1.Cardiac Protection during Open Heart Surgery: A View from Coronary Endothelial Function

Cardioplegic (and organ preservation) solutions were initially designed to protect the myocardium (cardiac myocytes) during cardiac surgery (and heart transplantation). Due to the differences between the cardiac myocytes and vascular (endothelial and smooth muscle) cells in structure and function, the solutions may have adverse effect on coronary vascular cells. However, such effect is often complicated by many other factors such as ischemiareperfusion injury, temperature, and perfusion pressure or duration. In evaluation of the effect of a solution on the coronary endothelial function, a number of points should be taken into consideration. First, the overall effect on endothelium should be identified. Second, the effect of the solution on the individual endothelium-derived relaxing factors (nitric oxide, prostacylin, and endothelium-derived hyperpolarizing factor) must be distinguished. Third, the effect of each major component of the solution should be investigated. Fourth, the effect of a variety of new additives in the solution may be studied. In the last decades, we have focused our research on the endothelial function during open heart surgery and have for the first time found that high potassium concentration impairs the EDHF-mediated function. This review attempts to discuss the above issues based on available literature in order to provide information for further development of cardioplegic or organ preservation solutions.
2.Combination of ESWL and endoscopic manipulation in the treatment of complex upper urinary tract calculi

Objective To investigate the treatment of complicated upper urinary tract calculi. Methods 138 cases of upper urinary tract calculi were treated by the combination of extracorporeal shork wave lithotripsy (ESWL),ureteroscopic lithotripsy and minimally invasive percutaneous nephrolithotomy were reviewed.There were 110 cases of complex ureteral stone and 28 cases of complex kidney stone among the group.All cases had variable degree of hydronephrosis.The treatment program for complex upper urinary tract calculi was initially,ESWL and followed by ureteroscopy and pneumatic lithotripsy in 24 hours.For the complex kidney stone and upper ureteral stone,which was difficult for ureteroscopy lithotripsy.The program was firstly percutuneous minimal pyelostomy,ESWL after 5~7 days,then urteroscopy lithotripsy and lithotomy through the fistula in 24 hours. Results Total stone-free rate was 97.1%(134/138),in ureteral stone group was 100%(110/110),in kidney stone group 85.7%(24/28).Satisfactory result has been observed in all cases. Conclusions The endoscopic manipulation become easy and fast by means of endoscopic manipulation in the treatment of complex upper urinary tract calculi.This technique is minimally invasive and has less complications,it increases the stone-free rate.
3.Recognition of factors influencing therapeutic efficacy of transurethral resection of the prostate (report of 600 cases)

Objective To recognize the factors which influence the therapeutic efficacy of transurethral resection of the prostate (TURP) and to improve the qualities of TURP. Methods The data of 600 patients with benign prostatic hyperplasia (BPH) were retrospectively studied based on the mastering of TURP.The patients’age ranged from 52 to 92 years,with a mean of 64.8 years.Preoperatively,the volume of the prostate was 16.3~210.4 ml,with a mean of 46.2 ml,measured by color Doppler B-ultrasound.The average maximum urinary flow rate (Qmax) was 8.4 ml/s,and average urinary flow rate (Qave) was 4.3 ml/s.The average residual urinary volume (RU) was 75 ml,and the average IPSS was 23. Results TURP was successfully performed on all the 600 patients.The average resected prostate tissue weight was 31.8 g.The average electric ablation time was 51 minutes.Intraoperative blood transfusion was performed in 38 cases. Aura of transurethral resection syndrome (TURS) occurred in 4 cases,but no TURS occurred.Of the 600 cases,508 cases (85%) were followed up for 1~32 months.Postoperative average Qmax increased to 16.9 ml/s,Qave increased to 10.8 ml/s and RU decreased to 15 ml.Compared with those of preoperation,the differences were statistically significant (P
4.Emergency treatment of obstructive acute renal failure by means of endoscopic manipulation (report of 23 cases)

