1.Prevalence and characteristics of overweight and obesity in Chinese children aged 0-5 years.

Objective: To study the prevalence and characteristics of overweight and obesity among Chinese children aged 0-5 years, in 2010-2013. Methods: Data was from the'China Nutrition and Health Surveillance-0-5-Years-Old Children and Lactating Women'project in 2013. Stratified multistage cluster sampling method was used to select 55 districts/counties from 30 provinces (autonomous regions, municipalities) with the sample size of children as 32 862. Definition of overweight and obesity were according to both the WHO 2006 growth standard in children less than 5-year-old and the WHO 2007 growth reference in children of 5-years-old. Results: were calculated by complex weight based on national census from the National Bureau of Statistics in 2010. Results The overall prevalence of overweight was 8.4% among the 0-5-year-old in 2013, with 9.4% in boys and 7.2% in girls. Both of the rates from urban and rural areas were the same, as 8.4%. The prevalence rates of overweight in the 0-, 6-, 12-, 24-, 36-, 48- and 60-71 months age groups appeared as 13.0%, 11.1%, 8.3%, 6.0%, 4.8%, 3.9% and 15.9%, respectively. The rates of overweight in low, medium and high income families were 8.0%, 8.8% and 8.9%, respectively. The prevalence of obesity was 3.1% among the 0-5-year-old, with 3.6% in boys and 2.5% in girls. There was no significant difference seen in urban (3.3%) and rural areas (2.9%). The prevalence rates for obesity in the 0-, 6-, 12-, 24-, 36-, 48- and 60-71 months age groups were 5.8%, 3.8%, 2.5%, 1.6%, 1.2%, 1.3% and 7.8%, respectively. The rates of obesity in low, medium and high income families were 2.8%, 3.3% and 3.5%, respectively. Conclusion: The prevalence rates of both overweight and obesity were increasing among the 0-5-year-olds in China, suggesting that it is necessary to timely conduct the surveillance and intervention programs on overweight and obesity in this target population.

2.Relationship between tunnel widening and different rehabilitation procedures after ACL reconstruction with quadrupled hamstring tendons.

OBJECTIVETo find out the relationship between different rehabilitation procedures and the tibial tunnel widening after ACL reconstruction.
METHODSSixty-five patients undergone ACL reconstructions by using quadrupled semitendinosus and gracilis tendons were divided into two groups. Group A had 33 patients, 19 male, 14 female, average age (31.2 +/- 12.4) years, only ACL reconstruction was performed with Germanic Professor Paessler's technique, and aggressive rehabilitation procedure was used for functional recovery. Group B had 32 patients, 20 male, 12 female, average age (30.3 +/- 10.3) years. Besides ACL reconstruction, every patient in Group B accepted meniscus repair or cartilage repair with microfracture technique, then postoperative conservative rehabilitation procedure was used. Six months after operation, standard posterior-anterior radiographic plates were taken for each patient, CorelDRAW 8.0 software was used to digitize all X-ray plates and measure the upper, middle and lower parts of the tibial tunnel. Magnification of X-ray plates was taken out after measurement. Statistic analysis was done by t-test.
RESULTSSix months after ACL reconstruction, on both the posterior-anterior and lateral X-ray plates the tibial tunnel widening of the upper, middle and lower parts in Group B with aggressive rehabilitation procedure was significantly serious than in Group A with conservative rehabilitation.
CONCLUSIONSRehabilitation procedure after ACL reconstruction is one of the reasons for tunnel widening. It perhaps influences the functional recovery and long-term clinical result of the operated knee by affecting the tunnel widening.

4.Quality assurance of TomoDirect treatment plans using I’mRT MatriXX

Purpose: To evaluate the performance of 2D-array I’mRT MatriXX for dose verification of TomoDirect treatment
plans.
Methods: In this study, a 2D-array ion chamber device – the I’mRT MatriXX and Multicube Phantom from IBA –
was used for dose verification of different TomoDirect plans. Pre-treatment megavoltage computed tomography (MVCT)
was performed on the phantom setup for position correction. After the irradiation of treatment plans on the I’mRT MatriXX and Multicube Phantom, the measured doses of coronal planes were compared with those from the planning calculations for verification. The results were evaluated by comparing the absolute dose difference in the high dose region as well as the gamma analysis of the 2D-dose distributions on the coronal plane. The comparison was then repeated with the measured dose corrected for angular dependence of the MatriXX.
Results: When angular dependence is taken into account, the passing rate of gamma analysis is over 90% for all
measurements using the MatriXX. If there is no angular dependence correction, the passing rate of gamma analysis
worsens for treatment plans with dose contribution from the rear. The passing rate can be as low as 53.55% in extreme cases, i.e. where all doses in the treatment plan are delivered from the rear.
Conclusion: It is important to correct the measured dose for angular dependence when verifying TomoDirect
treatment plans using the MatriXX. If left uncorrected, a large dose discrepancy may be introduced to the verification results.
5.MRI Staging of Legg - Calve - Perthes ( LCP ) Disease.

