Development of cataract is a well-documented ocular complication of systemic cortico steroid use in renal transplant patients. The purpose of this paper was to examine the result of cataracts in these patients and its correlation between development of cataract and steroid dose, rejection episodes, and human leukocyte antigen (HLA) type. Thirty patients (50 eyes) received cataract surgeries after renal transplantation. Thirty eight eyes (76%) were found to have posteiror subcapsular opacity. Forty seven eyes (96%) had a good postoperative visual acuity. Two eyes had a minimal macular edema, and other two eyes showed transient increase of intraocular pressure. The daily steroid dose of study group was significantly lower than that of control group. There was no significant correlation between cataract formation and rejection episode, cyclosporin dose and azathioprine use. HLA B13 was present in control group to a great extent (p<0.05). The surgical results of cataract in renal transplant patients were relatively good, and careful attention was needed when steroid dose was changed. We need further study about to prevent the formation of cataract.

BACKGROUND AND OBJECTIVES: This study was designed to investigate the influence of human leukocyte antigen (HLA) on genetic susceptibility to Meniere's disease and to evaluate the correlation between HLA genotypes and results of examination for various clinical factors of Meniere's disease. MATERIALS AND METHOD: The distribution of HLA class I was examined in 39 patients with MD and 199 healthy controls. For HLA-A and B, the serologic typing was performed according to a standard microlymphocytotoxicity technique. The HLA-C typing was performed by the ARMS-PCR method at DNA level. RESULTS: The frequencies of HLA-CwX0303 (RR=, p<0.02), and CwX0602 (RR=, p<0.03) were significantly increased in patients with Menieres disease, when compared to the controls. However, HLA-B44 (RR=, p<0.004) and CwX0102 (RR=, p<0.03) were significantly decreased in the patients compared to the controls. When an association between hearing level and the presence of HLA alleles was evaluated, the frequencies of HLA-B13 (RR=, p<0.004), CwX0303 (RR=5, p<0.02) and CwX0602 (RR=5, p<0.02) were significantly increased and the frequencies of B44 (RR=1, p*lt;0.02) and CwX0102 (RR=1, p<0.03) were significantly decreased in patients with the state of mild to profound hearing losses, compared to the controls. HLA-B13 showed a different distribution pattern between patients with and without hearing losses. CONCLUSION: These results suggest that some HLA alleles may be useful genetic markers in conferring the susceptibility to Meniere's disease and in implying a prognosis in Korean patients.

To study the gene polymorphism of HLA-A, B, DRB1 alleles in patients with chronic myelogenous leukemia and to explore the correlation of HLA with chronic myelogenous leukemia, the polymerase chain reaction-reverse sequence specific oligonucleotide (PCR-RSSO) was used to analyze the polymorphism of HLA-A, B, DRB1 alleles of 293 CML Patients and 406 randomized and synchronous blood donors (healthy and unrelated with patients) from Guangdong Han population. The results indicate that the gene frequency of HLA-A*24 in CML group was 15.53% lower than that of control group (22.09%, RR = 0.63, p = 0.005); the gene frequency of HLA-B*13 in CML group was 10.41% higher than that of control group (6.74%, RR = 1.68, p = 0.016). The gene frequency of HLA- DRB1*14 in CML group was 7.51% lower than that of control group (11.89%, RR = 0.58, p = 0.008). The differences were all statistically significant. It is concluded that the gene frequency of HLA-A*24, HLA- DRB1*14 in CML patients is significantly lower than normal people in Guangdong. The gene frequency of HLA-B*13 in CML patients is significantly higher than normal people in Guangdong. Further study is needed to make sure whether HLA-A*24 and HLA- DRB1*14 are protective gene markers for CML acquisition on Guangdong Chinese Han population and whether HLA-B*13 is a gene marker for CML susceptibility on this population.

According to recent studies, the immunogenetic factors are thought to be account for a part of the etiopathogenesis of autoimmune thyroid disease. In Korea, there was one report on the relationship between HLA DR5, DR8, B13 and autoimmune thyroid disease. There were also several reports on a familial hereditary transmission of autoimmune thyroid disease in other countries but not in Korea. We describe the occurrence of autoimmune thyroid disease that affected three members of a family through three generations. This is the first report on familial hereditary transmission of autoimmune thyroid disease in Korea. We report on an 80-year-old woman who presented with Hashimoto's thyroiditis, her 53-year-old daughter who had Graves' disease, and her 29-year-old grand-daughter who had Graves' disease. In order to identify the immunogenetic influence in these cases, HLA haplotypes & thyroid autoantibody were studied. HLA DRB3*02 was obseved in each of the patents. HLA DQB1*0301, DR11, DQB1*05031 and DR14 were observed in the two cases. However, HLA B13, DR5 and DR8 were not observed. The patients are currently undergoing follow-up using PTU, methimazole and synthyroid medication.