Objective To evaluate endoscopic manipulation such as ureteroscopy,air pressure lithotripsy and double J catheter placing for the emergency treatment of obstructive acute renal failure (OARF). Methods 23 cases of obstructive acute renal failure,with an average age of 53,were treated by instant endoscopic manipulation.Blood uria nitrogen (BUN) before treatment was 16.1~55.6 mmol/L and serum creatinine (SCr) 465~2 099 ?mol/L.The patients were followed up for 1~9 months. Results Satisfactory result has been observed in all. 24 h urine output reached 2 400~5 600 ml with BUN and SCr dropped significantly.Normal renal function has been regained in 19 whereas SCr dropped to 200 ?mol/L in the other 4 patients. Conclusions Endoscopic management has the advantages of minimal invasion,safety,quick recovery and bilateral ureteral obstruction could be dealt with simultaneously.Instant endoscopic management plays an important role in the treatment of obstructive acute renal failure.
5.Observation of Antibacterial Activity of 156 Chinese Medicinal Herbs Against Ureaplasma Urealyticum in Vitro

In order to study the antibacterial activity of Chinese medicinal herbs to Ureaplasma ure- alyticum(Uu),we examined the inhibitory effect of 156 Chinese medicinal herbs to 14 standard strains of Uu by microdilution method in vitro.The results showed that Uu had a higher sensitivity to Cortex phellodendri,Radix angelicae dahuricae,Fructus kochiae,Radix et rhizoma rhei(MIC_(90)≤7.81mg of dried medicinal herbs/ml);a moderate sensitivity to Radix glycyrrrhizae,Radix isatidis,Rhizoma cop- tidis,Herba andrographitis,Herba houttuyniae(15.63≤MIC_(90)≤62.5);a lower sensitivity to Cortex meliae,Herba leonuri(62.5250).It is our opinion that Chinese medicinal herbs are of momentous signfi- cance in the treatment of Uu infection.
6.Changes in complement factor H-related protein 2 in children with pulmonary arterial hypertension associated with congenital heart disease and its potential clinical significance

Objective To detect the changes and the clinical significance in plasma protein-complement factor H-related protein 2 (CFHR2) in pulmonary arterial hypertension (PAH) associated with congenital heart disease (CHD) children.Methods Various types of 66 CHD patients with or without PAH and 16 healthy children(healthy control group) were studied,including 11 ventricular septal defects (VSD) with PAH (VSD-PAH),11 isolated VSD,11 atrial septal defects with PAH (ASD-PAH),11 isolated ASD,11 mixed type of heart defects [two or more defects of VSD,ASD and patent ductus arteriosus (PDA)] with PAH (Mix-PAH) and 11 cases without PAH (Mix).CFHR2 was validated by enzyme linked immunosorbent assay in the sample plasma.Results Compared with the healthy control group,the CFHR2 concentration in VSD-PAH patients [(189.10 ±24.01) μg/L vs.(42.99 ±4.53) μg/L,t =4.975,P <0.01] and VSD patients [(189.10 ±24.01) μg/L vs.(165.00 ±23.17) μg/L,t =2.661,P < 0.05] were lower.The CFHR2 protein was also confirmed to be decreasing significantly in VSD-PAH patients compared with VSD patients (t =4.698,P < 0.01).The plasma CFHR2 level in ASD-PAH patients [(189.10 ± 24.01)μg/L vs.(70.92 ± 8.27) μg/L,t =3.951,P <0.01] and ASD patients [(189.10 ±24.01) μg/L vs.(72.48 ± 8.99) μg/L,t =3.880,P < 0.01] were significantly lower than those in the healthy control group,although there was no significant difference between ASD-PAH and ASD patients (t =0.128,P > 0.05).The plasma CFHR2 level in Mix-PAH patients [(189.10 ± 24.01) μg/L vs.(83.23 ± 15.96) μL,t =3.314,P < 0.05] was significantly lower than that in the healthy control group,while Mix patients [(189.10 ±24.01) μg/L vs.(170.40 ±33.15) μg/L,t =0.468,P > 0.05] had no difference compared with the healthy control group,but had statistical significance with M ix PAH group (t =2.370,P < 0.05).Conclusions The decrease of CFHR2 protein may demonstrate the deficiency of the immune system and coagulation mechanism in these patients and can be consi-dered as biomarker of CHD-PAH disease.
7.Association between PNPLA3 polymorphisms (rs738409) and non-alcoholic fatty liver disease

