1.Investigation of the morbidity and influencing infactors of plantar fasciitis in nurses

Objective To investigate the morbidity and influencing infactors of plantar fasciitis in nurses. Methods The clinical nursing staff of 829 people were included in this series. 71 people of them conform to the standard of plantar fasciitis. According to the proportion of 1:1, 71 nurses without plantar fasciitis were included in the control group. The thickness and hardness index of the plantar fascia were detected by ultrasound elasticity. The survey contents including age, the average working hours every day (h), the average standing time in working day (h), body weight, body mass index (BMI), number of night shift, average walking time every day (min) and the average weekly running time (hour) were conducted. The single factor and multi-factor regression method were used to analyze the influence factors of the onset of plantar fascia. Results 71 cases of 829 nurses were with plantar fasciitis, and the incidence was 8.56%. The thickness in observation group was (3.86±0.53) mm, which was significantly smaller than (2.67±0.39) mm of the control group (P<0.05). Hardness index was 2.01±0.23, lower than 3.83±0.70 in control group (t=14.09, P<0.05). Single factor analysis results showed that the average standing time in working day, weight, BMI, average walking time every day, the average weekly running time were different between the two groups (P<0.05). The regression equation: the plantar fascia hardness index =-25.34 + 4.78 × the average standing time in working day + 3.45 × weight + 1.22 × BMI. The equation of the ability to explain the plantar fascia hardness index reached 72.2%, and had a good effectiveness. Conclusions Body weight and BMI, prolonged standing work may induce the nurse plantar fasciitis, and they are independent risk factors.
2.Preliminary study on the efficacy and safety of levtiracetam in infant with epilepsy

Objective To preliminary evaluate the efficacy and safety of Lev therapy in infant with epilepsy .Methods To chil-dren with infant epilepsy in the Department of Neurology of Chongqing Children′s Hospital from May 2010 to February 2012 were enrolled .All of them took Lev mono-therapy or adjunctive therapy .Investigation was done by self-controlled and retrospective re-search .There were 70 patients received Lev therapy ,aging from one month to one year old ,and 39 patients received mono-therapy , 31 patients received adjunctive therapy .All subjects were divided into 2 groups ,12 patients in partial seizure group and 58 patients in generalized seizure group .Then all patients took a 7 months Lev therapy after initial medication .The seizure frequency change , seizure situation ,side effect were observed .Results During the follow-up ,5 patients lost their follow-up(3 case in mono-therapy group ,2 case in adjunctive therapy group)and 3 patients died .In mono-therapy group ,83 .33% of patients got responder and 16 . 67% of patients unmodified or worsen ,while those in adjunctive therapy group were 58 .62% and 41 .38% ,respectively .And there was significant difference between two groups(P<0 .05) .In the partial seizure group ,81 .81% of patients got responder and the 18 .18% of patients unmodified or worsen ,while those in generalized seizure group were 70 .37% and 29 .63% .And there was no significant difference between partial and generalized seizure group(P>0 .05) .During the study ,22 patients ended Lev therapy for some reasons .77 .27% of patients discontinued in 1-6 months and 22 .73% of patients in 7-34 months .Number of the adverse reac-tions was 14 .Main adverse reactions observed were anorexia and sleep disorders in early .3(4 .61% )of 65 patients discontinued ther-apies for the adverse reaction ,but there was no death caused by adverse reaction .Conclusion The Lev mono-therapy and adjunctive therapy are both effective in infant with epilepsy ,and Lev therapy is effective in infant epilepsy .
3.Efficiency Assessment of Improved Disinfection Method for Automatic Bottle Washer

Objective To find out the defects existing in the rinsing and sterilizing process of the automatic bottle washer and to put forward an improvement measure and efficiency assessment.Methods To make the washer more effective,an automatic brush pre-washing process and a rotary spraying device were designed and used.The total bacteria amount,the total coliform group and the disinfectant (ClO2) residual level were employed as the assessment parameters.Results An external automatic pre-washing machine was installed on the bottle washer;the bottle washer was innovated so that its spray nozzle could be in directions and the rinsing and sterilizing process could be enhanced.The results showed that the improved machine ran well in treating the polluted and recycled bottles and all the assessment indexes were better than that of before improvement.Conclusion The improved automatic bottle washer is considered effective in cleanout and sterilization of bottles with a satisfactory result.
4.Effect of Tetramethyl pyrazine on serum levels of IL-1beta, IL-6, and IL-2, and NO and PGE2 in the synovial fluid of CIA rats: an experimental research.

OBJECTIVETo observe the effect of Tetramethyl pyrazine (TMP) on the cytokines and inflammatory mediators in the serum and the synovial fluid of collagen-induced arthritis (CIA)rats, and further to investigate its possible mechanisms for treating rheumatoid arthritis (RA).
METHODSType II CIA rat model was established. Rats in the TMP group were administered with TMP at 50 mg/kg and 100 mg/kg, once daily. Dexamethasone at 2.0 mg/kg was intramuscularly injected to those in the Dexamethasone treated group, once daily. Normal saline at 2 mL/kg was given to those in the normal control group and the model group, once daily. All medication was started from the 7th day, lasting to the 35th day. CIA rats' foot swelling degree was observed. Contents of serum IL-1, IL-6, IL-2, NO and PGE2in the synovial fluid were detected by radioimmunoassay and nitrate reduction method.
RESULTSCompared with the normal group, the foot swelling obviously increased, contents of NO and PGE2 in the synovial fluid were obviously elevated in the model group (P < 0.01). Compared with the model group, the foot swelling could be obviously inhibited by 100 mg/kg TMP and Dexamethasone; serum levels of IL-1 and IL-6 obviously decreased, serum IL-2 level obviously increased, contents of NO and PGE, decreased (P < 0.01). TMP 50 mg/kg could obviously inhibit the foot swelling of CIA rats (P < 0.01). There was no statistical difference in other indices (P > 0.05).
CONCLUSIONSTMP at 100 mg/kg showed obvious inhibition on CIA rats. Its inhibitory effect might be correlated to inhibiting activities of endogenous cytokines and the generation of inflammatory mediators in inflammation local regions, improving contents of anti-inflammation cytokines, and inducing the balance of the inflammatory cytokine network.

9.Limb-shaking transient ischemic attack responsive to nimodipine: A case report

Limb-shaking transient ischemic attack (TIA), a rare manifestation, is commonly caused by severe
stenosis or occlusion of an extracranial internal carotid artery. Such patients are usually treated with
surgical revascularization or anti-platelet therapy. We present a 56-year-old woman with 6 months’
episodic attacks starting with mouth skewed to the right and a sensation of ‘weakness’ involving
predominantly her left arm, and at times, also involved the left leg. This was immediately followed by
rhythmic jerky movements of the left arm and at times, also involved the left leg. Magnetic resonance
angiography revealed severe stenosis of M1 segment of the right middle cerebral artery. The patient’s
symptoms were signifi cantly improved by treatment with anti-platelet drugs and nimodipine.

Indoleamine 2,3-dioxygenase (IDO) is a kind of immune regulation enzyme.IDO is the only rate-limiting enzyme that can catalyze the oxidative clearage of indole ring in tryptophan and its catabolism along the kynurenine pathway.IDO1 is one of the important mechanisms in maternal immune tolerance,which participates in the regulation of maternal-fetal immune relationship.The newly discovered IDO2 also plays an important role in this relationship.The abnormal expression of IDO2 is associated with the male infertility.
