1.Comparison of IOL refractive power caculation using different apparatus and various formulas in cataract eyes with silicon oil tamponade

Background Silicon oil tamponade eyes following vitrectomy accelerate and induce lens opacification,so the accurate measurement and calculation of intraocular lens (IOL) diopter before cataract extraction+ IOL implantation is very important for the recovery of visual function.Objective This study was to compare the differences of IOL powers measured and calculated by different apparatus and different IOL power formulas before cataract extraction + IOL implantation silicon oil tamponade combined with cataractous eyes.Methods A prospective,consecutive,nonrandomized study was performed.Thirty-six silicon oil tamponade with cataract eyes of 36 patients were included in the Second Affiliated Hospital of Soochow University from August 2011 to October 2013.Patients with silicone oil emulsification for 4 months to 2 years prepared to treat by cataract extraction + IOL implantation+silicon oil removal.Axial length (AL),corneal curvature (CC) and anterior chamber depth (ACD) were measured with IOLMaster and A-scan with manual kerameter (MK) in the eyes for the prediction of IOL power by SRK-Ⅱ,SRK/T,Hoffer Q,Holladay 1 and Haigis formulas under the oral informed consent.The mean predictive error (MPE) and mean absolute refractive error (MAE) between predictive IOL diopters before operation and actual IOL diopters after operation were evaluated.Results The AL and ACD values measured by IOLMaster was (25.43± 0.90)mm and (3.22±0.38)mm,which were significantly higher than (25.21±1.02)mm and (3.07±0.62) mm by A-scan+ MK respectively,the difference between the two measure methods was statistical significance (both at P =0.000).No significant difference was found in measured CC values between IOLMaster and A-scan+MK ([44.58±1.57] D vs.[44.56±1.62] D) (P=0.568).When the parameters measured by IOLMaster were used,the MAE from SRK/T formula was smaller than that from SRK-Ⅱ,Hoffer Q,Holladay 1 and Haigis formulas (P =0.017,0.009,0.012,0.001),and the MAE from Haigis formula was significantly larger than that from SRK-Ⅱ,Hoffer Q and Holladay 1 formulas (P =0.026,0.035,0.021).When measured by A-scan + MK,the MAE from Haigis formula was significantly larger than that from SRK-Ⅱ,SRK/T,Hoffer Q and Holladay 1 formulas (P=0.007,0.004,0.018,0.006).There was no significant difference in the number of eyes with MAE ≤ 1.0 D between IOLMaster and A-scan+MK measurements under the calculation of SRK-Ⅰ,SRK/T,Hoffer Q and Holladay 1 formulas (x2 =0.107,2.250,0.845,0.084,all at P>0.05).However,the number of eyes with MAE ≤ 1.0 D was significantly increased in IOLMaster measurement compared with A-scan+MK under the calculation of Haigis formula (x2 =4.431,P =0.035).Conclusions In silicon oil-filled cataract eyes,SRK/T formula appears to have a more accurate predictive value of IOL power than that of other formulas when employ IOLMaster;when use A-scan+MK measurement,the predictive value of IOL power seems to be better by using SRK-Ⅱ,SRK/T,Holladay 1 or Hoffer Q formulas.

Objective To make a summary of radiation-induced changes in ocular lens and retinal lesions based on dynamic follow-up of a patient exposed to a radiation exposure from 192 Ir source accident,in order to provide the information and experience for the clinical diagnosis and treatment of such injury in emergency of a nuclear or radiation accident.Methods The examination methods included eye lens and visual acuity,slit-lamp examination,fundus examination,automated perimetry,optical coherence tomography (OCT) examination,fundus photography,and fundus fluorescein angiography (FFA).The patient was followed-up for 20 months.Comparative analyses were made of the follow-up results.Results One month after 192Ir radiation exposure,the visual acuities in both eyes were 1.0.Mild age-related opacities were observed in peripheral cortex,but the fundus looked normal.Twenty months later,the patient was followed-up.The visual acuities reduced to about 0.6,evident powder-and punctuate-like opacities could be observed in the cortex of the right lens,and mild turbid foci in the posterior polar and capsule.Multiple scotomata appeared in the superior temporal and inferior nasal in the right eye.The macular fovea thickness was not changed significantly in the right eye.Examination with fundus photography demonstrated mild optical nerve atrophy,drusens,and pale optic discs,while FFA examination showed punctuate or flocculent leakages in both eyes.Conclusions The lens opacity could be induced or accelerated by radiation exposure.In addition to the posterior capsule and pole cataracts,punctuate opacity within cortical layer may also occur.It also suggests that cataract and fundus injuries may occur later during long-term follow-up,even though the eyes may look normal during early examination after the patient exposed to the radiation.
3.Morphologic features of retina pigment epithelial around fluorescein leakage sites in acute central serous chorioretinopathy before and after laser coagulation

