Objective To explore the clinical application value of serum procalcitonin(PCT)in infectious diseases.Methods 1 98 cases of patients with suspected infectious disease in this hospital,from June 2013 to June 2014,were enrolled in this study,and di-vided into virus infection group(67 cases),bacterial infection group(1 1 1 cases)and fungal infection group(20 cases)according to the results of blood cultures and fungus culture of infected tissues.Serum levels of PCT were detected and the results were statically analysed.Results Taking 0.5 ng/mL as positive threshold,the positive rate of PCT in the bacterial infection group,virus infection group and fungal infection group were 69.37%,8.96% and 5.00%,repectively,the positive rates of the three groups were statisti-cally significant different(P <0.05).Distribution of concentration of serum PCT in the virus infection group and fungal infection group were statistically significant different with that in the bacterial infection group(P <0.05 ).Conclusion Detection of serum PCT levels could be used in the diagnosis and differential diagnosis of bacterial infection,and the method is simple and reliable, which could provide clinical significance for prognosis and be worthy of clinical application.

Objective:To compare the level of low density lipoprotein cholesterol (LDL-C)before and after treat-ment between Uygur and Kazak nationality patients diagnosed as coronary heart disease (CHD).Methods:A total of 191 Uygur nationality patients and 184 Kazak nationality patients,who received coronary angiography and were diagnosed as CHD first time, and received measurement of LDL-C concentration.They received CHD secondary preventive treatment for 8~12 months,LDL-C concentration was measured again after treatment,and it's com-pared and analyzed between two nationalities.Results:After treatment,there were significant reductions in levels of total cholesterol (TC)and LDL-C in Uygur and Kazak nationality CHD patients.Compared with Kazak nationality pa-tients,there were significant reductions in levels of LDL-C [(2.74±0.13)mmol/L vs.(2.21±0.17)mmol/L]in Uygur nationality CHD patients (t=3.141,P<0.01).Conclusion:Effect lowering in Uygur nationality patients with coronary heart disease ,especially low density lipoprotein cholesterol is better than that of Kazak nationality.

AIM: To study the dynamic expression of protease - activated receptors - 1 (PAR- 1 ) after intracerebral hemorrhage (ICH) and the influence of Nao Xue Kang Tablet (NXKT) on it's expression. METHODS: 72 Wistar rats were divided into normal group, ICH model groups (ICH, 6 h, 24 h, 3 d, 7 d), Nao Xue Kang groups (NXKT, 6 h, 24 h, 3 d,7 d).ICH models were produced with the induction of collagenase typeⅦ - S, except normal group. Immunohistochemical method was used to detect PAR- 1 protein and RT- PCR technique was used to detect PAR- 1 mRNA in brain tissues around the haematoma at different time points of different groups. RESULTS: PAR - 1 protein and mRNA were mildly positive in normal group. In ICH model groups, intensity of PAR - 1 expression started to increase at 6 h, and further increased at 24 h. PAR - 1 expression reached the peak at 3 d and began to descend. At 7 d the decent was obvious. At 6 h, 24 h, 3 d, and 7 d time points, the PAR-1 protein positive cell number and PAR- 1 mRNA absorbance ratio in ICH model and NXKT groups were significantly higher than those in normal group ( P ＜0.05 or P ＜ 0.01). The PAR- 1 protein positive cell number and PAR- 1 mRNA absorbance ratio in NXKT group were significantly lower than in ICH model group ( P ＜ 0.05 or P ＜ 0.01 ). CONCLUSION: After ICH,PAR - 1 is continuously activated because of the stimulation of thrombin. Action of thrombin after ICH may be mediated by PAR- 1; NXKT may inhibit the activation of PAR- 1, so the praxiology is improved. This may be one of the main mechanisms that NXKT could facilitate the recovery of nervous function.

ObjectiveTo evaluate the long-term survival rate of precancerous lesions and early esophageal cancer after endoscopic mucosal resection (EMR).MethodsThe follow-up endoscopy was performed in 154 patients with early esophageal cancer and precancerous lesions who underwent EMR.The examinations were carried out at 1 month,6 months and every 1 year after the procedure,respectively,and argon plasma coagulation (APC) was applied if a recurrent lesion was suspected.The follow-up rate was 100%.ResultsAfter EMR,reecurence occurred in 6 cases (3.9%) in 5 years,of which 4 received esophagectmy,1 underwent APC,and the other one died from matastasis.The 5-year survival rate was 97.4% (150/154),with 1 died from matastasis and 3 from noncancerous diseases.The 5-year survival rate of 125 patients with high grade dysplasia,carcinoma in situ and intramucosal carcinoma was 97.6%( 122/ 125),with 1 died from noncancerous disease and 1 from metastasis.The 5-year survival rate of 29 patients with low grade dysplasia was 96.6% ( 28/29 ) with 1 patient died from noncancerous disease.ConclusionEMR,as a minimally invasive procedure,is suitable for precancerous lesions and early esophageal cancer,especially for high grade dysplasia,carcinoma in situ and intramueosal carcinoma,with little sequela and long-term survival.

