1.Detection of Human Papilloma Virus Type 16 and 18 in Adenocarcinoma in situ of the Uterine Cervix.

OBJECTIVE: The purpose of this study was to assess the role of HPV 16 and 18 in adenocarcinoma in situ(ACIS) of the uterine cervix. METHODS: Seventeen cases of primary cervical adenocarcinoma in situ were analyzed for HPV DNA by polymerase chain reaction. HPV 16 and 18 DNA extracted from formalin-fixed, paraffin-embedded histologic tissue sections by polymerase chain reaction. RESULTS: 35.3% and 23.5% of ACIS were positive for HPV 16 and HPV 18 DNA, respectively. From the normal tissue, 11.8% were positive for HPV 16. Human papillomavirus positive patients were younger than negative patients but statistically insignificant(mean age 42.1 vs 51.7 years). CONCLUSIONS: These results show that HPV type 16 and 18 were closely related to etiology of the ACIS of the uterine cervix.

2.Experience of Vascularized Joint Transfer for Segmental Loss of M-P Joint: A Case Report

Restoration of the M-P joint of a damaged or lost joint by trauma, tumor and others is necessary for adequate function of the hand. Several procedure have been described for restoration of the M-P joint. We experienced a case of vascularized joint transter for a lost joint by trauma. After 1 year and 2 months follow-up, there have been no degenerative change and no pain.

3.Clinical Experience in the Craniofacial Bone Reconstruction with Medpor(R)(Porous Polyethylene).

There has been growing interest in using density porous polyethylence(Medpor) for reconstrctive facial bone surgery because of their perceived increased biocompatibility. the implant is a highly stable and somewhat flexible porous alloplast that has shown to exhibit rapid tissue ingrowth into its pores. A total of 77 Medpor implants were placed in 65 patients over a one and half year period Implants were used for orbital reconstruction, correction of craniofacial contour deformities, malar and chin area. Before operation, adequate photographic and radiologic examination was performed to carry out operation plan. Within 65 patients 38 patients were due to acute trauma, 23 patients due to posttraumatic facial deformity, and 4 patients cosmetic. the result was satisfactory: 95.4% of the patients were improved after operation. One complication occurred due to previously existed sinusitis and it was cured by conservative treatment. On the basis of our results, the Medpor implant is an excellent alternative to existing implant materials. They are easy to handle, shape, contour, position and fixate. If applied under adequate indications, the use of these implants will permit satisfactory results with little complications.

Diabetic muscle infarction (DMI), also known as diabetic myonecrosis, is a rare complication of diabetes mellitus (DM); hence, it is often underdiagnosed. Thus, timely diagnosis and treatment are essential for a better prognosis. We describe a 24-year-old woman with Prader-Willi syndrome and an 8-year history of uncontrolled type 2 DM, who presented with a sudden onset of fever and subacute painful swelling of her left thigh. She was finally diagnosed with DMI based on magnetic resonance imaging and muscle biopsy after excluding other infectious and inflammatory diseases of proximal muscles. The patient was treated with bed rest, strict glycemic control, and analgesics, and her symptoms gradually resolved. DMI should be considered in the differential diagnosis of patients with poorly controlled DM, who present with subacute pain and swelling of lower extremity muscles, without a history of trauma.

6.Factors Influencing Patient Privacy Protection Behavior among Nursing Students.

PURPOSE: The purpose of this study was to identify factors influencing patient privacy protection behavior among nursing students and examine the relationships between these factors. METHODS: Participants in this study were 144 nursing students who have experienced clinical practice. The data were analyzed using descriptive statistics, one-way ANOVA, Scheffe test, Pearson's correlation coefficient, and multiple regression with IBM SPSS Win 23.0 program. RESULTS: Professional self-concept and ethical values were factors influencing patient privacy protection behavior among nursing students. These variables explained 21.9% of the variance for patient privacy protection behavior. A higher level of patient privacy protection behavior was associated with higher levels of professional self-concept and ethical values. CONCLUSION: The findings demonstrate that strategies for enhancing patient privacy protection behaviors of nursing students should include methods for forming images of positive nurses and firming ethical values.

7.The Effects of Paracetamol, Ketorolac, and Paracetamol Plus Morphine on Pain Control after Thyroidectomy.

