1.The effect of Shihusan on intracellular free calcium of human retinal neural cells

Objective To explore the effects of Shihusan on the intracellular free calcium of human retina cells and the protection from the damage of glutamate. To investigate its function and mechanism in retinal degeneration diseases, and to offer the basis on which to treat the similar disease clinically. Design Experimental study. Participants Retinal cells from normal human eye. Methods Cultivation of human retinal neural cells was perfonned. The changes of fluorescent density of intracellular free calcium of cultured cells labeled with Fluo3/AM before and after adding Shihusan and verapamil were detected with laser scanning confocal microscopy. In addition, the effect of Shihusan on calcium overload provoked by glutamic acid was observed. Main Outcome Measures The level of intracellular free calcium of retinal cells. Results The retinal cells adhered, spread and grew well in vitro. The fluorescence intensity increased 88% in glutamic acid group, decreased 14.8% and 57.3% in Shihusan group and verapamil group respectively. Shihusan couldn't decrease the intracellular Ca2+ that was increased by glutamic acid, but could inhibit the increasing tendency induced by glutamic acid. The difference was significant compared to control group (P=0.000). Conclusions Shihusan might decrease the level of intracellular free calcium in cultured human retinal cells, and resist the damage of glutamate. Its mechanism was related possibly to resisting calcium overload and inhibiting apoptosis.
2.A Multi-center Randomized Double-blind Comparative Clinical Trial of Levocetirizine and Cetirizine for the Treatment of Chronic Idiopathetic Urticaria

Objective To investigate and compare the effect and safety of levocetirizine and cetirizine for the treatment of chronic idiopathetic urticaria (CIU). Methods A multi-center, randomized and double-blind comparative clinical trial was employed. The patients with CIU were divided into levocetirizine group and cetirizine group. Levocetirizine (5mg/day) or cetirizine (10mg/day) were taken once daily for 28 days, and were followed up on the 7th day, 14th day and 28th day after starting treatment. Results One hundred and thirty cases were evaluable for the effect and safety at the end of the study. The effective rates in levocetirizine group and in cetirizine group were 73.44% and 77.27% on the 7th day after treatment, 82.81% and 81.82% on the 14th day, and 89.06% and 81.82% at the end of the therapy respectively. There was no significant difference between the two groups. The drug adverse reaction for levocetirizine group and cetirizine group were 14.06% and 18.18% respectively, which include mouth dryness, dizziness etc. Conclusion Levocetirizine is an effective and safe agent for the treatment of CIU.
3.Reconstruction and histologic characteristics of bilayer skin substitute inoculated of human hair follicle cells

Objective To study histologic characteristics of bilayer skin substitute reconstructed by cells from human hair follicle and whether the skin appendage can be induced based on the bilayer skin substitute.Methods After composite chitosan bilayer skin substitute was reconstructed with dermal papilla cells and outer root sheath cells or dermal sheath cells and outer root sheath cells,its histologic characteristics was investigated in vitro and after transplanted onto SD albino rats.Results Composite chitosan bilayer skin substitute reconstructed by cells from hair follicle had closely arranged epithelium cells and outstanding cornification;Epithelial cords linked with epidermis could be seen in dermis.However,there was no certain hair follicle-like structure formation either in vitro or in vivo.Conclusion Hair follicle cells are good source for skin substitute reconstruction,but it can not induce skin appendage formation through skin substitute by now.
4.Expressions of tissue factor and tissue factor pathway inhibitor in patients with acute graft-versus-host disease after allogeneic hematopoietic stem cell transplantation.

