Objective To explore the clinical application value of serum procalcitonin(PCT)in infectious diseases.Methods 1 98 cases of patients with suspected infectious disease in this hospital,from June 2013 to June 2014,were enrolled in this study,and di-vided into virus infection group(67 cases),bacterial infection group(1 1 1 cases)and fungal infection group(20 cases)according to the results of blood cultures and fungus culture of infected tissues.Serum levels of PCT were detected and the results were statically analysed.Results Taking 0.5 ng/mL as positive threshold,the positive rate of PCT in the bacterial infection group,virus infection group and fungal infection group were 69.37%,8.96% and 5.00%,repectively,the positive rates of the three groups were statisti-cally significant different(P <0.05).Distribution of concentration of serum PCT in the virus infection group and fungal infection group were statistically significant different with that in the bacterial infection group(P <0.05 ).Conclusion Detection of serum PCT levels could be used in the diagnosis and differential diagnosis of bacterial infection,and the method is simple and reliable, which could provide clinical significance for prognosis and be worthy of clinical application.

Objective:To compare the level of low density lipoprotein cholesterol (LDL-C)before and after treat-ment between Uygur and Kazak nationality patients diagnosed as coronary heart disease (CHD).Methods:A total of 191 Uygur nationality patients and 184 Kazak nationality patients,who received coronary angiography and were diagnosed as CHD first time, and received measurement of LDL-C concentration.They received CHD secondary preventive treatment for 8~12 months,LDL-C concentration was measured again after treatment,and it's com-pared and analyzed between two nationalities.Results:After treatment,there were significant reductions in levels of total cholesterol (TC)and LDL-C in Uygur and Kazak nationality CHD patients.Compared with Kazak nationality pa-tients,there were significant reductions in levels of LDL-C [(2.74±0.13)mmol/L vs.(2.21±0.17)mmol/L]in Uygur nationality CHD patients (t=3.141,P<0.01).Conclusion:Effect lowering in Uygur nationality patients with coronary heart disease ,especially low density lipoprotein cholesterol is better than that of Kazak nationality.

METHODS: 72 Wistar rats were divided into normal group, ICH model groups (ICH, 6 h, 24 h, 3 d, 7 d), Nao Xue Kang groups (NXKT, 6 h, 24 h, 3 d,7 d).ICH models were produced with the induction of collagenase typeⅦ - S, except normal group. Immunohistochemical method was used to detect PAR- 1 protein and RT- PCR technique was used to detect PAR- 1 mRNA in brain tissues around the haematoma at different time points of different groups. RESULTS: PAR - 1 protein and mRNA were mildly positive in normal group. In ICH model groups, intensity of PAR - 1 expression started to increase at 6 h, and further increased at 24 h. PAR - 1 expression reached the peak at 3 d and began to descend. At 7 d the decent was obvious. At 6 h, 24 h, 3 d, and 7 d time points, the PAR-1 protein positive cell number and PAR- 1 mRNA absorbance ratio in ICH model and NXKT groups were significantly higher than those in normal group ( P ＜0.05 or P ＜ 0.01). The PAR- 1 protein positive cell number and PAR- 1 mRNA absorbance ratio in NXKT group were significantly lower than in ICH model group ( P ＜ 0.05 or P ＜ 0.01 ). CONCLUSION: After ICH,PAR - 1 is continuously activated because of the stimulation of thrombin. Action of thrombin after ICH may be mediated by PAR- 1; NXKT may inhibit the activation of PAR- 1, so the praxiology is improved. This may be one of the main mechanisms that NXKT could facilitate the recovery of nervous function.

Objective To study high-frequency ultrasonic features of pigmented villonodular synovitis (PVNS).Methods High-frequency ultrasonic findings in 17 cases with PVNS were analyzed and a comparison between surgical and pathological results was made.Results Of 17 cases,10 cases were diffuse type,7 cases were local type.The main high-frequency ultrasonic signs were as follows: the diffused type synovial PVNS demonstrated as hypoecho,local synovial proliferation was revealed as nodulated and heterogeneous echo,hydrarthrosis of the knee joint was present in 10 cases,the involvement of meniscus occurred in 4 cases,intra-articular cartilage and cortex erosion were showed in 5 cases.Local type PVNS was mainly demonstrated as the nodulated and hypoechoic proliferation of synovium,hydrarthrosis of the joint and cartilage erosion were not found.Conclusions High-frequency ultrasound is a valuable imaging method for diagnosing PVNS in preoperative routine screening.

