Objective: To explore the application and clinical effects of humanistic service management in the management of NICU.Method: Humanistic service,management and education are conducted among neonate patients,their family members,nursing staff,and nursing students in NICU,and the outcome clinical effects are investigated.Result: The application of humanistic service dramatically increases the positivity,activeness,creativity,and cohesiveness of nursing staff,improves their overall quality,helps to establish a harmonious relationship between nurses,nursing students,and neonate patients' family members,enhances the satisfaction degree of neonate patients' family members and the nursing standard in NICU,and promotes the general improvement of hospital nursing service.Conclusion: The application of humanistic service in NICU initiates nursing service,improves hospital nursing standard,promotes the general development of NICU nursing,thus deserves to be further extended and applied.
2.Investigation on 6 genetical traits in Yi and Bai ethnic groups in Guizhou province

Objective To study distributive frequencies and the corelation among 6 genetical traits in Yi and Bai ethnic groups in Guizhou province. Methods According to the techniques of human population genetics and cluster random sampling,a survey on 6 genetical characters of 879 cases (Yi ethnic group 472 and Bai ethnic group 407) was carried out in October 2008,then dealt with data using SPSS13.0 software. Results The distributive frequencies in 6 dominant traits of Yi ethnic group are 20.6%,34.3%,74.4%,76.3%,25.4% and 60.8% respectively; Those of Bai ethnic group are 17.2%,39.3%,64.9%,86.2%,15.5% and 66.8% respectively. 3.Clinical Observation on 29 Cases of HIV Infection and AIDS Treated with Zhong Yan No. 2

All patients were treated with Zhong Yan No. 2 Prescription, 3 months constituting one course. Indexes of immunologic functions before and after treatment were compared and HIV -Rt - PCR detection was made in 16 cases. Results indicated that in absolute number of CD4, 8 cases increased, 14 cases decreased and 7 cases did not change; in CD4/CD8 ratio, 2 cases increased, one case decreased and 16 cases did not change; Two cases had increase of both CD4 and CD4/CD8 ratio: 4 cases had a decrease of new pterin, 11 cases increased and one case no change; 6 cases had a decrease of HIV - Rt - PCR, 8 cases increased and 2 cases no change. The total effective rate was 48.28% based on comprehensive assessment of virus, immunity, body weight and symptoms.
4.Association of hs-CRP with abnormality of glucose metabolism in patients with essential hypertension

Objective To investigate the association of high-sensitivity C-reactive protein(hs-CRP) with the abnormality of glucose metabolism in patients with essential hypertension.Methods Based on the level of fast plasma glucose(FPG) and 2-hours-plasma glucose(2hPG),120 patients with essential hypertension were divided into 3 groups:Normal glucose tolerance group(NGT),impaired glucose tolerance group(IGT)and diabetes mellitus group (DM).The hs-CRP was measured.Results The level of hs-CRP gradually increased from NGT to DM groups(P<0.05).The level of hs-CRP was positively correlated with SBP,DBP,2hPG and FPG in patients with essential hypertension(r =0.48,0.37,0.46,0.31,P<0.05).Conclusion The abnormality of glucose metabolism in patients with essential hypertension was related with hs-CRP.
5.Diagnosis and treatment of gastroparesis after abdominal surgery

Objective To investigate the possible contributing risk factors, diagnosis and treatment of gastroparesis after abdominal surgery. Methods Potential risk factors, clinical manifestations and therapeutic approaches of 22 cases of gastroparesis after abdominal operation were analyzed retrospectively. Results In this series, gastroparesis more frequently occured in those patients having a long-term preoperative gastric outlet obstruction, malignant tumor, O type blood group pancreaticoduodenectomy or simple gastrojejunostomy without gastric resection. Endoscopy, upper gastrointestinal radiography and radioisotope scintigraphy were effective methods in the diagnosis of gastroparesis, and normal gastrointestinal movement was achieved mostly in 5 weeks postoperatively by conservative treatments.Conclusions Postoperative gastroparesis can recover spontaneously after a longer period of supportive treatment combined with prokinetic drugs, and reoperation should be avoided.

7.Analysis of clinical features and risk factors of death in children with trauma

Objective To analyze the clinical features and risk factors of death in pediatric trauma,in order to provide preventing strategies and reduce mortality.Methods A retrospective study of 7936 traumatic children admitted to the emergency department of our hospital from April 2014 to March 2016 was conducted.We retrospectively summarized the clinical features and Logistic regression analysis was used to analyze the risk factors of death in children with trauma.Results Compared to females,male patients contributed higher percentages(1.73∶1).There were more migrant children(5535 cases) than local ones(2 401 cases)(χ2=14.314,P<0.05).Children in infancy stage(2 024 cases)and toddler period(3 097 cases) were more than in the other periods.Limb[41.9%(3 324/7 936)] and skin injuries[38.5%(3 058 324/7 936)] were the most common.Children trauma causes varied with age,fall,tumble and traffic accidents were common causes of children trauma.Pediatric trauma score of surviving group(n=1933),death group(n=5),general ward group(n=1852) and ICU group(n=86) were 8.53± 2.17,3.17± 1.29,9.72± 1.25,5.23± 1.84,respectively.Shock decompensation,mechanical ventilation,Glasgow coma scale ≤7 and pediatric trauma score<8 were the risk factors which caused the death of trauma.Conclusion Parents should improve the security sense,strengthen the care of children and obey the traffic rules to reduce the rate of children trauma.Clinicians should pay attention to shock decompensation,mechanical ventilation,Glasgow coma scale and pediatric trauma score in order to identify critically ill patients in early stage and improve the success rate of rescue.
8.Effect of dexmedetomidine combined with ropivacaine for continuous lumbar plexus block in early recov-ery of patients undergoing total hip arthroplasy

