1.Evaluation of Skeletal and Dental Maturity in Relation to Vertical Facial Types and the Sex of Growing Children

The purpose of this retrospective study was to evaluate the skeletal and dental maturity according to the vertical facial type and sex in Korean children in the developmental stage.
In total, 184 participants aged 8 - 14 years were selected and divided into three groups based on the mandibular plane angle. For the comparison between the sexes, the three groups were each divided into male and female subgroups. The skeletal and dental maturity were assessed using lateral cephalograms, hand-wrist radiographs and panoramic radiographs.
The vertical growth group showed significantly greater cervical vertebral and hand-wrist maturity than that in the horizontal growth group. Dental maturity was the highest in the vertical growth group. Girls showed greater skeletal maturity than boys, and no distinct difference was observed between the dental maturity of the sexes.
Analysis of the vertical facial type in children can provide ancillary indicators that may help determine the optimal timing for orthodontic treatment initiation. Earlier initiation of orthodontic treatment may be considered for patients with vertical facial growth patterns.
2.Comparison of Maximal Removal Rate of Indocyanine Green and Monoethylglycinexylidide Test in Quantitative Assessment of Hepatic Function.

BACKGROUND: The maximal removal rate of indocyanine green (ICG Rmax), which has been used as a useful indicator of quantitative assessment of the hepatic function, has some disadvantages such as high cost, requirement of multiple sampling, and long turn-around time. This study was designed to clarify that the measurement of the lidocaine metabolite, monoethylglycinekylidide (MEGX) test, can replace the ICG Rmax. And in healthy adults, MEGX forma pion was measured and compared according to methods of measurement and serf. METHOD: In 18 patients to whom ICG Rmax test was requested, ICG Rmax test was carried out at two doses of 0.5 mg/kg and 5 mg/kg and MEGX formation after 15 minute of 1 mg/kg lidocaine Injection was measured by fluorescence polarization immunoassay (FPIA) method. The correlation between them was analyzed, To 25 healthy volunteers included in this study as normal control, lidocaine was given intravenously at, a dose of 1 mg/kg and MEGX forma pion was measured IS and 30 minute later (MEGX15, MEGX30) using both high performance liquid chromatography (HPLC) and FPIA methods. RESULT: Patient group resealed significant correlation between ICG Rmax and MEGX15 (r=0.7674, p<0.001) and also between ICG Rl5 and MEGX15 (r=0.5612, p=0.008). There was significant difference between MEGX15 of 9 patients with chronic liver diseases and those of normal controls (22.24+/- 13.18 and 35.40+/- 14.43 ng/mL, respectively) (p=0.01). In normal controls, the correlation between methods was significant (p=0.001) and the values measured by FPIA method was significantly higher than that by HPLG (p(0.001). Of the normal controls, male group had higher MEGX15 values than female group in both methods (in HPLC method 33.89+/-15.95 and 22.53+/- 8.36, and in FPIA method 41.48+/-16.61 and 28.81+/-7.88 ng/mL, respectively), and in female group MEGX30 values was significantly elevated compared to MEGX15 (p<0.001). CONCLUSION: Inferred from the fact that the correlation between ICG Rmax and MEGX was good, MEGX test can be considered a replacement for ICG Rmax. In healthy adults, it is considered that there is serf-related difference In the rate of lidocaine metabolism so we should pay attention to it in interpreting the MEGX results.

3.The Cytogenetic Study of Acute and Chronic Leukemic Patients in Korea.