Objective To compare the effects of laparoscopy and open surgery of ligation of spermatic vessel cluster or internal spermatic vein for the treatment of varicocele.Methods By using randomized controlled trial,120 cases with varicocele who stayed and were treated in our hospital were divided into 4 groups(30 cases every group) from Jun.2007 to Oct.2011.The efficacy of 4 groups was observed and compared.Results Four operations were performed successfully.Laparoscopy approached less than open surgery in terms of the days of stay in hospital (P ＜ 0.05).The quality of semen in four groups was all increased by comparing spermatozoa density,motility rate before and after operation(P ＜ 0.05),but no difference was found among four groups (P ＞ 0.05).In 0.5-1 year follow-up,follow-up rate was 92.5％,without atrophy of testes.Conclusion The operation of laparoscopic spermatie vessel cluster ligation in patients with varicoceh has the advantage of confirmed effect,which is an alternative operative method of treatment for patients with varicocele.

Liver is the main target organ of the metastasis of colorectal cancer.More and more researches demonstrate that including micro-environment of cancer,signal transduction pathway,abnormality of chromosomes and the gene which control cell proliferation and apoptosis are the most important mechanisms in liver metastasis of colorectal cancer.The metastasis of colorectal cancer is the result of all these factors.To study the mechanism can shed some light on prevention and therapy in liver metastasis of colorectal cancer.

ObjectiveTo evaluate the long-term survival rate of precancerous lesions and early esophageal cancer after endoscopic mucosal resection (EMR).MethodsThe follow-up endoscopy was performed in 154 patients with early esophageal cancer and precancerous lesions who underwent EMR.The examinations were carried out at 1 month,6 months and every 1 year after the procedure,respectively,and argon plasma coagulation (APC) was applied if a recurrent lesion was suspected.The follow-up rate was 100%.ResultsAfter EMR,reecurence occurred in 6 cases (3.9%) in 5 years,of which 4 received esophagectmy,1 underwent APC,and the other one died from matastasis.The 5-year survival rate was 97.4% (150/154),with 1 died from matastasis and 3 from noncancerous diseases.The 5-year survival rate of 125 patients with high grade dysplasia,carcinoma in situ and intramucosal carcinoma was 97.6%( 122/ 125),with 1 died from noncancerous disease and 1 from metastasis.The 5-year survival rate of 29 patients with low grade dysplasia was 96.6% ( 28/29 ) with 1 patient died from noncancerous disease.ConclusionEMR,as a minimally invasive procedure,is suitable for precancerous lesions and early esophageal cancer,especially for high grade dysplasia,carcinoma in situ and intramueosal carcinoma,with little sequela and long-term survival.

Objective To compare the clinical curative effect between mini laparoscopic cholecystectomy (MLC) and conventional laparoscopic cholecystectomy (LC).Methods Using random number table method to divide the patients into conventional laparoscopic cholecystectomy group (30 cases) and mini laparoscopic cholecystectomy group (27 cases).Compared the operation time,bleeding volume,VAS score,postoperative hospital stay,complications.Results No conversion to laparotomy and serious complications,but each group has 6 cases from the three holes increase to four holes,and 4 cases of MLC group were converted to traditional LC.MLC group and LC group,average operation time (57.5 ± 17) min and (50 ± 15) min (P ＞ 0.05);the average postoperative hospital stay was (2.5 ± 1.4) d and (3.1 ± 1.5) d (P ＞ 0.05);postoperative pain degree of MLC group of 6 h and 12 hrespectively (4.8±2.6) and (3.5±1.1),LC group were (5.5 ±2.7) and (5.0±2.1) points (P＜ 0.05).Conclusion Postoperative pain of MLC is lighter than LC,in appropriate cases,is a safe and effective treatment method.

Objective: To observe the therapeutic effect of treating postoperative gastrointestinal syndrome with combined needles and herbs. Method: Altogether 117 in-patients were randomly allocated into the treatment group of 86 cases and control group of 31 cases. Point injection was mainly adopted in the treatment group plus Chinese herbal decoction, while Western medicine was adopted in the control group. And then the therapeutic effect of the two groups was compared. Result: The total effective rate of the treatment group and control group were 96.5% and 80.6% respectively, and the treatment course of the treatment group was significantly shorter than the control group (P ＜ 0.01). Conclusion: Combined needles and herbs have excellent effect in treating postoperative gastrointestinal syndrome and were superior to Western medicine.
