Objective To investigate the differences of platelet membrane glycoprotein PAC-1 and CD62P expression in patients with cerebral small vessel disease and its subtype and large artery atherosclerotic stroke and to compare the traditional platelet activation markers and inflammatory chemokine platelet factor 4(PF4).Methods Peripheral blood platelet PAC-1,CD62P positive rates and serum PF4 concentration in 30 patients with large artery atherosclerotic stroke,45 patients with cerebral small vessel disease and 30 controls were detected using flow cytometry and enzyme-linked immunosorbent assay.The differences between the groups were compared.Results The PAC-1,CD62P positive rates and serum PF4 concentration in the large artery atherosclerotic stroke group were 63.21% ± 9.78%,55.91% ± 8.17%,and 30.55 ± 15.56 ng/ml,respectively.They were significantly higher than 40.65% ± 17.42%,36.49% ± 14.60%,and 12.59 ±5.57 ng/mlin the cerebral small vessel disease group(all P=0.000),and the latter was still higher than 13.55% ± 3.14%,9.00% ± 2.32%,and 4.95 ±2.82 ng/ml in the control group(all P = 0.000).There were significant differences in all the subtypes groups of cerebral small vessel disease between the PAC-1,CD62P positive rates and serum PF4 concentration.The leukoaraiosis with lacunar infarction group(n = 15;47.72% ±15.52%,43.75% ± 12.54%,and 13.96 ± 5.23 ng/ml)and the simple lacunar infarction group (n = 15;49.87% ± 14.65%,43.98% ± 10.55%,and 14.41 ± 5.53 ng/ml)was significantly higher than the simple lacmar infarction group(n = 15;24.44% ± 8.45%,21.74% ± 7.19%,and 9.40 ±4.99 ng/ml)(P =0.000,0.000,and 0.013,respectively).There was no significant difference between the forrner 2 groups(P = 0.658,0.952,and 0.858,respectively).The peripheral blood platelet PAC-1 positive rate had significant correlation with CD62P positive rate in patients with ischemic cerebral disease(r= 0.964,P= 0.000),and the serum PF4 concentration showed linier correlation with the PAC-1(r =0.846,P =0.000)and CD62P(r =0.857,P =0.000)positive rates.Conelusions The platelet membrane glycoprotein PAC-1 and CD62P expression showed linear correlation,and they were consistent with the changes of PF4 concentration.This sugested that platelet activation and its mediated inflammatory mechanisms played an important pathophysiological role in the processes of atherosclerosis and thrombosis.This mechanism had significant difference between the different lesion types.

Objective To study the anti-asthma mechanism of water extract of Portulaca oleracea L( WEPO) related toβand M receptors.Methods Rabbits of 2.0 -2.5 kg were selected,whose isolated tracheal rings were prepared.The effect of WEPO on tracheal smooth muscle tension was evaluated using tracheal smooth muscle contraction frequency and contraction amplitude as indexes.Aminophylline( Ami) was used as a positive control medicine and propranolol( Pro) as theβreceptor blocking agent to observe the relaxation effect on tracheal smooth muscle of WEPO related to βreceptors. Acetylcholine chloride( AChCl) was used as the agonist for M receptors and atronine( Atr) as the M receptor blocking agent to observe the relaxation effect on tracheal smooth muscle of WEPO related to M receptors.Results ①The contraction frequency and amplitude of isolated tracheal smooth muscle in each dose group of WEPO were significantly lower after treatment.The difference was statistically significant(P <0.05,P <0.01).②The baseline,contraction frequency and amplitude of isolated tracheal smooth in Pro +WEPO group were higher than in WEPO group and the difference was statistically significant (P<0.01).③The baseline, contraction frequency and amplitude of isolated tracheal smooth muscle in AChCl group were significantly higher after treatment(P<0.01)significantly.④The baseline, contraction frequency and amplitude in WEPO +AChCl group were not different from those in AChCl group ( P >0.05 ) .⑤These indexes in Atr+WEPO group were not different those in WEPO group ( P >0.05 ) .Conclusion WEPO has relaxation effect on isolated tracheal smooth muscle of rabbits,which is related toβreceptors rather than M receptors.

