BACKGROUND: The purpose of study to present the basic data for enhancing the performance level of the dental clinic infection prevention through the actual condition of the implementation for dental treatment infection prevention aimed at dental hygienists in each hospital. METHODS: We carried out a survey targeting 69 dental hygienists in the dental college hospital, 42 in the dentist of the Dentistry in General Hospital, 51 in the dental clinic and 38 in the dentist's office and the medical institutions surveyed, and we used frequency analysis and chi-square analysis by using SPSS (Ver. 18.0). RESULTS: The experiences wounded in the body by sharp instruments or needles contaminated with patients' blood or saliva were on average in 86.5% of the dental hygienists, and people who disinfected the wound with disinfectants after washing their own hands and squeezing blood as the method to fight the wound was 73.4%. The dental clinic was the highest in case of the rate of wearing gloves, and the dental clinic was the highest in the case of the rate of wearing a mask. The use of the liquid soap containing antimicrobial was the highest as 28.0% in the dental college hospital in case of 'the cleanser to be used for washing their own hands'. In the multiple response for dental treatment mechanical equipment management apparatus and the actual condition to be equipped with medicines and the survey in sterilization, Povidone was the highest as 28% in the dental college, and surface disinfectants were being used in some hospitals. CONCLUSION: To prevent the infection in the consultation room, dental hygienists should thoroughly recognize Dental Treatment Infection Prevention Standards, and the research measures or administrative support of the government and the organs concerned is needed a lot.

Purpose: This study aimed to identify the longitudinal effects of children’s temperament on maternal depression. Methods: Data from a longitudinal cohort of the Panel Study of Korean Children (PSKC) from 2010 to 2012 were analyzed using hierarchical linear modeling. The survey included 1,721 mother-child dyads. The mothers reported on their children’s temperament and on maternal depression. The children’s temperament was measured by the Emotionality, Activity and Sociability-Temperament Survey for Children-Parental Ratings, while maternal depression was measured by the Kessler 6 Psychological Distress Scale. Results: The results showed that both children’s temperament and maternal depression were relatively stable when the children were between the ages of 2 to 4. The mean maternal depression scores were 11.83 in 2010, 11.88 in 2011, and 11.75 in 2012. There were significant negative correlations between the maternal depression scores and children’s ages, and sociability and activity subdomain scores ranged from r=-.05 to -.11 (p<.05). There was a significant positive correlation between children’s emotionality subdomain scores and maternal depression scores (r=.35, p<.001). Children’s temperament rament (emotionality: β=0.26, activity: β=-0.07, and sociability: β=-0.03) were significant factors in maternal depression. Conclusion These findings indicate the need for the early assessment of and intervention for children’s temperament and maternal depression. The results of this study will provide basic data for the development of nursing education programs related to early assessment and intervention to improve the health and quality of life of young children and mothers.

Once splinter hemorrhage can be considered as a pathognomonic sign of subacute bacterial endocarditis. But it can also be associated with a variety of systemic disorders that increase capillary fragility or primary nail bed involvement in dermatologic disorders. The cause of splinter hemorrhage can usually be established by careful history and physical examination. We report a case of 33-year-old man with splinter hemorrhages, who had polycythemia vera.

BACKGROUND: No-reflow is a specific type of vascular damage occuring when removal of coronary occlusion dose not lead to restoration of coronary flow. There are three major explanations for the no-reflow phenomenon such as endothelial cell edema, microvascular plugging by platelets or thrombi and coronary occlusion by ischemic contracture of the myocardium. But detailed mechanisms of no-reflow phenomenon are not known. The objects of this study are to elucidate the possibility whether elevation of cytosolic Ca2+ concentration during ischemic cardioplegic period is mechanism of no-reflow phenomenon or not. METHODS: Changes in cytosolic Ca2+ concentration were measured under varying experimental condition. Free [Ca2+] in the cytosole [Ca2+]i of single rabbit coronary artery cells was measured with fluorescent Ca2+ indicator, Fura-2. RESULTS: Resting [Ca2+]i was 134.2+/-34 nM (n=43). When single cells were perfused with cardioplegic or ischemic cardioplegic solution, [Ca2+]i was significantly increased and degree of [Ca2+]i elevation was further augmented by ischemic cardioplegic solution. Pretreatment of sarcoplasmic reticulum emptying agent (20mM caffeine) had no effect on cardioplegia-induced [Ca2+]i change, but application of Ca2+ channel blocker (5x10-7M nifedipine) or an antagonist of Na+/Ca2+ exchange (5mM Ni2+ ) partially (nifedipine) or completely (nickel) inhibited the [Ca2+]i elevation. Pretreament of caffeine had no effect on ischemic cardioplegia-induced [Ca2+]i change, but application of nifedipine or nickel partially inhibited the [Ca2+]i elevation. Magnitude of ischemic cardioplegia-induced [Ca2+]i elevation was dependent on the Ca2+ concentration of perfusate from 0 to 2.5mM. When Ni2+ was added to reperfusion solution, recovery of ischemic cardioplegia-induced [Ca2+]i elevation was very rapid compared with control. CONCLUSIONS: From the above results, it may be speculated that ischemic cardioplegia-induced [Ca2+]i elevation may act as one of the mechanism of no-reflow phenomenon in rabbit coronary artery.

