2.Trends of Meta-analysis in Upper Gastrointestinal Diseases.

Systematic review and meta-analysis is a research method that tries to provide the best evidence by comprehensive searching, combining of results, and evaluating methodological quality of studies focused on the undetermined clinical question. By keeping credibility of the research process and minimizing potential bias, authors can provide precise pooled estimates. Recently, meta-analysis publications have been increasing and there is a need for critical appraisal of this flow. This review aims to provide an overview of recent trends of meta-analysis in the upper gastrointestinal diseases by quantitative and qualitative analysis.

3.Epidemiology of Helicobacter pylori Infection in Korea.

Helicobacter pylori (H. pylori) is the most common chronic bacterial infection in humans and causes many upper abdominal diseases and symptoms. Infection is more frequent and acquired at an earlier age in developing countries compared to industrialized nations. In Korea, the nation-wide seroprevalence of H. pylori infection in 1998 was higher than that of the developed countries. At the year, the seroprevalence in children (neonate~15 years) and adult (16~79 years) were 17.2% and 66.9%, respectively. However, seroprevalence in 2005 (59.6%) significantly decreased compared with that of 1998 (66.9%), and the decrease was significant in subjects aged less than 70 years, living in Seoul and Gyeonggi province (which is close to Seoul). The major route of transmission remains poorly understood. However, according to multivariate analysis, sex, age, geographic area, crowding (number of person per room) in childhood, economic status in childhood, and types of housing in childhood were significantly and independently associated with H. pylori seroprevalence. As 13 years have already passed since the nation-wide study in 1998, well designed study including infants and children is warranted to elucidate the seroprevalence, epidemiological factors, and the route of transmission of H. pylori infection in Korea.

4.Helicobacter Pylori Infection and Related Gastrointestinal Diseases.

Helicobacter pylori is among the most common bacterial pathogens, but it has not yet been conquered. It is related to various gastrointestinal diseases, including chronic gastritis, peptic ulcer, gastric marginal zone B cell lymphoma, and gastric cancer. The association between Helicobacter pylori infection and functional dyspepsia or gastroesophageal reflux disease is not clear. Numerous factors determine disease course and outcome, such as bacterial virulence, genotype, host immunity, and environmental factors. The eradication of Helicobacter pylori in associated gastrointestinal diseases is important for the treatment and prevention of recurrent or severe outcomes, especially in Korea where gastric cancer is prevalent.

9.Treatment of Barrett's Esophagus.

Barrett's esophagus is a premalignant condition, with dysplasia usually preceding the development of adenocarcinoma. Although Barrett's esophagus rarely progresses to adenocarcinoma in Asian population, the diagnosis and optimal management is a matter of debate. Current treatment guidelines include relieving gastroesophageal reflux disease symptoms with medical or surgical measures and surveillance endoscopy. However, there is no verified treatment or management strategy shown to reduce moratlity rates of adenocarcinoma arising from Barrett's esophagus. In this review, the currently applied treatment modalities (medical, surgical and endoscopic treatment) of Barrett's esophagus will be introduced.
