1.The clinical analysis of Drug resistance of pseudomonas aeruginosa.

Objective To investigate the antibiotic resistance of pseudomonas aeruginosa isolated from our hospital dur- ing Janury 2005 to December 2006.Methods Disc diffusion method was used to detemine the susceptibility of 189 strains of pseudomonas aeruginosa to 10 antibiotics,WHONET 5.3 was applied for analysis.Results Among 189 strains of pseudomonas aeruginosa,resistance rates to piperacillin-tazobactam,imipenem,ceftazidime,cefepime and amikacin were less than 30%.The resistance rate to imipenem reached 22.5%.Conclusion The drug resistance of PAE is seri- ous.Supervision of drug resistance of PAE should be strengthened in hospital.It's very important to select antibiotics correctly according to the results of susceptibility test in clinic.
2.Progress of CLASS-01 trial of Chinese laparoscopic gastrointestinal surgery study group

The development of gastrointestinal minimally invasive surgery in China is changing during the past decade.The Chinese laparoscopic gastrointestinal surgery study (CLASS) group as the earliest and most closely cooperated organization for surgical clinical researches,under the support of Chinese Medical Association,Chinese Anti-cancer Association and Chinese Medical Doctor Association,the clinical research capacity and minimally invasive technology of the CLASS group continue to be developed.With the launch of the prospective,randomized,controlled CLASS-01 trial for the treatment of locally advanced gastric cancer in 2012 and the publication of safety results of laparoscopic surgery for gastric cancer in 2016,the international academic community is increasingly focusing its attention on China.This article attempts to review the history and development of the CLASS group in the past 7 years,aiming to prospect the future and attempt to create trends.
3.Surgical procedures and key points in laparoscopic total gastrectomy with D2 lymph node dissection

Correct implementation of laparoscopic total gastrectomy with D2 lymph node dissection depends on the understanding of anatomical features of peripancreatic space, the landmark of pancreas and blood vessels, and the diverse perigastric vascular anatomy and standardized surgical procedures designed according to the regional distribution of lymph nodes.From September 2006 to November 2009, laparoscopic total gastrectomy with D2 lymph node dissection surgery were completed in 12 cases in the Nanfang Hospital. A reasonable anatomy method and a simple, effective surgical procedure of laparoscopic D2 lymph node dissection were introduced.
4.Translucency of dental zirconia ceramics sintered in conventional and microwave ovens.

OBJECTIVETo evaluate the effect of microwave sintering on the translucency of zirconia and to compare these effect with those of conventional sintering. The relationship between the microstructure of specimens and translucency was investigated.
METHODSA total of 10 disc-shaped specimens were fabricated from 2 commercial brands of zirconia, namely, Zenostar and Lava. Each group included 5 discs. Conventional sintering was performed according to the manufacturers' specifications. The maximum temperature for Zenostar was 1,490 °C, whereas that for Lava was 1,500 °C. The dwelling time was 2 h. The sintering temperature for microwave sintering was 1,420 °C, heating rate was 15 °C · min⁻¹, and dwelling time was 30 min. After sintering, the translucency parameter (TP) of the specimens were measured with ShadeEye NCC. The sintered density of the specimens was determined by Archimedes' method. The grain size and microstructure of the specimens were investigated by scanning electron microscopy.
RESULTSDensity and translucency slightly increased by microwave sintering, but no significant difference was found between microwave and conventional sintering (P > 0.05). Small and uniform microstructure were obtained from microwave sintering. The mean TP of Lava was significantly higher than that of Zenostar (P < 0.001).
CONCLUSIONThe translucency of zirconia sintered by microwave sintering is similar to that of the zirconia sintered by conventional sintering.

