6.Role of Transvaginal Ultrasonography in Stress Urinary Incontinence.

Hypermobility of the bladder neck in response to increased intraabdominal pressure is the anatomical cause of female stress urinary incontinence ( SUI) and the degree of bladder neck movement has been used to classify SUI patients and to guide management decisions. We performed transvaginal ultrasonography in 14 SUI patients and 20 normal female as control to assess its role on the demonstration of the anatomical features associated with SUI. We adopted three anatomical factors in the sagittal plane which affect the bladder neck mobility , vertical (JY) and horizontal(JX) distance difference between pubic symphysis and bladder neck, and rotation angle difference(JZ) composed of by pubic symphysis and bladder neck during rest and stress states. There were significant differences in Y and Z between the two groups. Three months after corrective surgery for SUI these two factors showed significant improvement in all patients. It could be concluded that transvaginal ultrasonography is a safe and reliable method to diagnosis and evaluate the postoperative outcome for SUI.

7.Mental-Physical Comorbidity of Depression: Results From Korea National Health and Nutrition Examination Survey

Objectives:
:Depression is known to be very common for coexistence with physical diseases, but reports of this are not well known in Korea. The purpose of this study is to investigate demographic factors related to adult depression in Korea and the association between physical diseases and depression.
Methods:
:This study used raw data from the 7th National Health and Nutrition Survey of the Korea Centers for Disease Control and Prevention in 2016 and 2018, and analyzed by integrating data from adults over the age of 19. Patient Health Questionnaire-9 was applied to the diagnosis of depression. Cross-analysis and logistic regression analysis were used for the association with depression according to socio-demographic characteristics and health-related characteristics.
Results:
:The point-prevalence of depression in Korea using Patient Health Questionnaire-9 is 5.4%. Regarding sociodemographic characteristics, risks of depression were increased among the elderly, women, people with low household income, and people with low education levels. It was analyzed that the risk of coexistence of depression with physical diseases such as diabetes, stroke, myocardial infarction, arthritis, asthma, thyroid disease, renal failure, and cirrhosis was significantly high.
Conclusions
:In this study, the presence of chronic diseases greatly increased the risk of depression, and it was confirmed that certain groups were at high risk for depression.
8.Adherence of Slime

There were many studies to investigate the pathogenesis and prevention of infection in artificial joint replacement due to the difficulty in management of infected arthroplasty in spite of using large amounts of antibiotics. Biomaterials play a major role in the development of infection because of the way the body responds to their chemical and physical characteristics. Exopolysaccharide glycocalyx or biofilm(slime) which is produced by organisms adhered to the biomaterials has been detected and regarded as an important factor in pathogenesis. The production of slime on the biomaterials in turn makes the pathogens resistant to the antibiotics and therefore they survive. The objects of this study are to evaluate which materials are more susceptible to the adherence by Staphylococcus epidermidis, to evaluate the amount of antibiotics needed to kill the S. epidermidis adhered to the biomaterials(Polymethymethacrylate, Titanium-6Aluminum-4Vanadium alloy, Ultrahigh molecular weight polyethylene), and to evaluate the timing of administration of the antibiotics(cephradine, gentamicin) and potadine for prevention of postoperative infection. The results are as follows. 1. The materials in order of greatest adherence due to the number of organisms colonized are poly- methylmethacrylate(PMMA), ultrahigh molecular weight polyethylene(UHMWPE), and titanium alloy(Ti-6A1-4V alloy) being the least adherent. 2. With the production of biofilm the S. epidermidis becomes resistant to even that of 4 times the minimum bactericidal concentration(MBC) of antibiotics. 3. For prevention of postoperative infection, the prophylactic administration of cephradine if effective when used within 4 hours after contamination and the gentamicin and potadine are effective when used within 8 hours after the contamination with S. epidermidis.

9.The Etiological Role of Legionella Pneumophila in Patients with Community-Acquired Pneumonia in Korea.

BACKGROUND: Legionella pneumophila has been recognized as an important cause of community-acquired pneumonia(CAP) requiring hospitalization. However, epidemiological data on the occurrence of legionella-related pneumonia is unavailable in Korea. The purpose of this study was to evaluated the etiological imprtance of legionella pneumophila serogroup 1 in patients hospitalized with CAP. METHOD: The CAP patient over 16 year-old were recruited from July 1999 to June 2000 at the Chunchon Sacred Heart Hospital. Fifty four patients (male 29, female 25, average age 63.8±15.3) were included in this study. A diagnosis of a legionella pneumophila infection was based on a urinary antigen test using the Binax Company enzyme immunoassay. The severity of pneumonia was assessed using the Fine's PORT scoring system. RESULT: The average Fine's PORT score was 99.7(±44.9). According to the risk classification proposed by the Infectious Disease Society of America, the number of patients in each class(from class I to class V) were 6(11.1%), 13(24.1%), 9(16.7%), 14(25.8%), and 12(22.2%), respectively. Thirty two patients(59.3%) were initially admitted to the intensive care unit. The mortality rate was 16.7%(9 in 54). In all patients, urinary antigens to Legionella pneumophila serogroup 1 were not detected. CONCLUSION: Legionella pneumophila may play little role in causing adult CAP in Korea. Therefore, the routine use of macrolide in the empirical treatment of the CAP patients based upon the ATS guidelines(1993) in Korea should be reevaluated.

10.Mitochondrial Intestinal Pseudo-Obstruction with Neurogenic Bladder Syndrome: Point Mutation at T8356C: A New Mitochondrial Disease?.

We describe a unique patient with progressive external ophthalmoplegia, intestinal pseudo-obstruction, and neurogenic bladder. Genetic study in this patient shows point mutation at T8356C, the locus known as that of myoclonic epilepsy with ragged-red fibers. To the best of our knowledge, this is the first report of a mitochondrial syndrome consisting of intestinal pseudo-obstruction, neurogenic bladder, and progressive external ophthalmoplegia, point mutation at T8356C. We suggest that this could comprise a new mitochondrial disease rather than a new variant of mitochondrial neurogastrointestinal encephalomyopathy.
