Objective To investigate the prevalence of thyroid dysfunctions in the general population for routine physical examination in Taiyuan area. Methods Blood samples from 8125 individuals in Taiyuan were assayed for thyrotropin, free T 3, free T 4 and antithyroid antibodies. Results The prevalence of overt hyperthyroidism was 1.20%, that of subclinical hyperthyroidism was 0.87%. Objective To understand the status of mycoplasma infection and drug susceptibility to provide reference basis for clinical rational drug use.Methods The reagents kits produced by Shanghai Aopu Biopharmaceutical Co.,Ltd.were adopted to per-form the mycoplasma culture,identification and susceptibility test.Results Among 1 745 specimens,896 cases of mycoplasma in-fection were detected with the total detection rate of 51 .35%,including Ureaplasma urealyticum (Uu)infection in 841 cases (48.19% ),Uu combined Mycoplasma hominis(Mh)infection in 92 cases(5.27% )and Mh infection in 17 cases(0.97%).The de-tection rate was 9.46% in males and 41 .89% in females,the infection rate in females was obviously higher than that of males.The results of drug susceptibility test showed that the sensitive rate to doxycycline,josamycin and minocycline was higher than 95%, while the resistant rate to norfloxacin and lincomycin was higher than 75%.Conclusion Mycoplasma infection in this hospital is dominated by Uu.Doxycycline,josamycin and minocycline can be used as the empirical medication to treat mycoplasma infections in this hospital.But before clinical treatment,the mycoplasma culture and the drug susceptibility test should be performed as possible for rational and standardized medication.

ObjectiveTo investigate the optimal reconstruction windows for coronary angiography using 320-detector row dynamic volume CT (DVCT) and evaluate their effects on image quality,radiation dose and diagnostic accuracy.MethodsFrom 77 patients [mean heart rate (70 + 13) bpm,range:46-102 bmp] were scanned with retrospective ECG-gating 320-detector row DVCT.The relationship between heart rate and optimal reconstruction windows was analyzed.From 53 patients [mean heart rate ( 75 ±11 ) bpm,range:57-114 bpm] were scanned with prospective ECG-gating 320-detector row DVCT.The effects of prospective ECG-gating on image quality,radiation dose and diagnostic accuracy were evaluated.Friedman test was performed for image quality scores in random groups.Linear regression and Spearman correlation were performed to test bivariate data.ResultsThe proportion of systolic duration in the cardiac cycle increased significantly with higher heart rate (r =0.78,P ＜0.01 ).The image quality in systolic and diastolic phases were significantly influenced by heart rate ( r =0.38,0.82 ; P ＜ 0.01 ).According to the regression analysis of image quality score in different heart rate groups,the optimal reconstruction windows were determined as followed:when heart rate was ＜ 70 bpm,the optimal reconstruction windows should be preset at 65％-80％ ; 70 to 80 bpm,70％-85％ ; 81 to 90 bpm,70％-90％ ; ＞90 bpm,35％-50％.Compared with retrospective ECG-gating,prospective ECG-gating which preset reconstruction windows according to the phases mentioned above,could decrease radiation dose [(6.1 ± 3.8 )vs (12.4 ± 7.0) mSv],without deteriorating the image quality (t =6.5,P ＜ 0.01 ).However,in higher heart rate,the radiation dose of prospective ECG-gating DVCT was still higher ( r =0.64,P ＜ 0.01 ).ConclusionsDVCT can provide high image quality in a wide range of heart rate with prospective ECG gating.It is still recommended to control heart rate strictly since the radiation dose increases significantly in high heart rate.

