1.Clinical Application and Efficacy of Tazarotene Cream for Acne Vulgaris

OBJECTIVE:To provide optimal therapeutic scheme for patients with acne vulgaris. METHODS: By a single-blind randomized controlled method,a total of 122 patients with acne vulgaris were assigned to one of the following 3 groups: tazarotene cream group(n=42),Water chlorine tincture group(n=38) and combined treatment group(n=42). The curative efficacy was observed after treatment for 8 consecutive weeks. RESULTS: The number of skin lesions in all the 3 groups reduced as the course of treatment increased,and there were significant differences across the 3 groups after 8-week treatment as compared with before treatment (P0.05). CONCLUSION: Tazarotene cream is effective for non-inflammatory acne. The superior efficacy of water chlorine tincture as compared with tazarotene for patients with impetigo might be related to the antibacterial action of chloramphenicol. Tazarotene cream combined with local application of 0.5% water chlorine tincture is effective for inflammatory acne.
2.Clinical effect of proximal femoral nail anti-rotation on femoral intertrochanteric fracture in the elderly

Objective To investigate the clinical effect of proximal femoral nail anti-rotation (PFNA) on femoral intertrochanteric fracture in the elderly.Methods Eighty-seven elderly patients from June 2007 to March 2012,who had femoral intertrochanter fracture and received treatment of PFNA,were enrolled in this study.Patients were comprised by forty seven people with stable fracture and forty people with unstable fracture.Results They were all followed up from 4.5months to 14 months,with the average time of 7.2 months.No death were found in hospital.The operation time was 25 min in average.During operation,losing blood was 80 ml.Ambulation time was 5.5 d,and the fracture healing time was 22 weeks in average.However,there were significant differences in intraoperative resetpostoperative position of the blade between the stable and unstable fracture patients (P<0.01).Conclusions PFNA is one of the effective methods in the treatment of intertrochanter fracture,and it has shorter operation time,less losing blood,earlier ambulation,and hardly has complications.
3.Analysis of the use of oral hypoglycemic drugs in our hospital

Objective To analyzed the use of oral hypoglycemic drugs in our hospital for reference of clinical rational use of this kind of drugs. Methods The sorting methods of defined daily dose (DDDs) and the frequency of oral hypoglycenic drugs were used for analysis. Results The list of leading DDDs was glucosidase inhibitor, which accounted for 31.13 % of the total, followed by biguanides which accounted for 21.49%. Conclusions The glueosidase inhibitor, a new type oral hypoglycemic drugs used for lowering postprandial blood sugar level showed remarkable efficacy and safety, so it is currently the first choice for patients with diabetes.
4.Strengthening Hospital Infection Control and Link Management

OBJECTIVE To strengthen the hospital infection control and the link management, and to improve the medical care and nursing quality. METHODS The main methods were to strengthen the organization leadership, set up and enhance the management system of infection; strengthen the continuing education about the hospital infection knowledge, control the hospital infection, and standardize medical personnel′s hand hygiene; choose the correct washing and maintenance method for the apparatus and instruments; and strengthen the management of the medical wastes. RESULTS After that to set up the management of each link, eliminate the dangers in medical action, improve the antiseptic quality, and decrease the rate of hospital infection. CONCLUSIONS The hospital infection controls and the link management are two of the important indexes of appraising the management level of the hospital, and it is the important guarantee of improving medical care and nursing quality and supporting the medical treatment′s security.
5.Molecular biology research of recombinant human endostatin targeted therapy of non-small-cell lung cancer