This study examined the expressions of human serum tissue factor (TF) and tissue factor pathway inhibitor (TFPI) in patients with acute graft-versus-host disease (aGVHD) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) and their clinical significance. The serum TF and TFPI levels were detected by ELISA in 28 allo-HSCT recipients before and after the transplantation and the changes of TF and TFPI levels were dynamically monitored at different phases of the disease. No significant differences in the serum TF and TFPI levels were found in allo-HSCT recipients in the absence of aGVHD or with grade I aGVHD before and after the transplantation. The levels of serum TF and TFPI were substantially increased in the patients with gradeII aGVHD at the peak of aGVHD (P<0.05) and they were even higher in the patients with grade III-IV aGVHD (P<0.01). When the conditions became stable after treatment with immunosuppressive agents, the serum TFPI level was decreased to the baseline level (P>0.05) and the TF level was lowered but still higher than the baseline level (P<0.05). It was concluded that the levels of serum TF and TFPI were increased significantly in the patients with grade II-IV aGVHD after allo-HSCT and decreased markedly after the treatment. Monitoring the levels of serum TF and TFPI in the patients with allo-HSCT is important to predict the occurrence, outcome and prognosis of aGVHD.
5.Research of gestrinone-related abnormal uterine bleeding and the intervention in the treatment:a multi-center, randomized, controlled clinical trial

Objective To investigate the incidence, influencing factors and intervention of gestrinone-related abnormal uterine bleeding at different dosage of gestrinone in the clinical treatment. Methods This was a multicenter, randomized, control study of 195 Chinese women with endometriosis or adenomyosis from June 2011 to November 2013. The subjects were randomized into three groups with oral administration of gestrinone, 2.5 mg dose at one time;twice a week group:67 cases with oral administration twice a week last three months;double dose first month group:67 cases with oral administration triple times a week at first month, then twice a week for two months; three times a week group: 61 cases with oral administration three times a week last three months. The improvement of the abnormal uterine bleeding, the changes in estrogen, liver function and blood coagulation were evaluated. At the same time, B-ultrasound examination evaluation were performed. Results (1) Three months later, the incidence of abnormal uterine bleeding in twice a week group was 30%(20/67), in double dose first month group and three times a week group were 7%(5/67) and 16%(10/61) respectively, there were significant difference between three groups (P<0.05). The incidence in double dose first month group was the most lower. (2) Univariate analysis showed that the dosage and ovarian size were the significant factors for abnormal uterine bleeding (OR=0.461, P=0.003; OR=0.303, P=0.016); logistic regression analysis demonstrated that the risk of abnormal uterine bleeding in double dose first month group was the lowest when compared with twice a week group and three times a week group, the risk in twice a week group was 5-fold higher than that in double dose first month group (OR=0.211,P=0.011). The incidence of abnormal uterine bleeding in participants with abnormal ovarian volume results from ovarian cyst or ovarian surgery was significantly lower than those with normal ovarian volume (OR=0.304, P=0.018). (3) After the treatment of three months, there were no significant difference in alanine transaminase level between the groups (P>0.05). The body mass index significantly increased in three group (P<0.05), but there were no significant differences between the groups (P>0.05). As for blood coagulation, there were also no significant differences between the groups (P>0.05). Conclusions Double dose of gestrinone in the first month could significantly decrease the incidence of gestrinone-related abnormal uterine bleeding. It is a more optimied dosage of gestrinone and without severe side effects. Clinical trial registration Chinese Clinical Trial Registry, registration number: ChiCTR- TRC-12002327.

Objective In order to enhance the overall promotion of science and technology awards ,as well as administra-tion .Methods Combined with the particular procedures of the China Anti-Cancer Association Science and Technology Award , this paper analyzed the application of medical science and technology award evaluation system .Results The system improved the standardization of recommendation work ,increased the rigor of review process and enhanced the working efficiency of ad-ministrative department .However ,with the development of new technologies such as big data ,artificial intelligence ,along with the information needs of "healthy China" and other global ,relevance characteristics ,the system should bring in more brand-new design ideas and the function extension .Conclusions The system should constantly adapt to new needs ,focusing on the whole process of medical science and technology reward to expand the system function and strengthen the integration with related systems can promote the overall improvement of scientific and technological awards .
7.Applied value of routine ultrasound and shear wave elastography in the diagnosis of triple negative breast cancer

