1.Personality characteristics, depression and anxiety for patients with essential hypertension.

BACKGROUND: Majority of hypertension is essential in type, so its clear cue usually is not found, and the patients are tend to be persisted of psychopathy such as emotional tension, depression, anger, anxiety and psychologic conflict, also not to be adapted to stressful event. So we decide to survey the personality characteristics of essential hypertensive patients and factors of psychopathology. METHODS: Patient group-50 persons(male 23, female 27) are selected in the course of antihypertensive medication, being in or outpatient treatment at the dept. of F.M. or I.M. in Kae-jung, Dae-sung or Jung-ang Hospital from August 1, 1995 to July 31, 1996. Control group 52 persons(male 25, female 27) are selected. Exclusision criteria are any clinical disease hystory, over 140/90mmHg of BP, under 20-year old age or no cooperation. We assessment of the MMPI, BDI and STAI results about both group. RESULTS: Comparing hypertensive group with normal control group, significantly higher F, Hs, D, Hy, Pd, Pt and Sc scales of MMPI and BDI(p<0.01), and anxiety scale of STAI(p<0.05). There were no significant differences in all scales comparing between both sexs in the hypertensive and control groups, except comparing hypertensive male with hypertensive female in Pt scale(p<0.05) and control female with control male in Hy scale(p<0.05). CONCLUSIONS: Comparing essential hypertensive group with normal control group, significant higher score was found in scales of Hypochondriasis, Depression, Hysteria, Psychopathic deviate, Psychasthenia, Schizophrenia and anxiety. And in the treatment of hypertensive patient with antihypertensive drugs, appropriate additive psychotherapy appears useful in the case of revealing psychopathy.

4.A Case of Bleomycin induced Streaky Pigmentation and Scleroderma.

Bleomycin, a tumoricidal antibiotic agent, may produce unusual cutaneous manifestations such as pigmentation scleroderma, and gangrene. We report a case of the development of linear streaky pigmentation and cutaneous scleroderma in a patient treated with bleomycin for choriocarcinoma of undescented testis. The patient was 45-year-old male presented with linear brown and slate gray streaking over the trunk and extremities after three cycles of chemotherapy(bleomycin, etoposide, cisplatine). After the fourth cycle of the same chemotherapy, 18 weeks after initiation of bleomycin, the development of cutaneous scleroderma-like conditions was observed involving the same sites. Histopathologic examination showed increased basal pigmentation and thick collagen bundles through the entire dermis, extending to the subcutis. Herein, we describe a case of streaky pigmentation and scleroderma in association with bleomycin anticancer chemotherapy simultaneously in a patient.

5.Journal of Korean Neurosurgical Society for 30 Years.

The end of this month is the 30th birthday of Journal of Korean Neurosurgical Society. Since the first issue of Journal of Korean Neurosurgical Society published in October, 1972, it has been developed in its quality as well as the quantity. More than 4000 articles have been published and tremendous contribution for the development of Korean Neurosurgical Society was made by the journal. The authors review the history of Journal of Korean Neurosurgical Society including the problems in this situation and suggest the way to make the journal as an internationally famous one.

6.The Outcomes of the Out-of-Hospital Cardiac Arrest: A collaborative research of three hospitals.

BACKGROUND: There has been a lot of changes in prehospital medical environment with development of EMSS(emergency medical service systems). Especially in out-of-hospital cardiac arrest, the patients could survive when they are moved to the hospitals earlier. The purpose of this research is to know the status of EMSS in Korea by analyzing CPR(cardiopulmonary resuscitation) outcomes of out-of-hospital cardiac arrest patients at 3 hospital in the western area of Seoul and Incheon. METHODS: From July 1997 to June 1998, we collected data about out-of-hospital cardiac arrest victims at Ewha Womens University Mokdong Hospital, Catholic University Medical College St. Mary Hospital, and Kachon University Chung Ang Gil Hospital. We used same record form based on the 'Utstein Style'. RESULTS: CPR were performed in 265 out-of-hospital cardiac arrest patients at 3 hospitals. One hundred twelve(42.3%) patients recovered the spontaneous circulation at least once and eight(3.0%) patients discharged alive. One hundred ninety four(73.2%) patients died of medical causes, one hundred two(38.5%) cardiogenic and ninety two(34.7%) non-cardiogenic, and seventy(26.4%) patients died of traumatic causes. Initial EKG showed VT/VF(ventricular tachycardia/ventricular fibrillation) in thirty one(11.7%) patients, asystole in one hundred fifty one(57.0%) patients and other rhythms in eighty three(31.3%) patients. Among one hundred two cardiogenic cardiac arrest patients, two(2.0%) patients was discharged alive. CONCLUSION: Overall survival rate of out-of-hospital cardiac angst patients was 3% which was poorer than that of the western country. The proportion of the cardiogenic cause was 3% which was only hart of the western country. VT/VF is relatively not common as a initial EKG rhythm. These differences might be due to difference in the prevalence pattern of out-of-hospital cardiac arrest as well as prematurity of the EMSS.

7.Effects of the Photostability of Sunscreens on the in vivo Photoprotection.