Objective To investigate the relationship between molecular biological markers and recombinant human endostatin targeted therapy in non-small-cell lung cancer(NSCLC).Methods 68 cases patients with non-small-cell lung cancer to meet the requirements were randomly divided into group A (n=34)and group B (n=34).Group A received docetaxel and cisplatin(DP)and gefitinib;group B received recombinant human endostatin on the base of group A.The biological markers species,such as,EGFR,KRAS,VEGF,BRCA1,EML4-ALK,ERCC1,β-tubulin and CD3 were detected by immunohistochemical.The types of population for different programs were summarized according to the expression of biological markers and progression free survival (PFS)of the two groups. Results Group B median PFS was significantly longer than that in group A;no matter low or high expression of VEGF and BRCA1,group B median PFS was significantly longer than that in group A(P<0.05);high expression of ERCC1,KRAS,EML4-ALK andβ-tubulin,group B median PFS was significantly longer than that in group A(P<0.05);low expression of EGFR and CD3,group B median PFS was significantly longer than that in group A (P<0.05 ).Conclusion No matter low or high expression of VEGF and BRCA1 ,low expression of KRAS, ERCC1,EML4-ALK and β-tubulin and high expression of EGFR and CD3 in patients with non-small cell lung cancer may be more sensitive to the treatment of DP and gefitinib combined with recombinant human endostatin.
6.Study on the immune protective effect of Trichinella spiralis excretory-secretory antigen

Objective To compare the immune protective effect of different antigens of Trichinella spiralis:excretory-secretory(ES) antigen of adult worms, ES antigen of muscle larva and their mixed antigens in mice. Methods ES antigens of Trichinella spiralis adult worms and muscle larva were produced by the physiological saline culture methods. The ES antigens of Trichinella spiralis adult worms, muscle larva and the mix were used to immunize mice 3 times at 7 day interval, adjuvant control and normal control were set up. Seven day after the final immunization, each mouse was orally challenged with 200 Trichinella spiralis larva. Intestinal adult worms and muscle larva of Trichinella spiralis of each group were recoveried and examined on Day 7 and Day 30 post-challenge,respectively. Results The intestinal adult worms of Trichinella spiralis reduced by 87.95%, 69.48% and 84.34% in the adult worm ES antigen group, muscle larval ES antigen group and their mixed antigen group, respectively, while muscle larva reduced by 74.79%, 87.97% and 86.87% respectively. Objective To investigate the utilization of enteral nutrition and parenteral nutrition in an inpatient pharmacy.Method The utilization of enteral nutrition and parenteral nutrition formulations in our inpatient pharmacy from 2008 to 2010 was analyzed in terms of brand,volume,and cost using the defined daily dose method.Results The amount of the brands of enteral nutrition formulations increased from 7 to 9 from 2008 to 2010,with the expenditure increased from RMB 426 489 yuan in 2008 to RMB 673 717 yuan in 2010.The amount of brands of parenteral nutrition formulations increased from 43 in 2008 to 50 in 2010,with the expenditure increased from RMB 8 913 103 yuan in 2008 to RMB 13 637 343 yuan in 2010.Conclusion Parenteral nutrition remains the dominant nutritional mode in our hospital and the utilization of enteral nutrition is still low.
9.Epidemiological investigation on depression among 6~16 years old children and adolescents in Chengdu

Objective: To explore the prevalence and co-morbidity in depression disorder among children and adolescents in Chengdu City. Methods: Using a one-phase design, 4S8S children and adolescents in grades 1~10 were assessed with the Strength and Difficulty questionnaire (SDQ) and Development and Well-Being Assessment (DAWBA) . A structured interview with verbatim reports was reviewed by clinicians so that information from parents, teachers, and children were combined in a manner that emulated the clinical process. Results: (1) The rate of depressive disorder was 1.2% , and that in girls was much higher than that in boys after 12 years old (2.56% vs. 1.42%) . (2) The rate of depressive disorder increased with age from 0.10% (6~8 years) to 2.59% (≥ 15 years) . (3) The co-morbidity was 40% . The most common comorbidity diagnosis was anxiety disorders (29.1%) and behavior disorders (25.5%) . Conclusion: The prevalence of depressive disorder among children and adolescents in Chengdu is 1.2%. Gender and age may be the related factors. The co-morbidity is common in children and adolescents with depression disorder.
10.To simulate effects of different intensities of phototherapy on malondialdehyde, vitamin C and vitamin E levels in parenteral nutrition for premature infants

