1.Clinical and histopathologic findings of pathergy test sites in patients with Behcet's disease.

Through the clinicopathologic study of pathergy test sites of 16 patients with Behrets disease in the active or inactive stage, the following results were obtained; 1. In the active stage, six of 16 patients (37.5%) showed positive reactions in clinical pathergy tests, which are much higher than that in the inactive stage when only one patient showed a positive reaction. The severity of the ciniral pathergy reaction was directly related to the activity of the disease. 2. The characteristic histopathologic finding in the active stag was a dermal inflammatory cellular infiltration composed mainly of polymorphonuclear lukocytes and of lymphomononuclear cells accompanied by leukoctocylasia. Two of 16 patients showed true leukocytoclastic vasculitis. 3. Although the type of disease and the reactivity of the clinical pathergy test were related in terms of the severity of the histopathologic findings, there were some notible histopathologic changes even in the cases with negative clinical pathergy reactions. We concluded that the histopathologic findings of the pathergy test sites were more reliable for the diagnosis of Behcet's disease than the clinical findinigs.

2.Magnetic Resonance Imaging for the Evaluation of Acute Posterolateral Complex Injuries of the Knee.

The purpose of this study is to determine whether magnetic resonance imaging(MRI) can assist in deci- sion-making in the treatment of acute posterolateral complex injuries. We reviewed retrospectively seven patients with acute posterolateral complex injury, among the 79 cases that had taken cruciate ligament recon- struction. MRI findings were then correlated with the results of examination under anesthesia or operi lateral reconstruction. Based on surgical and clinical findings, there were six complete and four partial posterolateral complex injuries. MRI was able to accurately describe the extent of injury in each case. All ten patients had concomitant anterior cruciate ligament tears, and seven were three posterior cruciate ligament injuries. The iliotibial band was avulsed from Gerdys tubercle in seven patients. In knees with complete tears, separation of the coronary ligament of the lateral meniscus was noted on MRI. A characteristic bone contusion on the anteromedial femoral condyle was present in nine patients. Popliteal tendon tearing was observed in eight cases, and biceps femoris was torn in six patients. In knee with complete tears, tears of the lateral collateral ligament complex was noted both on MRI and intraoperatively. A high-quality MRI in patients sustaining these injuries will assist the surgeon in directly applying surgical techniques in a timely fashion.

3.Knee Ligament and Meniscus Injuries Associated with Ipsilateral Diaphyseal Femoral Fractures.

One hundred and ninety-four consecutive patients with two hundred and one diaphyseal femur fractures were retrospectively reviewed to evaluate the frequencies, types and the results of treatments for the associated ipsilateral knee ligaments and menisci injuries from Oct. 1990 to Feb. 1997 at Eulji Medical Center. Fifteen patients had associated ipsilateral knee ligament injuries. There were eleven(73.3%) males and four(26.7%) females with an average age of 35.7(17-68) years. Thirteen(86.7%) patients were injured by traffic accident. Ten patients were followed for average of 38 months(12-73 months). There were five(33.4%) partial and two(13.3%) complete tears of anterior cruciate ligaments, three(20.0%) partial and four(26.7%) complete tears of posterior cruciate ligaments, one(6.7%) partial and four(26.7%) complete tears of medial collateral ligaments, one(6.7%) complete and three(20.0%) partial tears of lateral collateral ligaments, five(33.3%) tears of medial menisci and four(26.7%) tears of lateral menisci. There was no relationship between specific ligament damage and the cause of the injury or level of fracture. Better range of knee motion and HSS knee score was obtained when both the femur and ligament injuries were surgically managed. The invention of the MRI and the development of arthroscopic techniques have made it easy to diagnose and take care for ligament injuries. Early diagnosis and proper treatment on ruptured ligaments and tom meniscus combined with diaphyseal femur fractures seem to be important for good prognosis.

4.Preliminary Results of Uncemented Metal on Metal Total Hip Replacement Arthroplasty.

Particulate wear debris(polyethylene, polymethylmethacrylate and metal particles, or combination of these) can initiate aseptic loosening. To eliminate polyethylene debris and reduce the total amount of wear, metal on metal bearings(Metasul) which were made of a wrought Co-28Cr-6Mo-0.2C alloy, were introduced. From May 1994 total hip replacements using a Metasul metal on metal articulation were performed by the authors. 34 patients with 40 hips with minimum follow-ups during 24 months were analysed in this study. The mean follow up period was 31 months(ranging from 24 to 47 months). We used Armor cups and standard cups as acetabular comporients and SL stems, CLS stems and cone stems as femoral components. Preoperative average Harris hip score of 48 points became postoperatively 93(from minimum 81 to maximum 100). And there were slight and occasional thigh pains in 2 cases(5%). There were no revisions for the early aseptic loosening of femoral stems or stem failures. Radiologic evaluation which revealed mild calcar atrophy was developed in 33 cases(82.5%), revealing cortical hypertrophy in 10 cases(25%). Osteolysis was not noted around the prosthetic stem nor around the cup. Radiolucent lines were found in 9 cases of femoral components(22.5%) and in 1 case of acetabular component(2.5%) and subsidence of more than 3mm in 1 case of femoral components(2.5%) were also found. There was 1 case of definitive loosening of acetabular component(2.5%). These also showed that endosteal bone formation adjacent to femoral prosthesis was developed in 22 cases(55%) but no distal pedestal and stem shift occurred. The short-term but satisfactory results of the contemporary metal on metal articulation investigated in this study were encouraging and warrant continued study.