Objective To analyze the differences of nutritional parameters and body composition in the protein energy wasting (PEW) patients on maintenance hemodialysis with or without diabetic kidney disease (DKD).Methods The patients with PEW were screened by subjective global assessment (SGA) from patients on maintenance hemodialysis from March 2014 to June 2015 in the Fourth Central Hospital of Tianjin.Anthropometric and nutritional parameters were collected before dialysis, and body composition and energy expenditure were examined by the bioelectrica] impedance method.The indexes of the two groups were compared.Results A total of 301 patients on maintenance hemodialysis (DKD 112, non-DKD 189), whose average age was (63.05±14.42) years old, were screened.Among which, 75 patients were diagnosed with PEW, and the overall prevalence rate was 24.92％, including 41 DKD cases with the prevalence of 36.61％ (41/112) and 34 non-DKD cases with the prevalence of 17.99％ (34/189).Compared with uon-PEW patients, the body mass index (BMI), hemoglobin (Hb), albumin (ALB), and total cholestero (TC) of patients with PEW were significantly lower, and C-reactive protein (CRP) were significantly higher (P＜0.05).In the DKD group, the average age of PEW onset ((59.61±13.74) years old), the maintenance dialysis time before PEW ((64± 12.50) months), ALB ((31.34±3.32) g/L), and TC ((4.17±0.95) mol/L) were all lower than those in the non-DKD group ((70.68±14.05) years old, (116.61 ±32.85) months, (32.34±4.64) g/L, (4.95±1.52) mol/L) (P＜ 0.05), respectively.The body composition results showed that muscle mass in the DKD group ((24.84±3.36) kg/ (45.32±6.95)％) was less than that of the non-DKD group ((28.53±3.75) kg/(50.67±7.28)％) (P＜0.05).Conclusions This study indicates that compared with non-DKD dialysis patients, the prevalence of PEW in dialysis patients with DKD is higher, onset age of PEW is younger, the duration of dialysis is shorter, and the muscle consumption is more obvious.

Object Isolation and structural determination of the chemical constituents from the 95% ethanol extract of the pericarp of Sphaerophysa salsula (Pall.) DC. Methods To isolate chemical constituents, solvent extraction together with column chromatography was used. Physico-chemical characters and spectroscopic analysis were employed for structural identification. Results Four compounds were identified as 5?-stigmast-3-one (Ⅰ), 5?-stigmast-3, 6-dione (Ⅱ), stigmast-4-en-3-one (Ⅲ), 3?, 6?-stigmast-4-en-3, 6-diol (Ⅳ). Conclusion All these compounds were first isolated from the plants of Sphaerophysa DC.

Objective To investigate the short-term effects of glucocorticoid therapy on patients with idiopathic interstitial pneumonia using high resolution CT semi quantitative scores. Methods According to the different HRCT scores will be January 2015 -2017 year in January in our hospital for diagnosis and treatment of idiopathic interstitial pneumonia patients with 60 cases of groups: control group with HRCT score < 20%, the observation group with HRCT score more than 20%; the patients in the two groups of experimental data were recorded in detail and analyzed and discussed the high resolution CT semi quantitative score judgment of idiopathic interstitial pneumonia patients treated with glucocorticoid efficacy. Results The observation group (HRCT score≥20%) accuracy is higher than that of the control group (HRCT score<20%), the difference between groups was statistically significant (P<0.05). Conclusion The treatment of idiopathic interstitial pneumonia in patients with glucocorticoid selection of high resolution CT;semi quantitative score to determine significant short-term effect, HRCT score of the best critical value was 20%, the diagnostic accordance rate can obtain excellent.

BACKGROUND: There are no effective treatments for spinal cord injury. Transplantation of olfactory ensheathing cells (OECs) has achieved great progress in repairing spinal cord injury. OBJECTIVE: To observe the effect of OECs transplantation on pathological and ultrastructural alterations of spinal cord, and the role in spinal cord injury developing.METHODS: A total of 60 SD rats were randomly divided into blank, model, transplantation and DF12 groups, with 15 animals in each group. The entire vertebral plate of T_(10), and partial vertebral plate of T_9 and T_(11) of blank group were cut open, and gelatin sponge was used for hemostasis. In the model group, the spinal cord was excised. In the transplantation and DF12 groups, OECs and DF12 culture solution were injected following spinal cord excision. The incision was sutured. Two rats from each group were anesthetized 1, 3, 7, 14, 28, 42, and 56 days following injury, and injured areas were observed by light microscopy and electron microscopy. RESULTS AND CONCLUSION: Following spinal cord injury, pathological and ultrastructural changes occurred, such as hemorrhage, edema, degeneration, necrosis, cavitation, gliacyte proliferation and nerve fiber regeneration. OECs transplantation attenuated neuronal and nerve fiber necrosis, relieved degree of pathological reaction, protected injured neurons, prevented gliacyte proliferation and increased nerve fiber regeneration. Results show that OECs transplantation ameliorated pathological reactions and promoted spinal cord injury repair.

Objective To find a novel operative modality with sphincter preservation in the treatment of middle and low rectal carcinoma. Methods Pull through lower resection was performed on 28 rectal cancer patients. The distance between the anal verge and the lower margin of the tumor was 6～8cm(20 patients) or 8～10cm(8 patients), including 8 patients in Dukes A stage, 16 Dukes B and 4 Dukes C. The resected line from tumor distal margin was 2cm, 3cm, and 4cm, respectively. Results There was no operative death, anastomotic fistula or anastomotic stenosis in these cases. Mean follow up period was 30 months. Local recurrence was found in two cases (7.1%) 18 months after the operation, and 26 cases were cancer free till the end of the follow up. Defecation was satisfactorily controlled 8～12 weeks after the operation. Conclusions Pull through Welch procedure could meet the criterion of the radical resection of rectal carcinoma,and keep the internal and external sphincter muscles intact in the superior lower anterior resection. The normal defecationcan can maintain after the operation due to the preservation of internal and external sphincter muscles.