Objective Study of Shuxuening combined with sodium phosphocreatine on acute severe carbon monoxide poisoning with ischemia modified albumin level and its curative effect in patients. Methods 66 cases of ASCOP patients were randomly divided into routine group and treatment group, all patients were detected for myocardial enzymes, cTnI, IMA before treatment, after treatment 6 h and 3 d. Results IMA was significantly decreased before treatment, while no significant changes occurred in myocardial enzymes and cTnIn levels. After treatment for 6 h, IMA in treatment group was higher than conventional group (P < 0.05). Myocardial enzymes and cTnI gradually increased, the two groups had no difference (P>0.05). After treatment for 3d, cTnI level of normal group increased obviously compare with the treatment group (P>0.05). Myocardial enzymes, IMA restored to the normal level (P>0.05). The level of cTnI after treatment for 3 d improved compare with 6 h. Conclusion Treatment of early ASCOP patients by Shuxuening combined with sodium phosphocreatine significantly improve the recovery of myocardial injury poisoning. The level of IMA can be used as evaluation index of myocardial ischemia and drug treatment.

Objective To study the value of copeptin and APACHE Ⅱ used for assessing acute paraquat poisoning (APP). Methods One hundred and twenty six APP patients were divided into survival group (n = 58) and death group (n = 68), with clinical death as the endpoint of observation. 3 mL blood was obtained from each subject at 2 h , 24 h and 7 d after admission for detecting the levels of copeptin , and APACHE Ⅱ scores were recorded at the same time. Results There was significant difference in dosages of paraquat taken in the death group and the survival group , but the differences were significant in the rescue time , the first time for hemoperfusion and the times for hemoperfusion. 2 h after admission , the death group and survival group had significant difference in elevated copeptin but no significant difference in scores by APACHE Ⅱ. 24 h after admission copeptin decreased to normal level in the survival group but maintained at a higher level in the death group, while the scores by APACHE Ⅱ had insignificant difference between two groups. 7 d after admission, copeptin kept at a higher level and the scores by APACHE Ⅱwere significantly increased in the death group , with significant differences compared to the survival group. The oral doses of pesticides the patients took were positively correlated with copeptin level and scores by APACHE Ⅱ. Conclusion Repeated detection of copeptin and APACHE Ⅱ score are of clinical importance for the assessment of prognosis of APP patients and reasonable distribution of medical resources.

A method was developed for determination of thirteen amines including seven aliphatic amines, two heterocyclic amines and four aromatic amines in atmospheric particulate matter (PM) by gas chromatography-mass spectrometry (GC-MS). Samples were ultrasonically extracted with ultra-pure water and derivatized with benzenesulfonyl chloride (BSC) under alkaline conditions. The derivatives were extracted with dichloromethane and then detected by GC-MS using DB-5MS chromatographic column. The method detection limit (S/N=3) and quantitation limit (S/N=10) were 0.00008-0.017 μg/mL and 0.00026-0.0565 μg/mL respectively, and the correlation coefficients were 0.9903-0.9996, which indicated that the standard curve had good linear correlation. In addition, the relative standard deviation was less than 30％ and the average recovery was 54.4％-159.7％ except for methylamine and benzylamine at spiked level of 1.0 μg/mL, showing high precision and accuracy. 9 kinds of amines were detected in the PM2.5 samples collected in Guangzhou city by this method, among which dimethylamine and butylamine accounted for 90％ of the total nine amines, which indicated that they were primary amines in PM2.5; while propylamine exhibited the lowest level in PM2.5 with the concentration less than 1.0 ng/m3.

Objective To investigate the effect of different sputum suction methods on the incidences of pulmonary infections in patients with mechanical ventilation.Methods Four hundred and sixty-six patients with mechanical ventilation were randomly divided into study group(n=219)and control group(n=247).The two groups were treated with continued balloon filling and airway humidification.Besides,the control group received conventional open sputum suction and the study group closed continuous negative pressure sputum suction.The two groups were compared in terms of incidence of pulmonary infections.Result The incidence of the study group(8.2%)was significantly lower than that of the control group(14.6%)(χ2=4.58,P<0.05).Conclusion Compared with the traditional open conventional sputum suction method,closed continuous negative pressure sputum suction may be more effective in reducing the incidence of pulmonary infection in patients with mechanical ventilation.

