2.Carcinosarcoma Arising from Mixed Tumor of the Parotid Gland: A case report.

A case of true malignant mixed tumor of the parotid gland is reported. The tumor, occuring in a 55-year-old man, started to grow rapidly after a long history of parotid mass. Total parotidectomy was carried out and the resected tumor measured 5x4x3 cm with a cut surface showing grayish-white solid and myxoid appearance. Microscopically, the tumor had both carcinomatous and sarcomatous elements, the former consisting of undifferentiated carcinoma with focal areas of ductal differentiation and the latter consisting of pleomorphic sarcoma with chondrosarcomatous differentiation. A remnant of benign pleomorphic adenoma could also be identified. Immunohistochemical study demonstrated focal cytokeratin reactivity in the carcinoma cells and vimentin in sarcomatous elements. It is assumed from these clinical and histological findings that the tumor had transformed from a pre-existing benign pleomorphic adenoma.

3.Medial Antebrachial Cutaneous Nerve Injury After Brachial Plexus Block: Two Case Reports.

Medial antebrachial cutaneous (MABC) nerve injury associated with iatrogenic causes has been rarely reported. Local anesthesia may be implicated in the etiology of such injury, but has not been reported. Two patients with numbness and painful paresthesia over the medial aspect of the unilateral forearm were referred for electrodiagnostic study, which revealed MABC nerve lesion in each case. The highly selective nature of the MABC nerve injuries strongly suggested that they were the result of direct nerve injury by an injection needle during previous brachial plexus block procedures. Electrodiagnostic studies can be helpful in evaluating cases of sensory disturbance after local anesthesia. To our knowledge, these are the first documented cases of isolated MABC nerve injury following ultrasound-guided axillary brachial plexus block.

5.Clinical analysis of laparoscopic surgery of tuboovarian abscess in acute pelvic inflammatory disease.

OBJECTIVE: To evaluate the clinical characteristics between patients with tuboovarian abscess and those presenting with non-tuboovarian abscess in acute pelvic inflammatory disease according to the operation findings. METHODS: The patients were divided into 2 groups based on the operation findings at laparoscopic surgery; 56 patients diagnosed with evidence of tuboovarian abscess and 74 patients diagnosed with acute pelvic inflammatory disease without tuboovarian abscess. Those were compared with respect to patient characteristics, clinical and laboratory findings depending on the significant differences. RESULTS: There were no significant differences between the two groups considering the percentage of age, parity, mean abortion rate and rate of the history of previous pelvic inflammatory disease. But there were differences in mean white blood cell count, erythrocyte sedimentation rate and C-reactive protein level. A higher number of patients having intrauterine devices were observed in the tuboovarian abscess group than in the non-tuboovarian abscess group. Also, statistical significance was noted in hospital stay after operation but there was no difference in number of sick days prior to operation. CONCLUSION: Our results suggest that some variables noted between the tuboovarian abscess group and acute pelvic inflammatory disease group. These results make easier to calculate diagnostic accuracy of patients prone to developing tuboovarian abscess and prevent subsequent complications from the delay of treatment if sensitivity, specificity, negative and positive predictive values were assessed.

6.Clinical Analysis of Contralateral Bulla of Lung on HRCT in the Patients Having Video-Assisted Thoracoscopic Surgery for Unilateral Primary Spontaneous Pneumothorax.

BACKGROUND: It is controversial whether the presence of bullae on the contralateral lung on HRCT plays a role in occurrence of contralateral primary spontaneous pneumothorax. We analyzed the significance of bullae on the contralateral lung and the risk factors associated with contralateral occurrence of primary spontaneous pneumothorax. MATERIAL AND METHOD: Three hundred ninety four patients who were undergone Video.Assisted Thoracoscopic Surgery for primary spontaneous pneumothorax between January 2004 and December 2009 were reviewed. The clinical features, HRCT and treatment of these patients were analyzed retrospectively. RESULT: Twenty eight of 394 patients had contralateral occurrence (7.10%). The average time was 13.06+/-9.79 months. A presence of contralateral bullae of lung on HRCT may not seem to be significant for occurrence of contralateral primary spontaneous pneumothorax (p=0.059). But bullae numbers were much more in contralateral pneumothorax patients (p=0.011). Younger than 20, being underweight (Body Mass Index<18.5 kg/m2) are independent risk factors for contralateral occurrence (odds ratio, 5.075 (1.679~5.339), 2.366 (1.048~5.339) respectively). CONCLUSION: The presence of bullae on the contralateral lung on HRCT was not significantly influenced the occurrence of contralateral primary spontaneous pneumothorax. However, age, body mass index, and the number of bullae were significant factors for the contralateral pneumothorax. We suggest that those high risk patients may require special attentions and general supportive care to prevent occurrence of contralateral primary spontaneous pneumothorax during the follow-up.

