1.The efficacy and safety of high-intensity focused ultrasound ablation of benign thyroid nodules.

High-intensity focused ultrasound (HIFU) is a promising form of thermal ablation of benign thyroid nodules, but evidence supporting its use is scarce. The present review evaluated the efficacy and safety of single-session HIFU treatment of benign thyroid nodules. As reported in the literature, the extent of nodule shrinkage following treatment ranged from 48.8% to 68.8%. Like other forms of ablation, the shrinkage rate was greatest in the first 3-6 months, and the best responders were patients with small (≤10 mL) nodules. Complications were uncommon, but temporary vocal cord palsy occurred in 3%-4% of patients, and was related to the distance between the HIFU beam and the recurrent laryngeal nerve. Despite being safe and efficacious, a larger-scale prospective trial is required.

2.Prevalence, awareness, treatment and control of hypertension in population older than 15 years of age in Beijing, 2013-2014.

Objective: To investigate the rates on prevalence, awareness, treatment and control of hypertension in population older than 15 years of age in Beijing, 2013-2014. Methods: A cross-sectional survey was conducted in Beijing between 2013-2014. Stratified multistage random sampling method was used to select representative sample of 13 057 Chinese individuals aged over 15 years, from the general population. Blood pressure was measured for three readings at sitting position after resting for at least five minutes with an average reading recorded. A standardized structured questionnaire was developed to collect history of hypertension and antihypertensive treatment. Results: A total of 4 663 community residents aged over 15 years were hypertensive among the 13 057 individuals, with the standardized prevalence rate as 32.7%, in Beijing area. The age-standardized prevalence rates of hypertension appeared 34.6% in men and 30.8% in women. The age-and sexstandardized prevalence of hypertension rates were 33.3% in urban and 24.6% in rural areas. The prevalence of hypertension increased with age and appeared higher in men than in women, in urban than in rural residents. Among the hypertensive patients, rates of awareness, treatment and control were 66.8%, 64.6% and 31.6%, respectively. Conclusion: High prevalence of hypertension with low rates on awareness and treatment and control, appeared in the general population of Beijing. Related strategies should be developed regarding prevention, control and management of hypertension, to reduce the burden of this disease.

3.Prophylactic Dual Catheter Technique to Prevent Side Branch Snowplowing Complications during Angioplasty and Stenting.

OBJECTIVE: Angioplasty and Stenting of intracranial atherosclerotic lesions have a higher complication rate and a large proportion of this is attributable to side branch arterial occlusion from forceful displacement of the atheroma into the ostia or snowplowing effect. This can result in severe disabilities when it result in small infarcts involving eloquent areas in the posterior circulation or the motor tracts. MATERIALS AND METHODS: We present a series of 6 cases utilizing a new dual catheter technique for maintaining the patency of at-risk vessels during angioplasty and stenting. There are several methods previously described to help reduce the incidence of stroke but because they do not have a physical presence in the ostia to protect it, they are unable to guarantee the patency of the vessel. RESULTS: All 6 patients underwent angioplasty and stenting with the technique. The patients were assessed for complications with post-procedure magnetic resonance imaging and no complications were found. CONCLUSION: In this preliminary series, the dual catheter technique appears to safe and effective in preventing occlusion of the adjacent branch arteries. This technique may facilitate the use of the Wingspan stent in the treatment of intracranial atherosclerotic stenotic segments by reducing the risk of peri-procedural stroke.

5.Diabetic ketoacidosis at diagnosis of type 1 diabetes mellitus in Malaysian children and adolescents

Background: Diabetic ketoacidosis (DKA) is a late presentation of newly diagnosed type 1 diabetes
mellitus (DM) in children. The aim of this study was to determine the clinical characteristics of type
1 DM at presentation so that appropriate actions can be taken to promote early diagnosis.
Methods: This was a retrospective cohort review from a patient registry database. Data on all
patients younger than 20 years old diagnosed with type 1 DM who had been registered with the
Malaysian Diabetes in Children and Adolescents Registry (DiCARE) from its inception in 2006
until 2009 were analysed.
Results: The study included 490 children and adolescents, out of which 57.1% were female. The
mean (SD) age at diagnosis was 7.5 (3.7) years, which increased from year 2000 to 2009 [6.6 (3.3)
years to 9.6 (3.5) years; p = 0.001]. An increasing percentage of DKA at diagnosis was observed from
year 2000 (54.5%) to year 2009 (66.7%), which remained high and leveled between 54.5% and
75.0%. DKA was more common in patients with normal weight (p = 0.002) with no significant
association with age, gender, ethnicity and status of family history of diabetes mellitus.
Conclusion: An increasing trend of age at diagnosis of patients with type 1 DM was observed.
Besides that, proportion of DKA at diagnosis had remained high over the past decade. This study
found that normal weight was associated with status of DKA, thus more detailed investigations are
required to determine the risk factors for DKA.
6.Effects of CYP2C9*3 Gene Polymorphism on Therapeutic Efficacy of Benzbromarone in Lowering Uric Acid and Its Hepatotoxicity