BACKGROUND: The aim of this study was to compare the efficacy of ketorolac, paracetamol, and paracetamol plus morphine on pain relief after thyroidectomy. METHODS: Eighty patients were randomly allocated to one of the 4 groups: normal saline (group C), ketorolac 30 mg (group K), paracetamol 1 g (group P), and paracetamol 700 mg plus morphine 3 mg (group PM). Each regimen was administered intravenously (IV) 30 min. before the end of surgery. If pain was not relieved, patients received an IV bolus of pethidine hydrochloride 25 mg. Pain intensity using a visual analogue scale (VAS) was recorded at 0.5, 1, 2, 4, and 6 hr after the end of surgery. RESULTS: VAS at 0.5 and 1 hr after the end of surgery were significantly lower in group K, group P, and group PM than in group C (P < 0.05). The number of patients receiving pethidine hydrochloride at 0.5 and 1 hr after the end of surgery was significantly lower in group K, group P, and group PM than in group C (P < 0.05). There was no significant difference among the groups in the incidences of adverse events associated with study medications and patient satisfaction (P > 0.05). CONCLUSIONS: Paracetamol 1 g IV possesses a similar analgesic efficacy to ketorolac 30 mg IV after thyroidectomy. Paracetamol may represent an alternative to ketorolac for pain prevention after mildly to moderately painful surgery in situations where the use of NSAIDs is unsuitable.

8.Is neoadjuvant androgen deprivation therapy beneficial in prostate cancer treated with definitive radiotherapy?.

PURPOSE: To determine whether neoadjuvant androgen deprivation therapy (NADT) improves clinical outcomes in patients with prostate cancer treated with definitive radiotherapy. MATERIALS AND METHODS: We retrospectively reviewed medical records of 201 patients with prostate cancer treated with radiotherapy between January 1991 and December 2008. Of these, 156 patients with more than 3 years of follow-up were the subjects of this study. The median duration of follow-up was 91.2 months. NADT was given in 103 patients (66%) with median duration of 3.3 months (range, 1.0 to 7.7 months). Radiation dose was escalated gradually from 64 Gy to 81 Gy using intensity-modulated radiotherapy technique. RESULTS: Biochemical relapse-free survival (BCRFS) and overall survival (OS) of all patients were 72.6% and 90.7% at 5 years, respectively. BCRFS and OS of NADT group were 79.5% and 89.8% at 5 years and those of radiotherapy alone group were 58.8% and 92.3% at 5 years, respectively. Risk group (p = 0.010) and radiation dose > or =70 Gy (p = 0.017) affected BCRFS independently. NADT was a significant prognostic factor in univariate analysis, but not in multivariate analysis (p = 0.073). Radiation dose > or =70 Gy was only an independent factor for OS (p = 0.007; hazard ratio, 0.261; 95% confidence interval, 0.071-0.963). CONCLUSION: NADT prior to definitive radiotherapy did not result in significant benefit in terms of BCRFS and OS. NADT should not be performed routinely in the era of dose-escalated radiotherapy.

9.Dermoscopic Features of Seborrheic Keratoses: Observations in 25 Korean Cases.

BACKGROUND: Dermoscopy is in vivo cutaneous surface microscopy at 10~20 X magnification. It has been gaining in popularity in clinical dermatology. The recognition of dermoscopic features of melanoma and other skin lesions including seborrheic keratosis (SK) is valuable for improving clinical diagnostic accuracy and for reducing unnecessary excisions. OBJECTIVE: The aim of this study was to clarify the dermoscopic features observed in SK, estimate the prevalence of the findings among Korean patients, and compare findings between Koreans and Caucasian peoples. METHODS: A total of twenty five patients with biopsy-proven SK were selected. The dermoscopic features of SK that we investigated have been well described by Braun et al. including 'milia-like cyst' and 'comedo-like opening'. We determined the frequency of each feature. RESULTS: Among the twenty five SK cases, the main dermoscopic features of SK such as 'milia-like cyst' and 'comedo-like opening' were found in about half of cases (52% and 52%, respectively). Additional dermoscopic features and frequencies (in parenthesis) were: 'sharp demarcation' (92%), 'fissure' (72%), 'moth-eaten border' (56%), 'hairpin blood vessel' (20%), 'network-like structure' (20%), 'exophytic papillary pattern' (20%), and 'globular pattern' (4%). The frequency of 'hairpin blood vessel' and 'network-like structure' was significantly lower in Koreans than in Caucasians (p<0.05). The small size is a limitation of the study. CONCLUSION: Most dermoscopic features (morphology and frequency) of SK observed in Korean patients (with the relatively rare exception of additional features) are similar to the features reported for Caucasians.