AIM: To investigate the role of Cx43 in the myocardialization of the proximal outflow tract(OFT) septum in the mouse heart.METHODS: C57/BL6 mice of ED11.5 to 1 day after birth were used in this study,which included Cx43 knockout homozygotes((Cx43-/-)),heterozygotes((Cx43+/-)) and wildtypes((Cx43+/+)).Pathohistological analysis was used to examine the structure of the hearts.The expression of alpha-sarcomeric actin(?-SCA),active caspase-3 and activator protein-2(AP-2) were detected by immunohistochemistry.RESULTS: Most(Cx43-/-) mice died within 24 h after birth with a swelling and blockage of the conotruncal region,which led to the obstruction of OFT and enlargement of right ventricle.HE staining showed plenty of abnormal tissues in this region forming many pouches.No apparent malformations were observed in(Cx43+/-) and(Cx43+/+) mice.The expression of ?-SCA in the proximal OFT septum was delayed obviously in(Cx43-/-).The apoptotic cells existed in the proximal OFT septum of(Cx43+/+) mostly during ED12.5 to ED15.5.However,there were less apoptotic cells observed in(Cx43+/-),and few in(Cx43-/-).The expression of AP-2,marker of neural crest cells,was increased in (Cx43-/-) and abnormally located in the proximal OFT septum.CONCLUSIONS: Cx43 KO mice are characterized by hyperplasia in conotruncal region,which may be associated with the delayed myocardialization of OFT septum.The decreased apoptosis and the abnormal distribution of cardiac neural crest cells are likely to contribute to the abnormal myocardialization in mice with Cx43 defects.

Objective To propose a technique and treatment of percutaneous polymethyl methacrylate(PMMA) cementoplasty for painful metastatic lesions of the pelvis, humerus, and tibia. Methods Percutaneous PMMA cementoplasty was performed in 24 cases, including the lesions of S1 in 9 cases, acetabulum in 7 cases, ischium in 5 cases, humerus in 2 cases, and tibia in 1 case. There were 26 local lesions. The puncture approach was performed under fluoroscopic guidance. The S1 vertebral body puncture was performed with lateral transsacroiliac joint approach. The needle progression was controlled in the anteroposterior projection with a needle course above the level of the S1 foramen. With the needle adjacent to the S1 vertebral body edge, the needle tip should center just in front of the spinal canal in the lateral fluoroscopic projection. The acetabular roof puncture was performed with lateral approach. The needle progression was controlled in the anteroposterior and lateral projections alternately with a needle course parallel to the body axial plane. The puncture needle arrived directly at the lesions. The puncture to the ischium was from ischium tubercle to the lesions. The puncture progression to the internal compartment of the acetabulum was with the trajectory of the needle from the ischium tubercle to the lesion. The puncture progression to the humerus should avoid conflicting with the radial nerve and upper extremity vessels. The needle course should be from the dorsal upper arm to the lesions. As for the lesions of the tibia, the needle was punctured from the front of tibia to the lesion. After the needle tip placement in the lesions, PMMA in paste condition was injected with the precession injector pressure device under continuous visual control with adequate filling and avoidance of important PMMA leakage. Results Partial or complete pain relief was obtained in all 24 patients (CR in 10 cases, PR in 14 cases) within 7 days after the operation. Clinical improvement was maintained in 23 patients at the follow-up of 2-7 months. One patient with lesion of humerus re-experienced severe pain in local area of PMMA injection 27 days after the operation when he got up by using the treated arm for the support of the body weight. The condition was considered as the fracture at the cement-bone interface, and the gradual pain relief was obtained in 2 days with the external fixation of plaster for limiting the motion of the treated arm, and the complete relief maintained. There was PMMA leakage in 2 cases without corresponding clinical importance. Conclusion The technique and treatment of percutaneous PMMA cementoplasty in osteolytic metastases of the pelvis and peripheral bone was safe and effective. Considering the incidence of the potential fracture, the external fixation for motion limitation should be given after PMMA injection to extremity lesions.
