1.A case of virilizing adrenocortical adenoma.

This is a case report of virilizing adrenal cortical adenoma in 22 month old male patient who was diagnosed by clinical features, endocrinological studies, radiologic studies and pathologic findings. During surgery, a 40gm well-encapsulated tumor was removed from the right adrenal gland. Histologic examination revealed no capsulr invasion and a diagnosis of adrenocortical adenoma was made. He is in well condition without the clinical or laboratory evidence of recurrence at 2 years of follow up.

2.Clinical Evaluation and Extracutaneous Hyperpigmentation in Nevus of Ota.

This study was undertaken to elucidate extracutaneous hyperpigmentation of nevus of Ota by analyzing the 70 patients who visited Cryosurgery Clinic in Seoul National University Hospital between January 1984 and March 1985 This results were as follows: 1. The age of onset showed two peaks: the first one was at birth or soon after birth; 27 cases(38. 6%) and the second une was between 11 and 17 years; 37 cases (45. 7%). 2 Seventy cases were classified by Taninos method: 15.7% was type I a, 27. l% type I b, 28. 6% type Il, 20. O% type g and 8. 6% type IV 3.Fifty two out of 70 cases who were examined at the department of ophtha1rnology: pigrnentation of sclera was observed in 29 cases(55. 8%.), of conjunctiva in 18 cases(34.6%), of uveal tract in 14 cases(26. 9A), of fundus in 14 cases (26. 9%) and of optic papilla in 2 cases(3. 8%). 4 Fifty two out of 70 cases who were examined at the department of Otorhinolaryngology. pigmentation of tympanum was observed in ]0 cases(23, 8%,), of nasal mucosa in 6 cases(14. 3p,), of oral mucosa in 5 cases(11, 9%), of external auditory canal in 4 cases(9 5%) and of pharynx in 1 case(2.4%,)

3.A Case of Aplasia Cutis Congenita Group 5.

Aplasia cutis congenita is a rare disorder characterized by localized or generalized absence of skin at birth. The various hypothesis and classification are proposed. Type V in Frieden's classification, which is associated with fetus papyraceus or placental infarcts, occurs as a large cutaneous defect on the trunk and extremities. We experienced the patient had skin defect affecting the lower abdomen symmetrically, with no family history of the any disorders or chromosomal anomalies and associated anomalies. A twin fetus was died in utero. We report a case of aplasia cutis congenita associated with fetus papyraceus with the review of the associated literatures.

4.A Study on the Serum Nickel Concentration During Delivery.

To evaluate the possible functional role of serum during the process of parturition, 15 serum samples were collected and analyzed for the nickel concentrations in each 3 groups(Group 1 for the period during parturition, Group 2 for the period from delivery of fetus to delivery of placenta, and Group 3 for the period after delivery of placenta) of normal, uncomplicated full term vaginal delivery and one control group composed of healthy unmarried women in 3rd decades of age. Data revealed that average serum nickel concentration of Group 2(26.6 microgram/l) and Group 1(22.2 microgram/l) were significantly higher than that of Control group(13.7 microgram/l), but Group 3's(13.8 microgram/l) was almost same as Control group's. There were significant negative correlation between age and serum nickel concentration in group 2, and a tendency of higher nickel concentration in women who have no previous experiences of pregnancy than who have previous experiences of pregnancy, although it was not significant. These result could be indicative of close causal relationship between serum nickel concentration and the entire process of parturition.

5.Isokinetic Muscle Strength and Clinical Evaluation of the Knee Following Arthroscopic Menisectomy.

PURPOSE: Postoperative strength of knee flexor and extensor, and functional evaluation after arthroscopic menisectomy of the knee was evaluated objectively at the time of returning to daily life. MATERIALS AND METHODS: The 31 knees (29 patients) had meniscal injuries without associated ligament injury. The muscle strength was tested with Cybex 340 isokinetic dynamometer preoperatively and at 2, 4, 6, 10 weeks postoperatively. Clinical evaluation by Lysholm scoring system was also done simultaneously. The statistical analysis was evaluated by repeated measures of ANOVA. RESULTS: Muscle strength of extensor (quadriceps) was abruptly decreased to about 45% torque of the preoperative level at postop 2 weeks, but slowly increased to about 90% torque at 10 weeks in 90 degree/sec and 180 degree/sec exercise. The flexor strength was also a little bit decreased at post-op 2 weeks, slowly increased to about 120%-150% torque of preoperative level in 90 degree/sec and 180 degree/sec exercise at post-op 10 weeks. The clinical evaluation by Lysholm scale showed 45 points at postop 2 weeks and slowly improved to 80 points at post-op 10 weeks. CONCLUSIONS: Muscle weakness of knee extensor and flexor remained in isokinetic test at post-op 4-6 weeks. The recovery rate for weakness of hamstrings was faster than quadriceps. The rehabilitation of quadriceps should be accentuated in the prevention of knee injury due to muscle weakness.

6.Deep Neck Abscesses in Children and Adolescents: 10 Year Experience in Two General Hospitals.

