.001) and between smoking temptation and nicotine dependency (r=.59, p<.001). There was a significant negative correlation between self-efficacy for smoking cessation and nicotine dependency (r=-.59, p<.001). The significant factors influencing nicotine dependency were stress (beta=.357, p<.001), self-efficacy for smoking cessation (beta=.359, p<.001), and smoking temptation (beta=.297, p<.001). This model explained 60.3% of variance in nicotine dependency (F=105.59, p<.001). CONCLUSION: The results suggest that an intervention program is needed to reduce the perception of stress and smoking temptation, and to increase the ability of self-efficacy for smoking cessation among male college students who smoke with the intention to quit smoking.

A 63-year-old man had a huge verrucous protruding mass over the suprasternal area. The lesion enlarged rapidly over 3 mooths, and measured about 10×8 cm. The histologic finding of the biopsy specimen showed nests of squamous epithelium with central keratinization, infiltrating the dermis. The neoplasm was treated successfully with surgical excision.

PURPOSE: The purpose of this study was to verify the correlation analysis between fluid intake on skin hydration and pH of college students. METHODS: The subjects were 129 female nursing students in D city. Data were collected by a self-administered questionnaire, using a skin moisture checker and skin pH meter on faces, hands, and feet. The collected data were analyzed by descriptive statistics, t-test, ANOVA, and Pearson correlation analysis, using SPSS WIN, 20. RESULTS: The daily fluid intake was composed of 57% pure water, 21% caffeinated beverages, 22% non-caffeinated beverages. There were significant differences in average skin hydration on the three body parts according to pure water, caffeinated beverages, and non-caffeinated beverages; however, there was no significant difference measured by fluid intake. There was a significant positive correlation between fluid intake and skin hydration: between pure water and skin hydration. There was significant negative correlation between caffeinated beverages and skin hydration: between non-caffeinated beverages and skin hydration. CONCLUSION: The results suggest that fluid intake, pure water, caffeinated beverages, and non-caffeinated beverages have an effect on skin hydration and pH. Therefore, it is good to increase the amount of fluid intake but, it is recommended to increase the amount of intake of pure water rather than beverages to improve skin status.

BACKGROUND: The aim of this study was to compare the cardiovascular changes followed by laryngoscopy with the McCoy laryngoscope blade with those followed by laryngoscopy with the Macintosh laryngoscope blade. METHODS: Forty eight patients were randomly divided into two groups. Following induction with fentanyl 2 mcg/kg and thiopental 5 mg/kg, and muscle relaxation with vecuronium 0.1 mg/kg, the vocal cords were visualized with either the McCoy or the Macintosh laryngoscope blade, then tracheal intubation was performed. Heart rate and arterial blood pressure were measured just before and after laryngoscopy, and 1, 3 and 5 min later. RESULTS: There was a significant increase in both heart rate and arterial blood pressure after tracheal intubation using the Macintosh laryngoscope. Also, use of the McCoy blade resulted in a significant increase in both heart rate and arterial blood pressure. CONCLUSIONS: There was no significant difference on arterial pressure and heart rate to laryngoscopy and tracheal intubation with either the McCoy blade or the Macintosh.
