1.Pathologic Study of Breast Cancer in Korea.

From a retrospective study of 366 cases of breast cancer, following results are obtained. 1) Relative incidence of breast cancer is increased during recent 7 years as compared with total female cancer. 2) Invasive ductal carcinoma and invasive ductal carcinoma with a predominant intraductal component (ICPI) account for 78% of total breast cancer, and its ratio shows little variance during recent 7 years and as compared with previous studies. 3) The incidence of invasive lobular carcinoma is increased as compared with 10 years ago, but it seems more likely that the increase is due to difference in diagnostic criteria rather than due to real increase in incidence. 4) The average patients' age of five major histologic types of breast cancer is 47.3 year. The patient's age of invasive ductal carcinoma is highest and that of ICPI is lowest. 5) Outer upper quadrant is the most frequent site of breast cancer. Left side is slightly more frequent than right side. There is little variance among the histologic types in tumor location. 6) The average symptom duration of patients of major types of breast cancer is 8.4 month. It is longest in invasive ductal carcinoma and shortest in ICPI. 7) Invasive lobular carcinoma is largest in tumor size and intraductal carcinoma is smallest, among the major histological types. 8) Intraductal carcinoma is most freqnently accompanied by nipple discharge and Paget's disease. 9) The rate of axillary lymph node metastasis is highest in invasive lobular carcinoma, followed by invasive ductal carcinoma, medullary carcinoma, and ICPI, in order of freqnency. 10) Distant metastasis is to the bone, lung, liver and brain, in order of freqnency. Medullary carcinoma shows the highest rate of distant metastasis. 11) The important prognostic factors of breast cancer are histologic types, status of axillary lymph node metastasis, size of tumor and histologic grade of malignancy. But there is little difference in prognosis between invasive ductal carcinoma and medullary carcinoma.

2.Hyperkalemic periodic paralysis and paramyotonia congenita caused by a de novo mutation in the SCN4A gene

Familial hyperkalemic periodic paralysis is an autosomal-dominant channelopathy characterized
by reversible paralysis associated with episodic hyperkalemia. Mutations in the skeletal muscle
voltage-gated sodium channel gene (SCN4A) have been reported to be responsible for this disorder.
Paramyotonia congenita is also caused by mutations in the SCN4A gene. Here, we report the case of a
17-year-old boy who presented with both hyperkalemic periodic paralysis and paramyotonia congenita.
A molecular analysis of the SCN4A gene revealed a heterozygous T>C transition at nucleotide 2078,
leading to an Ile693Thr mutation. This mutation was absent in the patient’s parents supporting a de
novo Ile693Thr mutation in our patient.
3.An Effect of Beach Ball Play for Depression, Powerlessness, Self-Esteem, Activity of Daily living in Stroke patients.

The purposes of this study was to determine the effect of beach ball play in stroke patients, and to define the strategy to promote their depression, powerlessness, self-esteem, activity of daily living. The experimental design was designed nonequivalent control group, non-synchroniged design. The study method had been done by investigating the experimental group and control group through the questionnaire on 57 patients who had been in patient department in D University hospital in Pusan from January 5th 1998 to the end of February 28th, 1998. Beach ball play was carried out experimental group once per day for 10 minute's for two weeks period from Jan 1998 to Feb 1998. Data was collected before and after the experimenation. Collected data was analyzed by means of frequency, percentage, chi-square test, mean, S.D, t-test with SPSS/PC. The results were summarized as follows: 1. The I st hypothesis: The experimental group which received the beach ball play should be higher in depression than the control group was supported(t=3.11, p=.003). 2. The 2nd hypothesis: The experimental group which received the beach ball play should be higher in powerlessness than the control group was supported(t=3.32. p=.002). 3. The 3rd hypothesis: The experimental group which received the beach ball play should be higher in self-esteem than the control group was not supported(t=-1.90, p=.064). 4. The 4th hypothesis: The experimental group which received the beach ball play should be higher in activity of daily living than the control group was not supported(t=-.47, p=.637). In conclusion, the patients who received beach ball play showed the increase in the degree of depression and powerlessness of stroke patients. So the beach ball play had been judged the nursing intervention to improve their emotional problem in stroke patients.

4.Studies on the Morphological Changes of Neurons in Dorsal Root Ganglion and Spinal Dorsal Horn of the Rat with an Experimental Peripheral Neuropathy.