PURPOSE: The purpose of this study was to review our recent experiences with deep neck abscesses in children and adolescents and to provide helpful information in treatment and diagnosis by comparing them with those in other available literatures. METHODS: Medical records of 36 children and adolescents admitted for deep neck abscess at two hospitals from January 2000 to October 2010 were reviewed retrospectively. RESULTS: Male to female ratio was 1.4 : 1 and the mean age was 6.5 years. Painful neck swelling and fever were the most frequent symptoms in patients under thirteen years of age whereas trismus and headache were frequent symptoms in patients over fourteen years of age. Submandibular space was the most common site of deep neck abscess in patients under thirteen years of age, whereas peritonsillar space was the most common site in patients over fourteen years of age. The results of bacterial cultures were positive in 61.5% of drained cases. Staphylococcus aureus was the most commonly identified bacteria in 6 patients (37.5%) and 5 of them were under 2 years of age. Twenty six patients received surgical drainage while the others were treated with antibiotics only. There were no statistically significant differences in the durations of admission, fever after admission, and antibiotic treatment between surgical and medical treatment groups. CONCLUSION: The common sites of deep neck abscess, associated symptoms, and causative organisms were different between children and adolescents. As there were no differences in durations of admission, fever, or antibiotics treatments between surgical and medical treatment groups, surgical drainage may be avoided by early recognition and suspicion. However, if there is no improvement of symptoms or size of abscesses within 48-72 hours of antibiotic treatment, surgical drainage should be considered.

7.Clinical Research on Surgical Treatment for Double-Head Pterygium.

PURPOSE: To evaluate two surgical methods-amniotic membrane transplantation (AMT) and split-conjunctival grafts (SCG)-for double-head pterygium, with regard to the postoperative outcome and recurrence rate. METHODS: In a total of 16 eyes (14 patients), 7 eyes (6 patients) receiving amniotic membrane transplantation and 9 eyes (8 patients) receiving split-conjunctival grafts were compared to evaluate recurrence and complications. RESULTS: Within the amniotic membrane transplantation group, two eyes (29%) had corneal recurrence, and 3 eyes (43%) had conjunctival recurrence. The mean follow-up period was 21.9+/-3.5 months, and all recurrences were on the nasal side. The average period preceding the corneal recurrences was 7.2+/-1.8 months. Within the split-conjunctival grafts group, the mean follow-up was 13.6+/-2.1 months, and neither the corneal nor conjunctival recurrences were observed. In addition, the eyes of this group were more aesthetically stable, with only one eye exhibiting pseudo-pterygium at the donor site. CONCLUSIONS: In cases of double-head pterygium without contraindication of conjunctival autograft, the split-conjunctival grafts produced fewer recurrences and showed enhanced cosmetic results, as compared to the amniotic membrane transplantation, indicating that the split-conjunctival grafts is the superior choice over amniotic membrane transplantation.

9.Clinical Study of Neonatal Adrenal Lemorrhage.

PURPOSE: Neonatal adrenal hemorrhage is not rare disease which can be caused by such risk factors as sepsis, large baby, birth trauma and asphyxia. The clinical manifestations include jaundice, anemia, abdominal mass and differentiation from neuroblastoma, renal vein thrombosis and adrenal abscess is needed. Through the clinical assessment of presenting features, we hope that this study be of any help to early detection and proper management of neonatal adrenal hemorrhage. METHODS: The 16 subjects out of neonates admitted to our hospital from July 1991 to June 1997 were diagnosed as neonatal adrenal hemorrhage. The risk factors, clinical manifestations, diagnostic methods and prognosis of neonatal adrenal hemorrhage were evaluated in these 16 cases. RESULTS: 1) Among 16 cases, males were 10 (62.5%) and females were 6 (37.5%). Mean birth weight was 3.601.08kg and mean gestational age was 39.82+1.08 week, and all were fullterm babies. In modes of delivery, vaginal deliveries were 12 cases (75.0%) and cesarean sections were 4 cases (25.0%). 2) 13 cases (81.2%) were involved in right side, 2 cases (12.5%) in left side and 1 case (6.3%) bilaterally. 3) The risk factors include sepsis in 6 cases (37.5%), large baby in 5 cases (31.3%), birth trauma in 5 cases<31.3%) and asphyxia in 3 cases (18.8%). 4) The clinical manifestations include jaundice in 7 cases (43.8%), anemia in 7 cases (43.8%), fever in 6 cases (37.5%) and abdominal mass in 3 cases (18.8%). 5) In the time of diagnosis, until 7 days of birth were 9 cases (56.2%), 8-14 days were 3 cases (18.8%), 15-21 days were 2 cases (12.5%) and 22-28 days were 2 cases (12.5%). Follow-up studies were done in 13 cases (81.2%), and tha lesions all decreased without any specific complications or sequelae. CONCLUSION: Neonatal adrenal hemorrhage can be diagnosed by abdominal ultrasono- gram in the presence of suggestive manifestations of jaundice, anemia, fever and abdominal mass, with relatively good prognosis. Differentiation from other conditions as well as avoidance of unnecessary explorations can be achieved by serial follow-up examinations of abdominal ultrasonogram.

10.Effects of a Multicultural Course on the Multicultural Acceptability and Competency of Nursing Students.

PURPOSE: This study was conducted to examine the effects of a multicultural course on nursing students' multicultural acceptability and competency. METHODS: The research design for this study was a non-equivalent control group, non-synchronized pre- and post-test. The experimental group (n=28) took 13 weeks of the multicultural course, while the control group (n=28) received only regular classes over the same period. Pre- and post-tests were done to identify the effects of the program. The data were collected using self-administered questionnaires and analyzed using the PASW 18.0 program. RESULTS: All indices of multicultural acceptability and competency in the experimental group were significantly higher than that of control group after the program, except for two: universality (a measure of multicultural acceptability) and multicultural awareness (a measure of multicultural competency). CONCLUSION: These results suggest that the multicultural course was effective in elevating the level of multicultural acceptability and competency among nursing students.
