1.THERAPEUTIC MANNERS AND TRENDS OF CERVICAL ESOPHAGEAL CARCINOMA

One hundred seventy of patients with cervical esophageal carcinoma (30 cases were treated by pre-operative radiation and surgery (combined group), 27 cases by surgery alone, 113 cases by radical radiation). The doses of the preoperative radiation and the radiacal radiation were D T40 Gy/4w and above D T 50 Gy/5w, respectively. The 5 year survival rates were 40% in the combined group, 15.3% in surgry alone, and 28% in radical radiation groups. The difference of survival rates in the combined group and surgery alone was significantly different statistically. Factors affecting the survivals were lymphnode metastasis in radical radiation and recurrent larygeal nerve paralysis in the combined group. The main causes of failure were local residual and recurrence (71%) in radical radiation and lymphnode metastasis (81%) in the combined group. The authors believe that pre-operative radiation combined with surgery could improve the survival of cervical esophageal carcinoma. Radical radiation is also one of effective treatment methods for cercical esophageal carcinoma.
2.Fractionated stereotatic radiotherapy for 50 patients with recurrent and residual nasopharyngeal carcinoma

Objective To evaluate the value of fractionated stereotatic radiation therapy (FSRT)(X knife) as a boost for recurrent or residual nasopharyngeal carcinoma (NPC) at the primary focus. Methods From Sept.20, 1995 to Dec.30,1998, 50 such patients were treated by FSRT with 6 MV photons. The total FSRT dose was 24~30?Gy prescribed at 1~4 centers on the 60%~90% isodose curves normalized to isocenter by multifractions of fraction size 6~8?Gy, 12,15?Gy, with interfraction duration of 4~6 days. Results CR 38/50 (76%), PR 9/50 (18%) and in evaluated 3/50 (6%). Total survivals(Kaplan meier):1 yr 83.6%, 2 yr 65.0% and 3 yr 59.6% . Conclusion Fractionated stereotatic radiotherapy (FSRT) is strongly indicated for recurrent or residual NPC at the primary focus.
3.Effect of Juebi Capsule on Experimental Prostatic Hyperplasia

Objective: To observe the effect of Juebi capsule (JBC) on experimental prostatic hyperplasia. Methods: Experimental prostatic hyperplasia was induced by castration testicular nourmone in the mice and rats respectively and the isolated bladder trigone muscle of the rabbit was used for contraction inhibition. Results: For mice, JBC 10 20g/kg was administered intragastrically for 10 days; for rats, JBC 1 2g/kg was taken for 3 weeks. The results showed that JBC can significantly inhibite prostatic hyperplasia and decrease wet weight of the gland, DNA content and the activity of serum acid phosphatase. Otherwise, 0.25%, 0.50% and 0.75% water suspension of JBC also can inhibite the contraction induced by norepinephrine in isolated bladder trigone muscle of the rabbit. The inhibition rates were 29.17%, 41.72% and 62.29%.Conclusion: JBC can significantly inhibit experimental prostatic hyperplasia and decrease contraction of bladder trigone muscle induced by ? adrenoceptor agonist.
4.Effects of supported employment on vocational rehabilitation for schizophrenia patients:a randomized controlled study

Objective To investigate the effects of supported employment(SE) on the vocational rehabilitation for schizophrenic rehabilitants.Methods 108 schizophrenic rehabilitants with stable conditions were recruited and randomly allocated into the control group and experimental group.Fifty-four cases in the control group received the general outpatient service and follow-up service with drug maintenance therapy and 54 cases in the experimental group received SE with drug maintenance therapy.At the baseline and the end of the sixth month,all of the 108 cases were assessed by the Positive and Negative Syndrome Scale (PANSS) and Work-related Social Skills Scale (WSSS) respectively.The competitive employment rate and average working days of the two groups were evaluated at the end of the sixth month.Results There was no significant difference in the total score of WSSS,and the total score of PANSS and its sub-scores on the Positive Scale,Negative Scale and General Psychopathological Scale betweeu the two groups at the baseline(P> 0.05).At the cnd of the sixth months,significant differences (P<0.01 ~ 0.05) were found in the total score of WSSS,the sub-score of the Negative Scale,the competitive employment rate and the average working days((35.11 ± 12.71) d vs(20.15 ± 8.04) d) between the experimental and control groups.Conclusion SE can increase the employment rate of schizophrenic rehabilitants,improve their abilities to acquire and maintain competitive employment,and meanwhile relieve their negative symptoms.
5.Clinical manifestation and pathological characteristics of intraspinal extramedullary peripheral primitive neuroectodermal tumour

