1.Effect of Qingluotongbi Granules on Bone Destruction in Rats with Adjuvant-induced Arthritis(AIA)

OBJECTIVE:To study the effect of Qinglutongbi granules on bone destruction in rats with adjuvant-induced arthritis (AIA). METHODS:AIA model was established in rats. Qinglutongbi granules were given to the model rats on day 7 for a total of 49 days. Roentgenography of joints of rats was performed for the observation of bone destruction. Synovial cells were taken from each group for cultivation of supernatant liquid, which was then added to cartilage cells that cultured in vitro. The outcome measures were proliferative response of cartilage cells, NO content and expression of antileptic type iNOS in supernatant of cartilage cells. RESULTS:Qinglutongbi granules (high, middle and low dosages) could improve the reduced bone density of joints in hind limb, widened articular cavity space and disorganization of joints in rats. Supernatant liquid that cultivated with synovial cells from AIA rats receiving Qinglutongbi granules at dosages of 7.2 g?kg-1 and 14.4 g?kg-1 could enhance the reproductive activity of chondrocytes and reduce iNOS expression as well as NO content. CONCLUSION:At a certain dosage, Qinglutongbi granules can inhibit the destruction of cartilage and bone in rats.
2.Expression of Gn protein of severe fever with thrombocytopenia syndrome virus and its immunogenicity

Objective To express the Gn protein of severe fever with thrombocytopenia syndrome virus(SFTSV) through adeno-associated virus 9(AAV9) expression system and evaluate its immunogenicity.Methods SFTSV Gn gene was inserted into viral vector pAAV-CMV-FH and the recombinant plasmid was transfected into HEK293T cells to obtain recombinant virus AAV9-Gn.The expression of Gn protein was determined by immunofluorescence and Western blot.Eighteen fernale BALB/c mice were randomly divided into three groups:Mock group(serum-free DMEM),AAV9-GFP group(1 × 10~(11) vg) and AAV9-Gn group(1 × 10~(11) vg),all of which were injected intramuscularly into the right hind limb at a dose of 100 μL per mouse.The body mass,diet,behavior and mental state of mice in each group were monitored continuously for 21 d,and the change rate of body mass was calculated;At 2,4,8 and 16 weeks after immunization,the levels of SFTSV neutralizing antibody in serum of mice in each group were detected by fluorescent reduction neutralization test(FRNT),and the levels of specific IgGl and IgG2a in serum of mice in AAV9-Gn group were detected by ELISA.Results After incubation with specific antibody,Vero cells transfected with AAV9-Gn showed specific green fluorescence under fluorescence microscope,and had specific binding to mouse anti-SFTSV Gn monoclonal antibody,and the specific binding bands were found at a relative molecular mass of about 61 000.The body mass of the three groups showed an increasing trend,there was no significant difference between the three groups(F=0.158—2.621,P> 0.05),and the diet,behavior and mental state were normal.At 2,4,8 and 16 weeks after immunization,the titer of SFTSV neutralizing antibody in serum of mice in AAV9-Gn group was significantly higher than that of Mock group and AAV9-GFP group(H=13.332—14.538,each P <0.001),and the titer peak appeared at 8 weeks;The level of specific IgG1 in serum of mice was significantly higher than that of IgG2a(F=4.373—12.975,each P <0.05) at different time points.Conclusion SFTSV Gn protein can be expressed correctly through AAV9 expression system,and has low toxicity to mice with good immunogenicity,which is expected to be a candidate component of SFTSV vaccine.
3.Clinical Analysis of 124 Children with Henoch-Sch(o)nlein Purpura

