2.A Comparison of Flexible and Rigid Rods System in Transpedicular Dcrew Fixation of Degenerative Lumbar Spine.

The transpedicular screw fixation is known to be capable of providing more secure correctional, force and rigid fixation to the spine. But it is often accompanied by various complications, including stress shield effects, loosening in osteoporotic spine, pseudoarthrosis, hardware problems and long-term change in motion behaviors at the adjacent motion segment and etc. The purpose of this study is to compare the flexible and rigid rods system in terms of complications and problems for various degenerative lumbar diseases. From September 1991 to November 1994, 41 patients were operated with the flexible rods system (group A) and 39 patients with the rigid rods system (group B). They were followed up for more than 4 years. The flexible rods system was composed of 41 cases of Wiltse system and the rigid rods system was composed of 29 cases of Diapason, 8 cases of TSRH and 2 cases of CD instrument. The resuits of the study were as follows: 1. By standard Cobb lateral measurement, 7 cases in group A and 10 cases in group B showed significant loss of sagittal angle (>4), post-operatively. 2. Hardware failure was noted in 7 cases in group A and 10 cases in group B. In group A, rod bending was seen in 4 cases, rod breakage in 2 and screw loosening in one. In group B, screw loosening was observed in 5 and screw-rod locking mechanism failure in 5 cases. 3. In spondylolisthesis patients, 10 cases in group A and 14 in group B, there was no statistically significant differences between the two groups in the reduction rate immediately after surgery and loss of reduction at the last follow up. 4. Clinical results were good to excellent in 85.4% of patients in group A and 82.1% of group B. No statistically significant differences between the two groups were found, 5. No statistically significant differences between the two groups were found with respect to degenerative changes at the adjacent motion segment to the fused level and pseudoarthrosis.

3.Localized Primary Thymic Amyloidosis Presenting as a Mediastinal Mass: A Case Report.

We herein describe a case of a 55-year-old healthy woman with localized primary thymic amyloidosis presented as a mediastinal mass, found incidentally by chest radiography. Computed tomography revealed a 4.1 cm soft tissue lesion with nodular calcification in the left anterior mediastinum. The resected specimen was a well-defined lobulating mass with calcification. Microscopically, the mass was consisted of amorphous eosinophilc hyalinized substances involving the thymus and intrathymic lymph nodes. These eosinophilic substances showed apple-green bi-refringence under polarized light after staining with Congo red. In immunohistochemical study, they were positive for kappa and lambda light chains and negative for amyloid A. There was no evidence of systemic amyloidosis in clinical investigations. A final diagnosis of localized primary thymic amyloidosis was made.

6.Study on the therapeutic effects of interferon and gamma-globulin in experimental Pneumocystis carinii pneumonia.

This study was performed to observe the therapeutic effects of interferon-gamma(IFN-gamma) and gamma-globulin(gamma-globulin) in experimental Pneumocystis carinii pneumonia of immune suppressed mice. After 9 weeks, trimethoprim-sulfamethoxazole(TMP-SMZ; 10-50 mg/mouse/day), mouse IFN-gamma(5 x 10(4) units/mouse/day) and mouse gamma-globulin(20 mg/mouse/day) were administered to the mice for 3 weeks by the experimental group. The therapeutic efficacy was evaluated by body weights, histopathologic and electron microscopic findings of the lungs, and number of P. carinii cysts by Gomori's methenamine silver stain. Body weights of the mice were significantly increased in the group of combination therapy of TMP-SMZ with IFN-gamma or gamma-globulin, and in the group of TMP-SMZ treatment (p < 0.05), however, little effect was found in the group of gamma-globulin alone. Histopathologic findings of P. carinii pneumonia were much improved in the group of combination therapy of TMP-SMZ with IFN-gamma. Treatment with either TMP-SMZ or IFN-gamma significantly reduced the number of cysts in the P. carinii pneumonia, but gamma-globulin alone was ineffective. In electron microscopic findings of P. carinii pneumonia, the number of trophozoites and cysts were reduced by treatment with either TMP-SMZ or IFN-gamma, and most of the cysts were empty or containing one or two intracystic bodies. The present results suggested, that combination therapy of TMP-SMZ with IFN-gamma had synergistic effects in treatment of P. carinii pneumonia in experimental mice.

7.The Effect of the Horticultural Activities Program on Loneliness and Life Satisfaction of the Disabled Female.

PURPOSE: The research was done to investigate the effect of a horticultural activities program on loneliness and life satisfaction of women who have a disability. METHODS: Participants were 50 disabled women from a welfare center of G district of G city. Data were collected from August 18 to October 20, 2010. The program was provided for 8 weeks. The tool for life satisfaction was Choi's scale (1986) and loneliness was measured with the UCLA Loneliness Scale developed by Russell and translated by Kim & Kim (1989). Data were analyzed using frequency, t-test, and paired t-test with SPSS/WIN 16.0. RESULTS: As a result of the horticultural activities program, the first hypothesis was supported by significantly decreasing of loneliness in the experimental group than in the control one (t=-3.27, p =.002). The second hypothesis was verified by testing variable which was life satisfaction (t=3.39, p =.001). CONCLUSION: The results of the study indicate that this kind of program is effective in decreasing loneliness of women with a disability and in improving their life satisfaction. Horticultural activities programs can be unique interventions leading to a satisfactory life for disabled women.

8.A Case of Lymphangioma Circumscriptum.

Lymphangioma circumscriptum is a rare disease and the lesions are indistinguishable, clinically and histologically, from lymphangiectasis. The only difference is that lymphangiectasis is usually bilateral and develops in adult or late life, ie, condary to irradiation, surgical interruption, or some malignancies, but lymphangioma circumscriptum is congenital disease or develops soon after birth.

10.The Changes of EEG Nonlinear Parameter in Sleep and Wakefulness States.

BACKGROUND: Up to now, sleep stages have traditionally been determined by the visual inspection of individual EEG waves. However, the exact physiological meaning of the sleep waves is not known. The purpose of this study was to try and find out the physiological parameters of the EEG of the sleep and wakefulness states by calculating one of the non-linear chaos parameter, the largest Lyapunov exponent (LLE), of EEG time series. METHODS: The digital EEG of the wakefulness with eye opening (WEO), wakefulness with eye closure (WEC), stage1 (S1), stage2 (S2), stage3 or 4 (S34) were recorded at centroparietal region (C4-P4 bipolar derivation) in 10 normal subjects. Lyapunov exponents of 50 EEG time series in different states were compared. RESULTS: LLE's of WEO, WEC, S1, S2, S34 showed an increas-ing tendency as states switched from wakefulness to sleep. LLE of sleep was larger than that of awake state. CONCLUSIONS: The EEG of the sleep state appeared to be more chaotic than that of the awake state. This nonlinear chaos parameter can be used as a physiological parameter of normal sleep and awake states.