Objective To observe the morphologic alterations of retina pigment epithelial around fluorescein leakage site using spectral-domain optical coherence tomography (SD-OCT) in acute central serous chorioretinopathy (CSC) before and after laser coagulation.Methods Twenty-four eyes of 24 consecutive patients with unilateral acute CSC who underwent laser photocoagulation (wave length:561 nm,energy:80-100 mW,spot diamteter:100 μm,exposure time:0.1 s) were enrolled in this retrospective study.The patients included 22 males and 2 females,with an average age of (39.2± 7.2) years and an average duration of (1.1 ± 0.6) months.The fluorescein leakage outside the foveal avascular zone and serous retinal detachment were found in all the eyes by fluorescein fundus angiography (FFA) and SD-OCT.Twenty-six leakage sites were observed in 24 eyes on FFA.There were 5 smoky leakage points (19.2%),21 inky leakage points (80.8 %).The retinal pigment epithelial (RPE) layer at leakage points were scanned by SD-OCT before and at 2 weeks,2 months after treatment.SD-OCT showed that the RPE abnormalities were detected at all of the fluorescein leakage points,including serous pigment epithelial detachment (PED) in 16 leakage points (64.5%),protruding RPE in 5 leakage points (19.5%),and irregular RPE in 5 leakage points (19.2%).The morphologic alterations of RPE around fluorescein leakage point before and after laser coagulation were observed.Results At 2 weeks after laser photocoagulation,PED existed in 7 leakage points (26.9%),protruding RPE in 3 leakage points (11.5%),and irregular RPE in 16 leakage points (61.5%).At 2 months after laser photocoagulation,protruding RPE existed in 3 leakage points (11.5%),and irregular RPE in 5 leakage points (19.2%),including RPE defect in 2 leakage points.And there were undetectable RPE abnormalities in other 16 leakage points (61.5%).Conclusions In acute CSC,SD-OCT shows that morphologic features of the RPE layer are changed,including PED,protruding RPE,irregular RPE around the leakage point.After laser photocoagulation the abnormalities of RPE can also be altered from PED to irregular RPE,and RPE defect is observed in restricted regions.
4.Expression of αB-crystallin in X-ray irradiated rat lenses

Objective To detect the changes of soluble αB-crystallin in X-ray irradiated rat lens,and to explore the potential role of αB-crystallin in the pathogenesis of radiation-induced cataract.Methods The radiation cataract model was established with the accelerator linear in male Sprague-Dawley(SD)rats.The rats were divided into normal control group,experimental control group and X-irradiated group(the doses were 5,15,and 25 Gy,respectively).The rats were killed at 3 morlths post-irradiation,and the lenses were carefully isolated and homogenized.The protein changes of αB-crystallin in lens supernatant were measured by Western blotting.Results Typical radiation-induced cataract was observed in 15 and 25 Gy groups,while the lenses of the normal control group,experimental control group and the 5 Gy irradiation group remained transparent.The αB-crystallin protein level was significandy decreased in a dose-dependent manner(5 Gy group:0.871±0.085;15 Gy group:0.643±0.096;25 Gy group:0.338±0.160;F=40.764,P<0.05).Conclusions The decreased expression of the molecular chaperone αB-crystallin in X-irradiated rat lens indicates its important role in the pathogenesis of irradiation cataract.
5.Tear film stability after pterygium excision and autologous corneal limbal stem cells transplantation versus simple pterygium excision

BACKGROUND:Treatment methods of pterygium mainly include pterygium excision,pterygium excision combined with local application of mitomycin,pterygium excision combined with conjunctival flap transfusion and pterygium excision combined with autologous corneal limbus stem cell transplantation.Dry eye commonly occurred in many patients following pterygium excision.OBJECTIVE:To evaluate the effects of stem cell transplantation of limbus cornea and simple excision of pterygium on tear film stability.METHODS:Eighty patients(eighty eyes)with pterygium were involved in this clinical experiment.All patients were randomly divided into 2 groups:Group A accepted simple excision of pterygium in 40 patients with 40 eyes under a 10-fold microscope,and Group B accepted excision of pterygium with stem cell transplantation of limbus cornea in 40 patients with 40 eyes under a 10-fold microscope,in which a free transplantation of the superotemporal limbus stem with an adjacent piece of conjunctiva was transplanted in the excision area.Slit-lamp examination,tear film break-up time and questionnaire on dry eye were performed before operation,at one week post-operation,and at three months post-operation.RESULTS AND CONCLUSION:In both groups,following surgery,some patients affected dryness,foreign body sensation,burning sensation.These symptoms were more in the group A compared with group B(P < 0.05).Implant was red 1 week following surgery in the group B,and confluence was found,without infection or rejection.The tear film break-up time was prolonged in the group B compared with the group A at 1 week following surgery,and no significant difference was determined at 3 months.Results indicated that compared with simple excision of pterygium,combined excision of pterygium with stem cell transplantation of limbus cornea obtained better outcomes,and could decrease the manifestations of dry eye and maintain better tear film stability in patients with pterygium.
6.Macular morphology and circumpapillary retinal nerve fiber layer thickness in Parkinson's disease ;evaluated by spectral-domain optical coherence tomography

