The limb-body wall malformation complex is a sporadic congenital anomaly characterized by protean manifestations. The diagnosis is based upon the presence of at least two out of craniofacial anomaly, body wall defect, and limb abnormalities. We present a case of limb-body wall malformation complex. This case shows abdominal and pelvic wall defects with eventration of the viscera. It also shows an absence of right kidney, polycystic left kidney, absence of external genitalia and anus, and lower extremity abnormalities. The right lower extremity is absent and the deformed left leg shows malformed foot-like structure attached to the shin in addition to a normally positioned left foot. Our patient is the first case of complete absence of the external genitalia associated with limb-body wall malformation complex in Korean publications and the seventh in English publications.

PURPOSE:To describe the features of displaced bucket-handle tears of the menisci on magnetic resonance (MR) images and to assess associated knee injuries. MATERIALS AND METHODS: We retrospectively reviewed coronal and sagittal MR images in 21 bucket-handle tears. The subjects were patients who had underwent preoperative MR evaluations of the knee and were identified from the arthroscopic surgical records as bucket-handle tear. We also described patterns of associated injuries. RESULTS:On coronal MR images, (a) in all cases, peripheral portion of the meniscus(bucket) had the appearance of a truncated or altered wedge;(b) central fragments(handle) were observed to be sitting in the intercondylar notch(16 cases) or located between the fernoral condyle and tibial plateau (5 cases). On sequential sagittal MR images, (c) the bow-tie appearance of the body of the meniscus was not seen (13 cases);(d) the bow-tie appearance of the displaced inner fragment was seen at the intercondylar notch level (9 cases);(e) "double posterior cruciate ligament" sign was presented (7 cases). Associated joint abnormalities included anterior cruciate ligament tears(l 1), contralateral meniscal tears(l 1), posterior cruciate ligament tears(3), medial collateral ligament tears(3), osteoarthritis(1), and Baker's cyst(l). CONCLUSION:Awareness of these characteristic MR findings(a-e) may increase the sensitivity of MR imaging in the diagnosis of displaced bucket-handle tears, and the MRI may be helpful to correctly characterize the displaced fragment and patterns of associated injury, providing arthroscopists a guide to appropriate surgical plans.

Angiomyolipoma is considered by many authors to be a hamartoma, occurring in a sporadic form or in association with tuberous sclerosis. This lesion consists of thick walled blood vessels, smooth muscle, and mature adipose tissue in varying amounts. We have experienced a case of the angiomyolipoma composed of monotypic epithelioid cells. The patient was a 69-year-old female. Clinically, there was no evidence of tuberous sclerosis. Microscopically, the tumor was composed of polygonal cells with abundant eosinophilic granular or clear cytoplasm, pleomorphic nuclei, prominent nucleoli, and multinucleated giant cells. The tumor cells showed positive reaction for HMB45, CD68, smooth muscle actin, and S-100, and negative reaction for epithelial membrane antigen, cytokeratin, vimentin, desmin, CD34, estrogen receptor, and progesterone receptor. Ultrastructual analysis showed the presence of glycogen, mitochondria, and other microorganelles in neoplastic cells. Melanosome or premelanosome was not identified.

Retinal dysplasia is an abnormal differentiation of the retina at embryonal retina stage with proliferation of its elements into rosettes, fold, and gliosis and it is very difficulat to distinguish between the various types of intraocular disease, especilaly retinoblastoma. We experienced a case of unilateral retinal dysplasia without systemic abnormalities occurring in a 5 months old boy. It was clinically suspected Retinoblastoma, which was proved to be Retinal Dysplasia by histopathologic examination.

PURPOSE: This study is to determine age-specific reference range of serum concentration of prostate-specific antigen(PSA) in each decades of age for whom PSA levels were checked in our Multipathic Health Checkup Center. MATERIALS AND METHODS: Between February 1996 and August 1998, the serum PSA of 10,691 men aged 20 to 79 years were checked from Multipathic Health Checkup Center. All cases of lower urinary tract infection(>4WBC/HPF in urinalysis) were excluded from the analysis. PSA was measured by Enzyme Immuno-Assay(AxSYM kit, Abbott Co.) techniques. Patients were grouped according to age by decade: group 20`s(n=941), group 30`s(n=3,963), group 40`s(n=3,607), group 50`s(n=1,735), group 60`s(n=392) and group 70`s(n=53). RESULTS: Based on linear regression analysis, serum PSA concentration was correlated with patient age significantly(r=0.091, p<0.005). The mean PSA values(ng/ml) of each age groups were 20`s: 0.927+/-.660, 30`s: 0.931+/-.664, 40`s: 0.951+/-.715, 50`s: 1.035+/-.883, 60`s: 1.369+/-.607 and 70`s: 2.244+/-.317. The upper limits of PSA(mean+2SD) according to age were 20`s: 2.25ng/ml, 30`s: 2.26ng/ml, 40`s: 2.38ng/ml, 50`s: 2.80ng/ml, 60`s: 4.58ng/ml and 70`s: 8.88ng/ml. CONCLUSIONS: These findings confirmed earlier observations that serum PSA were age-dependent and age-specific, and it were lower for Korean men than for white men. The age-specific reference ranges for Korean men should be established, and then the PSA assay could become more efficient as a screening test for prostatic cancer.

Bronchogenic cyst is an uncommon congenital lesion which is derived from the primitive foregut. Most bronchogenic cyst may develope at the tracheal bifurcation, both main bronchi, the lung parenchymeand the mediastinum. A 40-year old male was evaluated for dyspnea and chest tightness. Computed tomography revealed a well dermarcated, 7.2 x 7.9 cm sized, homogeneous mass compressing the left atrium. 2D-echo showed grade III mitral regurgitation. We completely removed the cystic mass and then confirmed the bronchogenic cyst in the pathological diagnosis. During the follow up period, the patient progressed well without any symptoms and showed grade I mitral regurgitation on the 2D-Echo. Therefore, we report a case of the bronchogenic cyst causing grade III mitral regurgitation.

BACKGROUND: Because Chlamydia trachomatis infection is the most frequent etiological agent in sexually transmitted diseases, accurate diagnostic methods are essential. The enzyme-linked fluorescence assay and Papanicolaou smear were evaluated for detection of C. trachomatis in patients with vaginitis. METHODS: Endocervical swabs from 78 women with symptoms of vaginal discharge, itching or lower abdominal pain in the obstetrics/gynecology department of Gyeongsang National University Hospital were tested by Vidas Chlamydia assay. Slides were stained with Papanicolaou and Giemsa stain to demonstrate inclusion body. RESULTS: Five (6.4%) of 78 endocervical specimens were positive by Vidas Chlamydia assay. Only one case was positve for C. trachomatis with Papanicolaou stain. We could not detect any C. trachomatis by Giemsa stain. CONCLUSION: The Vidas Chlamydia test allows sensitive and high-volume testing for chlamydiae and could be useful for the diagnosis of infection with C. trachomatis in patients with vaginitis and pelvic inflammatory disease.