5.Updates of intracorporeal esophagojejunal anastomosis during laparoscopic total gastrectomy for gastric cancer

In recent years,laparoscopic total gastrectomy (LTG) with lymphadenectomy is increasingly utilized for the management of gastric cancer located in the middle or upper third of the stomach.However,esophagojejunostomy is the key technical difficulty in operation.Compared with conventional extracorporeal esophagojejunostomy via mini-laparotomy,pioneers are attempting to perform intracorporeal anastomosis in order to gain better manipulation and minimally invasive benefits,as well as reducing the difficulties in digestive tract reconstruction.
6.Laparoscopic pancreas- and spleen-preserving splenic hilar lymph nodes dissection through retropancreatic approach in radical gastrectomy for advanced gastric cancer

Laparoscopic pancreas- and spleen-preserving splenic hilar lymph nodes dissection is still difficult to accomplish,which restrains its application in total gastrectomy for advanced proximal gastric cancer.Based on our anatomical understanding of pre- and retropancreatic spaces,features of splenic vessels and distribution of perigastric lymph nodes,we combined the characteristics of laparoscopic surgery and developed a novel technique for laparoscopic pancreas- and spleen-preserving splenic hilar lymph nodes dissection through retropancreatic space.The key lies in mobilization of the splenic pedicle through retropancreatic space,dissection of peri-vascular lymph nodes in sequence of trunk-to-branch,in-sheath vascularization of the splenic vessels.From August 2009 to December 2010,this technique was performed on 6 patients in Nanfang hospital.The initial results suggested that this technique could be safe and feasible for skillful surgeons.Further studies on the application of this technique in total gastrectomy for advanced proximal gastric cancer would be needed.
7.Rapid Determination of CYP2 C9 Activity by UPLC-MS/MS

Objective:To establish an ultra performance liquid chromatography-tandem quadruple mass spectrometry ( UPLC-MS/MS) method to determine CYP2C9 activity in vitro. Methods:An ACQUITY UPLC? BEH C18 (100 mm × 2. 1 mm, 1. 7 μm) column was used as the stationary phase at 30℃. The mobile phase consisted of acetonitrile-water ( containing 0. 1% formic acid and 0. 5%ammonia water) (40∶60, v/v). The flow rate was 0. 2 ml·min-1. Chlorpropamide was used as the internal standard. The MS condi-tions were as follows:ESI with positive ion detection mode. Self-prepared CYP2C9?1, ?2, ?3 and ?13 protein were incubated with tolbutamide at 37℃ and 800μl ethyl acetate was added to stop the reaction. After centrifuged at 10 000g, the organic layer was then dried using nitrogen, the residue was re-dissolved in 200μl mobile phase and determined by UPLC-MS/MS. Results: The reten-tion time of 4-hydroxytolbutamide was 1. 21 min. An excellent linear calibration curve of 4-hydroxytolbutamide was obtained within the concentration range of 0. 05-5 ng·μl-1(r=0. 999 8). The lower limit of quantification of 4-hydroxytolbutamide was 0. 01 ng·μl-1 with the average recovery of 99. 3%-100. 3%. The intra- and inter-day RSDs were all less than 5%. There was no interference from the endogenous substances existing in the incubation system. The catalytic activity of the variants CYP2C9?2,?3 and?13 after tol-butamide was incubated with CYP2C9?1,?2,?3 and?13 was 47. 3%, 11% and 0. 3% of wild type CYP2C9?1. Conclusion:The method is simple and stable, and suitable for the fast evaluation of cytochrome CYP2C9 activity in vitro and relevant studies on the inhibitors.

10.Studies on the effects of borneol on the pharmacokinetics of carbamazepine in rabbits

Aim To observe the effects of borneol on the pharmacokinetics of carbamazepine(CBZ) and 10,11-epoxide carbamazepine(ECBZ) in rabbits.Methods After ig administration of bornol and CBZ,the CBZ and ECBZ were determined with HPLC,then the pharmacokinetic parameters were obtained with 3P87 program on computer.Results In the CBZ-borneol combination group,the pharmacokinetic parameters of CBZ,T_(12(ka)),T_(peak) and AUC were increased,while Ka and CL were decreased.The parameters of ECBZ,T_(12(ke)) and T_(peak) were increased,while Ke was decreased.The ratio of CSF to plasma of ECBZ at the time of 0.5,1,2 hour after ig was increased.Conclusion Borneol significantly improved the bioavailability of CBZ,inhibited the metabolism of CBZ,and enhanced the ECBZ passing through blood brain barrier.
