2.Congenital coronary arteriovenous fistula combined with ASD.

Coronary arteriovenous fistula (CAF) is most common form of hemodynamically significant coronary anomaly. CAF combined with atrial septal defect (ASD), however, constitutes an uncommon subgroup of CAF. We present a case of congenital CAF combined with secondum ASD, which are manifested by congestive heart failure. These anomalies were identified by transthoracic echocardiography and coronary angiography respectively. These ASD and CAF were closed by operation successfully without any complication.

3.The Effect of Healon(R) and Metrol(R) on Cornea and Intraocular Pressure in Extracapsular Cataract Extraction with Posterior Chamber Lens Implantation.

The aim of this study is to evaluate the effect of viscoelastic substance on corneal thickness, corneal endothelium and intraocular pressure by measuring corneal thickness, corneal endothelial cell density and intraocular pressure in extracapsular cataract extraction with posterior chamber lens implantation. The study comprised of 50 patients, divided into two groups: extracapsular cataract extraction with posterior chamber lens implantation using Healon(R) or 2% methylcellulose(Metrol(R)). The txtracapsular cataract extraction was done by irrigation and aspiration technique and the posterior chamber lens was implanted into posterior chamber. The corneal thickness and endothelial cell density were measured preoperatively and at 6 weeks of postoperative period: intraocular pressure was measured preoperatively and at 24 hours of postoperative preiod. The results were as follows: 1. The preoperative corneal thickness, endothelial cell density and intraocular pressure were 499 +/- 34 micrometer, 2523 +/- 388 cells/mm2 and 15.9 +/- 2.4mmHg in average respectively. 2. The postoperative corneal thickness with Healon(R) was 501 +/- 42 micrometer, and with Metrol(R) was 491 +/- 28 micrometer. There was no significant difference between Healon(R) used and Metrol(R) used group(p<0.05). 3. The loss of endothelial cell with Healon(R) was 10.7%, and with Metrol(R) was 17.1%. The endothelial cell loss was lower in Healon(R) used group than that of Metrol(R) group. 4. The intraocular pressure with Healon(R) was 14.6 +/- 3.9mmHg, and with Metrol(R) was 12.3 +/- 4.4mmHg. There was no significant difference between Healon(R)-used and Metrol(R)-used group(p<0.05).

Neurilemmoma, also refired to as Schwannoma, neurinoma and perineural fibroblastoma, is well capsulated solitary tumor of Schwann cell origin. The most common sites for benign solitary neurilemmoma are the extremities or the head and neck region. In the retroperitoneum, they are commonly seen in the presacral region. It is incidentally detected and confirmed by pathologic findings. We report a case of retroperitoneal neurilemmoma arising from adjacent to kidney, in 49 years old man.

6.A Case of Cystadenocarcinoma of the Kidney.

Papillary renal cell carcinoma is a histological variant that may be cystic in appearance. This variety has been termed cystadenocarcinoma to distinguish it from a renal cell carcinoma that has been rendered cystic through necrosis and hemorrhage. Because a greater proportion presented as stage I lesion, papillary renal cell carcinoma has been reported to have a better prognosis than renal cell carcinoma in general. We report a case of cystadenocarcinoma of the kidney that was confirmed by surgical exploration.

As part of an initial inquiry into the function of the outer membrane proteins (OMPs) of Helicobacter pylori Korean strain 51, we have conducted an extensive proteome analysis via quadrupole time of flight (Q-TOF) mass spectrometry (MS). Fifty one OMPs of H. pylori were purified using sarcosine and resolved via two-dimensional electrophoresis with immobilized pH gradient strips. The most abundant proteins were observed in the alkaline pI regions (6.0~11.0) at molecular masses between 10~100 KDa. Here, 15 spots were identified, representing 9 types of genes (KHP0852, KHP0853, KHP1353, KHP1017, KHP0172, KHP0076, KHP0617, KHP1069, KHP0614) from the sarcosin-insoluble fraction of H. pylori 51. These may be employed in the characterization of the OMPs of H. pylori 51, which will help to identify new potential target proteins for vaccine development and drug therapy.