Anthrax toxin consists of three separate proteins, protective antigen (PA), edema factor (EF), and lethal factor (LF). PA binds to the receptor on mammalian cells and facilitates translocation of EF or LF into its cytosol. PA is the primary component of anthrax vaccines. In this study we purified PA from culture filtrates of Bacillus anthracis. The purification involved sequential chromatography through hydroxylapatite, DEAE-Sepharose CL-4B, followed by Mono-Q. The purified PA was judged to be homogeneous on SDS-PAGE, and consisted of a single polypeptide chain with a relative molecular weight of 85,000.

BACKGROUND: Although anticardiolipin antibody(aCL)-related stroke is far more frequently identified in younger populations, the role of aCL in the pathogenesis of cerebral infarction may be important also in the old, stroke-prone population. We studied the clinical profiles of aCL- related stroke in elderly patient to look at its role on the pathogenesis of ischemic stroke. METHODS: We analyzed unselected patients with acute ischemic stroke and age matched controls for the presence of aCL, prospectively. Also, we studied the characteristics of these patients based on the conventional risk factors and other clinical, laboratory and radiological features in them. RESULTS: aCL was positive 30 (14.7%) of 203 stroke patients, but 11 (6.1%) in 193 control patients. The proportion of patients having more than one of the risk factors was significantly greater in aCL-positive (24/30, 80.0%) than in aCL-negative patients (100/173, 57.8%). The incidence of aCL positivity was significantly greater in patients having one or more risk factors (24/124, 19.3%) than in patients not having any of the risks (6/79, 7.6%). Two of the patients had prolonged aPTT, and only one had a lupus anticoagulant. A positive ANA and false-positive VDRL were not found in our patients. Radiological findings confer subcortical infarction. CONCLUSION: The elevated aCL are a risk marker for stroke also in the elderly population. Our aCL-positive patients generally had multiple risk factors for stroke and are associated with subcortical infarctions, contrary to previous studies. The role of aCL as a disease marker for ischemic stroke in elderly patients warrants further investigations.

PURPOSE: The diagnosis of Budd-Chiari syndrome on CT is difficult if CT do not demonstrate obstruction of the IVC or hepatic vein and other parameter is needed for the correct diagnosis. The purpose of our study was to determine the usefulness of measuring the diameter of azygos-hemiazygos vein on CT to differentiate Budd-Chiari syndrome from advanced liver cirrhosis. MATERIALS AND METHODS: Fourteen patients who were proven as Budd-Chiari syndrome on vena cavography were studied for analysis. All patients showed evidence of liver cirrhosis on CT. As a control group fifteen cases of advanced liver cirrhosis who underwent endoscopic sclerotheraphy due to esophageal variceal bleeding were also included for comparison. The largest short axis diameter of azygos-hemiazygos vein was measured in all patients at the level of diaphragm on axial CT and the results were compared in both groups. RESULTS: In patients with Budd-Chiari syndrome the largest short axial diameter of azygos-hemiazygos vein ranged from 0.5cm to 2.5cm(mean ;1.5cm). Only one patient who showed hepatic venous obstruction demonstrated a diameter of less than 1 cm(0. Scm). In contrast, the diameter in patients with advanced liver cirrhosis without obstruction of IVC or hepatic vein was less than 1 cm with a range from 0.2cm to 1 cm(mean ;0.6cm). CONCLUSION: The short axis diameter of azygos-hemiazygos vein was an indicator of IVC obstruction (Budd-Chiari syndrome).

Acupuncture with gold needle insertion is utilized in Oriental medicine, and the effect of therapy is represented by anti-inflammatory and analgesic function, But sometimes the needle is erroneously inserted into abdominal organs, and then it may cause dangerous complications. We report a case of multiple gold needles in the abdominal organs without having definete clinical symptoms.

When the oculomotor nerve is completely paralyzed, the affected eye shows severe outward displacement and poor cosmetic appearance. Past results of many surgical procedures for oculomotor palsy have been generally unsatisfactory. We tried a new surgical approach experimentally, in which the disinserted lateral rectus muscle was used as an adductor by medial transposition of the muscle. Five adult cats underwent disinsertion of the medial rectus muscle of both eyes to induce iatrogenic medial rectus paralysis. The disinserted medial rectus was removed as far back as possible to prevent reattachment. Then, the right lateral rectus muscle was disinserted and passed beneath the superior rectus muscle and resutured to the sclera 4mm superoposterior to the medial rectus insertion site. After excision of the bilateral medial rectus, a large exotropia of an average 47.6 delta (42.0-55.5 delta) was induced. The medial transposition of the right lateral rectus produced an average 36.6 delta (24.8-45.8 delta) correction of the exotropia. A satisfactory cosmetic result was achieved by this procedure.
