[Objective] To summarize the clinical experience of the national physician master professor GUO Chengjie application of malt, provide reference for clinical application. [Methods] By following GUO Lao clinical practice study and consulting GUO Lao of previous clinical cases, collection, analysis, summarize the experiences of application of malt of GUO Lao and features, case example is given. [Results] Guo Lao clinical application of this product, FaGu and innovation, to create, dosage, many decoction is exquisite, in prescription compatibility, such as liver and qi, clearly purging liver and gall, disinhibiting dampness and relieving jaundice shall use raw product, lactation most appropriate for raw and fry, back to milk needs fry and large dosage, the food is fried with appropriate medium doses, good clinical results will often begot. [Conclusion] GUO Lao unique method of clinical application of malt, clinical each fine effect, is worth learning to imitate.

3.A analysis of clinical value about endoscopic nipple modified resection in duodenal papillary adenoma resection

Objective: To analyze the clinical value of modified endoscopic nipple resection in duodenal papillary adenoma resection Methods: 40 patients with duodenal papillary adenoma were randomly divided into observation group (20 cases) and control group (20 cases)according to the random number table. The patients of control group were treated by conventional resection of papillary adenoma, while the patients of observation group were treated by modified endoscopic nipple resection. The resection rate and the implantation of stent between the two groups were compared. Besides, the occurrence rate of complication and postoperative follow-up between the two groups also were compared. Results: The complete resection rate of observation group (95.00%) was not significant difference with that of control group (90.00%), while en bloc resection rate of observation group (85.00%) significant higher than that of control group (55.00%) (x2=4.28, P<0.05).There was not statistically significant difference between the two groups about the implantation of stent (x2=0.36, P>0.05). In postoperative three months, the difference of recently complication between the two groups was significant (x2=6.14, P<0.05). While the difference of long-term complication between the two groups was no significant (x2=0.36, P>0.05) in more than postoperative three months. After the follow-up of one year, the recurrence rate of observation group (10.34%) was no significant with that of control group (14.29%)(x2=0.17, P>0.05). Conclusion: The modified endoscopic nipple resection can enhance the en bloc resection rate, and reduce the incidence of complications and the recurrence rate. Besides, it has many dominances including higher feasibility in clinical practices. Therefore, its promotion is worthy.
4.Clinical presentations of multiple sclerosis:an analysis of 413 cases

Objective To summarize the features of multiple sclerosis (MS) in China. Methods The clinical manifestations and inducements of 413 MS patients are comprehensively analyzed using clinical statistical method. Other parameters are also analyzed by electromyography and brainstem auditory evoked potential. Results MS in China mainly invades young and middle-aged people, with a higher ratio in female than in male. MS was found mostly acute or subacute in onset and usually due to an inducement mainly consisting of common cold, fever, cyesis and fatigue. The first seen symptoms were various and complicated, but mainly showed different types of visual disturbance such as the acuity of vision decrease, constriction of visual field and diplopia. Limb paralysis, sensory abnormality and visual disturbance were found the commonest symptoms. Lhermitte’s sign was common. Paroxysmal symptoms as algospasm and seizure were relatively common. Electromyography and brainstem evoked potential showed that peripheral nervous system damage might occur in MS. Spinal cord and visual nerve were found the mostly invaded according to the clinical data. Conclusion The features of MS in China should be different from those in the western countries.
5.Effect of Huangqi Injectio on Abnormal Expression of Aquaporin-1 Caused by Glucose Lactate Peritoneal Dialysis Solution

Objective To observe the effect of Huangqi (Radix Astragali seu Hedysari) Injectio on the abnormal expression of aquaporin-1 (AQP-1) caused by glucose lactate peritoneal dialysis solution. Methods Different glucose concentration lactate peritoneal dialysis solutions and Huangqi Injectio given to the rats intraperitoneally. 6.Pathogenetic investigation of multiple sclerosis.

Objective TO explore the etiology and pathogenesis of multiple sclerosis. Methods The information of pa-tients from 413 MS was analyzed comprehensively by the clinical statistical methods. Results Common cold was the com-monest factor causing MS, and fever,fatigue and childbirth were also the common causes. Immunological disturbance exis-ted in MS. CD~+_4 cell decreased in most of MS patients, but the distribution of CD~+_8 cell was not regular. The level of sexhormones was abnormal in MS,but it was not the same in male and female. There were three patients having family histo-ry of MS. Conclusion Virus infection,sex hormones and heredity factors are all related to MS through facilitating immu-nological disturbance.
7.CT EVALUATION OF INCREASED INTRACRANIAL PRESSURE WITHOUT LOCALIZING SIGNS

