1.The Supracondylar Process of Humerus: A Case Report

Occasionally, a bony spine of variable size, the supracondylar process, projects distally from the anteromedial surface about 5cm proximal to the medial epicondyle, to which it is joined by a band of fibrous tissue. It was found in one per cent of living white subjects and less often in the colored races; only in rare instances it is associated with clinical signs, usually median nerve neuralgia. Here, we reported a case of incidental finding of supracondylar process without a definite symptom.

2.Calcific Tendinitis of Supraspinatus Tendon

Of the shoulder syndromes, acute calcific tendinitis of the rotator cuff is the least difficult to differentiate, for the mechanical and inflammatory aspects combine to produce a picture of severe, and localized pain which presents in obvious distress, splinting the affected shoulder by holding the arm to his side. The radiologic picture corresponds poorly to the clinical picture, with visible calcium deposits ranging from thin crescents to large fluffy amorphous masses. Acute calcific tendinitis is a self-limited process with a natural history of 6 to 14 days. Relief is dramatic when the inflammatory and mechanical cycle is broken, but the potential remains for exacerbation as for more serious sequellae such as adhesive capsulitis. We expereinced 5 lesions of calcific tendinitis in supraspinatus tendon treated by surgical removal in 4 lesions and by conservative measure in one lesion.

3.An Imported Case of Intestinal Capillariasis Presenting as Protein-Losing Enteropathy.

Intestinal capillariasis is caused by a small trichurid nematode, Capillaria philippinensis, which infects freshwater fish as intermediate hosts and subsequently fish eating birds as definitive hosts. It has occurred in areas such as the Philippines and Thailand where people eat raw fish. We report an imported case of intestinal capillariasis in a 31-year-old Korean man who lived in Saipan for ten years. He suffered from diarrhea with weight loss, abdominal distension, and pitting edema in the lower extremities for two years. He ate raw freshwater fish in the Saipan 2 years ago. The diagnosis was confirmed by jejunal biopsy. Flat mucosal surface without villi contained sections of numerous round worms with stichosome, larvae, and eggs which were identified as Capillaria philippinensis. The patient was successfully treated with albendazole.

4.A study of the Sports Injury of Volleyball Players in Korean Young Women

Certain motion in the sports of volleyball can cause injury. For examples, excessive spiking, jumping, receiving and rolling may produce strain, sprain and stress fracture, etc. of all extremities and spine. The authors analized injuries of 160 Korean young women volleyball players treated and managed at Department of Orthopedic Surgery, National Police Hospital from September, 1973 to October, 1977. The results were as followings: 1. Of 160 cases, age distribution was ranged from 15 to 26 years and average was 20. 7 years. 2. Out of 495 injuries, 113 (23%) were upper extremities, 93 (19%) were lumbar and lumbosacral regions and 289(58%) were lower extremities. The most common site of injury was knee joint. 3. As to the ratio of the side, in the upper extremities 77% were in the right side and 23% in the left, but in the lower extremities both sides were nearly equal. 4. The injuries were composed of strain, sprain and the others, including stress fracture in 14, linear fracture of patella in 2, chip fracture of ulnar styloid process in 4, H.I.V.D. in 4 and meniscus tear in 6, etc.. 5. The injuries were supposed to be prevalent for the first one month after change from one to another team (14%). 6. The injury was usually more frequent in incidence among the exercise in winter and the heating in Gymnasium also one of the important factors to induce the injury, the injuries were less frequent at the good heating (40%). 7. It was concluded that the career of play did not affect significantly to incidence of the injury.

6.Genetic Variants of Thromobomodulin Gene as Risk Factors for Myocardial Infarction.

