1.Clinical Evaluation and Extracutaneous Hyperpigmentation in Nevus of Ota.

This study was undertaken to elucidate extracutaneous hyperpigmentation of nevus of Ota by analyzing the 70 patients who visited Cryosurgery Clinic in Seoul National University Hospital between January 1984 and March 1985 This results were as follows: 1. The age of onset showed two peaks: the first one was at birth or soon after birth; 27 cases(38. 6%) and the second une was between 11 and 17 years; 37 cases (45. 7%). 2 Seventy cases were classified by Taninos method: 15.7% was type I a, 27. l% type I b, 28. 6% type Il, 20. O% type g and 8. 6% type IV 3.Fifty two out of 70 cases who were examined at the department of ophtha1rnology: pigrnentation of sclera was observed in 29 cases(55. 8%.), of conjunctiva in 18 cases(34.6%), of uveal tract in 14 cases(26. 9A), of fundus in 14 cases (26. 9%) and of optic papilla in 2 cases(3. 8%). 4 Fifty two out of 70 cases who were examined at the department of Otorhinolaryngology. pigmentation of tympanum was observed in ]0 cases(23, 8%,), of nasal mucosa in 6 cases(14. 3p,), of oral mucosa in 5 cases(11, 9%), of external auditory canal in 4 cases(9 5%) and of pharynx in 1 case(2.4%,)

2.A Case of Targetoid Hemosiderotic Hemangioma.

Targetoid hemosiderotic hemangioma is a benign vascular tumor with a peculiar clinical presentation and histologic features sometimes difficult to differentiate from the patch stage of Kaposi's sarcoma, progressive lymphangioma, eosinophilic hemangioma, retiform hemangioendothelioma. We report a case of targetoid hemosiderotic hemangioma on upper back and right upper arm of a 28-year-old male. He has a 5mm × 5mm sized slightly raised violaceous papule surrounded by an ecchymotic halo on upper back and right upper arm. We took a biopsy of the lesion and identified it as targetoid hemosiderotic hemangioma. This is a case of targetoid hemosiderotic hemangioma with typical clinical and histopathological findings.

3.Isokinetic Muscle Strength and Clinical Evaluation of the Knee Following Arthroscopic Menisectomy.

PURPOSE: Postoperative strength of knee flexor and extensor, and functional evaluation after arthroscopic menisectomy of the knee was evaluated objectively at the time of returning to daily life. MATERIALS AND METHODS: The 31 knees (29 patients) had meniscal injuries without associated ligament injury. The muscle strength was tested with Cybex 340 isokinetic dynamometer preoperatively and at 2, 4, 6, 10 weeks postoperatively. Clinical evaluation by Lysholm scoring system was also done simultaneously. The statistical analysis was evaluated by repeated measures of ANOVA. RESULTS: Muscle strength of extensor (quadriceps) was abruptly decreased to about 45% torque of the preoperative level at postop 2 weeks, but slowly increased to about 90% torque at 10 weeks in 90 degree/sec and 180 degree/sec exercise. The flexor strength was also a little bit decreased at post-op 2 weeks, slowly increased to about 120%-150% torque of preoperative level in 90 degree/sec and 180 degree/sec exercise at post-op 10 weeks. The clinical evaluation by Lysholm scale showed 45 points at postop 2 weeks and slowly improved to 80 points at post-op 10 weeks. CONCLUSIONS: Muscle weakness of knee extensor and flexor remained in isokinetic test at post-op 4-6 weeks. The recovery rate for weakness of hamstrings was faster than quadriceps. The rehabilitation of quadriceps should be accentuated in the prevention of knee injury due to muscle weakness.

4.Expression of Angiogeneis-related Factors in Retinal Pigment Epithelial Cells under Hypoxia.