Objective To observe the macular morphology and circumpapillary retinal nerve fiber layer thickness (RNFL) in Parkinson's disease (PD) evaluated by spectral-domain optical coherence tomography (SD-OCT). Methods A total of 37 patients (74 eyes) with PD were in the PD group, 32 age-and sex-matched healthy subjects (64 eyes) in the control group. All subjects underwent SD-OCT examination with 5 line scanning, macular cube 512×128 scanning and optic disc volume 200×200 scanning. The retinal thickness, central foveal thickness (CFT), macular volume and thickness of circumpapillary, superior, inferior, nasal, and temporal of RNFL between two groups were comparatively analyzed. The relationship between SD-OCT parameters and age, disease duration, scores of Hoehn-Yahr and unified PD rating scale (UPDRS) in PD patients was analyzed by Pearson correlation analysis. Results Both of the retinal thickness and macular volume in PD group were significantly reduced than those in control group (t=?2.546,?3.410;P=0.012, 0.001). There was no difference of CFT (t=?0.463, P=0.644) and the thickness of circumpapillary (t=?1.645, P=0.102), superior (t=?0.775, P=0.439), inferior (t=?1.844, P=0.067), nasal (t=?0.344, P=0.732) and temporal (t=?0.541, P=0.590) of RNFL between two groups. The retinal thickness, macular volume, CFT and the thickness of circumpapillary, superior, inferior, nasal, temporal of RNFL had no relationship with age, disease duration and scores of Hoehn-Yahr and UPDRS in PD patients (P>0.05). Conclusions In PD patients, the retinal thickness and macular volume are decreased, however, the circumpapillary RNFL have no obvious alterations.
7.Characteristics of multifocal electroretinogram in patients with Parkinson′s disease

Objective To investigate the changes of multifocal electroretinogram ( mf-ERG ) in patients with Parkinson′s disease.Methods Forty-five Parkinson′s disease patients ( 70 eyes ) and 35 normal controls (60 eyes) enrolled in the Second Affiliated Hospital of Soochow University from February 2015 to February 2016 received mf-ERG examination and general ophthalmology examinations including best corrected vision acuity, slit-lamp test, fundus examination, intraocular pressure and vision field test. Results The amplitude density of P 1 wave in Parkinson′s disease patients was dramatically decreased in ring 1, ring 2 and ring 5 compared with the normal controls (amplitude density of P1 in ring 1:(100.58 ± 41.19) nV/deg2 vs (138.10 ±17.16) nV/deg2,t=5.086,P<0.01; amplitude density of P1 in ring 2:(21.93 ±7.46) nV/deg2 vs (37.56 ±9.39) nV/deg2, t=9.844, P<0.01; amplitude density of P1 in ring 5:(7.07 ±3.04) nV/deg2 vs (10.22 ±3.68) nV/deg2 ,t=4.924, P<0.01).The latency of P1 wave in ring 1, ring 3 in Parkinson′s disease patients was significantly higher than the control group (latency of P1 wave in ring 1:(42.72 ±8.09) ms vs (37.90 ±5.95) ms, t=-3.155, P<0.01;latency of P1 wave in ring 3:(41.20 ±7.63) ms vs (37.80 ±4.59) ms, t=-2.995, P<0.01).The implicit time of N1 wave in ring 1 in Parkinson′s disease patients was delayed , which had statistically significant difference compared with the normal controls ((21.92 ±7.87) ms vs (19.04 ±4.19) ms, t=-2.012, P=0.046).However, in the amplitude of N1 wave, there was no statistically significant difference between the two groups . Conclusions The vision function has already decreased in Parkinson′s disease patients , even before the vision acuity is normal .The mf-ERG test can find the changes of vision function in Parkinson′s disease patients, which are the decrease of amplitude density of P 1 , the latency of implicit times of P 1 wave and N1 wave in some regions of the retina .
8.Effects of anemoside B4 on the expression of apoptosis-associated proteins in the retina of diabetic rats