Objective To explore the effect of dexmedetomidine combined with ropivacaine for continuous lumbar plexus block on the quality of early recovery in patients undergoing total hip ar-throplasty.Methods Sixty patients (35 males and 25 females)with ASA physical status Ⅱ or Ⅲ, aged 65-84 years,undergoing total hip replacement were randomly assigned to dexmedetomidine com-bined with ropivacaine group (group D)or ropivacaine group (group C).All patients received ropiva-caine which was administered via continuous lumbar plexus block as patient-controlled analgesia (PCA)after surgery.The PCA were programmed with a background infusion 8 ml/h of ropivacaine, bolus dose was 4 m1 and the block time was 30 min;the patients in group C received 0.2% ropiva-caine,and the patients in group D received 0.1% ropivaciane combined with 1 μg/ml of dexmedeto-midine by the way of intravenous infusion.In addition,all patients received another patient-controlled intravenous analgesia (PCIA)with 1 mg/ml of morphine for relieving the explosive pain.The PCIA was programmed with a lobus dose of morphine 1 mg without background dose,the block time was 5 min.Consumption of morphine and visual analog scale (VAS)score,muscle strength and maximum flexion and abduction of hip joint were recorded at the time points of 6,12,24 and 48 h after opera-tion.The side-effect reactions such as nausea,vomiting,drowsiness and itching were recorded.The sleep quality was assessed with Pittsburgh sleep quality index (PSQI)on day 1 before operation,day 1 and day 7 after operation.Results The consumption of morphine in group D was significantly fewer than group C (P <0.05).Compared with group C,muscle strength and maximum flexion and abduc-tion of hip joint in group D were increased at 6,12,24 and 48 h after operation (P <0.05 ).PSQI scores were decreased on the day 1 and day 7 after operation.The incidence of delirium,nausea and vomiting,drowsiness and itching in group D were decreased (P <0.05).Conclusion 0.1% ropiva-ciane continuous lumbar plexus block combined with 1 μg/ml of dexmedetomidine by the way of in-travenous infusion could provide satisfied analgesia for the operation of total hip arthroplasty;the sleep quality is improved,the functional exercise of hip joint is promoted.

The purpose of education is to cultivate talents who can master the ability of self-learning lifelong. With the rapid development of multimedia technology, the knowledge carrier represented by micro curriculum plays a very important role in improving students' self-learning ability. In traditional Chinese medicine college, due to the short of time, weak learning foundation, the ability of self-learning is hard to improve in the modern medical courses such as biochemistry. This is not conducive to the cultivation of modern talents of Chinese medicine. In this paper, we chose the biochemistry teaching in TCM college as an example, and discuss how we can make the application of micro courses reasonably in the teaching process. This study was regarded as a starting to improve the students' self-learning ability effectively.
10.Comparison of continuous lumbar plexus block with different concentrations of dexmedetomidine added to ropivacaine for postoperative analgesia in elderly patients undergoing hip replacement

Objective To compare the efficacy of continuous lumbar plexus block (LPB) with different concentrations of dexmedetomidine added to ropivacaine for postoperative analgesia in elderly patients undergoing hip replacement.Methods Sixty elderly patients of both sexes,aged 65-85 yr,weighing 50-71 kg,of American Society of Anesthesiologists physical status Ⅱ or Ⅲ,scheduled for elective total hip replacement,were divided into 2 groups (n =30 each) using a random number table:0.5 μg/ml dexmedetomidine+0.1% ropivacaine group (group D1) and 1.0 μg/ml dexmedetomidine + 0.1% ropivacaine group (group D2).Lumbar-sacral plexus block combined with general anesthesia was used.At the end of operation,LPB pump was started and set up to deliver a 4 ml bolus dose with a 30 min lockout interval and background infusion at a rate of 8 ml/h.The analgesia solution contained 0.5 μg/ml dexmedetomidine and 0.1% ropivacaine in group D1 and 1.0 μg/ml dexmedetomidine and 0.1% ropivacaine in group D2.Postoperative analgesia lasted for 48 h,and the visual analogue scale score was maintained ≤ 3.Patientcontrolled intravenous analgesia (PCIA) pump was connected when break-through pain happened.PCIA solution contained morphine 50 mg diluted to 50 ml in 0.9% sodium chloride.PCIA pump was set up to deliver a 1 ml bolus dose with a 5 min lockout interval and no background infusion.When the visual analogue scale score >3,the patient-controlled LPB pump was used first,and 15 min later if analgesia was still ineffective,PCIA pump was applied.The number of patients in whom analgesia was effective and occurrence of adverse reactions were recorded.Results Compared with group D1,the rate of effective analgesia was significantly increased,and the incidence of nausea,vomiting and pruritus was decreased in group D2 (P< 0.05).No bradycardia,hypotension,over-sedation,respiratory depression,urinary retention or local skin infection was found in the two groups.Conclusion Continuous LPB with 1.0 μg/ml dexmedetomidine added to 0.1% ropivacaine provides better efficacy for postoperative analgesia in elderly patients undergoing hip replacement.