Objective To observe the clinical manifestation and pathological immunohistochemical features of intraspinal extramedullary peripheral primitive neuroeetodermal tumour (PNET). Methods From Apr. 1996 to Aug. 2007, totally 5 patients with intraspinal extramedullary peripheral PNET were treated, including 3 male and 2 female, aging from 11 to 48 years. The primary site was cervical vertebral in 2 (C5-C7 and C5-T1 ) and thoracic vertebral in 4( T4-T6, T6-T8 ,T4-T5 and C5-T1 ). The clinical manifestation was paresis of limbs in 5 and incontinence in 4. All the patients were pathologically diagnosed Results All of the three patients who had CD99 staining expressed CD99 ( + ). All the 5 patients received surgery followed by radiotherapy and 4 received postoperative chemotherapy. The most common treatment failure was local recurrence. The overall survival time of the 5 patients was 4.8year,2.5year,2. 1ycar, 1.6year and 0.8 year respectively. Conclusions Intraspinal extramedullary peripheral PNET is highly malignant tumor.Patients have very poor survival and local control New treatment protocol should be studied to improve the local disease control and increase remission rate. To distinguish central PNET from peripheral PNET has crucial significance since different entities have different treatment protocol and prognosis.
6.Neuroprotection of progesterone on retinal ganglion cells in chronic ocular hypertension model rats

Objective To observe the protective effect of progesterone(PROG) on rat retinal ganglion cells(RGCs) in chronic ocular hypertention models.Methods The chronic ocular hypertention rat model was made by cauterizating three episcleral veins.Rats were divided into control group,high level PROG group,middle level PROG group,low level PROG group according to different concentrations of PROG injected intraperitoneally.The left eye was model eye and the right eye was control eye.Three months later,the animals were executed and the eyeballs were enucleated.The RGCs were detected by HE staining and Thy-1.1 immunohistological staining.The apoptosis of RGCs was detected by TdT-dUDP terminal nick end-labeling(TUNEL) technique.Results The number of RGCs of model eyes in high level PROG injection group was more than those in control group,low level and middle level groups(P
7.Short and long term results of percutaneous transhepatic cholangioscopic lithotomy in the treatment of intrahepatic duct stones

Objective To explore the short and long term curative effects of percutaneous transhepatic cholangioscopic lithotomy (PTCSL) in the treatment of intrahepatic stone (IHS).Methods 38 IHS patients were enrolled,who were treated with PTCSL between January 2008 and July 2013.Results PTCSL was successfully completed in all the 38 IHS cases.Stone clearance rate was 84.2% and the average episode of stone removal was (2.6 ± 0.9) times.Average diameter of percutaneous transhepatic fistula was (18.4 ± 0.6) F and the average time from percutaneous transhepatic puncture and fistulization to cholangioscopic lithotomy was (7.2 ± 0.7)d.The average operation time was (68 ± 20) min,intraoperative blood loss was (20 ± 13) ml,and hospitalization was (4 ± 2) days.The hepatolith recurrence rate in patients with stones completely removed was 37.5% (12/32),and 1 case developed into biliary cirrhosis.Patients with calculi residual suffered from higher hepatolith recurrence rate of 83.8% (5/6),with biliary cirrhosis found in 1 case.Conclusions PTCSL is safe and effective in treating primary IHS,which is indicated in multiple recurrent IHS especially in after biliary surgery patients.It has the advantages of minimally invasion,less bleeding,less postoperative pain,less complications,and fast postoperative recovery.
8.Thanssphenoidal mircrosurgery for the elderly patients aged over 70 years with pituitary adenoma

