1.Giant Follicular Ovarian Cyst with Torsion in Juvenile Primary Hypothyroidism.

Multicystic ovary was first described by Silver in 1958 as a rare case feature of acquired primary hypothyroidism. In 1980, Lindsay reported four girls with hypothyroidism associated with multicystic ovary diagnosed by pelvic ultrasonography. The mechanism of ovarian cyst formation and pseudoprecocious puberty in severe hypothyroidism in childhood are unknown. Increased ovarian sensitivity to GnRH and increased TSH level which acts on FSH receptor is one of the mechanism explained. We report a case of a girl with primary hypothyroidism presented with pseudoprecocious puberty and giant follicular ovarian cyst with torsion.

2.Task Analysis and Education Need of Dietitians in the Contracted Business & Industry Foodservice.

The purposes of this study were to investigate importance level and performance frequency of the dietitians' duties and task elements, to examine the actual condition of education and need for education, to analyze the interrelation between their tasks and education, and to provide the direction of education for the contracted Business & Industry (B & I) foodservice dietitians. The task elements of receiving, ordering, HACCP management and directing serving process were done almost every day. In terms of the importance of tasks, food sanitation management, personnel sanitation management, receiving and ordering were high. Meanwhile, the computerization of their works was being carried out on the whole, showing a higher frequency in all the details of procurement management and accounting management, as well as task elements such as menu planning, leftover and food waste management, HACCP management and human resource management. In the past three years, HACCP management, cost management, planning work schedule and allotting a task, general business and sale bond management were increased most and rapidly. For the actual condition of education, dietitians got more education for the duty of sanitation, safe and facility/utility management than any other duty, while they did less education of procurement management and office management than others. Meanwhile, the education for sanitation, safe and facility/utility management and accounting management were very much required. For the relationships of frequency of duty and the necessity of education, seven task elements including food sanitation management were correlated positively. Eighteen tasks besides menu planning had a significant positive correlation between the importance of duties and the necessity of education.

5.Treatment of Pierre-robin sequence Using Bilateral Mandibular Distractors.

The purpose of this study is to present our experience of distraction osteogenesis procedure used to improve the functional and consmetic problems of two patients associated with Pierre-Robin sequence, which is an uncommon congenital disease in combination with micrognathia, glossoptosis, and respiratory obstruction. A female child of 42 months and a male infant of 4 months suffering from pneumonia, difficult breathing, cyanosis and insomnia were admitted. We performed temporary tongue to lip dhesion by modified Routledge method and bilateral mandibular distraction. After the operation, symptoms of insomnia and impaired breathing had improved. We found an extension of the mandibular length. In addition, the posterior airway space and lower face area were expanded in the cephalometric tracing and profilogram. We report that bilateral distraction provedure could be an effective and safe method for improving of impaired breathing and cosmetic problems of patients with Pierre-Robin sequence.

8.Heart Rate Variability and Lipid Profile in Patients with Major Depressive Disorder.

OBJECTIVES: The analysis of heart rate variability (HRV) is a useful non-invasive tool to investigate the autonomic nerve function. Previous studies on the relationship between HRV and depression have been reported controversial results. Similarly, the correlation between the serum lipids and depression is debatable. The purpose of this study is to examine the relationship between heart rate variability, lipid profile and depression. METHODS: A total of 42 patients with major depressive disorder (MDD) and 32 age and sex-matched normal subjects who had no previous history of major medical and mental illnesses were recruited for this study. A structured-interview was used to assess the general characteristics and psychiatric illness. HRV measures were assessed by time-domain and frequency-domain analyses. Psychological symptoms were measured using the Hamilton rating scale for anxiety (HAM-A), Hamilton rating scale for depression (HAM-D). In addition, the evaluation for lipid profile was performed by blood test. RESULTS: In serum lipid profile test, MDD group showed higher cholesterol (197.68+/-42.94 mg/dL vs. 176.85+/-34.68 mg/dL, p=0.044), TG (139.45+/-92.54 mg/dL vs. 91.4+/-65.68 mg/dL, p=0.018), LDL (130.03+/-33.18 vs. 106.62+/-27.08, p=0.004) level than normal control group. In HRV time domain analyses, the standard deviation of the NN interval (SDNN) was decreased in MDD group than normal control group, but was not significant (32.82+/-14.33 ms vs. 40.36+/-21.40ms, p=0.078). ApEn (Approximate Entrophy) was significantly increased in MDD group than normal control group (1.13+/-0.11 vs. 0.91+/-0.18, p<0.001). ApEn was correlated with LDL level (r=0.277, p=0.028), HAM-D scores (r=0.534, p<0.001) and HAM-A scores (r=0.470, p<0.001). CONCLUSION: MDD patients showed increased ApEn, one of the HRV measurement. And this ApEn was correlated with LDL, HAM-D and HAM-A scores. In this study, the analysis of ApEn would be a useful test of MDD.

10.Factors Related to Parenting Satisfaction among Mothers of Preschoolers.

PURPOSE: The purpose of this study was to identify factors related to level of satisfaction among mothers of preschoolers. METHODS: Participants were 359 mothers of firstborn healthy preschoolers. The questionnaires were collected from August 1 to November 21, 2012. Data were analyzed using SPSS 20.0. RESULTS: The mean score for degree of mothers' parenting satisfaction was 5.94 out of 7. There were significant differences in the level of parenting satisfaction according to mother's job (t=2.48, p=.014), planned pregnancy (t=3.07, p=.002), number of children (t=2.22, p=.027), and parenting education (F=4.33, p=.005). The environmental variables of age of spouse (F=3.06, p=.048) and monthly income of the household (F=2.87, p=.036) also seemed to have an influence. Analysis of predictors such as the mother's job, planned pregnancy, parenting education, overall health problems, parenting stress, perception of emotional status and behaviors in children, age of spouse, and spouse's support explained 31.5% of the variance in mother's parenting satisfaction. CONCLUSION: The results suggest a need to develop nursing intervention programs for parenting stress reduction, and health programs for fathers to support mothers' child rearing.
