1.The Trend Change of Incidence and Treatment of Urolithiasis between the 1980s and 2000s.

PURPOSE: The incidence of urolithiasis has recently shown an increasing tendency in relation to improvements in living conditions in Korea. With the development of extracorporeal shock wave lithotriptor (ESWL), endourology and other new instruments, urolithiasis has become easier to treat, without surgical intervention. The incidence and treatment of urolithiasis, between the 1980 and 2000, were evaluated. MATERIALS AND METHODS: 328 and 1,142 patients with urolithiasis, either admitted between January 1981 and December 1984 or treated at the out-patient clinic between January 2001 and December 2004, respectively, were analyzed. RESULTS: In the early 1980s, 328 (27.3%) patients of the total 1,203 admitted to the urology department had urolithiasis. In the early 2000s, 1,142 patients were treated for urolithiasis. The total number and incidence constantly increased over the stated period. The ratio of males to females was 1.3:1 in the 1980s and 2.0:1 in the 2000s. The occurrence rate for those under 20 years decreased, but increased after the 6th decade. And the seasonal occurrence was highest during the summer of the early 1980s, but there was no seasonal difference during the early 2000s. The incidences of lower ureteral, bladder and urethral calculi were decreased, but those of renal and upper ureteral calculi increased. In the management of urolithiasis, open surgery and expectant therapy decreased during the 2000s. In the endourological management of urolithiasis, the success rates of ESWL, percutaneous nephrolithotomy (PNL) and ureteroscopic removal of stone (URS) were 94.5, 86.6 and 96.4%, respectively. CONCLUSIONS: The incidence and treatment modalities of urolithiasis have changed, especially since the late 1980s.

2.The Impact of Symptom Experience and Self-Care Agency on Quality of Life in Patients with Hemodialysis

Purpose:
This was a descriptive study investigating the effect of symptom experience and self-care agency on quality of life among patients with stage 5 chronic kidney disease undergoing regular hemodialysis.
Methods:
The participants were recruited from one general hospital and two private hospitals located in J city. 154 participated and completed structured questionnaires from June 30 to July 18, 2022. The data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Scheffé test, Pearson’s correlation coefficient, and hierarchical multiple regression.
Results:
The mean symptom experience score was 0.91±0.67 (out of 5), self-care agency was 4.19±0.71 (out of 6), and quality of life was 3.02±0.49 (out of 5). A negative correlation was found between quality of life and physical symptom experience (r=-.39, p<.001) and emotional symptom experience (r=-.39, p<.001). A positive correlation was found between quality of life and self-care agency (r=.66, p<.001). The regression analysis showed self-care agency (β=.48, p<.001), emotional symptom experience (β=-.27, p=.001), and monthly family income (β=.19, p=.002) significantly influenced quality of life, and explained 54.0% of the quality of life.
Conclusion
These results suggest to improve the quality of life among patients on hemodialysis, it is necessary to assess and intervene with emotional symptom experiences and develop effective programs with specific strategies to enhance self-care agency.
3.Recurrent Desaturation Events due to Opioid-Induced Chest Wall Rigidity after Low Dose Fentanyl Administration.

Opioid-induced chest wall rigidity is an uncommon complication of opioids. Because of this, it is often difficult to make a differential diagnosis in a mechanically ventilated patient who experiences increased airway pressure and difficulty with ventilation. A 76-year-old female patient was admitted to the intensive care unit (ICU) after surgery for periprosthetic fracture of the femur neck. On completion of the surgery, airway pressure was increased, and oxygen saturation fell below 95% after a bolus dose of fentanyl. After ICU admission, the same event recurred. Manual ventilation was immediately started, and a muscle relaxant relieved the symptoms. There was no sign or symptom suggesting airway obstruction or asthma on physical examination. Early recognition and treatment should be made in a mechanically ventilated patient experiencing increased airway pressure in order to prevent further deterioration.

4.Study on the therapeutic effects of interferon and gamma-globulin in experimental Pneumocystis carinii pneumonia.

This study was performed to observe the therapeutic effects of interferon-gamma(IFN-gamma) and gamma-globulin(gamma-globulin) in experimental Pneumocystis carinii pneumonia of immune suppressed mice. After 9 weeks, trimethoprim-sulfamethoxazole(TMP-SMZ; 10-50 mg/mouse/day), mouse IFN-gamma(5 x 10(4) units/mouse/day) and mouse gamma-globulin(20 mg/mouse/day) were administered to the mice for 3 weeks by the experimental group. The therapeutic efficacy was evaluated by body weights, histopathologic and electron microscopic findings of the lungs, and number of P. carinii cysts by Gomori's methenamine silver stain. Body weights of the mice were significantly increased in the group of combination therapy of TMP-SMZ with IFN-gamma or gamma-globulin, and in the group of TMP-SMZ treatment (p < 0.05), however, little effect was found in the group of gamma-globulin alone. Histopathologic findings of P. carinii pneumonia were much improved in the group of combination therapy of TMP-SMZ with IFN-gamma. Treatment with either TMP-SMZ or IFN-gamma significantly reduced the number of cysts in the P. carinii pneumonia, but gamma-globulin alone was ineffective. In electron microscopic findings of P. carinii pneumonia, the number of trophozoites and cysts were reduced by treatment with either TMP-SMZ or IFN-gamma, and most of the cysts were empty or containing one or two intracystic bodies. The present results suggested, that combination therapy of TMP-SMZ with IFN-gamma had synergistic effects in treatment of P. carinii pneumonia in experimental mice.