Objective To investigate the applied value of routine ultrasound and shear wave elastography (SWE) in the comparison between triple negative breast cancer ( TNBC) and non-triple negative breast cancer (non-TNBC) . Methods A total of 120 lesions in 120 patients with pathologically confirmed breast cancer were retrospectively reviewed ,which were examined by routine ultrasound and SWE before surgery . According to immunohistochemical analysis ,those were divided into TNBC group and non-TNBC group . The features of routine ultrasound ( including shape ,orientation ,margin ,boundary ,echo pattern ,microcalcification ,posterior features of each lesion and blood flow characteristics) and the SWE index[including the average value of the lesion stiffness(Emean) ,the maximum value (Emax) ,the minimum value (Emin) ,the standard deviation (SD) ,and the ratio of the normal breast gland tissue (Eratio)] in the two groups were analyzed and compared . Results ① Two-dimensional ultrasound :TNBC lesions mostly showed the margin of microlobulated ,abrupt boundaries and no internal microcalcification ,but non-TNBC lesions were more likely to have an angular or spiculated margin ,an echogenic halo of boundary and a few microcalcification in the interior ,which were statistically different( P <0 .05) . The grade of blood flow and resistance index between TNBC and non-TNBC group showed no statistical difference( P >0 .05) . ② The values of Emax ,Emean and Eratio were statistically different( P <0 .05) ,with AUCs of 0 .811 ,0 .781 ,and 0 .770 , respectively . Conclusions Routine ultrasound and SWE can comprehensively analyze the morphology ,blood flow and stiffness features of breast lesions ,which provides more valid information to identify T NBC and non-T NBC .

9.Flipped classroom teaching method based on micro-lecture in bedside teaching of standardized training in pediatric residents

Objective:To explore the effect of flipped classroom teaching method based on micro-lecture in bedside teaching of standardized training in pediatric residents.Method:s A total of 30 residents in pediatric rotation were selected as subjects, with 15 subjects in each group. The students in the experimental group carried out the bedside teaching of "flipped classroom based on micro-lectures", while the students in the control group carried out traditional teaching rounds. The bedside teaching of the two groups were conducted every two weeks, and the same cases were selected with the same instructors. Both groups completed the quizzes before the bedside teaching. After six months' training, the difference of training effect between the two groups was compared through the examination (case analysis and clinical skills).Result:s The scores of the quizzes in the experimental group were significantly higher than those in the control group ( P<0.05). The scores of case analysis and clinical skills in the experimental group were significantly higher than those in the control group ( P<0.05), and logical thinking and doctor-patient communication abilities in the experimental group were significantly better than those in the control group( P<0.05). The results of the questionnaire survey in the experimental group showed that the students believed that the new bedside teaching method could promote learning initiative, help to inspire clinical thinking and improve doctor-patient communication abilities. Conclusion:Flipped classroom teaching method based on micro-lecture in bedside teaching of pediatric residents not only strengthens their basic theoretical knowledge, but also improves their clinical skills, especially in establishing correct clinical thinking and mastering doctor-patient communication skills.
10.Clinical phenotype and genetic analysis of a fetus with recombinant chromosome 8 syndrome.

OBJECTIVE:
To explore the clinical characteristics and molecular genetic mechanism of a fetus with recombinant chromosome 8 (Rec8) syndrome.
METHODS:
A fetus who was diagnosed with Rec8 syndrome at the Provincial Hospital Affiliated to Shandong First Medical University on July 20, 2021 due to high risk for sex chromosomal aneuploidy indicated by non-invasive prenatal testing (NIPT) (at 21st gestational week) was selected as the study subject. Clinical data of the fetus was collected. G-banded karyotyping and chromosomal microarray analysis (CMA) were carried out on the amniotic fluid sample. Peripheral blood samples of the couple were also subjected to G banded karyotyping analysis.
RESULTS:
Prenatal ultrasonography at 23rd gestational week revealed hypertelorism, thick lips, renal pelvis separation, intrahepatic echogenic foci, and ventricular septal defect. The karyotype of amniotic fluid was 46,XX,rec(8)(qter→q22.3::p23.1→qter), and CMA was arr[GRCh37]8p23.3p23.1(158049_6793322)×1, 8q22.3q24.3(101712402_146295771)×3. The karyotype of the pregnant woman was 46,XX,inv(8)(p23.1q22.3), whilst that of her husband was normal.
CONCLUSION
The Rec8 syndrome in the fetus may be attributed to the pericentric inversion of chromosome 8 in its mother. Molecular testing revealed that the breakpoints of this Rec8 have differed from previously reported ones.
