1.A Case of Acute Encephlopathy with Bilateral Thalamotegmental Involvement.

We experienced an unusual case of acute encephalopathy in a 4 month-old boy He was admitted to our hospital because of lethargy and seizures after preceding symptoms of upper respiratory tract infection a few days before admission. On admission, he was in semicomatous mental state with decorticated rigidity. Laboratory tests showed normal blood sugar, ammonia, and transaminase levels. CSF was acellular and sterile. Brain MRI in both 71 and 72 weighted-image showed high signal density in both thalami and caudate nucleus head. After recovery, neurologic sequales of developmental delay, mental retardation, right hemiplegia and seizure remained. We report a case of acute encephalopathy that have clinical course similar to Reye syndrome but have specific brain image.

2.Attitude of Korean Lawyers toward Withdrawal of Life Sustaining Treatment.

PURPOSE: This study was conducted to study the attitude of Korean lawyers toward withdrawal of life sustaining treatment, and compare and analyze different types of their attitudes. METHODS: Research design of this project was Q methodology approach. The study population was 24 lawyers, aged from 32 to 69 years. Q sample to investigate the attitude of the lawyers toward withdrawal of life sustaining treatment included 34 statements obtained from literatures, TV debate, and depth interviews of 5 lawyers among the lawyers included. After listening to the purpose and method of the study, the 24 lawyers agreed to fill out a survey asking sociodemographic information, and the information was distributed in 9 scale Q-sample. RESULTS: The collected data were processed through QUANL PC program and sorted into 5 types as follows: The first type was 'Choosing to withdraw life sustaining treatment', the second 'Withholding life sustaining treatment' regardless of the cost, the third is neutral type that claims that humans have the right to decide the death and life, and demands the proper legalization to protect such rights, the fourth type agrees to withdrawal of life sustaining treatment, nevertheless, admits that one has a rigt to withhold one's own life treatment, categorized as self contradiction type. The fifth type believed that 'Life and death are providential' with the faith, therefore, such authority to decide life and death belongs to God, but not human beings. CONCLUSION: In conclusion, the lawyer's attitudes toward withdrawal of life sustaining treatment were grouped into five different types as follows: 'Choosing to withdraw life sustaining treatment', 'Withholding life sustaining treatment', 'Demanding legalization', 'Self contradiction type', and 'Life and death are providential'.

3.Clustered microcalcifications on mammogram: Loss of microcalcifications during tissue processing? radiologic-pathologic correlation.

Mammography is the only examination capable of depicting malignant calcifications in breast. In some instances calcifications evident in mammogram and specimen mammograms are not observed in the histologic specimen. Therefore, wer retrospectively analyzed 27 cases in which hook-guided wire localization and biopsy had been performed to determine if microcalcifications were lost during histopathologic processing. Calcium was apparently lost during preparation of the block in about 7% of the cases and after slide preparation in about 11%, for a total possible loss of 18%. The authors recommend a protocol for avoiding these loss that involves performance of a more comprehensive examination of histologic sections by the pathologist and radiographic reexamination of paraffin-embedded tissue blocks with subsequent step sectioning by the pathologist.

6.Structural Analysis of the Health Related Quality of Life in Working Women with Preschool Children

Purpose:
The purpose of this study was to construct a model that describes the health related quality of life in working women with preschool children and to verify the goodness of fitness of the model.
Methods:
The data were collected with a structured self-report questionnaire from 483 working women with preschool children.
Results:
The modified model showed a reasonable fitness to the data. Social support, job satisfaction, parenting stress, sense of coherence and depression accounted for 78% of the health related quality of life in working women with preschool children.
Conclusion
This study suggests that the health related quality of life in working women with preschool children can be improved by reducing parenting stress and depression level, and by increasing social support, job satisfaction, and sense of coherence. Depression can also be managed by reducing parenting stress and by increasing job satisfaction and sense of coherence. Based on the results of this study, it is necessary to develop intervention programs to increase the health related quality of life or to decrease depression in working women.
7.Palate bone exposure from flexible denture: a case report.

Recently, flexible denture is widely used with some advantages such as esthetics, flexibility and biocompatibility. However, there is an opposite opinion about stability because of the movement of denture by the material's flexibility. As the denture moves to tissue surface during mastication, it irritates the supporting tissue. It can lead to trauma and rapid resorption of residual ridges if this irritation lasts for a long time. In this case, the patient has used flexible denture with insufficient stability, retention and support for several years and thus continuous irritation of the supporting tissue resulted in palate bone exposure. The patient discomfort and palate bone exposure underwent improvements by the new denture with stability and retention. A careful case selection for flexible denture, regular checkup and proper treatment are necessary to prevent the side effects.

