2.Xeroderma Pigmentosum: The Treatment of Associated Skin Cancer.

Xeroderma pigmentosum is characterized by sunlight sensitivit.y, autosomal recessive inheritance, and multiple cutaneous malignancies, usually basal and squamous cell carcinomas. In this work the effects of bleomycin and topical 5-fluorouracil on basal and squamous cell careinomas appearing in the patient of xeroderma pigmentosum were studied and followed up for two years. The material was a 50 year old female patient of xeroderma pigmentosum associated with basal and squamous cell carrinomas occuring in the exposed area, and its mixed tumor in the nose for thirty years. Duringfirst admission blecmycin was given intravenously in a dosage of 15 mg every other day or twice a week for basal cell carcinomas in the exposed area and mixed tumor in the nose. The total amount of bleomycin given was 315 mg. A year later another squamous cell carcinomas developed on the exposed area. She was treated with topical application of 5% 5-fluorouracil cream daily for 40 days during second admission. The results obtained are summarized as follows: l. The effect of bleomycin is more effective in the squamous cell carcinoma than in the basal cell carcinoma which recurred after 2 years. 2. Topical therapy of 5% 5-fluorouracil cream appeared almost complete effectiveness to the squamous cell carcinoma.

4.Dermatitis Herpetiformis Resembing to Atopic Dermatitis: Report of a Case.

A case of dermatitis herpetiformis resembling atopic dermatitis is reported. The patient is 45 year-o1d male, who 6 months ago developed the pruritic erythematous vesicular eruptions on the left side of back and it gradually spread to the margin. He was treated with steroid but the lession had undergone exacerbation and remissions. Biopsy was performed on the vesicular lesion and revealed non-acantholytic subepidermal bulla with mild inflammatory cells infiltration and then he was treated with DDS for 2 months with good response.

5.Allergic Contact Dermatitis Due to Nail lacquer.

Nail lacquer is a cause of allergic contact dermatitis. Nail lacquer is composed of nitrocellulose(film former), adhesives(also maintain glass), plasticizer, solvent and coloring agent, among these the adhesives-sulfonamide and formaldehyde resin-are the moat common and important antigen. Predilection area of this dermatitis is periungual, face especially upper eye lid, forehead and cheek, and neck. The lesion is usually eczematous, and characteristic lesion is also seen such as mottled, linear or band-like hyperpigmentation and hypopigmentation. Recently allergic contact dermatitis due to nail lacquer is increasing in number, so report with the result of patch test and review of literatures.

6.A Case of solar Urticaria.

A 23-year-old woman had solar urticaria that was activated by visible light and UVA, but the intensity of the urticarial reaction of visible light was more pronounced than that of UVA. The results of passive and reverse passive transfer studies were negative, We report a case of solar urticaria which is characterized by overlapping features of 60th type Il and Il[ in the classification of Harber.

7.Total radical TRapping antioxidant parameter, calculated.

BACKGROUND: It has been suggested that diabetic patients are under high oxidative stress and plasma MDA concentration is a reliable marker for oxidative stress. However, some studies showed that plasma MDA is not a good marker for oxidative stress. Reeently, the total radical-trapping antioxidant parameter (TRAPc) has been proposed as a marker for the overall antioxidant property of plasma samples. Therefore, in this study, we tried to evaluate whether MDA and TRAPc are reliable markers of the oxidative stress-antioxidant system or not. METHODS: The plasma samples from 67 type 2 diabetic patients and 31 normal subjects were collected. The plasma MDA, protein-bound SH groups, uric acid and vitamin C were determined by fluorophotometry or spectrophotometry. Plasma vitamin E concentration was analyzed by HPLC. Calculated TRAP (TRAPc) were determined by the proposed calculation methods. RESULTS: 1. Diabetic patients had significantly lower TRAPc, compared with normal subjects. 2. SH groups, uric acid, vitamin C and vitamin E were not different between the two groups. 3. MDA and MDA/TG were significantly higher in diabetic subjects. CONCLUSION: From the results of this study, TRAPc seems to be a reliable parameter of overall plasma antioxidant system and the plasma MDA may be used as a marker of oxidative stress, but further long-term logitudinal studies are needed.

9.Disseminated Superficial Actinic Porokeratosis (DSAP): Report of Two Cases.

Disseminated superficial actinic porokeratosis (DSAP) is one of the recently found skin disorderes, which was differentiated from classicaI porokeratosis (Mibelli) and senile keratosis etc. in view of clinical features and hietogenesis by Marvin E. Chernosky on 1967. The one was 60 years old female patient, who had been suffered from multiple dark-brownish keratotic uniform minute papules on dorsa of the hands, forearms and neck with accompanying itching sensation since about 20 year's old of her age. The another one was 38years old housewffe, wha had been suffered from same lesions of the former on the face, neck, extremities especially sun-light exposed area. In both cases, skin lesions were exacerbated during each summer season and increase in numbers of the skin lesions with more severe itching sensation. Chernosky postulated none of a dark-sklnned race affording natural protection against sun-light had skin lesion of D S A P, we were abIe to find out two cases of typical D S A P ih the Korea.

10.Extramammaty Paget's Disease: Electron Microscopic Study for Observation of Bleomycin Effect.

Extramammary Paget's disease is a dermatosis which pathologically and clinically resembles Paget's disease of the breast. The disease occurs in regions with apocrine gIands; most often in the anogenital area, seldom in the axilla. umbilicus, mucous membrane of the mouth, nose, ear and Moll's gland of tne eyelid. In this work the effect of bleomycin for extramammary Paget's disease was studied on ultrastructural aspect. The material was a case of extramammary Paget's disease of 7 years standing occuring in the scrotum and the pubic area by serial sections after IV injection of 60mg, 120mg and 240mg of bleomycin, and examined under the electron microscope. The results obtained are summarized as follows: 1. The size of Paget's cell and its nucleus was gradually decreased. 2. The numbers of lysosome, dense body, autophagic vacuolization and tonofilament was decreased. From these fidings, it is suggested that bleomycin induces retrograde degenerative change of Pagct's cell, and cell necrosis finally.
