AIM: To investigate the damages of adrenal gland in rabbits infected with neisseria meningococci(MC).METHODS: 18 rabbits were injected with MC (1?10 11 organism/kg, iv, model group, n=10 ) or normal saline (control group, n=8 ).TNF-? was detected in baseline (before challenged), 60 min and 120 min after injection. 120 min after injection, TNF-? immunochemical localization of adrenal gland was detected with ABC methods by electron microscopy. RESULTS: In model group, TNF-? positive cells were located in medulla and TNF-? released into simus of medulla. The medulla damages were appeared but gland cells of cortex was normal. No damages were showed in control group. TNF-? value of plasm was elevated at 60 min and 120 min after injection compared with control group. CONCLUSION: TNF-? was released into medulla and resulted in medulla damages after MC challenging. Adrenal cortex didnt damaged in early phase of MC septic shock. [

Objective:The present study was designed to investigate whether transfecting DC with tumor derived total RNA is an effective way to induce CTL and antitumor immunity.Methods:DC were propagated from bone marrow(BM) of C57BL/6J(H 2k b?I A b)mice in vitro with GM CSF+IL 4.Tumor derived total RNA extracted from actively growing Hepa 1 6 cells was used to transfected DC.The phenotypes of DC were detected by FACS,the cytotoxicity of CTL was assayed by MTT method.Results:The tumor derived total RNA transfected DC exhibited much more and longer plasm membrane processes and increased expression of MHC Ⅰ?MHC Ⅱ?CD80(B7 1)?CD86(B7 2).Conclusion:This experiment has shown that DC transfected with tumor derived total RNA of C57BL/6J cells could stimulate effectively the responsiveness of syngenic splenic T cells to induce specific CTL against C57BL/6J cells.

Objective To evaluate the effect of mini-abdominal incision for choledochoscopy via cystic duct in exploration of the common bile duct(CBD).Methods The right subcostal incision was 4-6 cm in length.Choledochoscopy via cystic duct stump for exploration and extraction of CBD stones was done in 135 cases.Results The CBD stone was removed in 109 of 135 patients(80.7%).The CBD exploration was negative in 26 cases(19.3%)and retained CBD stones occurred in 4 cases(3.0%).One case(0.7%)had intraoperative injury of common bile duct,2 cases(1.5%)had tear of cystic duct stump,and 2 cases(1.5%)had biliary leakage,all the 5 cases were recovered after treatment.Conclusions The mini-abdominal incision with choledochoscopy via cystic duct for exploration of the CBD is safe and feasible.

Objective To investigate the clinical effectiveness of gamma knife and microsurgery operation for the treatment of bilateral parasagittal meningiomas.Methods Twenty-six patients with bilateral parasagittal meningiomas were operated by microscopy,and the most parts of the tumor was removed.The remains of the tumor were treated with gamma knife.Results All patients were followed up 5 years.The tumors disappeared in 8 cases,reduced in 11 cases,did not change in 5 cases,and recurred in 2 cases.No severe complications occurred after gamma knife treatment.Conclusion Combined treatment of gamma knife and micriosurgery operation are effective for bialateral parasagittal meningiomas.It not only greatly improves the effectiveness,but also reduces the complications and death rate in patients with bilateral parasagittal meningiomas.

ObjectiveTo compare orthotopic liver transplantation (OLT)and combined liverkidney transplantation (CLKT) in the treatment of severe hepatitis B.MethodsIn this study 52 patients of severe hepatitis B were allocated to OLT (40 cases) or CLKT( 12 cases) at our department from Jan.2001 to Sep.2005.The perioperative complications and the result of follow-up were analyzed.ResultsThe preoperative renal functions in CLKT cases were severer than that in OLT cases.Postoperative severe infection was more common in CLKT cases than that in OLT cases.In OLT group 28 patients (70％)suffered from early posttransplant renal dysfunction,among them 11 patients needed dialysis,whilst there were 2 (16.7％ ) patients who needed dialysis in CLKT group (P ＜0.01 ).The posttransplant mortality in OLT group was 40％ ( n =16),significantly higher than that in CLKT ( 16.7％,n =2) ( P ＜ 0.01 ).In OLT group,9 cases developed severe renal failure and died.No one died of renal failure in CLKT group.ConclusionsThe prognosis is more favorable to perform CLKT in patients who suffered from severe hepatitis B with chronic renal dysfunction before transplantation.

Objective To investigate the expression and clinical significance of Survivin and Ki-67 in breast carcinoma,and explore their correlation.Method The expression of Survivin and Ki-67 in breast carcinoma(49 cases)and normal breast tissues(12 cases)were examined by immunohistochemical method.Results The positive expression rates of Survivin and Ki-67 in breast carcinoma were significantly higher than those in normal breast tissues(P<0.05).The expression of Survivin was significantly related to clinical stage and lymph node metastasis(P<0.05).The expression of Ki-67 Was significantly related to clinical stage,lymph node metastasis,and pathological grade(P<0.05).The expression of Survivin was obviously related with Ki-67 in breast carcinoma(r=0.734,P<0.05).Conclusions The overexpression of Survivin and Ki-67 may play a cooperative role in the occurrence and development of breast carcinoma.They may serve as useful markers for assessment of biological behavior of breast carcinoma.

Objective To evaluate the information of head injury caused by the major earthquake of Wenehuan and the diagnostic value of muhi-slice spiral CT(MSCT). Methods A total of 1557 earthquake victims were hospitalized, of which head CT scan was performed in 292 eases, and abnormalities were found in 225 of them. The composition, type of injury and MSCT findings of head injury were retrospectively analyzed. Results CT abnormalities were found in 225 victims, including 66 cases of skull fracture,50 eases of cerebral contusion and laceration, 55 eases of intracranial hematoma, 16 cases of subaraehnoid hemorrhage, 16 eases of subdural collection of fluid,20 eases of foreign substance of scalp,and 2 eases of traumatic cerebral infarction. Operation was performed in 22 of them. Conclusion The incidence of head injury in earthquake was high, and most of them were not severe. The examination of MSCT was rapid, MPR and 3D images can clearly show the lesion of craniocerebral trauma. MSCT can provide more valuable information for clinical therapy in craniecerebral trauma caused by earthquake.

Objective To investigate the causes of acute renal failure(ARF)after orthtopic liver transplantation(OLT)in patients of acute liver failure(ALF)and the effects of systemic therapy based on continuous renal replacement(CRRT).Methods Clinical data of 412 patients who underwent liver transplantations between January 2001 and June 2006 were analyzed retrospectively (all the cases were followed up to June 2007).According to UNOS grading scale,54 patients were of acute liver failure(UNOS 1 and 2A).Posttransplant ARF developing in 17 cases underwent a systemic therapy based on CRRT as well as anti-rejection,anti-infection and nutrition support.The perioperative courses,complications,causes of death and follow up results were analyzed.Results There were no severe complications during CRRT.Perioperative mortality was 5.4%and 58.8%in patients without ARF and those with ARF respectively.the rate of complications was 35.1%vs 100%.1 year survival rate Was 89.2% vs 41.2%.3 year survival rate was 81.1% vs 41.2%.Condusions The effect of surgery mainly depends on the function of liver and other vital organs.The ALF recipients suffered from a high perioperative mortality,especially those with posttransplant ARL.The systemic therapy based on CRRT benefits patients with postoperative ARF.