The CT findings in 200 cases of increased intracranial pressure without localizing signs were reported 132 out of the 200 cases (63.1%) were CT positive, among them there were 8 cases with brain metastatic cancer, 11 cerebral cysticercosis, 14 intracranial inflammatory lesions, 18 hydrocephalus with symmetrical ventricular enlargement, 66 mass lesions in the brain and 15 others. 77 out of the 209 cases (36.9%) were CT negative, among whom the diagnosis of the 61 cases coincided with that of BIH. It seems that our BIH cases were much different from those typical cases reported abroad, and the cause would be pending further investigation. It is to be noted that the following remarks, according to our study, would be associated with the positive findings in CT examination: 1. age, 60 years; 2. course of disease, within 6 months; 3. papilloedema and/or "soft" signs; and 4. local and severe abnomalities in EEG.
8.Investigation of brain-derived neurotrophic factor and glial cell line-derived neurotrophic factor concentrations in serum and cerebrospinal fluid of patients with multiple sclerosis and neuromyelitis optica

Objective To investigate brain-derived neurotrophic factor (BDNF) and glial cell line-derived neurotrophic factor (GDNF) concentrations in serum and cerebrospinal fluid (CSF) in patients with multiple sclerosis (MS) and neuromyelitis optica (NMO),and their neuroprotective effects.Methods Sixty-two patients (49 patients were MS and 13 patients were NMO) and 21 controls were investigated in our studies.The disability severity in MS and NMO patients in their relapse period was assessed by the Expanded Disability Status Scale (EDSS).MRI scanning of brain,spinal cord or optic nerve was examined and the oligoclonal band in serum and CSF were detected.BDNF and GDNF concentrations in serum and CSF were assessed by Liquid Assay.Results There were no significant differences of BDNF (μg/L,5.616±0.650 in serum and 0.186±0.012 in CSF of MS patients;6.584±0.929 in serum and 0.176± 0.006 in CSF of NMO patients) and GDNF (μg/L,0.039 in serum and 0.080 in CSF of MS patients;0.029 in serum and 0.050 in CSF of NMO patients) concentrations in serum and CSF in patients with MS and NMO in relapse,compared with those in controls.There was a positive correlation between BDNF and GDNF concentrations in CSF (r=0.756,P=0.000),and a negative correlation between BDNF and GDNF concentrations in serum (r=-0.329,P=0.018).There were no correlations of BDNF and GDNF concentrations in serum and CSF with EDSS,blood brain barrier index,Delpech index and Tourtellotte synthesis rate.There were no significant differences of BDNF and GDNF concentration in serum and CSF between NMO/MS patients with and without atrophy.Conclusions The level of BDNF in patients with MS and NMO is correlated with that of GDNF,which may have a synergistic neurotrophic effect on MS and NMO.BDNF and GDNF are not associated with the blood-brain harrier destruction and lgG synthesis in central nervous system.However,associations of BDNF and GDNF with functional disability and neuron atrophy in NMO and MS patients still need further studies.
9.Perioperative nursing management of 151 hospitalized elderly patients without caregiver

Objective To explore an effective perioperative nursing management mode for elderly patients without caregivers. Methods The perioperative nursing management mode was implemented for 151 elderly patients without caregivers from 9 surgery departments. The work mode included admission safety assessment,nursing care classification,design of clinical nursing pathway,establishment of nursing group and quality control. Results The new work mode provided elderly patients with high-quality nursing care,ensured patient safety,increased patient and relatives satisfaction. Conclusion The perioperative nursing management mode is an effective way to ensure patient safety and promote patient comfort during perioperative period for elderly patients without caregivers.
10.Effect of venlafaxine on cervical cancer patients with depressive symptoms

Objective To study the effect and reliability of venlafaxine on depression and anxiety symptoms in cervical cancer patient,and to assess the overall quality of life before and after treatment.Methods We treated 63 cervical cancer patients with depression by use of venlafaxine for 8 weeks,at the same time,evaluated their emotion state.change of quality of life and the adverse effect with HAMD,HAMA,TESS and laboratory tests.Results After 8 weeks.patients' scores of anxiety and depression decreased significantly from the baseline(P<0.01),effective rates were 65%and 52%respectively.Three domains of quality of life(physiology,psychology and independence)became much better than those of baseline(P<0.05).Conclusions This studyshows that venlafaxine can reduce the depression and anxiety symptoms in cervical cancer patients,and the react time is 2 to 4 weeks.Venlafaxine with the treatment of depression and enxiety can improve the quality 0f patients' life.
