We report a case of squamous metaplasia of the pleura observed in a 51 year old man. Squamous metaplasia of serosal surface, and in particular of the pleura, is uncommon. The pathogenesis of squamous metaplasia is obscure, but it may be associated with chronic irritation.
2.Limb-Body Wall Malformation Complex with Absence of External Genitalia.

The limb-body wall malformation complex is a sporadic congenital anomaly characterized by protean manifestations. The diagnosis is based upon the presence of at least two out of craniofacial anomaly, body wall defect, and limb abnormalities. We present a case of limb-body wall malformation complex. This case shows abdominal and pelvic wall defects with eventration of the viscera. It also shows an absence of right kidney, polycystic left kidney, absence of external genitalia and anus, and lower extremity abnormalities. The right lower extremity is absent and the deformed left leg shows malformed foot-like structure attached to the shin in addition to a normally positioned left foot. Our patient is the first case of complete absence of the external genitalia associated with limb-body wall malformation complex in Korean publications and the seventh in English publications.

5.The Detection of Enterotoxin Gene from Bacteroides fragilis Isolates in Korea by Polymerase Chain Reaction.

Bacteroides fragilis is a Gram negative nonsporulating anaerobic rod bacterium that makes up about 1 to 2% of the norrnal human colonic microflora. In 1984, Myer et al. reported that some strains of B. fragilis produce enterotoxin and cause diarrheal disease in cattle and human. Since then it has been termed enterotoxigenic B. fragilis (ETBF). In this study, we tried to detect enterotoxin gene from 37 B. fragilis strains, isolated in Korean patients, to confirm the existence of ETBF and usefulness of PCR as a rapid diagnosis method. By this method, we identified 9 ETBF strains and confirmed their pathogenesis by cytotoxicity test. No significant cross- reactivity with other anaerobes or aerobes was observed. Thus, the PCR method may be considered useful for the sensitive and rapid detection of anaerobic infections. And the entire amplified PCR mixture was ligated into a pT7Blue T-vector and transformed into E. coli. When the nucleotide sequences of cloned PCR products were compared with reported enterotoxin gene, pBF529 inserted DNA sequence was nearly in good agreement with it but pBF570 inserted DNA sequence showed some difference at nucleotide 270-300. A search for nucleotide sequence homologies revealed that pBF529 exhibited 99%, but pBF570 indicated only 90% identity with reported enterotoxin gene. According to these results, it was suggested that ETBF toxin can be differentiated into at least 2 subtypes.

7.A Case of Letterer-Siwe Disease.

The patient was presented characteristic clinical, histopathological and X-ray findings, in cluding generalized edema, petechial rash, lymphadenopathy, bone lesions, pulmonary infiltration and hepatomegaly with anemia. Most of these signs developed soon after birth. Diagnosis was confirmed by microscopic examin ation of lymph node biopsy and clinical X-ray findings. The patient was received antibiotics, corticosteroid and vinblastine and discharged against doctors order without improvement.

10.Nasolacrimal Ostium after External Dacryocystorhinostomy.

PURPOSE: To evaluate the internal ostium and lacrimal sac change after external dacryocystorhinostomy (DCR) using digital subtraction dacryocystography (DCG). METHODS: Between May 2003 and July 2004, digital subtraction DCG was performed to assess the ostium and lacrimal sac 6 months after external DCR in 48 patients (n=50 eyes). RESULTS: The lacrimal sac had reformed in 45 (90%) eyes. Mean ostium size was 2.4 mm (0.5~6.1 mm) and these were located at the inferior part of the reformed sac in 39 eyes (78%). Patients with a fistula-type internal ostium, of which the lacrimal sac had not reformed, often complained of epiphora (p=0.005). CONCLUSIONS: Digital subtraction DCG showed successful anatomic changes in the distal lacrimal pathway and confirmed that the internal ostium contracts into a relatively small ostium after DCR. A fistula-type internal ostium may explain the pathophysiology of functional failure after successful DCR.