H. pylori produces urease abundantly amounting to 6% of total protein of bacterial mass. Urease genes are composed of a cluster of 9 genes of ureC, ureD, ureA, ureB, ureI, ureE, ureF, ureG, ureH. Production of H. pylori urease in E. coli was studied with genetic cotransformation. Structural genes ureA and ureB produce urease apoprotein in E, coli but the apoprotein has no enzymatic activity. ureC and ureD do not affect urease production nor enzyme activity ureF, ureG, and ureH are essential to produce the catalytically active H. pylori urease of structural genes (ureA and ureB) in E.coli. The kinetics of activation of H. pylori urease apoprotein were examined to understand the production of active H. pylori urease. Activation of H. pylori urease apoprotein, pH dependency, reversibility of CO2 binding, irreversibility of CO2 and Ni2+ incorporation, and CO2 dependency of initial rate of urease activity have been observed in vitro. The intrinsic reactivity (ko) for carbamylation of urease apoprotein coexpressed with accessory genes was 17-fold greater than that of urease apoprotein expressed without accessory genes. It is concluded that accessory genes function in maximizing the carbamylating deprotonated E-amino group of Lys 219 of urease B subunit and metallocenter of urease apoprotein is supposed to be assembled by reaction of a deprotonated protein side chain with an activating CO2 molecule to generate ligands that facilitate productive nickel binding.

9.Ciliary Beat Frequency in the Airways of Humans : Influences of Lidocaine, Age, Sex and Smoking.

We measured the ciliary beat frequencies (CBFs) in respiratory ciliated epithelia obtained from the nasal cavity, trachea, and bronchus of 40 subjects during fiberoptic bronchoscopy, using a video-computerized analysis technique. The subjects were classified into various groups based on a range of parameters, including sex, age, history of smoking and whether or not a local anesthetic has been used and mean values of CBFs were analyzed between these groups. The ciliary beat of nasal epithelial cells was significantly faster than that of the trachea or bronchus (p=0.008). There were significant positive correlations between the CBFs at the nasal cavity and trachea (R2=0.467, p<0.001), nasal cavity and bronchus (R2=0.566, p<0.001), and trachea and bronchus (R2=0.541, p<0.001). Subjects older than 60 years of age had significantly slower CBFs at all three sites and than their younger counterparts (p<0.001). Neither sex nor smoking affected the ciliary motility at any site of the respiratory tract. There was no significant difference in the nasal CBFs between the anesthetized and non-anesthetized sides. In conclusion, this study can provide important clinical data on the CBFs of respiratory tracts in humans.

10.Suppression of Fibrotic Reactions of Chitosan-Alginate Microcapsules Containing Porcine Islets by Dexamethasone Surface Coating

Background:
The microencapsulation is an ideal solution to overcome immune rejection without immunosuppressive treatment. Poor biocompatibility and small molecular antigens secreted from encapsulated islets induce fibrosis infiltration. Therefore, the aims of this study were to improve the biocompatibility of microcapsules by dexamethasone coating and to verify its effect after xenogeneic transplantation in a streptozotocin-induced diabetes mice.
Methods:
Dexamethasone 21-phosphate (Dexa) was dissolved in 1% chitosan and was cross-linked with the alginate microcapsule surface. Insulin secretion and viability assays were performed 14 days after microencapsulation. Dexa-containing chitosan-coated alginate (Dexa-chitosan) or alginate microencapsulated porcine islets were transplanted into diabetic mice. The fibrosis infiltration score was calculated from the harvested microcapsules. The harvested microcapsules were stained with trichrome and for insulin and macrophages.
Results:
No significant differences in glucose-stimulated insulin secretion and islet viability were noted among naked, alginate, and Dexa-chitosan microencapsulated islets. After transplantation of microencapsulated porcine islets, nonfasting blood glucose were normalized in both the Dexa-chitosan and alginate groups until 231 days. The average glucose after transplantation were lower in the Dexa-chitosan group than the alginate group. Pericapsular fibrosis and inflammatory cell infiltration of microcapsules were significantly reduced in Dexa-chitosan compared with alginate microcapsules. Dithizone and insulin were positive in Dexa-chitosan capsules. Although fibrosis and macrophage infiltration was noted on the surface, some alginate microcapsules were stained with insulin.
Conclusion
Dexa coating on microcapsules significantly suppressed the fibrotic reaction on the capsule surface after transplantation of xenogenic islets containing microcapsules without any harmful effects on the function and survival of the islets.