Objective To develop a novel method for genotyping rs 738409 ( C >G ) single nucleotide polymorphism ( SNP ) , and explore the association between the rs 738409 genotypes and non-alcoholic fatty liver disease ( NAFLD ) .Methods Method establishment and analysis of genetic susceptibility.The principle of amplification refractory mutation system ( ARMS) and combined a TaqMan fluorogenic probe as signal report were used , by monitoring the difference of cycle threshold (ΔCt=C allele-special primer Ct values minus G allele-special primer Ct values ) between the two PCR reactions in a real-time PCR, the method for rs738409 genotyping was established ( ARMS-TaqMan) .618 subjects ( men:401;women: 217 ) , in an annual health check-up program from January 2011 to December 2014 in Aerospace Center Hospital , were performed for rs738409 genotyping by the ARMS-TaqMan assay.Fatty liver was diagnosed based on abdominal ultrasonography .The chi-square test and multiple logistic analyses were used to analyze the relationship of rs 738409 genotypes and NAFLD.Results The ΔCt by ARMS-TaqMan for rs738409 genotyping were -13.1 ±1.4 of CC alleles ( 243 cases ) , 0.01 ±0.45 of CG alleles ( 282 cases), and 12.7 ±1.9 of GG alleles (93 cases), respectively.The GG alleles frequency of rs738409 were significantly higher in patients with NAFLD compared with subjects without NAFLD (21.5%vs 12.3%,χ2=8.677, P =0.003).In comparison to subjects with CC alleles, the OR (95% confidence interval) adjusted for age, gender, total cholesterol, triglyceride, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol, fasting blood glucose and body mass index was 1.35 (0.91-2.00) in subjects with CG alleles and was 2.21 ( 1.32 -3.71 ) in subjects with GG alleles ( P =0.013 ) .Variant rs738409 genotypes were associated with significant increased trend in alanine aminotransferase ( ALT) level from CC alleles, CG alleles to GG alleles (F=8.980, P<0.001), and in aspartate aminotansferase (AST) between GG alleles and CC alleles (F=6.491, P<0.001).Conclusions The novel ARMS-TaqMan assay had the features of accuracy , one step and high-throughput for rs738409 genotyping.The G allele of rs738409 was a risk factor of NAFLD susceptibility and associated with higher level serum ALT and AST .
8.Development and application of triphenyltetrazolium chloride (TTC) method for determining the survival of cultured cells

Objective To develop a precise and reliable method to determine mammalian cell survival using the triphe-nyltetrazolium chloride (TTC) solution.Methods Different amounts of TTC solution were added to the cultured cells in order to find the optimal concentration .The correlation between the cell number and the measured optic density value was analyzed.In addition, the TTC method was compared with traditional MTT assay .Results The TTC assay could precisely reveal the viability of cultured cells as well as the MTT method , but the non-specific reaction of the MTT reagent by other antioxidants could result in unreliable overestimation of the cell number .Conclusion The TTC assay can be used to deter-mine the survival of cultured cells more precisely .
9.Analysis of the relationship between the composition of stones and the age and sex in patients with kidney stones