Objective To study the clinical characteristics of Henoch-Sch(o)nlein purpura (HSP) in children.Methods We retrospectively analyzed the clinical data of 124 children with HSP.Results Among these 124 children,54.84% of the children presented upper respiratory tract infection before the onset of HSP.The purpura recurred in 30 of 47 children with Henoch-Sch(o)nlein purpura nephritis and in 22 of 77 children without Henoch-Schonlein purpura nephritis,respectively (P<0.05).The incidence of Henoch-Sch(o)nlein purpura nephritis was significantly higher in children who had purpura,gastrointestinal symptoms,and joint symptoms simultaneously than in those only with purpura (P<0.05).The elevated levels of IgA and C reactive protein were found in 67.35% and 39.39% of the children,respectively.Conclusion The most important predisposing factor of HSP is upper respiratory tract infection.The children with recurrence of purpura or those who have purpura,gastrointestinal symptoms,and joint symptoms simultaneously are more likely to present renal damage.
4.Efficacy of Reduning injection combined with piperacillin sulbactam in the treatment of community acquired pneumonia

Objective To observe the clinical efficacy and safety of Reduning injection combined with pip-eracillin sulbactam in the treatment of community acquired pneumonia(CAP).Methods 122 cases with CAP were randomly divided into treatment group and control group by digital table method.The two groups were given piperacil-lin sodium and sulbactam sodium 3.75g,once every 12 hours,course 7 -10d.The treatment group was treated with Reduning injection,20ml/d treatment for 7 -10d.The clinical symptoms of the two groups before and after treatment, the total number of white blood cells(WBC)and C reactive protein(CRP)to resume normal time were observed. Results The effective rate was 94.91% in the treatment group and 80% in the control group,the difference between the two groups was statistically significant(χ2 =10.286,P <0.05).The treatment group was significantly shorter than the control group in controlling fever and cough symptoms(all P <0.05).3 months after treatment,the WBC and CRP recovery to the normal rate of the two groups had statistically significant differences (χ2 =2.880,3.926,all P <0.05).No obvious toxic side effects and adverse reactions were observed in the two groups.Conclusion Reduning injection combined with piperacillin sodium and sulbactam sodium in the treatment of CAP can alleviate the clinical symptoms of fever.This method has good clinical curative effect and high safety.It can be used as an effective method for treatment of CAP.
5.Progress in drug therapy for retinoblastoma

Retinoblastoma is the most common intraoeular malignant tumor in children.In its treatment,drug therapy is one of the most effective and irreplaceable methods.To reduce the toxicity of drugs,enhance the therapeutic effect and lessen drug resistance, some new drug deliver systems and new medicines for retinoblastoma are coming into being.Ophthalmic arterial infusion,fibrin sealant and iontophoresis are newly-found drug deliver systems.And the latest drugs under research include nutlins,phenoxazine derivative Phx-1,combretastatin A4 phosphate,2-deoxy-d-glucose and histone deacetylase inhibitors,etc.The following text is focused on the two aspects of the drug therapy for retinoblastoma.
6.The Diagnostic Value of MR Cholangiopancreatography in Gallstones

Objective: To evaluate the diagnostic value of magnetic resonance cholangiopancreatography (MRCP) in gallstones.Methods: Seventy-three patients with gallstones were examined with MRCP. Results: MRCP showed the lesions directly in69 patients,of which 58 patients were proved by surgery. The characteristic imaging appearance included a round,oval ormultifaceted area of filling defect and crateriform depression within the lumen of the bile ducts. MRCP could show false-positive result caused by air within the bile ducts. In the other 4 patients with tiny intra-hepatic stones ( <4 mm) ,MRCP couldnot show the lesions except for the proximal bile duct dilation. Conclusion:MRCP is a safe,non-invasive and reliable examination and may act as substitute method for patients unsuitable for endoscopic retrograde cholangiopancreatography (ER-CP).

OBJECTIVE: To put forward proposals for improving essential medicines system in China.METHODS: The problems existing in the essential medicine system in China and the successful experience of some other countries were analyzed.RESULTS & CONCLUSION: Considering the current medical economy level and the capacity on research & development(R&D) of drugs,China should draw inspiration from WHO and other developed countries about their successful experiences on essential drugs to improve the screening methods for essential medicines list,assign legal status for essential medicines and build up system of production,procurement and distribution of essential medicines according to the basic national situation of our country and finally establish the essential medicine system suitable to our country's condition.
8.MicroRNA in the injury of glomerular mesangial cells