7.Evaluation of the Back Muscle Exercise for the Lumbar Discectomy Patients.

STUDY DESIGN: Retrospective study. OBJECTIVES: To identify the implementation of self-exercise therapy and analyze the effects of exercise therapy after lumbar discectomy. MATERIALS AND METHODS: Studied 47 patients who underwent partial lumbar discectomy and laminectomy from January, 2009 to December, 2009. They were instructed on 16 kinds of exercise therapy in total from postoperative 1day to postoperative 6weeks. Group A (n=25), whose frequency of outpatient clinic visit is above the average, and Group B (n=22), whose frequency of visit is relatively low;, below 5 times. We tested the visual analog scale (VAS scale) of back, Oswestry disability index and the strength of the two groups before surgery and at postoperative 3, 6, and 12 months. RESULTS: The degree of pain was significantly different (26.6+/-9.4 and 53.5+/-18.6) between group A and group B at postoperative 6 months. Function of daily life and strength test were significantly different (6.6+/-4.8 and 11.3+/-4.0 at group A, 3.6+/-0.9 and 3.0+/-1.1 at group B) between the two groups at postoperative 12 months. The characteristics of low compliance patients are low accessibility to the hospital and lack of knowledge on the importance of exercise according to the degree of pain. CONCLUSION: These results suggested that postoperative exercise program has significant effects on the pain, the function of daily life. It also increases flexibility and strengthens the muscle of patients with high compliance of outpatient clinic visit. The factors influencing the results are the age of patients, the willingness to exercise, and the environment in which each patient lives/resides.

8.Coexistence of Membranous Glomerulonephritis and IgA Nephropathy in a Patient with Psoriasis Vulgaris.

A 17-year-old man with psoriasis developed albuminuria and microscopic hematuria. Renal biopsy revealed a glomerulonephritis with features of both membranous glomerulonephritis and IgA nephropathy. Histologically the glomeruli exhibited variable degree of mesangial expansion and hypercellularity, three of which showed segmental hyalinosis and/or sclerosis. Direct immunofluorescence demonstrated granular IgG-bearing deposits along the peripheral glomerular capillaries and IgA deposits in the mesangium. His urinary abnormalities persisted after the remission of skin lesion induced by PUVA treatment. It suggests that although the psoriasis may induce the renal lesion, it is insufficient to treat only the skin lesion for clinical improvement of glomerulonephritis after the renal lesion is already established.

Mesenteric fibromatosis is a rare nonmetastasizing fi brous tumor, characterized by fibroblastic proliferation without evidence of inflammatory response or definite neoplasia. It appears at any age, but mainly in the second to fifth decades as incidence of two to four desmoid tu mor cases per 1 million people per annum. It is pseudoen capsulated and locally invasive and has a tendency to recur, but metastases are rare. It is more frequent with Gardner's syndrome(adenomatosis coli) in which 50% of them will appear as mesenteric fibromatosis. Trauma, especially that of previous surgery, has been associated with the subsequent development of desmoid tumor. The treatment of choice is a wide margin resection of the tumor. we experienced a case of mesenteric fibromatosis without Gardner's syndrome 2 years after appendectomy in a 31-year old man and recurred at 21 months after resection and report with a review of literatures. 6

10.Plasma Total Homocysteine Concentrations in Patients with Chronic Renal Failure.

Hyperhomocysteinemia, an independent risk factor of vascular disease, is common in patients with chronic renal failure(CRF) patients including dialysis patients. We measured fasting plasma concentrations of total homocysteine(tHcy) by high-performance liquid chromatography in 114 chronic renal patients and 37 healthy controls. The CRF patients were divided into four groups : chronic renal failure with serum creatinine >1.4mg/dl and creatinine clearance >10ml/min(CRF group, n=27), non-dialyzed ESRD patients with creatinine clearance <10ml/min(ESRD group, n=38), patients on maintenance hemodialysis(HD group, n=20) and patients on continuous ambulatory peritoneal dialysis(PD group, n=29). Mean(+/-SD) tHcy in each of CRF(14.2+/-5.6micromol/L), ESRD(21.6+/-14.1micromol/L), HD(21.0+/-9.2micromol/L) and PD(17.2+/-7.7micromol/L) group was significantly higher than that in controls(9.0+/-3.1micromol/L, P=0.001). In 87 ESRD, HD and PD patients, mean(SD) tHcy in 45 patients who received routine folate supplementation (1mg/day) was lower(17.5+/-8.3micromol/L) than that in 42 patients without supplementation(22.6+/-13.4micromol/ L, P=0.03), but was higher than that in controls (9.13.1micromol/L, P=0.001). In conclusion, hyperhomocysteinemia was present in patients with varying degree of chronic renal failure and increased in parallel with progression or renal failure.
