AIM: To investigate the damages of adrenal gland in rabbits infected with neisseria meningococci(MC).METHODS: 18 rabbits were injected with MC (1?10 11 organism/kg, iv, model group, n=10 ) or normal saline (control group, n=8 ).TNF-? was detected in baseline (before challenged), 60 min and 120 min after injection. 120 min after injection, TNF-? immunochemical localization of adrenal gland was detected with ABC methods by electron microscopy. RESULTS: In model group, TNF-? positive cells were located in medulla and TNF-? released into simus of medulla. The medulla damages were appeared but gland cells of cortex was normal. No damages were showed in control group. TNF-? value of plasm was elevated at 60 min and 120 min after injection compared with control group. CONCLUSION: TNF-? was released into medulla and resulted in medulla damages after MC challenging. Adrenal cortex didnt damaged in early phase of MC septic shock. [

3.Treatment of Unstable Angina Pectoris with Fleabane Injection

Objective To observe clinical effects of fleabane injection treating patients with unstable angina pectoris (UAP). Methods One hundred and sixty cases with UAP were randomly recruited into two groups: a control group (including 80 patients) received Asprin and Nitrate medication, and a treatment group(including 80 patients) received fleabane injection (20 mg/d, ivdrip) on the basis of the control group. Both groups were treated for two weeks. Results Compared with the control group, the treatment group showed significantly better clinical effects and electrocardiograph. Conclusion Fleabane injection has obvious clinical effects for treating patients with UAP.
4.Curative effect analysis on treatment of transabdominal extraperitoneal hernia sac exclusion for children with hernia

Objective To explore the treatment of children with hernia,and the advantage of transabdominal extraperitoneal hernia sac exclusion was analyzed.Methods A total of 102 patients in our hospital from January 2005 to January 2015 were randomly divided into 2 groups,55 cases who treated by the surgery of transabdominal extraperitoneal hernia sac exclusion were treatment group,47 cases who treated by the surgery of high ligation of the hernia sac throug the inguinal incision were control group.The operative time,length of hospital stay, length of incision,postoperative pain and hospitalization costs between two groups were compared.Results The effect of the treatment group was superior to the control group on operative time,length of incision and postoperative pain,the difference was significant(P <0.05).Con-clusion There are some advantages of more simplified operation,shorter time of the surgery,less complication,lower recurrence rate and more reliable curative effect in the treatment of transabdominal extraperitoneal hernia sac exclusion for children with hernia,which is worthy of clinical promotion.
5.Practice of open heart operation on beating heart in congenital heart diseases

Objective To discuss the feasibility of open heart operation on beating in the congenital heart diseases.Method This kind of operation was put to use in 181 cases of the congenitalheart diseases in order to observe the effects of myocardial protection, prevention from air embolism,et al.Results Operation field exposure was clear enough to finish the manipulation. All but one case was uneventiful postoperatively,especially without air embolism. Conclusion Open heart operation on beating with slight hypothermia is highly feasible in the common congenital heart diseases.
6.Comparison of tongue postures at rest position between anterior cross-bite and normal bite

ObjectiveTo compare tongue positions at rest between normal occlusion people and patients with anterior cross-bite malocclusions,and to explore whether there is any abnormality in the tongue position of the anterior cross-bite malocclusions.Methods Twenty adults of normal bite (8 males and 12 females,with an average age of 23.8 years) and 20 adults of anterior cross-bite (1 1males and 9 females,with an average age of 24.6 years) were involved in this study.The position habits of tongue were investigated by a questionnaire.The tongue positions,which were enhanced by applying barium to tongue and palatal surface,were compared by cephalometric roentgenogram.Two groups were compared by t-test to determine the variation of anterior cross-bite. Results In normal occlusion group,the dorsum of tongues was sealed with all palatal surfaces except uvula site.In anterior cross bite group,tongues were far from palate,and were difficult to lift.The profile of tongue showed standing upright forward and up in normal occlusion group,while inclining back and low in anterior cross-bite group.The tops of tongue dorsum sites were at the border of soft and hard palate in normal occlusion group.While in anterior cross bite group,the tops of tongue dorsum sites were at the border of uvula and soft palate.In anterior cross-bite group,the tongue-to palate distance was bigger than that of normal occlusion group (P<0.01).However,both length and height of the tongue had no significant differences between two groups (P>0.05).ConclusionsIn anterior cross-bite patients,tongue positions were lower than that of normal occlusion people.
7.The Diagnosis and Treatment of 18 Cases of Rectal Gastrointestinal Stromal Tumors

