1.Sacral pressure sore treatment with gluteal perforator-based flap.

Sacral pressure sores have been treated by a variety of surgical methods. complete treatment needs wide excision and coverage with healthy tissue which has constant and sufficient blood supply. Use of gluteus maximus muscle flap with or without overlying skin is a revolutionary method because of the reliability of blood supply. However, it is technically a little bit complicated, and future reconstruction for recurrent decubitus is especially limited in paraplegic patients. The development of gluteal perforator-based flap with para-sacral perforator introduce a new treatment modality for the sacral pressure sores. Total 10 cases of sacral pressure sores were treated with gluteal perforator-based flap. There were minimal postoperative complications except wound dehiscence in one case. This flap has a many advantage of no transection or sacrifice of the gluteus maximus muscle, elevation time for the flap is short, reliable blood flow of the perforator, large rotation arc and no post-operative hindrance to walking in patients who are not paraplegic. The disadvantages of this perforator-based flaps are the anatomical variation in the location of perforators and the need for technically careful dissection.

2.Noninvasive Evaluation of Coronary Artery Bypass Graft Patency by Electron Beam Tomography.

Recently non-invasive diagnostic imaging replaced the invasive catheter angiography in the diagnosis of vascular disease. Catheter methods are now almost confined to the purpose of intervention. Coronary artery or coronary artery bypass graft still needs catheter technique because of small diameter and the cardiac motion. The last challenge for radiologists in this domain is to obtain a non-invasive imaging. Electron beam tomography(EBT) for high temporal resolution is able to obtain a coronary arteriogram or coronary artery bypass graft (CABG), of which CABG imaging is quite useful for the evaluation of patency. In our experience as well as others, the accuracy of EBT angiogram in evaluating CABG patency revealed that the accuracy of patency of saphenous vein grafts(SVG) is high due to relatively wide lumen, short and straight course and less influence from cardiac motion. The sensitivity and specificity of patency of SVGs were 92%, 97% respectively in the prospective evaluat on and 100% each in the retrospective evaluation. A false positive and a false negative case are rudimentary errors in the initial learing period. In contrast the analysis of left internal mammary artery(LIMA) graft was difficult due to the inherent small size and the adjacent surgical clips provoking beam-hardening artifact; therefore, the method of combining 3 dimensional reconstruction and flow mode study was important in improving the accuracy of LIMA patency. The sensitivity and specificity of LIMA patency were 100% and 80% in both prospective and retrospective evaluation. Therefore, EBT angiography is an accurate non-invasive diagnostic modality for evaluating the patency of CABG, particularly in SVGs. The accuracy can be improved with the improvement of the EBT and the development of the image reconstruction software.

4.A Statistical Study of Dermatoses in Goyang City (2009~2013).

BACKGROUND: Data regarding the incidence rates of skin diseases that are held by general hospitals provide readily available sources of information that may be representative of the provinces they serve. The population of Goyang city has increased rapidly. Hence, a statistical study of the latest data is necessary. OBJECTIVE: This study aimed to investigate the prevalence rates of common dermatoses in Goyang city and to compare these with previous findings. METHODS: We reviewed the records of 21,693 outpatients who visited the Department of Dermatology at Myongji Hospital from 2009 to 2013, and we statistically analyzed the findings. RESULTS: Of the 21,693 outpatients' records reviewed, 9,939 (45.8%) were male patients and 11,754 (54.2%) were female patients. Patients most frequently presented with dermatoses in the fourth decade of life (16.0%), followed by the first (15.2%), fifth (15.0%), and third (13.1%) decades of life, and these four age groups comprised 59.3% of the total number of outpatients. The 15 most common dermatoses were dermatophytoses, alopecia, other eczematous conditions, urticaria, herpes zoster infections, viral warts, atopic dermatitis, allergic contact dermatitis, seborrheic dermatitis, acne, irritant contact dermatitis, benign epidermal tumors, melanocytic nevi, molluscum contagiosum, and pruritus. Diseases that occurred frequently during the summer were dermatophytoses, urticaria, irritant contact dermatitis, herpes zoster infections, and molluscum contagiosum. Diseases that occurred frequently during the winter were other eczematous conditions, atopic dermatitis, and melanocytic nevi. Viral warts and acne occurred frequently in summer and winter. Seborrheic dermatitis, dermatophytoses, molluscum contagiosum, viral warts, alopecia, and benign epidermal tumors showed higher frequencies in males than in females, whereas allergic contact dermatitis, irritant contact dermatitis, herpes zoster infections, melanocytic nevi, urticaria, pruritus, acne, and other eczematous conditions showed higher frequencies in females than in males. CONCLUSION: Compared with previous studies, the current population of Goyang city showed higher levels of herpes zoster infections and viral warts.

10.The Period of Overcorrection after Surgery According to Age in Pediatric Patients with Intermittent Exotropia

PURPOSE: To investigate the recovery period of overcorrection and related factors after surgery in pediatric patients with basic intermittent exotropia (XT). METHODS: Retrospective chart reviews of the medical records of patients who underwent bilateral lateral rectus recession for basic XT were analyzed. Preoperative age, sex, angle of deviation (prism diopters [PD]), and suppression at distance were measured. Patients were observed every week when the angle of deviation was > 2 PD of overcorrection at postoperative day 1. Recovery of overcorrection was defined as improvement of overcorrection with orthotropia. Patients were divided into two groups according to age: younger (group 1) and older (group 2) than 10 years of age. Success was defined as an angle of deviation between 10 PD of exodeviation and 5 PD of esodeviation at the final visit. RESULTS: A total of 88 patients were included. At postoperative day 1, the angle of deviation at distance was −6.9 ± 2.2 PD, and the near angle of deviation was −6.9 ± 2.4 PD. Esodeviation presented as a minus value. The recovery period of overcorrection was 1.9 ± 3.9 weeks and the success rate was 80.7% (71 patients). The success rates of group 1 and group 2 were not statistically significant (p = 0.51). The recovery period of overcorrection in group 2 (2.7 ± 5.9 weeks) was significantly longer than in group 1 (1.8 ± 3.4 weeks) (p = 0.02). CONCLUSIONS: In pediatric adolescents with basic XT, the surgical success rates did not differ significantly according to age, but recovery of overcorrection after strabismus surgery took longer in patients ≥ 10 years of age.
