PURPOSE: Surfactant replacement therapy is now recognized as a life saving and safe intervention in small premature infants with respiratory distress syndrome. The incidence of pulmonary hemorrhage is increased in association with artificial surfactant therapy in extremely low birth weight infants, but the pathogenesis is not clear. We conducted this study to prove whether exposure of RBC or type II alveolar epithelial cell membrane to various artificial pulmonary surfactants may lead to increased membrane permeability. METHODS: 1) Washed packed red blood cells(30 pl) with various concentration of SurfactenR, NewfactanR, CurosurfR, and ExosurfR were incubated for either 2 or 24 hours at 37 degrees C. Hemolysis was measured by spectrophotometry. 2) Type II alveolar epithelial cell(HTB-181) (105cell/Ml) with various concentrations of above artificial surfactants were incubated for 24 hours at 37 degrees C. Lactate dehydrogenase (LDH) release was measured as an indicator of cytotoxicity. RESULTS: 1) Concentrationdent hemolysis of RBC membrane with each artificial surfactant for 2 or 24 hours was observed. Especially for 24 hours incubation, hemolysis with ExosurfR was significantly higher than with other artificial surfactants at concentration up to 2.5 mg/2 mL(P<0.05). 2) Exposure of 105 cells to 6 mg of all artificial surfactants led to a maximum release of LDH from cells into the media. At the concentration of 6 mg, release of LDH with ExosurfR was significantly higher than SurfactenR and NewfactanR (P< 0,05). There was no difference between SurfactenR, NewfactanR, and control group. CONCLUSION: Artificial surfactant may be associated with in vitro cytotoxicity and this property differs for different dosage and exposure time. Especially purely synthetic artificial surfactant(ExosurfR) is more potent in inducing lysis of RBC and type II alveolar epithelial cell.

Purpose: This study aimed to explore the perceptions held by practicing dietitians of the importance of their tasks performed in current work environments, the frequency at which those tasks are performed, and predictions about the importance of those tasks in future work environments. Methods: This was a cross-sectional survey study. An online survey was administered to 350 practicing dietitians. They were asked to assess the importance, performance frequency, and predicted changes in the importance of 27 tasks using a 5-point scale. Descriptive statistics were calculated, and the means of the variables were compared across categorized work environments using analysis of variance. Results: The importance scores of all surveyed tasks were higher than 3.0, except for the marketing management task. Self-development, nutrition education/counseling, menu planning, food safety management, and documentation/data management were all rated higher than 4.0. The highest performance frequency score was related to documentation/data management. The importance scores of all duties, except for professional development, differed significantly by workplace. As for predictions about the future importance of the tasks surveyed, dietitians responded that the importance of all 27 tasks would either remain at current levels or increase in the future. Conclusion Twenty-seven tasks were confirmed to represent dietitians’ job functions in various workplaces. These tasks can be used to improve the test specifications of the Korean Dietitian Licensing Examination and the curriculum of dietetic education programs.

BACKGROUND: Bisphosphonates are now well established as successful antiresorptive agents for the prevention and treatment of osteoporosis. We investigated the effect of cyclic intravenous treatment with an aminobisphosphonate, pamidronate in cases of primary osteoporosis. METHODS: Eighteen patients with primary osteoporosis (bone mineral density BMD t-score < -2.5) received four courses of pamidronate (30 mg with 500 mL normal saline over 2 hours every 3 months). The serum biochemical parameters and bone turnover markers were measured before each treatment. The bone pain score, medication score, and the side effects were also monitored. BMD and simple spine X-ray were performed before and 1 year after of treatment. RESULTS: BMD at the lumbar spine (L2-4) significantly increased from 0.798+/-0.110 g/cm2 to 0.860+/-0.107 g/cm2 after 1 year of treatment with pamidronate: by +8.3+/-9.4% of baseline. BMDs at the femoral neck, Ward s triangle and the trochanter also increased, but not significantly. Serum total alkaline phosphatase (p<0.05) and urine deoxypyridinoline/creatinine (p=0.069) decreased with treatment. Other bone turnover markers were unchanged. The bone pain score decreased significantly. None of the patients experienced a new fracture during treatment. The frequency of the side effects following the first infusion was 61.1% (a transient fever and myalgia with flu-like symptoms in 10 patients and mild phlebitis in 1 patient). However, only two patients complained of flu-like symptoms after second infusion, and no patient complained following the third infusion. CONCLUSION: Cyclic intravenous treatment of pamidronate every three months was effective in increasing BMD and in the decreasing bone turnover rate, and was relatively well tolerated in primary osteoporotic women.

This study was performed to observe the changes of glucose-related hormones and the morphological change including ultrastructure of the pancreatic islets in the male Otsuka Long-Evans Tokushima Fatty rat. Area under the curve (AUC) of glucose at the 30th (709 +/- 73 mg.h/dL) and at the 40th week (746 +/- 87 mg.h/ dL) of age were significantly higher than that at the 10th week (360 +/- 25 mg.h/ dL). AUC of insulin of the 10th week was 2.4 +/- 0.9 ng.h/mL, increased gradually to 10.8 +/- 8.3 ng.h/mL at the 30th week, and decreased to 1.8 +/- 1.2 ng.h/mL at the 40th week. The size of islet was increased at 20th week of age and the distribution of peripheral alpha cells and central beta cells at the 10th and 20th weeks was changed to a mixed pattern at the 40th week. On electron microscopic examination, beta cells at the 20th week showed many immature secretory granules, increased mitochondria, and hypertrophied Golgi complex and endoplasmic reticulum. At the 40th week, beta cell contained scanty intracellular organelles and secretory granules and apoptosis of acinar cell was observed. In conclusion, as diabetes progressed, increased secretion of insulin was accompanied by increases in size of islets and number of beta-cells in male OLETF rats showing obese type 2 diabetes. However, these compensatory changes could not overcome the requirement of insulin according to the continuous hyperglycemia after development of diabetes.

A simple method for early selection of the highest risk infants would optimize intervention trials. The purpose of this study was to develop an scoring system to predict the bronchopulmonary dysplasia (BPD) in preterm infants. METHODS: A retrospective analysis of clinical, respiratory and radiographic variables of a 2.5-year experience was done with data of preterm infants less than 34 weeks of gestation admitted to Ewha Womans University Hospital from January 1998 to July 2000. RESULTS: Multiple regression analysis identified four independent predictors of BPD; longer duration of assisted ventilation, lower Apgar score at 5 minute, higher PaCO2 at birth, presence of patent ductus arteriosus after 7 days. A clinical predictive scoring system (CPSS) was developed with these four predictors. CONCLUSION: We have come up with a CPSS for early identification of preterm infants with high risk of BPD. A prospective study is on going to evaluate the predictivity of this scoring system.

Extranodal natural killer (NK)/T-cell lymphoma, nasal type is a rare non-Hodgkin lymphoma originating in the nasal cavity or in the paranasal sinuses, and is etiologically closely related with the Epstein Barr Virus infection. It is more commonly found in East Asia, South America, and Mexico than in Europe or North America. Extranodal NK/T-cell lymphoma, nasal type typically shows a crust on the nasal mucosal surface, necrosis, and inflammation. Pathologically, it presents as significant vascular damage and destruction, with apparent tissue necrosis and cytotoxicity. These neoplasms are very aggressive and can show septal perforation, diffuse ulcer lesion on the nasal mucosa, fistula or epistaxis. We experienced a case of the extranodal NK/T-cell lymphoma, nasal type at the nasal septum, which was delayed in diagnosis due to septal perforation, which occurred after septoplasty in a 47-year-old female. Thus, we report this case with a review of literatures.