Objective To summarize the clinical features and transsphenoidal microsurgical experiences of pituitary adenomas in the elderly patients over 70 years old Methods Twenty four elderly patients accepted transsphenoidal microsurgery were conductded a retrospective review,whose clinical data including clinical main presentation,histological types,surgical conditions,perioperative treatment and outcome were also analyzed Results There were 16 non functional adenomas (66 7%) and 18 giant macroadenomas(75%) respectively Visual deterioration was the commonest complain in 20 (83 3%) Despite the majority of patients having coexisting medical conditions,we had no mortality and no postoperative adjunctive morbidity with an average hospital stay of 17 8 days For an average follow up of 3 5 years,we did observe major patients(79 2%) vision had improved Conclusions Pituitary adenomas in the elderly are most frequently non functional macroadenomas that present with visual deterioration and hypopituitarism If special care is devoted to the perioperative treatment,in particular to fluid and electrolyte balances,transsphenoidal microsurgery is safe and effective in hands of an experienced team
9.Effects of Monocrotaline on Right Ventricular Function and Expression of Cardiac Canonical Transient Receptor Potential Channels Subfamily in Experimental Rats

Objective: To explore the effects of monocrotaline (MCT) on right ventricular function and expression of cardiac canonical transient receptor potential channels (TRPC) subfamily in experimental rats.
Methods: The SD male rats were randomly divided into 2 groups:Control group, the rats were normally fed and MCT group, the rats received a single dose injection of MCT 60 mg/kg to induce myocardial hypertrophy. n=10 in each group and all animals were treated for 3 weeks. The right ventricular hemodynamics parameters and right ventricular hypertrophy index (RVHI) were measured, right ventricular myocardium tissue section was observed by HE staining, the mRNA and protein expressions of TRPC subfamily were examined by RT-PCR and Western blot analysis.
Results: Compared with Control group, MCT group had increased RVSP, RVHI, RV+dp/dtmax and decreased RV-dp/dtmax, all P<0.01. In MCT group, the right ventricular myocardial cells had the thinker ifber, deeply stained nuclei with irregular shape. Compared with Control group, the mRNA and protein expressions of right ventricular TRPC6 were elevated in MCT group, n=6 and n=4 respectively, all P<0.05.
Conclusion:Right ventricular hypertrophy could be induced by 3 weeks MCT treatment, it up-regulating the mRNA and protein expressions of TRPC6 which might be involved in the occurrence and development of cardiac hypertrophy in experimental rats.
10.Necessity of replanning for the change of anatomy and dosimetry during intensity modulated radiotherapy for nasopharyngeal carcinoma

Objective To study the dynamic change of anatomy and dosimetry distribution and its influence during intensity modulated radiation therapy(IMRT) for nasopharyngeal carcinoma(NPC). Methods From June 2006 to August 2007,12 patients with stage Ⅲ-Ⅳ NPC receiving initial IMRT concurrently combined with chemotherapy were included in the study. The target volumes and,involving organs were contoured on the first set of spiral CT images. When IMRT planning was finished,a second CT scans was acquired to rectify the treatment center. Weekly spiral CT images acquired during the treatment period according to the same treatment center were fused with the first CT images. In order to determine the best time of replanning, modified plan target volume( PTV1 ) and normal organs( parotids and outline) were contoured on the fusion interface by a single physician. Changes of each structure throughout treatment period were measured by a system software. Then the dose distributions were computed and evaluated for replanning CT using the same beams arrangement in the original plan. Cumulative dose was estimated compared with the original plan. Results The volume of outlines and parotids decreased gradually, and the change came to peak in the fifth week. So the fifth and first week CT scans were selected as research objectives. No significant changes in maximal and mean dose was observed in the brainstem, spinal cord or mandible. Despite volume changes,D99 and D95 of the PTV1 did not change siguificantly(P>0.05). D95 of the bilateral parotids increased significantly ( PL = 0.03,PR = 0.01 ). Conclusions During IMRT for NPC, the volume of PTV1, the outlines and parotids decrease,and the change come to peak in the fifth week. Comparing to the first treatment plan, the dose parameters of the parotids increase significantly in the second plan based on the fifth week CT,but those of the brainstem,cord,mandible and PTV1 change slightly.
