1.Application and progress of surgical nutrition
Chinese Journal of General Surgery 2001;0(08):-
Nutrition support is extensively used in many clinical fields. The rest energy requirement in Chinese is only 5021~5858kJ/d.In our country 40% to 50% of hospitalized patients need nutrition support. In nutrition support, attention should be paied to :(1)The energy giving to the patient should be appropriate;(2)hormone resistance;(3)immunonut rition.

Objective To study the clinic, neuro-electrophysiology and molecular biology of Machado-Joseph disease (MJD).Methods Family visiting, physical examination and the blood samples were analysed on molecular biology in 44 members of a family with MJD.The cases of inpatients were examined on cerebrospinal fluid and neuro-electrophysiology.Results 10 patients of the family attacked, which were consisted with autosomal dominant inheritance type. Age of the onset was 8~38 years old. The clinical characteristic was progressive severe spinocerebellar of ataxia, faciolingual myokymia, bulging eyes.Change of denervated muscle was revealed by neuro-etectrophysiological examination. Light atrophy was observed in cerebellar, brain stem, spinal cord. The genetic defect of MJD was located the long arm of chromosome 14 between D14S280 and D14S81, their distance was 3.0 cm.All tested patients had their CAG repeated expansion from 72 to 84 in the MJD gene. Conclusion MJD is a neuro-degenerative disorder of autosomal dominant inheritance. The disease was clinically characterized by progressive severe spinocerebellar ataxia, no obvious changes of cerebrospinal fluid,neuro-electrophysiology, CT and MRI.The genetic defect of MJD was located the long arm of chromosome 14.The number of CAG repeated expansion mutation was associated with the age of the onset.

Objective:To observe the dynamic expression of mRNA of TLR4 and TLR9 in Lewis rats with experimental autoimmune neuritis(EAN) and the effect of TWP on the disease. Methods:Male Lewis rats were immunized with P0 180-199(100 microgram),TWP was profused into post-immunization rats’ stomach daily. The clinical signs of rats and pathological changes in the sciatic nerves were observed.TLR4 and TLR9 were detected by RT-PCR dynamically which spleens, sciatic nerves and peripheral blood lymphonodes as sample. Results:EAN group got the peak of clinical score at the 17 d.p.i,and ameliorated obviously at 33 d.p.i,and the mRNA expression of TLR4 got the peak at the 16 d.p.i,then reduced gradually(P

Objective To investigate the relationship between the gene polymorphism of TLR3c.1377,TLR9-1486,and TLR9 2848 and susceptibility to multiple sclerosis(MS)in Han people of south China. Methods A total of 123 unrelated MS patients from South China with a clinical or laboratory definition MS according to 2005 Revisions to the McDonald Criteria were studied. Another 126 controls were randomly selected from hospital staff of non-autoimmune diseases and healthy individuals. Toll like receptor (TLR) 3 and TLR 9 genotypes were determined by PCR and digested by specific restriction enzymes. Results There was significant difference in genotype and allele distribution of TLR3c.1377 polymorphism between the MS patients and the controls (P<0.05), and the MS patients with T allele had a lower risk (OR=0.532, P=0.014). There was no significant difference in genotypes and allele distribution of TLR9-1486 polymorphism between the MS patients and the controls. There was higher TLR9 2848 A allele frequency in the MS patients than in the controls ((39.8%) vs. 30.6%;P=0.037), and higher risk in MS patients with A allele than those without ((OR=)(1.837), P=0.020). There was no significant interaction among the TLR3c.1377, TLR9-1486 and TLR9 2848 allele. Strong linkage disequilibrium was found between TLR9-1486 and TLR9 2848, but there was no significant interaction between the polymorphism of TLR9-1486 and TLR9 2848 in the MS patients. Conclusion TLR3c.1377 and TLR9 2848 polymorphisms may be related to MS in Han people in south China. TLR3c.1377 and TLR9 2848 may be linked with susceptibility genes.

Acta Anatomica Sinica 1957;0(04):-
The contents of dopamine (DA) and its metabolites, homovanillic acid (HVA) and 3,4-dihydroxyphenyl acetic acid(DOPAC), in both striata were determined with HPLC-EC technique in each of 28 albino rats, which were divided into 3 groups (bilateral lesioned, unilateral lesioned and healthy control groups). Among the unilateral lesioned rats, three indices such as DA, HVA/DA and DOPAC/DA in the lesioned striata were 22.27%, 420.00% and 199.75% of those of the intact striata, respectively. This suggests that there may be an enhanced compensatory DA release in the striatum ipsilateral to the lesion of the rat. Having compared the lesioned striata of unilateral lesioned rats with those of bilateral lesioned animals, and the intact striata of unilateral lesioned rats with those of the control ones, we did not find any significant differences in DA, HVA/DA or DOPAC/DA. The result implies that the compensatory DA release in the lesioned striatum probably accounts for the mechanism of the ipsilateral nigrostriatal system, not for the crossed nigrostriatal fibers.

Objective To evaluate the immediate and short term therapeutic efficacy of percutaneous transluminal septal myocardial ablation (PTSMA) in patients with hypertrophic obstructive cardiomyopathy (HOCM) and to investigate the prevention of the related complications. Methods PTSMA was conducted in 3 patients with HOCM refractory to medication. The immediate effect was evaluated by pressure monitor, but the follow up effect by observation of the clinical symptoms and ultrasonic cardiogram. Results The left ventricular outflow tract gradients triggered by ventricular premature beats decreased from 143 mmHg (ranging from 70 to 180 mmHg) to 53 mmHg (ranging from 30 to 80 mmHg). Complete atrioventricular block (Ⅲ?AVB) with atrioventricular node rhythm was found in 1 case, and complete right branch bundle block (CRBBB) in another one. Follow up of the 3 cases at 6 months after operation revealed remarkable improvement of the patients' cardiac function (NYHA), disappearance of angina pectoris, and obvious attenuation of ventricular septal hypertrophy and SAM (systolic anterior motion of mitral) phenomenon. Conclusion PTSMA is an effective method with satisfactory short term effect for the patients with HOCM refractory to medication.

Objective To investigate the expression of TROP2 and MMP-9 protein expression in cholangiocarcinomas and their relationship between the pathological behavior and prognosis. Methods A total of 54 patients who were diagnosed with cholangiocarcinoma in the People's Hospital of Binzhou, were retrospectively reviewed.Immunohistochemical staining and Log rank test were used to detect the expression of TROP2 and MMP-9 protein in 54 cases of cholangiocarcinomas and 18 cases of normal bile duct tissues achieved by partial hepatectomy of hepatolithiasis. Results The positive expression rate of TROP2 in cholangiocarcinoma tissues (55.6%) was higher than that of normal bile duct tissues (5.6%).The positive expression rate of MMP-9 in cholangiocarcinoma tissues (51.9%) was higher than that of normal bile duct tissues (11.1%).The differences of the expression of TROP2 and MMP-9 in cholangiocarcinoma of TNM stage, lymph node metastasis and neural invasion were significant(all P < 0.05).There was significant positive correlation between TROP-2 and MMP-9 expression by using spearman correlation analysis (r =0.555,P < 0.001).Survival analysis showed that TROP2 expression was an independent prognostic factor in cholangiocarcinoma. Conclusions TROP2 plays a important role in the development and metastasis of cholangiocarcinoma.Thus,TROP2 may be a prognostic indicator for cholangiocarcinoma.