Objective:To investigate the effects of different phototherapy intensities on the levels of malondialdehyde, a peroxidation product of intralipid, vitamin C and vitamin E in parenteral nutrition for premature infants.Methods:The parenteral nutrition for premature infants was prepared under strict aseptic condition and was divided into four groups based on different phototherapy intensities in simulated clinical settings, which were indoor light group, single-, double-, and three-sided phototherapy group. According to whether the nutrient solution shielded for light or not, each group was further divided into two subgroups: exposure or non-exposure group. The levels of malondialdehyde, vitamin C and vitamin E in all groups before phototherapy and 6, 12, 18, and 24 h after phototherapy were measured. Ten samples of parenteral nutrient solutions were prepared for each group, of which 2 ml were extracted for test at different time points. Repeated measurement analysis of variance was used for data analysis and the results were adjusted using Greenhouse-Geisser method if failed in Mauchly sphere test.Results:With the increase of phototherapy time, the malondialdehyde level increased in the exposure and the non-exposure subgroups in the one-sided phototherapy group [before phototherapy: (3.777±0.112) vs (3.746±0.141) nmol/ml; phototherapy for 6 h: (3.808±0.122) vs (3.715±0.145) nmol/ml; 12 h: (4.546±0.138) vs (4.507±0.136) nmol/ml; 18 h: (6.116±0.151) vs (5.239±0.156) nmol/ml; 24 h: (7.569±0.136) vs (5.300±0.200) nmol/ml; all P<0.05], but the level of vitamin C [before phototherapy: (62.507±0.205) vs (62.341±0.144)μg/ml; phototherapy for 6 h: (51.211±0.086) vs (58.128±0.076) μg/ml; 12 h: (43.288±0.084) vs (55.351±0.050) μg/ml; 18 h: (35.758±0.113) vs (51.215±0.093) μg/ml; 24 h: (33.473±0.075) vs (48.473±0.080)μg/ml] and vitamin E decreased [before phototherapy: (4.101±0.132) vs (4.084±0.141) μg/ml; phototherapy for 6 h: (3.761±0.119) vs (3.904±0.075) μg/ml; 12 h: (3.654±0.092) vs (3.729±0.087) μg/ml; 18 h: (3.385±0.102) vs (3.582±0.119) μg/ml; 24 h: (3.313±0.127) vs (3.438±0.113) μg/ml, all P<0.05]. The same situation was also observed in indoor light group, double-, and three-sided phototherapy groups. The malondialdehyde level at different time in the exposure subgroups were higher but the vitamin C and vitamin E levels were lower than those in the non-exposure subgroups, regardless of the phototherapy intensities (all P<0.001). (2) The analysis of all exposure phototherapy subgroups showed that the higher the intensity of light therapy, the higher the malondialdehyde level, and the lower the level of vitamin C and vitamin E, with statistical significance differences in any pairwise comparison. Analysis of all non-exposure subgroups showed statistically significant differences in the malondialdehyde level in any pairwise comparison (all P<0.05) except for the comparison between indoor light group and single-sided phototherapy group ( F=2.383. P=0.140). Moreover, the greater the phototherapy intensities, the lower vitamin C level, with statistically significant differences in any pairwise comparison. And statistical significance differences were observed in the vitamin E level in any pairwise comparison (all P<0.05) except for the comparison between double- and three-sided phototherapy groups ( F=1.358, P=0.259). Conclusions:Phototherapy can increase the malondialdehyde level in parenteral nutrient solution for premature infants and the degree of intralipid peroxidation, but can also lead to vitamin C and vitamin E loss in the parenteral nutrient and weaken its antioxidant capacity.