BACKGROUND: Because specific chromosomal abnormalities are associated with certain hematologic disorders, cytogenetic studies can help classifing the diseases, providing the clues of disease progression and being used to monitor remission after chemotherapy. In this study, cytogenetic analysis was performed. In acute and chronic leukemic patients in Korea and the results were compared with foreign cytogenetic reports, and the typical acute lymphocytic leukemia (ALL) and acute myelocytic leukemia (AML) associated chromosome aberrations were analysed by some calculated parameters to clarify if the specific chromosomal abberations in the specific types or subtypes of leukemias had diagnostic value or not. METHOD: Chromosome studies were done in bone marrow or peripheral blood samples by high resolution banding technique. Sensitivity, specificity, and positive and negative predictive values of finding or not finding a given aberration were calculated for followings : for the differential diagnosis between ALL and AML when a patient is known to have acute leukemia, for the differential diagnosis among AML and ALL FAB subtypes in a patient with known AML and ALL. RESULTS: The high positive predictive values (1.0) in the AML versus ALL comparison were found for -7, del(7) (q11-34q22-36), +8s, t(8;21) (q22;q22), t(15;17) (q22;q11), inv (16) (q13;q22) and -Y. Among the AML subtypes, the highest sensitivity, positive and negative predictive values were 0.85, 0.97, 0.94 for t(15;17) (q22;q11) in M3, respectively. The high positive predictive values and specificity in the ALL versus AML comparison were found for t(1;19) (q23;p13) ,t(4;11) (q21 ;23) and t(8; 14) (q24;q32) Among the ALL subtypes, the highest negative predictive value was 0.99 for t (8;14) (q24;q32) in L3. Among 398 CML cases, Philadelphia chromosome positive CML were shown in 81.9% that were classic t(9;22) (q34;all) (94.5%), complex variant traslocation(1.8%) and additional secondary chromosome aberrations (3.7%) . CONCLUSION: Total chromosomal aberration rate in acute and chronic leukemia in Korea was lower than that in foreign reports, but the patterns of chromosome aberrations were similar except for t(15;17) (q22;q11) in AML patients. Quantitativly calculated data of sensitivity, specificity and positive and negative predictive values in the specific chromosomal aberration might be used for diagnostic markers of acute leukemia.

4.Relationships among Malnutrition, Depression and Quality of Life in Patients with Gynecologic Cancer receiving Chemotherapy.

PURPOSE: The purpose of this study was to identify nutritional status, and relationships among malnutrition, depression and quality of life in patients with gynecologic cancer who were receiving chemotherapy. METHODS: For this study a descriptive cross-sectional design was used. Participants were 111 women who were enrolled and agreed to undergo a face-to-face interviews including administration of the structured questionnaires: Patient-Generated Subjective Global Assessment (PG-SGA), Beck Depression Inventory (BDI), Simplified Nutritional Appetite Questionnaire (SNAQ), and Functional Assessment of Cancer Therapy-General (FACT-G). RESULTS: Mean body mass index was 23.3 and mean body weight was 56.5 kg. Sixty-three (57%) of the 111 patients were malnourished according to the PG-SGA. The malnourished patients showed higher levels of depression and lower quality of life compared to the non-malnourished patients. In addition, malnutrition was associated with BMI level, depression, appetite and quality of life. CONCLUSION: The findings indicate that the prevalence of malnutrition is high and malnutrition in patients with gynecologic cancer influences depression and adversely affects the quality of life of these women. To improve the patient's quality of life, nutritional assessment and appropriate management is important to decrease malnutrition in patients with gynecologic cancer.

6.Recurrent Impetigo Herpetiformis of Pregnancy Successfully Treated with Acitretin.

Impetigo herpetiformis (IH) is an extremely rare pustular dermatosis of pregnancy with typical onset during the second or third trimester of pregnancy and generally rapid resolution after delivery. Recurrent case of IH in subsequent pregnancy tend to earlier onset and greater severity. We report a 33-year-old pregnant woman, with a history of mild IH in the first pregnancy, who presented with generalized lesions at nearly 29 weeks' gestation. Her condition had become worse suddenly at 34 weeks' gestation. She developed fever, small for gestational age, and gait disturbance due to ache in both thighs. So we decided to terminate the pregnancy by repeat cesarean section. After delivery, her skin lesions had worsen rapidly in spite of treatment with corticosteroids. So she was treated with a large dose of acitretin. Three months after her delivery, her skin was returned to original state, except for residual pigmentation. In conclusion, our case indicates that clinicians should be aware of the possibility of sudden deterioration of the maternal lesions and fetal condition associated with IH.

7.A Case of Duodenal Intramural Hematoma and Hemoperitoneum after Therapeutic Endoscopy in a Patient with Chronic Renal Failure.

Duodenal intramural hematoma is mostly caused by blunt abdominal trauma. It is also less commonly reported as a complication of anticoagulation therapy or as a blood dyscrasia, and as a complication of diagnostic/ therapeutic endoscopy. The presentation of these patients is abdominal pain, vomiting, fever and hematochezia, and this is rarely accompanied with intestinal obstruction, severe pancreatitis and acute peritonitis as its complications. The diagnosis is made clear by performing abdominal ultrasonography and abdominal computed tomography. We reported here on one case of intramural duodenal hematoma and hemoperitoneum after performing endoscopic hemostasis in a chronic renal failure patient who was on maintenance hemodialysis.