Objective According to the infrared spectrum of renal stone composition analysis,discuss the relationship between gender,age and kidney stone.Methods Kidney stone composition analysis of 408 cases had been done in our hospital from the 2002 to 2011 by using Fourier transform infrared spectroscopy.There were 271 males (66.4%) and 137 females (33.6%).Results There were 359 cases with calcium stones,accounting for 88.0% (359/408); 197 cases with calcium oxalate stones alone,accounting for 48.3%; 180 cases with phosphate stones,accounting for 44.1%; 211 cases with mixed composition stones,accounting for 51.7%.Main chemical composition of the stones were:calcium oxalate 48.3%,carbonate apatite 34.8%,urates 5.4%,ammonium magnesium phosphate 4.4%,ammonium carbonate eutectic 3.4%,brushite 1.5%,cystine 2.0% and protein 0.3%.Male patients accounted for 66.4% of all cases and the peak onset age was 30-59 years; Female patient accounted for 33.6% of all cases and the peak onset age was 50-59 years.Conclusions Calcium oxalate and phosphate are the most common components of kidney stones.Males age between 30-59 years and females age between 50-59 years may suffer from of kidney stone formation.
10.Efficacy and safety of different surgical approaches for early cervical cancer in patients

Objective:To investigate the efficacy and safety of laparoscopic versus open surgery for early cervical cancer and analyze the factors that influence laparoscopic surgery for early cervical cancer. Methods:The clinical data of 411 patients with International Federation of Gynecology and Obstetrics (FIGO)stage IA1-IIA2 cervical cancer who received treatment in Liuzhou Worker's Hospital, China between January 1, 2000 and August 31, 2016 were retrospectively analyzed. The clinical data were divided into laparoscopic surgery ( n = 320) and open surgery ( n = 91) groups according to surgical approaches. The operation time, intraoperative injury, intraoperative blood loss, time to first postoperative anal exhaust, postoperative complications, time to extubation, and postoperative recurrence rate were compared between laparoscopic surgery and open surgery groups. Multivariable COX regression analysis was performed to analyze the recurrence of cervical cancer after laparoscopic surgery. Results:The operation time in the laparoscopic surgery group was significantly shorter than that in the open surgery [(223.4 ± 53.2) min vs. (251.0 ± 50.0) min, t = 20.200, P < 0.05]. The intraoperative blood loss, time to extubation and the time to first postoperative anal exhaust in the laparoscopic surgery group were less or shorter than those in the open surgery group (all P < 0.05). Under the premise that there were no significant differences in vascular and lymphatic space invasion, tumor size, parametrial invasion, depth of muscle invasion, lymph node metastasis, and vaginal margin between the laparoscopic surgery and open surgery groups, the 1-year and 3-year recurrence rate of cervical cancer was 9.4% (30/320) and 15.9% (51/320), respectively and the 1-year and 3-year tumor-free survival rate was 90.6% and 84.1%, respectively in the laparoscopic surgery group; the 1-year and 3-year recurrence rate of cervical cancer was 4.4% (4/91) and 9.9% (9/91), respectively and the 1-year and 3-year tumor-free survival rate was 95.6% and 89.6%, respectively in the open surgery group. There were no significant differences in 1-year and 3-year recurrence rate of cervical cancer and 1-year and 3-year tumor-free survival rate between laparoscopic surgery and open surgery groups (all P > 0.05). During the 5-year follow-up, there were no significant differences in the recurrence rate of cervical cancer (18.7% vs. 12.2%) and the tumor-free survival rate (81.9% vs. 86.8%) between the laparoscopic surgery and open surgery groups (both P > 0.05). Univariate and multivariate analysis showed that positive lymph node metastasis, tumor size ≥ 2 cm, FIGO stage > IB1 and non-squamous cell carcinoma pathological type are the risk factors for the recurrence of cervical cancer. Conclusion:Laparoscopic surgery for early cervical cancer can reduce the intraoperative and postoperative complications. Positive lymph node metastasis, tumor size ≥ 2 cm, FIGO stage > IB1, and non-squamous cell carcinoma pathological type are the risk factors of recurrence of cervical cancer after laparoscopic surgery.