MicroRNA could participate in a variety of physiological and pathological processes via iden-tifying the target genes by their seed sequences and regulating gene expression at post-transcriptional level. Glo-merular mesangial cell is one of the most chief cells of the glomerulus and plays an important role in the renal fi-brosis. The mesangial cellular proliferation and dysfunction may appear once attacked by pathogenic factors. Then,the extracellular matrix increases in the stimulation of inflammatory cytokines,eventually,resulting in glo-merular sclerosis and renal fibrosis. Reports have shown that series of microRNA could take part in the process of injury of mesangial cells and lead to renal fibrosis. MicroRNA could be a potential therapeutic target for early chronic kidney diseases.
9.Physical morphology of the root canal taper in the apical region of human permanent first morlars

Objective:To investigate the physical morphology of the root canal taper in human maxillary and mandibular permanent first molars.Methods: Forty maxillary and 44 mandibular extracted human permanent first molars were investigated.The roots were dissected transversely from their apical foramen vertical to the long axis of the root,and 1-mm-thick serial sections were made.The root apex side of each section was observed with a stereomicroscope at 30? magnification.The maximum and minimum canal diameters were measured at 0,1,2,and 3 mm from the apical foramen.The physical canal taper in the apical section was calculated.Results: 47.5%-60.0% of the narrowest diameter of root canals were present at the foramen,and 25.0%~42.5% located at the level of 1 mm from apical foramen.Almost all physical configurations of root canals in 3 mm apical region showed a bottleneck-like taper.Canal dia-meters increased rapidly at 1-2 mm with the biggest taper,and the taper had a bigger variance(0.07-0.35).The ring of shrunken bottleneck was formed at the level of 1 mm from apical foramen.The taper of canal in maximal diameter was larger than the one in minimal diameter.Conclusion: The physical morphology of bottleneck-like taper in apical canal was helpful to locate the canal apical stop in root canal treatment.
10.Assessment of the upper motor neuron lesion in amyotrophic lateral sclerosis with the triple stimulation technique

Objective To quantitatively estimate upper motor neuron (UMN) lesion with the triple stimulation technique (TST) in amyotrophic lateral sclerosis (ALS). Methods Fifty ALS patients and 22 normal controls were enrolled in the study. Patients were examined clinically with conventional transcranial magnetic stimulation (TMS) and TST at abductor digiti minimi. Central motor conduction time (CMCT),motor evoked potential ( MEP), resting motor threshold ( RMT), compound muscle action potential ( CMAP), modified ashworth scale ( MAS), Medical Research Council scale and modified ALS scale were assessed and their correlation to disease progress was analyzed. Results The TST amplitude ratio was significantly decreased in ALS patients with UMN signs(28 cases 62. 0% (40. 7%, 75.9% ) ), compared with controls ( 96. 9% ± 2. 6% ) and ALS patients without UMN signs ( 22 cases 95.6% ( 85.4%,100. 0% ) ;Z = -4. 827, -5.435, both P =0. 000). The abnormal rates of the TST amplitude in ALS with UMN signs, ALS without UMN signs and controls were 89. 3%, 27.3% ,9. 1% respectively. The abnormal rates of the TST amplitude, the latency of MEP, CMCT, RMT in detecting UMN lesions were 89. 3%,64. 3%, 53.6%, 64. 3% , 78. 6%, respectively. The TST amplitude was significantly correlated to tendon reflex in right arm ( r = 0. 690, P = 0. 000), with modified ashworth scale ( MAS, r = - 0. 772,P = 0. 000),with diagnostic degree ( r = 0. 483, P = 0. 000), with RMT ( r = - 0. 774,P = 0. 000), the latency of MEP (r = - 0. 444, P = 0. 005 ), motor evoked potential/compound muscle action potential of erb' s ( MEP/ CMAPerb, r = 0. 685, P = 0. 000 ), MEP/CMAPerb in facilitation ( r = 0. 770, P = 0. 000). Conclusions TST appears to be a more accurate and sensitive measure of detecting and quantifying UMN abnormality in ALS patients than the other parameters. TST may reveal the subclinical UMN impairment in ALS and provide an accurate diagnosis assessment for UMN loss in ALS and an objective scale for monitoring the progression of disease.