Animal models for human chronic pain syndromes were developed and widely used for pain research. One of thsese neuropathic pain model by Kim and Chung[1992] has many advantages for operation and pain elicitation. We have examined the c-fos protein, substance P, CGRP immunoreactivity in dorsal root ganglia and dorsal horn in this neuropathic model. About 50 Sprague-Dawley rats were used for this study. L5 and L6 spinal nerve were ligated tightly to produce neuropathic pain model. After 2, 4, 8, 16, 24 hours and 1 week of surgery, rats were anesthesized and sacrificed by perfusion through the left ventricle with saline followed by 0.1M phosphate buffer[pH 7.4] containing 3% paraformaldehyde, 3% glutaraldehyde, and 0.1% picric acid. After confirmation of the roots transected by the surgery, the L5 and L6 dorsal root ganglion and spinal cord were removed and processed for immunohistochemistry. All tissue sections were immunohistochemically stained for substance P, CGRP and c-fos by using the peroxidase-antiperoxidase[PAP] method. Count the number of immunostained substance P and CGRP dorsal root ganglion cells and c-fos immunoreactive dorsal horn cells and analyzed statistically with Mann-Whitney U test. The results are as follows. 1. The number of c-fos protein immunoreactive neurons in the superficial layer of dorsal horn were increased markedly at 2 hours after operation, gradually decreased to normal level 1 week after operation. 2. The number of c-fos protein immunoreactive neurons in the deep layer of dorsal horn were gradually increased to the peak 24 hours after operation, decreased to normal level 1 week after operation. 3. The number of substance P and CGRP immunoreactive L5 and L6 dorsal root ganglion neurons were decreased markedly at 1 week after pain model operation. In conclusion, after neuropathic pain model operation, c-fos protein were immediately expressed in the superficial layer of spinal dorsal horn, thereafter c-fos protein in the deep layer of spinal dorsal horn were expressed. CGRP and substance P immunoreactive neurons were decreased markedly 1 week after neuropathic pain model operation.

5.Squamous Metaplasia of the Pleura.

We report a case of squamous metaplasia of the pleura observed in a 51 year old man. Squamous metaplasia of serosal surface, and in particular of the pleura, is uncommon. The pathogenesis of squamous metaplasia is obscure, but it may be associated with chronic irritation.
6.Effect of the inhibition of PLA2 and PAF on the neutrophilic respiratory burst and apoptosis.

BACKGROUND: Since the exact pathogenesis of sepsis-induced ARDS has not been elucidated, the mechanisms of enhanced neutrophilic respiratory burst were probed in endotoxin primed neutrophils associated with the roles of phospholipase A2 (PLA2), platelet activating factor (PAF) and apoptosis. METHODS: In isolated fresh human neutrophils, effects of the inhibition of PLA2 and PAF on the apoptosis were examined by the method of Annexin-FITC/dual PI flow cytometry. The roles of PLA2 and PAF on the neutrophilic respiratory burst were also examined by measuring oxidant generation in cytochrome-c reduction assay. Activities of the PLA2 and lysoPAF acetyltransferase (lysoPAF AT) of the neutrophils were determined to understand the effect of endotoxin on these enzymatic activities which may be related to the neutrophilic respiratory burst and apoptosis. In addition, the role roles of PLA2 and PAF in neutrophilic adhesion to bovine endothelial cells were examined in vitro by neutrophil adhesion assay. To investigate the effect of oxidants on pulmonary surfactant, cytochemical ultrastructural microscopy was performed. To inhibit PLA2 and PAF, non-specific PLA2 inhibitor mepacrine (100 nM) and WEB 2086 (100 nM) or ketotifen fumarate (10 kg/ml) were used respectively in all in vitro experimental sets. WEB 2086 is PAF receptor antagonist, and ketotifen fumarate is a lyso PAF AT inhibitor. RESULTS: The mapacrine treatment, provided after the endotoxin (ETX) treatment, resulted in increased apoptosis of neutrophils (p<0.001) while treatments of WEB 2086 and ketotifen did not. The inhibition of PLA2 and PAF decreased (p<0.001) production of oxidants from PMA-stimulated neutrophils. While endotoxin increased the PLA2 activity of neutrophils (p<0.01), mepacrine supressed (p<0.001) the activity, provided after treatment of ETX. The lyso PAF actyltransferase activity (lyso PAF AT) increased(p<0.01) after treatment of ETX. In contrast, mepacrine, WEB 2086 and ketotifen showed a tendency of decreasing the activity after treatment of ETX. The Treatment of ETX increased (p<0.001) neutrophil adhesion to endothelial cells, which was reversed by inhibition of PLA2 and PAF (p<0.001). The binding of oxidants to pulmonary surfactant was identified histologically. CONCLUSIONS: The enhanced neutrophilic respiratory burst by ETX plays a pivotal role in the pathogenesis of ARDS in term of oxidayive oxidative stress. Increased production of oxidants form neutrophils is mediated by the activations of PLA2 and lyso PAF AT.