8.Radiographic evaluation of computer aided design/computer aided manufacturing (CAD/CAM) customized abutment of implant.

PURPOSE: In this study, the retrospective radiographic study is executed to evaluate amount of bone loss of various conditions in patients using customized abutment for 4 years of follow-up. MATERIALS AND METHODS: The subjects of this study were implant fixed dental prosthesis using CAD/CAM customized abutments. CAD/CAM customized abutment and fixed dental prosthesis were manufactured by the Prosthodontics Department of Chosun University Dental Hospital from August 1, 2011 to July 31, 2012. Radiological assessments were performed on the patients who were treated by the fixed prosthodontics. After each treatment, a retrospective study was performed for a total of 4 years at 3 months, 6 months, 1 year, 2 years, 3 years, and 4 years. RESULTS: As a result of the study, the customized abutment using CAD/CAM showed less bone loss than the results of existing research. There was no statistically significant differences at alveolar bone loss between splinting group and non-splinting group (respectively 0.27 mm, 0.5 mm). Also, there were statistically significant differences at alveolar bone loss in mx. anterior, mx. posterior, mn. anterior and mn. posterior part (respectively 1.37 mm, 0.39 mm, 0.00 mm, 0.30 mm). CONCLUSION: The customized abutment using CAD/CAM showed less bone loss than the results of existing research, there were statistically significant differences at alveolar bone loss in implant positions.

In treating young women for gestational trophoblastic tumor ( G.T.T. ), all efforts are made to preserve fertility and hysterectomy is avoided unless absolutely essential. However, in some cases the operation may be lifesaving. During the last 20 years among the 310 cases of G.T.T. treated at Il Sin Christian Hospital, there were 3 cases in which hysterectomy was avoided by the excision of a localized uterine lesion thus preserving fertility.

10.Comparison of Maximal Removal Rate of Indocyanine Green and Monoethylglycinexylidide Test in Quantitative Assessment of Hepatic Function.

BACKGROUND: The maximal removal rate of indocyanine green (ICG Rmax), which has been used as a useful indicator of quantitative assessment of the hepatic function, has some disadvantages such as high cost, requirement of multiple sampling, and long turn-around time. This study was designed to clarify that the measurement of the lidocaine metabolite, monoethylglycinekylidide (MEGX) test, can replace the ICG Rmax. And in healthy adults, MEGX forma pion was measured and compared according to methods of measurement and serf. METHOD: In 18 patients to whom ICG Rmax test was requested, ICG Rmax test was carried out at two doses of 0.5 mg/kg and 5 mg/kg and MEGX formation after 15 minute of 1 mg/kg lidocaine Injection was measured by fluorescence polarization immunoassay (FPIA) method. The correlation between them was analyzed, To 25 healthy volunteers included in this study as normal control, lidocaine was given intravenously at, a dose of 1 mg/kg and MEGX forma pion was measured IS and 30 minute later (MEGX15, MEGX30) using both high performance liquid chromatography (HPLC) and FPIA methods. RESULT: Patient group resealed significant correlation between ICG Rmax and MEGX15 (r=0.7674, p<0.001) and also between ICG Rl5 and MEGX15 (r=0.5612, p=0.008). There was significant difference between MEGX15 of 9 patients with chronic liver diseases and those of normal controls (22.24+/- 13.18 and 35.40+/- 14.43 ng/mL, respectively) (p=0.01). In normal controls, the correlation between methods was significant (p=0.001) and the values measured by FPIA method was significantly higher than that by HPLG (p(0.001). Of the normal controls, male group had higher MEGX15 values than female group in both methods (in HPLC method 33.89+/-15.95 and 22.53+/- 8.36, and in FPIA method 41.48+/-16.61 and 28.81+/-7.88 ng/mL, respectively), and in female group MEGX30 values was significantly elevated compared to MEGX15 (p<0.001). CONCLUSION: Inferred from the fact that the correlation between ICG Rmax and MEGX was good, MEGX test can be considered a replacement for ICG Rmax. In healthy adults, it is considered that there is serf-related difference In the rate of lidocaine metabolism so we should pay attention to it in interpreting the MEGX results.