Thrombomodulin (TM) is thrombin receptor present on the luminal surface of endothelial cells. Because the thrombin-TM complex acts as an anticoagulant, the functional variants or deficiency of TM may lead to increment of thrombotic tendency. In this study, we screened the genetic variants of the TM gene in patients with myocardial infarction (MI) and analyzed the genotype to elucidate the effects of genetic variations of TM gene on the development of the MI. We screened a promoter region and coding sequence of the TM gene using single strand conformation polymorphism-heteroduplex analysis and identified three common genetic variants: those were TM G-33A, TM Ala455Val, and TM C1922T. The genotype frequencies were investigated in the patients with MI (n=234) and control subjects (n=291) by the method of allele-specific oligomer hybridization. The frequencies of mutant genotypes (TM -33A, TM 455Val, and TM 1922T) were higher in patient group compared to the control subjects in males while there were no significant differences in females. In the multiple logistic regression analysis, TM 455Val and TM 1922T alleles were independent risk factors for MI (OR[95% CI: 1.799[1.125-2.878] p=0.014 and 5.624[1.019-31.025], p=0.048, respectively) in males. However, the genetic variations were not independent risk factors for MI in females. There were significant linkage disequilibriums among three genetic variants. These linkage disequilibriums explain the similar effects of three genetic variants on the development of MI. To investigate the effect of the TM G-33A mutation on TM promoter activity, the two TM promoter constructs (pTM-355 and pTM-125, bearing TM -33G or TM -33A) containing of firefly luciferase gene were transfected into HepG2, BAE, and CHO cells. The promoter activities were higher in the promoter constructs with TM -33G compared to the constructs with TM -33A in pTM-355. These results suggest the possibility of the positive predisposing effect of TM -33A allele on MI in males. The functional study for TM Ala455Val and TM C1922T should be followed to elucidate the genotype effects of these mutations on the development of MI. In this study, we identified three genetic variants of TM gene and showed the significant associations between genetic variants and MI in males. These results proposed that TM gene is an attractive candidate for genetic risk factor for MI in Koreans.

7.Expression of Trans forming Growth Factor-a and Proliferating Cell Nuclear Antigen in Human Gliomas.

To evaluate the expression of transforming growth factor-alpha(TGF-alpha) and proliferating cell nuclear antigen(PCNA) and its relation to the differentiation of the tumors, immunohistochemical studies were performed in 49 human gliomas. Tumors were graded by a 3-grade-system; grade I=low grade glioma, grade Il=anaplastic glioma, grade III=glioblastoma multiforme. TGF-A and PCNA were predominantly expressed in malignant gliomas compared with benign gliomas. Malignant gliomas revealed 87% TGF-A reactivity, while benign gliomas revealed 26% TGF-A reactivity. The proliferation index with PCNA was 26%+/-7%(mean+/-standard deviation) in malignant gliomas and 5%?% in benign gliomas. A strong positive correlation between tumor grade and extent of TGF-A and PCNA expression was found(P<0.0001, Chi square and P<0.002, T-test). Synchronous expression of TGF-A and PCNA was observed in 16 cases(33%). The results of this study support the suggestion that the expression of TGF-A might be a useful prognostic indicator in human gliomas.

8.Clinical Observation of Geut

Gout is a hereditary condition of disturbed uric acid metabolism in which urate salts are deposited in articular, periarticular and subcutaneous tissues. Clinically, it is characterized by recurring attacks of acute arthritis, intervals of freedom from pain, and in late stages by crippling deforming arthritis, nephritis, urinary calculi and cardiovascular lesions. Seventeen patients with gout managed at the department of orthopedic surgery, National Police Hospital, from February, 1972 to December, 1977, were reviewed and the following results were obtained. 1. Most of cases were between 30 and 49 years old and all the cases were male. 2. The most common season of occurrence of acute attack was winter and the least common season was summer. 3. The recurring attacks were more frequent than the first attacks. 4. The precipitating factors could be found usually including family history, dietary indiscretions, exposure to cold and stress conditions. 5. The average serum uric acid level during acute attacks showed 12. 9mg% and all the cases except 2 cases noted increased ESR with average 33.9mm/hr. The leucocytosis were noted in 6 cases. 6. The first involved joints were the M-P joint of big toe in 13 cases and the knee joint in 3 cases. 7. The radiological multiple punched-out bone lesions were found in 8 cases. 8. One tablet (0. 65mg) of colchicine was given every hour until the diarrhea appeared or the pain subsided, in 13 cases. Among them, when 10 tablets of colchicine were taken, the diarrhea appeared in 10 cases and the pain subsided in 8 cases. So we recommend 7~8 tablets for relief of G-I discomfort in management of gout. 9. The cases of gouty arthritis may be found more and more in our country because the stress condition and meat intake might be increased such as in Europe and America.

A simple out-patient's procedure with the use of a simple 17 or 18G hypodermic needle for the percutaneous release of trigger finger is described. The authors are reporting the satisfactory result obtained by means of percutaneous release of tendon sheath without skin incision in 21 trigger fingers of 17 patients treated at the out-patient-basis.
