1.Application of SBAR communication mode in the patients with acute myocardial infarction

Objective To explore the application value of the situation-background-assessment-recommendation(SBAR) communication mode in the patients with acute myocardial infarction (AMI).Methods Ninety-four cases of AMI patients who were hospitalized in the department of cardiology in Wuxi No.2 People's Hospital of Nanjing Medical University were included and were randomly divided into the conventional group and the SBAR group. There were 47 cases in each group. The patients in the conventional group were given the routine care; the patient in the SBAR group was given the SBAR communication mode. The nursing satisfaction and the incidence of adverse events in the 2 groups were compared.Results The nursing satisfaction in the SBAR group was 95.74%, which was significantly higher compared to the conventional group (70.21%,P<0.05). The total incidence of adverse events in the SBAR group was 4.25%; in the conventional group the incidence was 25.53%. The difference between the 2 group was significant (χ2=6.42,P<0.05).Conclusions The SBAR communication model can effectively improve the quality of the nurses' disease observation and the nursing satisfaction and reduce the incidence of clinical adverse events, which was worth clinical promotion.
2.Application of evidence-based nursing in CHD patients with PCI in selected days

Objective To explore the application of evidence-based nursing in patients with coronary atherosclerosis heart disease (CHD)who received percutaneous coronary intervention (PCI)in selected days.Methods A total of 85 CHD patients undergoing PCI in selected days were randomly divided into observation group (43 cases)and control group (42 cases).Control group was treated with routine nursing while observation group with evidence-based nursing.Re-sults The compliance rates of observation group was 95.35%,evidently higher than the 78.57%in control group (P <0.05).Self-rating anxiety scale (SAS)and self-rating depression scale (SDS)scores decreased markedly in both groups after interventions than intervention before, which were apparently lower in observation group than in control group,and there was significant difference (P <0.05).One month after operation,observation group was obviously higher in seat-tle angina questionnaire (SAQ)than control group (P <0.01).Conclusion Evidence-based nursing can increase the therapeutic compliance,promote the negative emotions and improve the postoperative quality of life of CHD patients with PCI.

4.Application of meticulous management in the clinical nursing pathway

Objective To apply meticulous management in clinical nursing pathway so as to make it scale and effectively advance .Methods The management of clinical nursing pathway focusing on hierarchical management , process system , training implementation and performance appraisal in meticulous management were carried out through the daily , mid-term, terminal evaluation .The patients with pilot disease and ward nurses who did not receive or received the meticulous management were divided into the control group and the intervention group .The patient satisfaction , the nursing quality and the comprehensive ability of clinical nursing were respectively evaluated by the questionnaire about patient satisfaction , the nursing quality assessment and the clinical skills assessment .Results The average hospital days and costs after the application of meticulous management were respectively (8.62 ±0.41) d, (12 920.54 ±415.19)yuan, and were lower than (10.46 ± 0.54) d, (14 061.74 ±423.17) yuan before the application of meticulous management , and the differences were statistically significant (t =37.41, 24.35, respectively;P <0.01).The patients satisfaction after the application of meticulous management was increased from (86.18 ±3.41) % to (95.27 ±3.62) %, and the score of the comprehensive ability of clinical nursing was increased from (73.21 ±4.21) to (81.24 ±4.37), and the differences were statistically significant (t=-23.12, -13.23, respectively;P<0.01).Conclusions The application of meticulous management in the process of clinical nursing pathway is practical .
5.Effects of Orem support-education system on negative emotions and self-management capabilities of lung cancer patients with chemotherapy

Objective To analyze the effects of Orem support-education system on negative emotions and self-management capabilities of lung cancer patients with chemotherapy.Methods A total of 85 cases of lung cancer patients treated with chemotherapy were selected and divided into,according to random number table method,observation group with 44 cases intervened by Orem support-education system,and control group with 41 cases treated by conventional health education.Changes of anxiety,depression and self-management capabilities before and after the intervention in the two groups were compared.Results After the intervention, anxiety and depression scores in the observation group were (41 .55 ±4.81 )points and (43.36 ±5.58)points, significantly lower than (46.73 ±3.91 )points and (48.22 ±6.1 0)points in the control group;capabilities of stress reduction,positive attitude,decision-making and self-management in the observation group were all higher than that in the control group (P <0.05).Conclusions Orem support-education system is significant for lung cancer patients with chemotherapy in alleviating their emotions of anxiety and depression,and enhancing their self-management capabilities.
6.Application effects of appointment interview policy between head nurse and nurses in ICU