Objective To comprehensively know clinical pathological change of recurrent anterior dislocation of shoulder joint by arthroscopy. Methods 52 patients with recurrent anterior shoulder dislocation were inspected by arthroscopy in our institute, 38 cases were male, 14 cases were female; the mean age were 24.7, from 16 to 49 years old. 2 epilepsy patients were excluded, the other 50 cases were divided into two groups, the first group included 32 shoulder dislocated patients accompanied with shoulder hyper-laxity. The other group included 18 purely traumatic shoulder dislocated patients. All patients were explored by arthroscopy and all pathological changes were recorded and the difference of pathological change between the two groups were contrasted. Results 51 cases showed anterior-inferior labrum injury, 50 cases showed Hill-Sachs injury, 32 cases showed biceps-tendon inflammation or injury, 30 cases showed bone or cartilage injury of anterior-inferior glenoid, 29 cases showed SLAP injury, 7 cases showed rotator cuff injury. Anterior-inferior labrum injury: labrum abraded disappear was more occurred in pure traumatic group(P=0.055) and ALPSA injury was more in hyper-laxity group (59.4% to 38.9%, P=0.164). Bone or cartilage injury of anterior-inferior glenoid were more occurred in purely traumatic group(P=0.083, P=0.052). Hill-Sachs injury was more superficial or only cartilage injury in hyper-laxity group but wider and deeper in purely traumatic group. Conclusion The general pathological changes of recurrent anterior shoulder dislocation sucessively are: anterior-inferior labrum injury, Hill-Sachs injury, biceps-tendon inflammation or injury, bone or cartilage injury of anterior-inferior glenoid, SLAP injury and rotator cuff injury. The pathological changes of the hyper-laxity group were lighter than purely traumatic group.

Objective To investigate the expression and significance of KAI1/CD82 and E-Cadherin in human squamous cervical carcinoma.Methods The expressions of KAI1/CD82 and E-Cadherin were examined respectively by immunohistochemical S-P method in 10 cases of normal cervical tissue,15 cases of atypical hyperplasia of cervical epithelia and 64 cases of squamous cervical carcinoma.We analyzed statistically the correlation between the immunohistochemical results and the clinicopathological features.Results From normal cervical tissue,Cervical intraepithelial neoplasis(CIN) to cervical cancer,the positive expression rates of KAI1/CD82 and E-Cadherin were gradually decreased.Through statistic test,the positive expression rates of both invasive squamous carcinoma of the cervix were significantly lower in normal cervical tissue and CIN(P0.05).Of all the samples,direct correlation was showed in the expressions of KAI1/CD82 and E-Cadherin.Furthermore,the low-expression of both revealed significant correlation with lymphonode metastasis in human squamous cervical carcinoma.Conclusion KAI1/CD82 and(E-Cadherin) interactions may depress lymphonode metastasis in cervical carcinoma.Logistic analysis reveals that(E-Cadherin) gene,specific and interactions of several genes may be more significant during tumor metastasis.

Objective To investigate the correla tion between the plasma endothelin (ET) and gastric mucosal pathology.Methods In this experiment,seven dogs who exper ienced common bile-duct ligation were in experiment group, the other three dogs who underwent sham-operation were control group. By using specific radio immun ological method, the plasma ET levels were determined in dogs before and after 1 ,4,7 d. The total bilirubin levels were determined before and after 1,4,7 d afte r operation by using carbazotic acid method.The gastric mucosa,7 d after operati on, was cut into slices and was examined under the electron microscope and the l ight microscope.Results In the experiment group,the levels of ET and total bilirubin rose continually and were higher than those in control gr oup. Vascular congestion and oedema were seen in gastric mucosa of common bile- duct ligated dogs. Under the electron microscope, we could see the dilated bloo d capillary and sludging of blood,the intenstitial mucusa cells oedema. Conclusion The increase of plasma ET might lea d to impairment of mucosal micro-circulation,which might be one of the pathogen s that could induce the gastric mucosal damages in common bile-duct ligated dog s.

Objective To evaluate the application and effect of endoscopic screening directly with i-odine stain in high risked area of esophageal cancer and compare the detecting rate of cancer and dysplasia before and after iodine stains. Methods In the high risked area of esophageal cancer, endoscopic exam were directly performed in 3 022 people, aged 40 - 69 years with iodine staining and biopsy, then observing and recording the alternation of color and morphology of mucbsa and texture of submucosal blood vessels before and after iodine staining. Results One hundred and thirty one cases of esophageal carcinoma and 659 cases of moderate and severe dysplasia were diagnosed by biopsy; the discovery of esophageal cancer before and after iodine stain were 57 ( 1. 9% ) and 111(3.7%) cases, while of moderate and severe dysplasia were 154(5. 0% ) and 659(21. 8% ) cases respectively with significant statistical differences. Conclusion The mucosal iodine staining under endoscopy markedly increased the detecting rates of early superficial esophageal cancer and dysplasia ( moderate and severe) .

Gelofusine is a plasma substitute made from modified fluid gelatin,it's hemodynamic and biochemical effects on organism were studied in 6 anesthetized dogs and 20 patients scheduled for undergoing open heart surgery(OHS). Different degree of isovolemic acute hemodilution was achieved by supplying the gelofusion at the same amount of blood withdrawn. The results showed,in patients group, after hemodilution, no significant changs were observed in HR, MAP, DO_2 and CaO_2, wherease, CO, CI, and VO_2 increased. In dog group, HR and MAP remained, CaO_2 and SVRI decreased, CO, CI, DO_2 and VO_2 increased at the degree of isovolemic hemodilution less than 20 ml/kg, DO_2 began to decrease at the degree of hemodilution greater than 20ml/kg. In both groups, there's no significant changes in the concentration of K~+, Na~+ ,CI~- and pH value after hemodilution. Our findings supports previous observation about gelofusion's effect to maintain the blood volume and reserve the hemostasis,in addition,it has no deterious effect on coagulation