One of the most important prognostic factors in LCP disease is the extent of epiphyseal involvement. Magnetic resonance imaging is considered to be the technique of choice for early diagnosis of Legg-Calve-Perthes disease. Gadolinium-enhanced spin-echo MR images were obtained after nonenhanced Tl-weighted(spin-echo) and T2-weighted(gradient-echo) images. Four different areas were identified in the femoral epiphysis(necrosis, regenerative, cartilaginous and normal fatty bone tissue). The histological evolution of LCP is well described by Catterall and others. Comparing their description with our MRI finding, we suggest classification of LCP into three phases: (I) necrosis, (II) regeneration(IIa-early and IIb-late) and (III) reossification and sequale. T2 weighted image was useful in the early stage and Tl weighted image was useful in the later stage for evaluation of involved extent of the disease. With MRI, we think that we can find out the stage of LCP more early and rationally, pathological factors more easily and appropriate time for operation exactly. we believe that MRI is more adequate method to decide the stage of LCP disease.

6.Management of severe hidradenitis suppurativa with biologic therapy and wide excision

Hidradenitis suppurativa (HS) is a chronic inflammatory follicular occlusive disease that involves the intertriginous areas. Treatment methods include conventional topical and systemic medication, radiotherapy, biologic agents, and surgical excision. Of late, there has been an increased focus on the use of biologic agents in patients with moderate to severe HS. Here, we present the case of a 46-year-old man with Hurley stage III HS for whom wide excision was ultimately curative, after aggressive medical therapy with the use of infliximab and adalimumab had succeeded in limiting the body surface area affected by the disease. This case demonstrates the effective treatment of severe HS with a combination of biologic therapy and surgery.

7.Prognostic Value of Early Postoperative Intraperitoneal Chemotherapy in Resectable Advanced Gastric Cancer.

PURPOSE: There are variants of gastric cancer associated with predominantly peritoneal spread or with haematogenous metastases. Perioperative intraperitoneal chemotherapy as an adjuvant to surgery is considered as a rational therapeutic modality to prevent peritoneal spread. We evaluated the influence of early postoperative intraperitoneal chemotherapy on the prognosis of resectable advanced gastric cancer. MATENRIALS AND METHODS: From 1990 to 1995, 246 patients with biopsy proven advanced gastric cancer were enrolled in the study. Among them 123 patients received early postoperative intraperitoneal mitomycin C and 5-fluorouracil. The survival rate was calculated using by the Kaplan-Meier method and was compared using the log-rank test according to 13 clinico-pathologic factors. Multivariate analysis was performed with the Cox proportional hazards model. RESULTS: Gastric resection plus early postoperative intraperitoneal chemotherapy showed an improved survival rate as compared to surgery alone (54.1% versus 40.3%; P= 0.0325). Depth of tumor invasion, degree of regional lymph node metastasis, distant metastasis, tumor size, tumor location, extent of gastric resection, and curability of surgery significantly influenced survival. When a multivariate analysis was performed, depth of tumor invasion, lymph node metastasis, early postoperative intraperitoneal chemotherapy, curability of surgery, and extent of gastric resection emerged as the statistically significant and independent prognostic factors. CONCLUSION: Early postoperative intraperitoneal chemotherapy is one of the independent prognostic indicators of resectable advanced gastric cancer.

BACKGROUNDIt has been demonstrated that bone tunnel widening could appear after knee anterior cruciate ligament (ACL) reconstruction, especially for those patients whose ruptured ACL were reconstructed with semitendinosus and gracilis tendons. Many factors can influence the extent of tunnel widening. Few studies have investigated the relationship between bone tunnel widening and rehabilitation procedures. This research was carried out to find the rehabilitation procedures' influence on the tibial bone tunnel widening after ACL reconstruction.

Six months post operation, standard posterior-anterior radiographic plates were taken for each case, CorelDRAW 8.0 software was used to digitize all X-ray plates and measure the upper, middle and lower parts of the tibial tunnel. Magnification effect of X-ray plates was taken out after measurement.
RESULTSSix months after ACL reconstruction the tibial tunnel widening of the upper, middle and lower parts on both the posterior-anterior and lateral X-ray plates in Group A with aggressive rehabilitation procedure was much more serious than in Group B with conservative rehabilitation. KT-1000 knee stability measurement and clinical manifestation showed no difference between the two groups.
CONCLUSIONSRehabilitation procedure after ACL reconstruction is one of the reasons for tunnel widening. It not only can directly influence the function recovery of ACL reconstructed knee, but also perhaps indirectly influence the function recovery and long-term clinical result of the operated knee by influencing the tunnel widening.

AIM: To study the effects of Sodium Hyaluronate (HA) and Bion Tears on corneal thickness in adult myopic patients.METHODS: A total of 38 cases (76 eyes) were involved in this study. Three consecutive corneal measurements (the thinnest point of the cornea,THN) were evaluated before and half an hour after the instillation of one drop of HA in one eye and Bion Tears in the other at random with the Orbscan Corneal Topography System II (Orbscan,Inc,Salt Lake City,UT,USA,Version 3.00E).RESULTS: There were no significant between-group differences in baseline variable (t=0.264). Thirty minutes after the instillation of HA and Bion tears,THN were significantly increased by 5.57±7.00μm (t=4.906,P<0.01) and 7.89±7.64μm (t=6.369,P<0.01) respectively. However,there were no between-group differences in THN changes(t=1.381,P>0.05).Increase in the corneal thickness were found in 32 eyes (84%) and 33 eyes (87%) for the HA and Bion tears group,respectively.CONCLUSION: Artificial tears including HA and Bion Tears can significantly increase the corneal thickness in a short period of time. Corneal thickness can be used as one of the objective indices for evaluating the quality and therapeutic role of artificial tears.