6.A cases of sarcoidosis with annular cutaneous lesions.

Cutaneous manifestations of sarcoidosis are highly variable and can be devided into the specific lesions which reveal noncaseating granuloma and the nonspecific lesio is which are referred to as cutaneous changes. Among speciific lesions, annular lesions may be foimied from coalescing papular lesions that may clear centrally and elevate peripherally. We report a case of sarcoidosis with multiple annular lesions on the face in a 33-year-old female who had systemic involvements in the intrathoracic organs and eyes. The main histopathologir. findings were well-demarcated islands of epitheloid cells with a few giant cells in the dermis. In addition, a slight admixfure of lymphoid cells was present at the margins of the epitheloid cell granulomas. Complete regression of t!he skin lesions was obtained with systemic cortiocosteroid therapy and no recurrence has been found for 2 1/2years.

7.A case of Kaposi's sarcoma Treated with radiotherapy.

Kaposi's sarcoma is a neoplasm of multifocal origin which manifest primarily as vascular nodules in the skin and other organs. We report a case of Kaposis sarcoma of classic type in a 17-year-old boy, who had a 4 x 5cm sized, dusky erythematous plaque with multiple spots of acupuncture on the left foot for 1 year. Histopathologic examination showed dilated, bizarre-shaped vessels, ndothelial cell proliferation, and spindle cell formation with vascular slits throughout the dermis. He was treated with 3000 rads of radiation(300 rad per day for l0 successive days). Six months after radiotherapy, gross findings were much improved and histopathologic findings showed only fibrosismc hemosiderin deposits in the dermis.

Air pollution does harm to the respiratory tracts. Fine particulate matter (PM(2.5)) attacks the lung directly passing through mucosal ciliary clearance, causes new-onset asthma, or exacerbates asthma symptoms in children. Oxidative stresses, immunologic changes, allergic sensitization, and epigenetic modification are associated with bronchial asthma. Furthermore, it causes respiratory tract infection and lung function decline. We have to protect the children who are more vulnerable to PM(2.5) than adults. We will investigate individual exposure, influences by the components of air pollution, and genetic susceptibility.

9.Cervical Spinal Cord Infarction Presenting as Chest Pain in Patients with Acute Cerebellar Infarction

Spinal cord infarction is rare, especially cervical cord infarction is lesser than thoracic and lumbar level. We describe two cases of cervical cord infarction following cerebellar infarction with vertebral artery occlusion, which initially presented with chest pain and dyspnea. Blood supply for the cervical cord comes from anterior and posterior spinal artery, which originated from the distal vertebral or posterior inferior cerebellar arteries. Therefore, occlusion of unilateral vertebral artery could cause a cervical cord infarction.
10.The Determinant of the Severity Who was Hospitalized with Asthma-like Symptoms in Influenza A (H1N1) Season.

PURPOSE: The aim of study was to identify the predictors of severe asthma exacerbation with influenza A (H1N1) infection so that intensive care could be initiated immediately. METHODS: Patients were diagnosed influenza A (H1N1), using a real-time reverse transcriptase polymerase chain reaction (RT-PCR) from the nasal aspirates in St. Mary's Medical Center, Busan, Korea, between September, 2009 and February, 2010. Medical records were retrospectively reviewed to collect data. Data were analyzed in two groups by severity of asthma exacerbation. The groups were severe groups and mild-moderate. Statistical analysis was performed by SPSS ver. 12.0. RESULTS: A total of 1,054 children were diagnosed influenza A (H1N1) infection. Of 1,054, 318 (30.1%) were hospitalized. There were 200 boys (62.9%) and median age was 5.0 years (0.1 to 18.0 years). Among hospitalized, 25.2% (80/318) were diagnosed asthma exacerbation. Among 80 patients, 16 (20.0%) were severe group. and 64 (80.0%) were mild-moderate. High leukocyte counts, neutrophil counts, and C-reactive protein (P<0.05) in serum had a greater risk for severe asthma exacerbation. Age, gender, early antiviral medication, gastrointestinal or neurological cosymptoms, allergic family history, sinusitis in water's view, total immunoglobulin E, Dermatophagoides pteronyssinus, Dermatophagoides farinae, mycoplasma coinfection, eosinophil and fever duration (P> or =0.05) were not reliable predictors. CONCLUSION: Among who had influenza A (H1N1) infection with asthma-like symptoms, patients who had not been managed asthma before, had high leukocyte, neutrophil, and C-reactive protein in serum, are likely to progress severe asthma exacerbation.