8.A Study on the Serum Lipid, Apolipoprotein Levels and Their Correlations in Healthy Adults of Gyeongnam Area.

The purpose of this study was investigating serum lipid, apolipoprotein levels and their correlations in healthy adults of Gyeongnam area. The BMI (body mass index) was significantly higher (p < 0.001) in male (25.2 +/- 2.7 kg/m2) than female (23.8 +/- 1.5 kg/m2), however PBF (percent body fat) was significantly higher (p < 0.001) in female (29.6 +/- 4.3%) than male (22.7 +/- 5.0%). The WHR (waist to hip ratio) and blood pressure in the groups showed there was no significant differences. The levels of serum total cholesterol, LDL-cholesterol and apolipoprotein B were significantly higher (p < 0.01) in male (208.7 +/- 27.7 mg/dl, 129.0 +/- 26.9 mg/dl, 1.0 +/- 0.2 g/L) than female (193.6 +/- 29.1 mg/dl, 112.5 +/- 29.5 mg/dl, 0.9 +/- 0.2 g/L, but HDL-cholesterol level was significantly higher (p < 0.01) in female (54.9 +/- 6.6 mg/dl) than male (49.9 +/- 7.3 mg/dl). The LDL-C/HDL-C, Apo B/Apo A-I and AI (atherogenic index) were significantly higher (p < 0.001) in male (2.6 +/- 0.6, 0.8 +/- 0.2, 3.3 +/- 0.7) than female (2.1 +/- 0.5, 0.6 +/- 0.2, 2.6 +/- 0.5). The triglyceride level was positively correlated with apolipoprotein B concentration (p < 0.05) and negatively correlated with HDL-cholesterol concentration (p < 0.05), however no significant correlation was found with apolipoprotein A-I. According to these results, we conclude that male adults are expecting higher incidence of cardiovascular disease than female adults and we suggest the serum triglyceride should be kept normal level for the prevention of these diseases.

9.A Case of Morvan Syndrome.

Neuromyotonia, or Isaacs' syndrome, consists of continuous muscle fiber activity caused by hyperexcitability of the peripheral nerves. Rarely, these patients also develop CNS symptoms characterized by confusion, insomnia, hallucinations, and agitation. A rare disease consisting of neuromyotonia, autonomic symptoms, and CNS dysfunction is called Morvan's syndrome. We report a 24-year-old man who presented with insomnia, malaise, anorexia, hyperhidrosis, palpitation and myokymia in both the lower extremities. The pathomechanism of Morvan's syndrome is related to the voltage-gated K+ channel (VGKC) antibodies.

10.Oral health educational contents in primary, middle, and high school textbooks.

OBJECTIVES: This study was conducted in order to analyze oral health content and its importance in primary, middle, and high school textbooks. METHODS: A total of 1,176 textbooks was reviewed, and the items under investigation included: textbook division (national/authorized), titles; grade levels, semesters taught, inclusion of educational content for general and oral health, and the number of pages and rows containing the oral health information. The data were analyzed via paired t-tests, frequency analyses, and one-way ANOVAs. RESULTS: Oral health content was found in 12.1 percent of the primary school textbooks, 6.5% of the middle school texts, and 2.1% of the high school textbooks. The total average was 5.4%. Among the textbooks containing health (general and oral) content, the average number of pages (P<0.001) containing general and oral health information were 22.1 and 0.5, respectively. The average number of lines (P<0.001) for both types of health information were 475.1 and 6.2, respectively, in the primary school textbooks. Middle school textbooks included average numbers of health information-related pages (P<0.001) of 45.7 and 0.4 and average numbers of lines (P<0.001) of 1,086.9 and 5.3, respectively. The average number of pages (P<0.001) in the high school books containing general and oral health content were 83.0 and 0.9, respectively, and the average number of lines (P<0.001) were 2,128.6 and 10.7. Our research showed that, regarding the degree of reflecting oral health education objectives, high school textbooks gained 1.00 point (1 objective), middle school books obtained 0.67 point (5 objectives), and primary school books received 0.18 point (39 objectives; P<0.05). CONCLUSIONS: Oral health content in textbooks decreased as school level increased. The objectives of oral health education for primary school were not properly reflected in the textbooks because the objectives were relatively diverse, and the textbook treatment of the material was rather static.
