4.Effects of Breast-Feeding Adaptation and Quality of Sleep on Postpartum Depression in Puerperal Women

PURPOSE: This study aimed to provide preliminary data for a nursing intervention plan for puerperal women to reduce postpartum depression by investigating factors that affect depressive disorder during the puerperal period. METHODS: A total of 153 pregnant women were recruited from a university hospital in Gimhae city in Korea. Data were collected using self-report questionnaires. RESULTS: Mothers' quality of sleep, breast-feeding confidence, discomfort with breastfeeding, depressive feeling during pregnancy, and baby's feeding capability and growth were significant predictors of postpartum depression. These variables explained 36.1% of the variance in postpartum depression in puerperal women. CONCLUSION: The aforementioned results indicate that puerperal women are less likely to experience postpartum depression when their quality of sleep is higher, breast-feeding confidence is higher, discomfort with breast-feeding is lower, when they did not feel depressed during pregnancy, and when baby's feeding capability and growth is better. Therefore, to decrease puerperal women's depression, a nursing intervention program and a research study to verify the effects of the program are necessary to increase mother's sleep quality and breast-feeding adaptation and prevent depression during pregnancy.

OBJECTIVE : To analyze the methylenetetrahydrofolate reductase (MTHFR) mutation in recurrent spontaneous abortion associated with hyperhomocysteinemia. MATERIAL AND METHOD: The blood Sample of habitual aborter with high fasting homocysteine level was tested by PCR-RFLP method. RESULTS: The patient was found to be a homozygosity for MTHFR gene mutation that was confirmed by the finding which is consistent with the mutation at the nucleotide 677 C to T, Corresponding to Ala to Val. CONCLUSIONS: Hyperhomocysteinemia due to MTHFR mutation is a cause of recurrent spontaneous abortion. Therefore, the MTHFR mutation should be examined in the workup of recurrent spontaneous abortion showing hyperhomocysteinemia.

8.Recurrent Desaturation Events due to Opioid-Induced Chest Wall Rigidity after Low Dose Fentanyl Administration.

Opioid-induced chest wall rigidity is an uncommon complication of opioids. Because of this, it is often difficult to make a differential diagnosis in a mechanically ventilated patient who experiences increased airway pressure and difficulty with ventilation. A 76-year-old female patient was admitted to the intensive care unit (ICU) after surgery for periprosthetic fracture of the femur neck. On completion of the surgery, airway pressure was increased, and oxygen saturation fell below 95% after a bolus dose of fentanyl. After ICU admission, the same event recurred. Manual ventilation was immediately started, and a muscle relaxant relieved the symptoms. There was no sign or symptom suggesting airway obstruction or asthma on physical examination. Early recognition and treatment should be made in a mechanically ventilated patient experiencing increased airway pressure in order to prevent further deterioration.

10.Leiomyoma of the Lung: A case report.

We present a 37-year-old male who was found to had mas within the bronchus. This patients was admitted for the evaluation of cough. Chest CT scan showed endobronchial mass in the bifurcation of LUL and LLL bronchus. The left lower lobe was atelectatic. Lobectomy of the left lower lobe was done. On opening of the bronchus, there was a 2x1x1 cm sized endobronchial mass. Histologically, the mass was smooth muscle tumor composed of densely packed eosinophilic spindle cells in interlacing bundles with pale elongated nuclei covered by bronchial epithelium. On immunohistochemical staining, the tumor cells were positive for desmin. Ultrastructurally, the tumor cells exhibited numberous cytoplasmic microfilaments with focal densities, pinocytotic vesicles, and a thick basal lamina.
