1.Effect of irradiation-induced wt-p53 positive circuit on cell life cycle,apoptosis and irradiation sensitivity of lung adenocarcinoma cells

Objective To study the effects of irradiation-induced wt-p53 positive feedback circuit on cell life cycle,apoptosis rate and irradiation sensitivity of adenocarcinoma of lung in vitro.Methods The therapeutic group pE6R4-p53-EGFP/H1299 cells and the other 3 control groups H1299(p53-/-) ,pE6-p53EGFP/H1299 and pR4-p53-EGFP/H1299 cells were irradiated with 8-Mev electron beam generated by a linear accelerator at a dose rate of 400 cGy/min and a source-skin distance(SSD) of 100 cm,12 h exposure to irradiation.The cell life cycle and the rate of apoptosis were analyzed by FCM.Mean lethal dose(Do) and sensitive enhancement ratio(SER) index were calculated from the irradiation dose-survival curve.Results In 12 h post 4-Gy 8MV-X-ray irradiation,FCM analysis showed that most cells in therapeutic group were arrested at G0/G1 stage(75.13?1.42) %,compared with the 3 control groups(38.47?0.87) %,(62.57? 0.76) %and(51.23?2.41) %,respectively.After irradiation,the cell apoptotic rate in each group was higher than that in the cells without irradiation.The apoptotic rate in the therapeutic group was(23.73? 0.21) %,which was 5.69,1.51 and 2.57 folds respectively higher in the 3 control groups(4.17?0.12) %,(15.67?0.32) %and(9.23?0.15) %.Do values were 0.91,1.073 and 1.413 Gy respectively in pE6R4-p53-EGFP/H1299,pE6-p53-EGFP/H1299 and H1299 cells.The SER,derived from Do values,was 2.63 and 1.34,respectively.Conclusion Irradiation up-regulates wt-p53 gene expression,regulates cell cycle and induces cell apoptosis.Thus this positive feedback circuit increases the sensitivity of lung adenocarcinoma to irradiation.

Objective To explore the methods of thoracoscopic intrapleural perfusion hyperthermic chemotherapy(TIPHC)on diagnosing and treating malignant pleural effusion caused by lung cancer,as well as its effect.Methods From February 1999 to March 2006,seventy patients with malignant pleural effusion caused by lung cancer were randomly divided into therapeutic group(35 cases)and control group(35 cases).Pleural biopsy and TIPHC under general anesthesia with unilateral ventilation were performed in the therapeutic group,and intrapleural injection of cisplatin was administered in the control group after drainage of pleural effusion.The effect on malignant pleural effusion,the change for the concentration of carcino-embryonic antigen(CEA),cytokeratin-19 fragments(CYFRA21-1),neuron-specific enolase(NSE)and the side effect were compared before and after the treatment.Results The therapeutic group achieved total response rate of 100%,but only 54.3% in the control group,with significant difference(P
3.Constructing a scale on KAP of nurses ′ on stroke-dysphagia and test of its reliability and validity

Objective:To develop a rating scale on the knowledge, attitude and practice of stroke-dysphagia for nurses, and to test the reliability and validity.Methods:Based on theoretical framework of the Knowledge, Attitude, Practice (KAP), the scale items were determined through literature review, expert consultation, pre-investigation, and qualitative interviews. 633 nurses in the department of stroke that from 18 hospitals in Hubei Province were selected for the survey, to further screen the items and test reliability and validity of the scale.Results:The positive coefficient of the two-round expert consultation were both 100%; the expert authority coefficient was 0.930; the level of coordination of experts ′ opinions was good. Exploratory factor analysis extracted 4 factors, the accumulative variance contribution rate was 44.278%, Content validity index S-CVI was 0.984, I-CVI was 0.857-1.000; the Cronbach α coefficient of scale was 0.855, test-retest reliability was 0.909. Eventually, the scale was consisted of 3 dimensions, 32 items. Conclusion:The scale has good reliability and validity, and can be used as a survey tool to test the current knowledge, attitude, practice of clinical nurses about patient with stroke-dysphagia.
4.Influence factors cause prolonged ICU stay in cardiac rupture patients after operations

Objective To investigate the influence factors on length of ICU stay in cardiac rupture patients after surgical repair.Methods A retrospective study was conducted and enrolled survived cardiac rupture patients admitted to ICU of Wuxi People's Hospital from January 2009 to January 2017.Clinical data of patients were recorded including age,causes,complications,blood transfusion,duration of mechanical ventilation,duration of vasoactive agent employment,and length of ICU stay.At the same time,the laboratory findings were documented including hemoglobin(Hb),hematocrit (Hot),serum creatinine,blood lactic acid and creatine kinase muscle b(CK-MB).Patients were divided into two groups according to length of ICU stay(A group >3 d,B group ≤ 3 d).Data were analyzed using SPSS20.0 and MedCalc software.Pearson correlation test and ROC curve were taken to analyze the predicting factors for the prognosis.Results Twenty-two patients were referred to ICU after operation and survived.The length of ICU stay varied from 2 to 12 d.The statistical analysis showed the levels of Hb and Hct in group A were lower than those in group B (P<0.05);the levels of CK-MB and lactic acid in group A were higher than those in group B (P<0.05);and larger volume of blood transfusion (P=0.002),higher complications (P=0.024),longer duration of mechanical ventilation (P=0.001) and longer duration of vasopressor application (P=0.003) were found in group A.Pearson correlation test showed Hb and Hct had negative correlation with duration of mechanical ventilation,duration of vasoactive agent application and length of ICU stay (P<0.05),whereas the CK-MB and lactic acid had positive correlation with duration of mechanical ventilation,duration of vasoactive agent employment and length of ICU stay (P<0.05).ROC curve showed lactic acid,Hct,and CK-MB all had significance in prediction of prolonged ICU stay,and there were no statistically significant differences in areas under the curve(AUC) among these three biomarkers (P>0.05).With the employment of ROC curve,the optimal cut-off points of three biomarkers for clinical diagnosis were lactic acid ≥ 3.4 mmol/L,Hct ≤ 39%,CK-MB ≥ 58 U/L.Conclusion Hb,Hct,lactic acid,CK-MB,blood transfusion,complications,duration of mechanical ventilation,duration of vasoactive agent employment were factors influencing on length of ICU stay in cardiac rupture patients after operations.The levels of lactic acid,Hct,and CK-MB had significance in prediction of prolonged ICU stay.

Objective:To analyze the radiological protection situation in the workplace of medical X-ray diagnostic equipment in primary medical institutions in south Xinjiang Uygur Autonomous Region, and then put forward necessary measures and suggestions.Methods:In accordance with the national medical radiation protection monitoring program and the requirements of relevant standards for radiological health, medical X-ray diagnostic equipment in radialogical diagnosis and treatment institutions was tested selectively for workplace radiological protection, with the result statistically analyzed.Results:From 2018 to 2021, radiological protection test was conducted for 84 workplaces in 15 radiological diagnosis and treatment institutions in 4 districts of south Xinjiang, with a pass rate of 98.8%, which was consistent with the mainland including the eastern and central regions in the country.Conclusions:Based on the current situation in radiological diagnosis and treatment institutions on medical radiation protection in south Xinjiang, it is recommended to strengthen the supervision and management of radiological diagnosis and treatment equipment, improve the testing and technical capabilities of the local medical radiation monitoring technical teams and raise the level of radiological health work in the south Xinjiang.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