Objective:To investigate the effects of anemoside B4 on apoptosis of retinal cells in diabetic rats.Methods:Sixty Sprague-Dawley rats were randomized into three groups: the normal control(control), diabetic rats(DM)and diabetic rats treated with Anemoside B4(B4)groups(n=20, each group). Rats in the DM and B4 groups were rendered diabetic with an intraperitoneal injection of streptozotocin(STZ, 60 mg/kg). After 3 days of successful modeling, rats in the B4 group were intraperitoneally injected with anemoside B4(5 mg/kg), twice/day, for 8 weeks, while rats in the control and DM groups were injected with an equivalent volume of normal saline.After 8 weeks of anemoside B4 and normal saline injection, rats were sacrificed and retinas were harvested for examination.Paraffin sections of retina were stained with the hematoxylin-eosin(H-E)method for morphological evaluation.Protein levels of Bax and Bcl-2 were detected by using Western blot.The expression of caspase-3 mRNA was detected with quantitative PCR.Results:H-E staining results showed the control group had intact retinal structure and clear morphological features, whereas disordered retinal structure, thinner layers, and sparse and disorganized cells were seen in the DM group.However, retinal structure and morphology were improved after treatment with anemoside B4.Compared with the control group, the protein expression of Bcl-2 was lower( t=57.81, P<0.01), the protein expression of Bax was higher( t=10.47, P<0.01), and the Bcl-2/Bax ratio was lower( t=23.98, P<0.01)in the DM group.Compared with the DM group, the protein expression of Bcl-2 was higher( t=41.07, P<0.01), the protein expression of Bax was lower( t=6.811, P<0.01), and the Bcl-2/Bax ratio was higher( t=14.70, P<0.01)in the B4 group.Caspase-3 mRNA expression was higher in the DM group than in the control group( t=7.916, P<0.01), but was lower in the B4 group compared with the DM group( t=6.221, P<0.01). Conclusions:Anemoside B4 can inhibit the apoptosis of retinal cells by up-regulating Bcl-2 expression and down-regulating Bax and caspase-3 expression in diabetic rats.
9.UPLC Fingerprint of the Root of Rosa Laevigata Michx from Different Regions

Objective:To establish the fingerprint analysis method for the root of Rosa laevigata Michx from different regions by UPLC. Methods:The column was ACQUITY UPLC? Phenyl(2.1 mm × 100 mm,1.7 μm). The mobile phase consisted of methanol-water with gradient elution. The flow rate was 0. 2 ml·min-1 , the detection wavelength was 210 nm, the column temperature was 30℃, and the injection volume was 3 μl. Results:The fingerprint consisted of 15 common peaks. The range of similarity for twelve bat-ches of the root of R. laevigata Michx was 0. 489-0. 942. And the reference fingerprint of the root of R. laevigata Michx was estab-lished by UPLC. Conclusion:The fingerprint method is simple and reproducible, which can provide basis for the quality control and the medicinal resources exploration.
10.The effects of Curcumin on the cellular apoptosis induced by high glucose in rat retinal vascular endothelial cells

Objective To observe the effects of Curcumin on the cellular apoptosis of rat retinal vascular endothelial cells (RRVEC) induced by high glucose. Methods Generation 4 cultured RRVEC were used in this experiment, and identified with anti-vWF factor antibody by immunochemistry and immunofluorescence. The RRVEC were divided into control group (5.5 mmol/L glucose), high glucose group (30 mmol/L glucose), and treatment group (30 mmol/L glucose+30 μmol/L Curcumin), respectively. Flow cytometry was used to measure the cellular reactive oxygen species (ROS) level and apoptosis. The expression intensity and location of nuclear factor (NF)-κB p65 in the cells of the three groups were detected by immunochemistory. The expression of Bcl-2 and Bax protein was detected by Western blot test. Results Immunostaining showed that RRVEC were positive for vWF factor. The flow cytometry showed that the cellular ROS level in treatment group was higher than that in the control group (t=8.677, P=0.000), but less than that in the high glucose group (t=40.957, P=0.000). Compared with the high glucose group, the cellular ROS level in the treatment group was decreased significantly (t=6.568, P=0.000). The cellular apoptosis were significantly different among the three groups (F=325.137, P=0.000). Compared with the high glucose group, the cellular apoptosis in the treatment group was decreased significantly (t=12.818, P=0.000). Immunochemistry showed that NF-κB p65 was expressed strongly in the cellular nuclei and cytoplasm in the high glucose group than that in the control group and the treatment group with the significant differences (t=8.322, P=0.000). Western blot results demonstrated that compared with the control group, the expression of Bcl-2 of RRVEC and Bcl-2/Bax ratio decreased (t=4.362, 6.449; P=0.005, 0.001) and Bax increased (t=3.813, P=0.009)in the high glucose group, with statistically significant differences. Compared with the high glucose group, the expression of NF-κB and Bax decreased (t=2.577, 3.059; P=0.042, 0.022) and Bcl-2/Bax ratio increased significantly (t=3.831, P=0.009) in the treatment group. Conclusion Curcumin could suppress the cellular apoptosis of RRVEC induced by high glucose. The mechanism of Curcumin protecting RRVEC may be via regulating NF-κB signal pathway.