The data were analyzed by SAS/PC program using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient. The results of this study are as follows : 1. The mean score for the level of self- disclosure was 3.29+/-0.98. 2. The mean score for the level of clinical competency was 3.93+/-1.00. The mean score for the dimension of skills, professional attitudes, teaching and coordinating, nursing process, interpersonal relationships were 4.09+/-0.77, 4.08+/-0.82, 3.97+/- 0.90, 3.77+/-0.76 and 3.37+/- 0.75 respectively. 3. The statistically significant difference in the score of the self-disclosure according to the educational background (F=9.42, p<.01), grade (F=5.59, p<.01), religion (F=2.68, p<.05), satisfaction of nursing major (F=14.20, p<.0001), and satisfaction of nursing practice (F=5.42, p<.01) was obtained. 4. The statistically significant difference in the score of the clinical competency according to the grade (F=32.44, p<.0001), achieved performance records (F=3.52, p<.05), satisfaction of nursing major (F=12.06, p<.0001), satisfaction of nursing practice (F=27.35, p<.0001) was obtained. 5. The data shows the positive correlations between self-disclosure and skill (r=.3231, p<.0001), between self- disclosure and teaching/coordinating (r=.1912, p<.0001), between self-disclosure and interpersonal relationship (r=.3064, p<.0001), between self-disclosure and professional attitude (r=.2789, p<.0001), between self-disclosure and nursing process (r=.2766, p<.0001).

8.Effects of Gratitude Disposition, Social Support, and Occupational Stress of Clinical Nurses on Grit

Purpose:
This is a descriptive research that aims to identify the effects of clinical nurses' gratitude disposition, social support, and occupational stress on grit and provide basic data for the development of future grit promotion among clinical nurses.
Methods:
The study participants were 164 nurses who had worked at three general hospitals in Jeollabuk-do, and data were collected using a structured questionnaire from May 2022 to June 2022. For data analysis, descriptive statistics, independent t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis were conducted using SPSS 23.0.
Results:
The average grit score was 42.26±5.48. Grit showed a positive correlation with gratitude disposition and social support and a negative correlation with occupational stress. The factors influencing grit were gratitude disposition, clinical experience of 12~60 months, and occupational stress; social support did not have any effect on grit. The explanatory power was 25.0%.
Conclusion
Results confirmed that the higher the gratitude disposition among clinical nurses, the higher the grit; and the lower the occupational stress, the higher the grit. In order to increase grit among clinical nurses, measures at the nursing organization level, including training and programs that can strengthen gratitude disposition and reduce occupational stress, are needed.
9.Mediating effects of academic self-efficacy in the relationship between academic stress and sleep quality among academic high school students

Purpose:
This study aimed to confirm mediating effects of academic self-efficacy in the relationship between academic stress and sleep quality among academic high school students.
Methods:
The participants included 195 academic high school students who were attending a private educational institution in the Gwangju or Pusan metropolitan city. Data were collected from September 1 to September 30, 2019. The data were analyzed using PROCESS macro SPSS/WIN 3.2 and bootstrapping was used to test the mediating effects.
Results:
There were significant relationships between sleep quality and academic stress (r=-.55, p<.001), sleep quality and academic self-efficacy (r=.35, p<.001), and academic stress and academic self-efficacy (r=-.16, p<.020). Academic self-efficacy showed partial mediating effects in the relationship between academic stress and sleep quality.
Conclusion
These findings suggest that the development of a program that improves the academic self-efficacy of academic high school students who are experiencing academic stress is a way to improve the quality of their sleep.
10.Role of Leukemia Inhibitory Factor in the Effect of Co-Culture on Preimplantation Embryo Develpement.

OBJECTIVE: To assess the effect of recombinant human leukemia inhibitory factor on in vitro development of 1-cell ICR mouse embryo. MATERIALS AND METHOD: ICR mice were superovulated with PMSG/hCG and 1-cell stage mouse embryos were recruited. 1-cell mouse embryo were cocultured on human oviductal cells in a CO2 incubator (coculture group) and were cultured on 0.4% BSA+HTF media (control group). And anti-hLIF Ab was added the cocultured group in a different concentration (1pg, 10pg, 100pg, 1ng) and developmental rate was compaired to the control group, and rhLIF was added to the preincubated 0.4% BSA+HTF media in a different concentration (2000U, 1000U, 100U, 10U) and its developmental rate was compaired to group which was cultured on 0.4% BSA+HTF media only. RESULT: 1. The cleavage rate of 2-cell mouse embryo co-cultured with human tubal epithelial cell was significantly higher than that of cultured with media alone (HTF with 0.4% BSA) (p<0.05). 2. When LIF antibody was added to the medium with human tubal epitherlial cell, the mouse embryo could not cleave more than 2-cell in 1 ng of LIF antibody, and less than 1 ng, the cleavage rate was lower than cultured without LIF antibody group(p<0.05). 3. Two cell blocked ICR mouse embryos were developed into four cells under LIF(p<0.05), but no further development was observed. CONCLUSIONS: These results shows that LIF enhances the development of preimplantation embryo, and when rhLIF is applicated in vitro, it has positive effects on the development of early mouse embryo and can help overcoming the two-cell block.