Objective: To assess the clinical features, diagnosis and treatment of gastrointestinal stromal tumor in the rectum.Methods: Records of 18 patients diagnosed as GIST in the rectum between January 2002 and April 2009 were re-viewed and the major clinical features, treatment modalities and outcomes were analyzed.Results: The clinical features of GIST in the rectum were nonspecific.Most patients manifested with bloody stool or changes in bowel habits.CT scan or MRI findings showed necrosis and/or hemorrhage in the tumor and well defined tumor margins.Even in the case of large GIST, no lymphadenopathy was not found, which could be a factor for the differential diagnosis of GIST from other rectal neoplasms.All of the resected tumor specimens showed positive expression of CD117 and CD34 in immunohistochemical staining.Low and very low risk patients accounted for 44.4% (8/18).All patients received surgery.Twelve patients were treated with local excision with different approaches.Anterior resection of the rectum (Dixon) was undertaken in three pa-tients and abdominoperineal resection (Miles) in three patients.Neoadjuvant therapy with imatinib was applied for three pa-tients with partial response.After a median follow-up of 34 months (1~84 months), recurrence and/or metastasis occurred in five patients, and three of them were treated with imatinib.One patient received Miles surgery after repeated local exci-sions.Only one patient died of bone metastasis.Recurrence-free survival (RFS) of the local excision group was longer than that of abdominoperineal resection (APR) group (75.0±8.4 months vs 26.0±11.1 months, P=0.023).Conclusion: The treatment for rectal GIST should be individualized and be different from that of rectal cancer.Treatment decision and choice of procedures should be based on careful preoperative evaluation of tumor size, location, extent and risk level.Most of the anorectal GIST were rated as low-risk in this cohort and could be excised locally by different approaches with satisfactory outcome.Neoadjuvant therapy with imatinib may benefit some patients to obtain the opportunity of sphincter-saving.
8.Diagnosis value of copeptin and cTnI in early diagnosis of non-ST-segment elevation myocardial infarction

Objective To investigate the diagnostic accuracy and the clinical usefulness of the combination of troponin I (cTnI) and copeptin detected in patients with suspected non-ST elevation myocardial infarction. Methods 176 patients presenting to the emergency departments with chest chocking or chest pain within 6 hours and without ST elevation on a 12-lead electrocardiogram (ECG) were enrolled in this study. The level of copeptin and cTnI was measured. The diagnosis was adjudicated by 2 independent experts.The diagnostic performance of them was assessed using ROC analysis , and the sensitivity and specificity of them were inferred based on the positive rate of two cardiac markers. Results (1)The levels of copeptin and cTnI in NSTEMI patients were markedly higher than other groups (P<0.05).(2)The AUCs of copeptin and cTnI were 0.846 and 0.683, and the 95%CI of two markers were 0.786 ~ 0.906 and 0.577 ~ 0.789, respectively. (3)Using 10.85 pmol/L as cut off value,the sensitivity and specificity of copeptin were 90% and 64%,and the positive predictive value and the negative predictive valueof NSTEMI diagnose were 42.4% and 95.6%,respectively.Using 0.05 ng/mL as cut off value,the sensitivity and specificity of cTnI were 42.5% and 94.1%,the positive predictive value and the negative predictive value were 68%and 84.8% for diagnosis of NSTEMI. (4)The copeptin level over 10.85 pmol/L in combination with cTnI could be used to detect NSTEMI with higher sensitivity than that of copeptin or cTnI alone (95% vs 90% vs 42.5%). The negative predictive value of the combination of copeptin and cTnI was increased , compared to that of copeptin or cTnI alone (97.7% vs 95.6% vs 85.7%). Conclusions Determination of copeptin in addition to cTnI can improves diagnostic performance , especially early after chest pain onset. It seems to allow a rapid and reliable rule out of NSTEMI.
9.Changes of interleukin-18 mRNA in lipopolysaccharide induced monkey endotoxic shock

Objective:To evaluate the levels of interleukin 18(IL-18) mRNA in peripheral blood mononuclear cells(PBMCs),livers and spleens in monkey endotoxic shock models.Methods:Cynomolgus monkeys were injected i.v. with lipopolysaccharide(LPS) (2.8 mg/kg) to prepare the endotoxic shock models. The mRNA levels of IL-18 in PBMCs,livers and spleens were tested by fluorescence semi-quantitative realtime reverse transcription(RT)-PCR and compared with that of TNF-? and IL-1?.Results:IL-18 mRNA expression in PBMCs remarkably increased at 120 min after LPS administration, so did in livers and in spleens at the same time. mRNA levels of TNF-? in PBMCs peaked at 60 min after LPS injection, and also increased markedly at 120 min in livers and spleens. IL-1? mRNA levels peaked at 60 min in PBMCs, and did not change so much in livers and in spleens.Conclusion:IL-18 mRNA expression in PBMCs can be up-regulated by LPS, but changed latterly than TNF-? and IL-1? in endotoxic shock cynomolgus monkeys. It is presumed that IL-18 can be produced by PBMCs and liver Kupffer cells(maybe splenic macrophages),and TNF-? is produced by a variety of cells, but IL-1? in bloodstream mainly come from PBMCs after LPS challenge.

Bile collected from 28 cases with various hepatic or biliary diseases during operation was assayed for the ?-glucuronidase level. In Group I consisting of 14 patients with primary intrahepatic stone, the ?-glucuronidase level averaged 2250.67 u/dl, while in Group II consisting of 14 patieats with other hepatic diseases, it averaged only 463.99 u/dl(P = 0.01).The patients of both groups showed a declination of the enzyme level gostoperatively, but in Group I, the enzyme level raised with other hepatic diseases, he T-tube. The AMPS and MP contents of the bile .remarkable inhibitory effect on two substances exert their function in positive remarkable inhibitory effect on at pH4,6. It is believed that. ?-glucuronidase, Group 11 consisting the results of thisles in the reformation of biliary stones reformation biliaryWhen ?-glucuronidase level is lower than 200u/dl.Chinese traditional prescriptions with glycynrhizae liquid extract shows a remarkable inhibitory effect on this enzyme. In Chinese traditonal prescriptions, the drugs which can promote circulation and clear deposit are usually added. According to the results of this study, the authors suggest that the therapeutic effect of glycyrrhizae be emphasized.