OBJECTIVE:To study the effects o f CYP2C9*3 gene polymorphism on therapeutic efficacy of benzbromarone in lowering uric acid and its hepatotoxicity. METHODS :A retrospective study was conducted to analyze the relevant clinical indicators and genotypes of 196 gout patients who received benzbromarone and CYP2C9*3 gene polymorphism test in Wuhan third hospital from Jan. 2018 to Sept. 2019. RESULTS :Among 196 patients,179,15 and 2 patients with CYP2C9*3 genotypes * 1/*1, *1/*3 and * 3/*3 genotypes were found ,respectively,and the distribution of each genotype was in line with Hardy-Weinberg balance(P>0.05). Before treatment ,there were no significant differences in the levels of UA ,Scr,ALT,AST and CRP between *1/*1 genotype and * 1/*3+*3/*3 genotype(P>0.05). After 4 weeks of treatment ,the UA ,Scr,CRP levels of patients with * 1/*1 genotype as well as the UA and CRP levels of patients with * 1/*3+*3/*3 genotype were significantly reduced ,the UA level of patients with * 1/*1 genotype was significantly lower than that of patients with * 1/*3+*3/*3 genotype(P<0.05 or P<0.01). The ALT and AST levels had no obvious changes in patients with different genotype before and after treatment ,and they were in the normal range. No serious abnormal liver function was observed during the treatment. CONCLUSIONS :Therapeutic efficacy of benzbromarone in lowering uric acid in gout patients with CYP2C9*3 genotypes * 1/*1 genotype is better than that of * 1/*3 and * 3/*3 genotypes. However ,the gene polymorphism may be not associated with its hepatotoxicity.
7.Rosiglitazone and all-trans retinoic acid inhibit human myeloma cell proliferation via apoptosis signaling pathway modulation.

OBJECTIVETo investigate the effects of artificial ligand of peroxisome proliferators activated receptors (PPARs), rosiglitazone (RGZ) and all trans-retinoic acid (ATRA) on human myeloma cell line growth in vitro and in vivo.
METHODSU266 and RPMI 8226 cells were treated with different concentration of RGZ in the presence or absence of ATRA and the results were studied by 3H-TdR thymidine incorporation (for cells proliferation), Annexin V-PI staining and caspase-3 activity assay (for cells apoptosis), RT-PCR (for FLIP, XIAP and survivin mRNA expression), and tumor formation test in BALB/c nude mice.
RESULTSExposure to RGZ induced proliferation inhibition in a dose-dependent manner in both U266 (r = 0.991, P < 0.01) and RPMI 8226 cells (r = 0.961, P < 0.01). A combination of RGZ with ATRA could enhance the inhibition effect (P < 0.001 in U266, P < 0.01 in RPMI8226). 10 micromol/L of RGZ induced apoptosis of (9.8 +/- 1.7)% in U266 cells and (10.7 +/- 3.3)% in RPMI8226 cells, in a time and dose dependent manner and combined with ATRA intensified the apoptosis induction effects (P < 0.01 in both cell lines). The FLIP, XIAP and survivin mRNAs were expressed in both cell lines and their levels decreased significantly after cultured with RGZ. The addition of RGZ + ATRA in the culture further decreased the levels. Caspase-3 activity increased substantially with the increase of RGZ concentration and the addition of RGZ + ATRA in the culture medium showed similar synergism effect on caspase-3 activation (P < 0.01). The xenograft of U266 cells in BALB/c nude mice were inhibited by RGZ and so did more by the combination of RGZ and ATRA (P < 0.01).
CONCLUSIONThe down-regulation of FLIP, XIAP and Survivin induced by RGZ can activate caspase-3, whereby induced apoptosis and proliferation inhibition in myeloma cells. ATRA can enhance these effects of RGZ.

9.Secondary stability of cemented and non-cemented acetabular implants ex-vivo under dynamic load.

OBJECTIVETo measure the bone-prosthetic implant interface micromovement during the application of physiological load by using a material testing system (MTS).
METHODSThe cadaveric hip specimens were used to simulate a single leg stance and the joint in the neutral position. Micromovement was recorded via a 3-dimensional transducer in the acetabula of postmortem specimens, which had been preserved in formalin. The study data of the cemented and uncemented prosthsis refereed to the lone-term clinical process and the radiological status and experimental results.
RESULTSCemented cups showed higher transverse relative motion up to 90 microm, whereas the maximum transverse movement of the non-cemented cup was 60 microm. Orthogonal motion perpendicular to the implant surface showed compression for all cups at all sites.
CONCLUSIONThe results indicate that there are large differences in survival time between 2 groups. That could not be compared statistically in secondary stability. Nevertheless, according to the results, the amount of micromotion of press-fit cup is relatively less than that of cemented polyethylene cup, which is instrumental in bone ingrowth and secondary stability.