7.Heterotopic Glial Nodule in the Lung of an Anencephaly Patient : An autopsy case.

The heterotopic and tissues may be divided into two categories: those that are found in the head and neck region, and those that arise elsewhere. The latter type is rare and most cases are found in the lungs of patients with neural tube defect, particularly anencephaly. Our report descrives anencephalic male infant with heterotopic glial nodules in both lungs. The largest nodule is 2x1.5x1 cm, locates in the lower lobe of the left lung, and has a round gray-white cut surface with cystic spaces. Microscopically, the nodules consist of irregularly arranged astrocytes and glial fibers, in which are embedded gland-like or cystically dilated bronchioles. The astrocytes and glial fibers are strongly positive for glial fibrillary acidic protein and show astrocytic filaments on electron microscopy. This will be an additional case supporting the amniotic fluid aspiration/implantation theory of pathogenetic mechanism of distal heerotopic glial tissue.

9.Relationship among Powerlessness. Self-Efficacy and Quality of Life in Hemodialysis Patients.

The purpose of this study was to the relationship between powerlessness, self-efficacy and quality of life in hemodialysis patients. The subjects were compromised of 198 hemodialysis patients from Pusan, Kyung-Sang Namdo and Kyung-Sang Bukdo 6 hemodialysis clinics. There were significant positive correlation between self-efficacy and quality of life(r= .512, p .00l). Data were collected by questionnaires from Sep. 19 to Oct. 27, 2001. The instrument used for study were the Powerlessness Behavioral Assessment Tool (Miller, 1983) and self-efficacy developed by Kim, Ju-Hyun(1995) and quality of life developed by Kim, Ok-Soo(1993). Collected data were analyzed by frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheffe test, pearson correlation The results were as follows. 1. The mean score of powerlessness was 52.41+/-.93, mean mark 3.28+/-.43, and the mea score of self-efficacy was 45.44+/-.75, mean mark 2.87+/-.49, and the mea score of quality of life was 121.27+/-3.81. mean mark 2.96+/-.58. 2. There were significant differences in the level of powerlessness according to sex (1,=2.148, p= .033), occupation(t=3.682, p=.000), economic status(F=3.094, p=.048), experience of hospitalization (1,=-2.002. p= .047). 3. There were significant differences in the level of self-efficacy according to age(F=3.271. p=.013), economic status (F=5.759, p=.004), religion(F=2.667, p= .048), hemodialysis period(F=2.991. p =.032), hemodialysis frequency(t = 9.045, p = .003), experience of hospitalization (1,=4.40, p=.037). 4. There were significant differences in the level of quality of life according to occupation(t=3.796, p=.053), economic status(F=11.478, p=.000), hemodialysis frequency(t=7.573, p=.006). 5 There were significant negative correlation between powerlessness and self-efficacy (r=- .401. p .001), powerlessness and quality of .life(r=- .562, p .001),

10.Cardiac Fibroma: A surgically excised case.

Primary cardiac tumors in infancy and childhood are rare, with fibromas being the second most common tumor after rhabdomyomas. Although cardiac fibromas are characteristically benign intramural tumors, they may exhibit exhibit expansile growth resulting in obstruction, valvular dysfunction, as well as other problems so early diagnosis and successful surgical excision are important. We report a case of cardiac fibroma in a 2 month-old male infant. He presented with generalized cyanosis from birth. Echocardiography showed oval round large mass filing the right atrium and ventricle which infiltrated into the lateral wall of the ventricle. Partial excision of the tumor was done after another echocardiogram showed a pericardial effusion and restriction of blood flow to the right ventricle due to the tumor. The resected tumor was ovoid, gray-tan, slightly firm and measuring 5x3x2.5 cm. Histologically, the tumor was composed of spindle-shaped fibroblasts and hyalinized fibrous tissue interdigitating with the surrounding myocardium.