Objective To establish and implement an appointment interview policy between head nurse and nurses in ICU,in order to reduce their job burnout and to improve the job satisfaction and nursing quality. Methods A total of 68 appointment interviews were performed between head nurse and 28 nurses in ICU from February 2014 to September 2015.The job burnout level,job satisfaction,and the nursing quality were compared before and after the intervention.Results After the intervention,the score of emotional exhaustion reduced from (29.17 ±7.90)to (23.08 ±6.58);the personal achievement increased from (31.63 ±5.20)to (36.13 ±5.90);the job satisfaction significantly improved from (58.96 ±7.17)to (64.38 ±4.92);the nursing quality also improved from (80.13 ±1.65)to (88.88 ±1.89).The differences of the above scores had statistical significance (P <0.05 ).Conclusions The establishment of the appointment interview policy can effectively improve the relationship between the head nurse and nurses and provide supports to reduce their job burnout and improve the job satisfaction and nursing quality.
7.Exploration and practice of health education manager model in ambulatory surgery wards

Objective To explore innovative forms of health education in nursing management of ambulatory surgery under the health care reform situation.Methods Single disease health education guidelines were established according to standard clinical nursing path.Health education manager model was formed by combination of health education information platform and personal responsible nurses.By applying four-grade quality control management,individualized and overall health education was conducted to the patients.Therapeutic effect of patients in the ambulatory surgery ward was evaluated.1 300 discharged patients were surveyed by phone about their satisfaction to the nursing.Results Hospitalized days and expenses of patients undergone ambolatory surgery were fewer than patients with the same disease yet in general wards;0.08% of the patients were transferred to general wards,and another 0.08% were readmitted because of accident.Patients' satisfaction to the nurses' technique,service,communication and education was 100.0%,98.2%,100.0% and 100.0%.Conclusions Health education manager model in ambulatory surgery ward can guarantee therapeutic effect of the surgery and meet the patients' need of health education.
8.Design and application of index cards for the duty shift of ICU nurses based on SBAR communication model

Objective To design an index card based on SBAR communication model for the shift changes of intensive care unit ( ICU) nurses and increase the effective communication between nurses and the quality of shift changes, and ensure the safety of patients. Methods According to the standard SBAR communication mode, we designed the index cards for ICU nursing shift handover and applied it to the process of nursing shift from May 2015. Nurses′mastery of the patient′s condition, the quality of shift, and the proportion of adverse events caused by poor communication were compared before( May 2014 to April 2015) and after( May 2015 to April 2016) the use of index cards. Results A total of 383 cases of patients used index cards, the nurses′mastery degree of the patient′s illness condition increased from (86.62±5.23) to (91.68±4.71), and the quality of shift improved from (90.89±3.35) to (94.44±2.66) (t=5.57,5.58;P<0.05). Nursing adverse events due to poor communication was significantly decreased, and the incidence rate of pressure scores decreased from 3.17% to 1.04% (χ2=4.17,P<0.05) . Conclusions The index cards for the duty shift of ICU nurses based on the SBAR communication model helps to promote the effective communication among ICU nurses. It improves the quality of shift and guarantees the patient safety.
9.Effect of companion support on oral medication compliance and anxiety in tumor patients

Objective To investigate the effects of companion support on oral medication compliance and anxiety in tumor patients.Methods A total of 78 cases of tumor patients with pain were randomly divided into intervention group (n=39) and control group (n=39) and investigated using randomized controlled study. Patients in control group received routine analgesic care, while patients in intervention group received additional 3-month companion support intervention. Treatment compliance and anxiety of patients were compared between two groups.Results After the intervention, patients′ treatment and medication compliance including number of oral medication, medication time, dosage, adherence to continuous medication in intervention group were better than patients in control group. The differences were statistically significant (t=6.366, 2.646, 3.552, 2.588, 3.551;P<0.05). The scores of anxiety of the patients in intervention group were significantly lower than that in control group (t=-2.542,P<0.05).Conclusions Companion support intervention can increase oral medication compliance and improve the anxiety of tumor patients.

Medical security is the core of hospital management at home and abroad. To ensure patient safety, it is the responsibility of the medical staff. However, the effective communications of physicians and nurses and among nurses are patient's safety guarantee. Situation-background-assessment-recommendation (SBAR) communication mode in clinical communication has much positive effect in aspects of validation, gradually become important communication mode. From the origin and framework of SBAR communication mode, this article summarize from carrying out the form and the enlightenment to today's management, in order to provide theoretical reference for clinical application.