PURPOSE: To elucidate the mechanism of neoangiogenesis of human retinal pigment epithelium (HRPE) under hypoxia. METHODS: HRPE cells were cultured for 2 and 24 hours in a hypoxic chamber. Expression and production of the angiogenic factor, vascular endothelial growth factor (VEGF) and the anti-angiogenic factor, pigment epithelium-derived factor (PEDF) were analyzed by RT-PCR and Western blot, respectively. Neoangiogenesis was induced by adding culture supernatant harvested from cells exposed to hypoxic conditions. Neoangeogenesis was measured with a tube formation assay that uses ECV 304 cells and with a migration assay that uses human dermal microvascular endothelial cells. RESULTS: Competitive RT-PCR showed that the expression of the PEDF gene in HRPE cells under hypoxic state decreased compared to normoxic state (p<0.01) but the expression of the VEGF gene increased (p<0.01) when exposed to hypoxic conditions. These results corresponded to those of the Western blot analysis which revealed a significant increase of VEGF production (p<0.01) and a decrease of PEDF production (p<0.01). Moreover, the tube formation and migration assays demonstrated that angiogenesis was increased by exposure to hypoxic stress. Taken together, HRPE cells under hypoxic stress produce more VEGF and less PEDF, which lead to neoangiogenesis. CONCLUSIONS: These results suggest that the subretinal neovascularization that occurs under hypoxic stress might be caused by an imbalance of angiogenesis-related factors in HRPE cells.

6.A Case of Meningioma Compatible with Metastatic Glioblastoma Multiforme.

We report a case of 54-year-old female with two intracranial tumors, a meningioma and multifocal glioblastoma multiforme with extracranial metastasis. The simultaneous occurrence of meningioma and glioblastoma multiforme and extracranial metastasis of glioblastoma multiforme are rare. We demonstrated additional clinical and pathological features of interest and reviewed the relevant literatures, and have speculated on the causes of this phenomenon.

7.Intrauterine Growth Curve of Twins on the Basis of Gestational Age.

OBJECTIVE: In order to investigate more accurate patterns of intrauterine growth in twin fetuses, we constructed the twin-specific growth curve based on gestational age in live-born non-anomalous infants. METHODS: 563 twin pairs, 1,126 infants delivered between January 1994 and October 1999 were comprised our study population. The newborns were grouped according to gestational age and the median, 10th, 50th, and 90th percentile birth weight for each gestational week were calculated. Curves were plotted for the general population and for each gender and parity and compared with previously reported singleton's growth curve. RESULTS: After 32 weeks gestation, birth weight of twins falls below that of singleton, so that by 38 weeks the 50th percentile for twins falls below the singleton 10th percentile. This difference was also present among all subgroups of twins, such as gender and parity. And the mean birth weight in male twin infant was heavier than in female twin infant throughout all gestational week. CONCLUSION: The pattern of growth in twin infants differed from those of singleton. We recommend twin-specific growth curve for clinical use in the management of twin gestations.

8.Results of Intramedullary Nailing of Femoral Shaft Fracture: Trochanteric Entry Portal (Sirus Nail) versus Piriformis Entry Portal (M/DN Nail).

PURPOSE: To compare treatment results obtained using the trochanteric (Sirus nail) entry portal with those obtained using the Piriformis fossa (M/DN) entry portal during intramedullary (IM) nailing of femur shaft fractures. MATERIALS AND METHODS: Four hundreds and thirty-two patients treated for femur shaft fracture using IM nails from February, 2001 to May, 2010 were divided into two groups. group 1 was composed of 180 patients treated through the trochanteric (Sirus nail; n=180) entry portal, while group 2 contained 170 patients treated through the piriformis fossa (M/DN nail; n=170) entry portal. We compared the clinical and radiographic findings of both groups to evaluate the treatment results. RESULTS: Functional result, range of motion and union time (18, 20 weeks) were similar in both groups. The operation time of patients in the over-weighted group was 90 minutes in group 1 and 120 minutes in group 2 (p<0.05). Additionally, the blood loss was 280 ml in group 1 and 335 ml in group 2, and in case of over-weight patients, group 2 showed more blood loss (p<0.05). The duration of exposure to fluoroscopy differed slightly, with group 1 being less exposed than group 2; however, this difference was not significant (p>0.05). There were 18 iatrogenic fractures in group 1 and 4 in group 2 (p<0.05). CONCLUSION: There was not much difference in complications based on clinical and radiographic findings of both groups. For groups using the trochanteric entry portal, the operation time was shorter and blood loss was lower than in groups using the piriformis entry portal. Iatrogenic fracture occurred more often in the group using the trochanteric entry portal than in the group using the piriformis entry portal.