BACKGROUND: Solar ultraviolet (UV) radiation induces sunburn, immune suppression, and various pigmentary disorders. Sunscreens are widely used to protect those untoward effects by UV but there are reports of phototoxicity or stability problems of sunscreens after exposure to UV. OBJECTIVE: We tried to compare sunscreens with different photostability in terms of their protection against various biologic responses like sunburn, immune suppression or pigmentation. METHODS: Three different sunscreens with SPF around 30 were used; Sunscreen-A (Sc-A) was photochemically inert, sunscreen-B (Sc-B) showed intermediate level of photostability, and sunscreen-C (Sc-C) was the least stable. To observe their in vivo effects, we measured sunscreen-protection against sunburn by back-skin swelling and sunburn cell formation, against immune suppression measured by depletion of Langerhans cells, local and systemic suppression of contact hypersensitivity (CHS), and against pigmentation by irradiation with mixed light source with UVA and UVB lamps that mimic solar UV spectrum. RESULTS: Back skin swellings by 5 kJ/m2 of UVB were protected well by sunscreens, but protection of Sc-C against 50 kJ/m2 of UVB was worse than Sc-A or Sc-B. Sunburn cells were increased significantly in mice irradiated with 5 kJ/m2 of UVB and it was protected by sunscreens, and the effect of photostability was minimal. Depletion of epidermal Langerhans cells by 5 kJ/m2 of UVB was protected completely by sunscreens. Local suppression of CHS by 5 kJ/m2 of UVB was protected by sunscreens, and Sc-A had better protection. But, in the experiment with 50 kJ/m2 of UVB, the protective efficacy was reversed; Sc-A showed worse protection. Systemic suppression of CHS by 10 kJ/m2 of UVB was protected well by sunscreens, and Sc-A had better protection and Sc-C had worse protection. In the experiment irradiated with 100 kJ/m2 of UVB, the protection of sunscreens was decreased, and Sc-B showed better protection, whereas Sc-C showed worse protection. In UV-induced pigmentation, all three sunscreens showed significant protection both by L* value and individual topographic angle (ITA) with the best protection by Sc-A and the worst protection by Sc-B. CONCLUSION: These data showed sunscreens can protect various in vivo responses and photostability of sunscreens played important roles particularly in the back-skin swelling and systemic suppression of CHS by high dose of UVB.

8.Somatosensory Evoked Potentials in Adult Polio-like Syndrome Following Acute Hemorrhagic Conjunctivitis.

Electrodiagnostic studies were carried out in 4 patients with neurological complications of acute hemorrhatic conjunctivitis at Severance Hospital on 1983. Both motor and sensory nerve conduction studies were normal even in the involved limbs. The electromyography, however, reveals denervation potentials and neuropathic MUPs. The somatosensory evoked potentials taken in median nerve stimulated at the wrist and peroneal nerve stimulated around the knee showed normal latencies, shapes and amplitudes almost symmetrically.

9.A Clinical Observation on Donor Nephrectomy for Renal Transplantation Using horacoretroperitoneal Approach.

A clinical observation was made on 10 donors for renal transplantation which were done at dept. of urology university hospital from April 1978. to August 1978. The following results were observed: 1. Donors were 3 male and 7 female, aged ranging from 1970 to years old. The most common age group was fifties 2. The related donors were 9 and unrelated was 1. 3. Seven donors had sam ABO blood type with the recipient and 3 had different. The HL-A match grade showed that 2 were B grade and 1 was C grade and 7 were D grade. The failed 3 patient had same ABO blood type with donor and all in D grade. 4. Seven were nephrectomized left and 3 were right. The postoperative course were all uneventful and discharged from POD 9 to 13. 5. The thoracoretroperitoneal approach used for donor nephrectomy: gave good operative field and easy operative technique.

10.Comparative Study on Function and Stability of Sunscreening Products.

BACKGROUND: A lot of protective tools such as sunshades (sun-cap), sun-shielding tints and various sunscreens are widely used to protect solar ultraviolet (UV) radiation. Although dermatologists are aware of these products, they do not know the exact protective efficacy or their stability after strong UV irradiation. OBJECTIVE: We tried to measure the spectal absorbance and transmittance of various sunscreening products. In addition, we measured change of sunscreens's absorbance or transmittance after strong UV irradiation for 30, 60 and 90 minutes. METHODS: We purchased five commercially available sunshades (product A-E), five sun-tinting films (product A-E), and eight sunsceens (product A-H) with similar sun-protection factor (SPF) around 30, and measured spectral absorbance and transmittance of those agents. For the sunscreens, they were irradiated with 250 watt Xenon-Arc lamp and change of spectral responses were evaluated. RESULTS: In absorption and transmission spectra of five different sun-caps, sun-cap C showed very good protection in entire UV range whereas all others protected UV only partially. Absorption and transmission spectra of six different sun-shielding tints showed all of them protected UV fairly well and tint C showed the best protection. Eight different sunscreens showed profound differences in spectal absorbances or transmittances. Sunscreen-A showed the best protection and there was no relationship between price and spectral-protection of sunscreens. In the photo-stability of eight sunscreens after UV-irradiation, there were big differences. Only sunscreen-A showed the least change after UV irradiation, and all other sunscreens showed a change of specta by increased UV-irradiation time. CONCLUSION: Protection efficacies of sunscreening products were variable, and most sunscreens were unstable to strong UV irradiation. Further studies would be necessary to give proper information for protecting UV effectively to dermatologists and consumers.
