Purpose: To investigate the relationship between the expression of ET, the content of DNA and prognosis in primary lung cancer. Methods: The expressions of ET and the contents of DNA in 66 cancer specimens of different pathological grades were studied by immunohistochemistry and image - analysis technique, all cases were followed up more than 2 years. Results: The positive rate of ET in lung cancer was 73%, which was significantly higher than that in benign bronchial disease (P0.05). Objective To analyze the reasons for accidental extubation in infants during mechanical ventilation and to explore effective intervention countermeasures.Methods A total of 148 infants with accidental extubation from May 2013 to February 2014 who given routine nursing care were selected as control group.Another 152 infants with accidental extubation in pediatric intensive care unit (PICU) who given optimization nursing care from March to December 2014 were selected as observation group.Retrospective analysis of accidental extubation,artificial airway way and intervention care were conducted.Results The total intratracheal tube time was 283 d in control group and 253 d in observation group.Nineteen cases (6.7%) suffered accidental exudation in control group and that of 8 cases(3.2%) in observation group,and there was significant difference between two groups,x2=5.25,P<0.05.The primary reason of unplanned extubation was that infants with unconsciousness,the second cause was the improper catheter fixed.The third cause was the patient's comfort level and so on the improper nursing operation leading to accidental extubation too short or too shallow.Conclusion Infants given mechanical ventilation should pay more attention to the intervention of accidental extubation,appropriate calm,physical constraints and correct fixed pipeline.
3.Preparation and in vitro targeting of sterically stabilized liposomes modified with chimeric TNT-3 monoclonal antibody

Aim To prepare sterically stabilized liposomes modified with chimeric TNT-3 monoclonal antibody (chTNT-3) and investigate their immunoreactivity and in vitro targeting. Methods An endgroup functionalized polyethylene glycol-lipid derivative (pyridylthiopropionoylamino-polyethylene glycolhydrogenated soy phosphatidylethanolamine, PDP-PEG-HSPE ) was synthesized and incorporated to sterically stabilized liposomes. After mild thiolysis of the PDP groups by dithiothreitol, liposomes were covalently linked with maleimide-derivatized chTNT-3 and formed sterically stabilized immunoliposomes.Coupling efficiency, antibody density, size distribution, immunoreactivity of chTNT-3-modified sterically stabilized liposomes (chTNT-3-SLs) and specific binding properties of the chTNT-3-SLs to fixed Raji cells were determined, separately. Results Higher initial Ab/PDP-PEG-HSPE molar ratios resulted in higher antibody density on the surface of liposomes but lower coupling efficiency. The optimal coupling efficiency of 71% was obtained while antibody density in liposome was 106 μg antibody/μmol phospholipids (as initial antibody/PDP-PEG-HSPE = 1: 10). The chTNT-3-SLs had a narrow size distribution after extrusion and the mean size of this immunoliposomes was (115 ± 33) nm. The immunoreactivity of chTNT-3 can be preserved after efficient attachment of maleimide-derivatized chTNT-3 to the surface of liposomes. But calculated per antibody concentration, the immunoreactivity of chTNT-3-SLs would obviously decrease compared to that of chTNT-3 or chTNT-3 derivatives. Significantly higher binding of chTNT-3-SLs to fixed Raji cells directed by chTNT-3 was obtained compared to other preparations in serial dilutions (P<0.01). Conclusion chTNT-3-SLs prepared by PDP-PEG-HSPE method remained most immunoreactivity of chTNT-3 and was able to bind nuclear antigens in vitro.
4.Relationship between hypertriglyceridemic-waist phenotype and clustering of cardiovascular risk factors in adolescents

Objective The aim of the study was to explore the relationship between hypertriglyceridemic-waist(HTWC) phenotype and clustering of cardiovascular risk factors in adolescents. Methods A cross-sectional study was conducted in 3 136 Han adolescents aged 13-17 years in Qinhuangdao. The HTWC phenotype was defined as serum triglyceride concentrations ≥1. 47 mmol/ L and waist circumference ≥90 th percentile for age and sex. Results The prevalence of HTWC phenotype was 3. 3% . The prevalence of clustering of cardiovascular risk factors was 0. 4%in adolescents with normal waist circumference and serum triglyceride and 14. 7% in adolescents with HTWC phenotype. After adjustment for sex, age, body mass index, and family history of diabetes and hypertension, the prevalence of clustering of cardiovascular risk factors in adolescents with HTWC phenotype was 9. 286 fold(95% CI 3. 161-27. 282, P<0. 01) of that in adolescents with normal waist circumference and triglyceride. Conclusion There was a significant correlation between HTWC phenotype and clustering of cardiovascular risk factors, suggesting that this phenotype may be applied as a simple marker for identifying adolescents with clustering of cardiovascular risk factors.
5.Analysis and management of complications caused by holmium laser ureterolithotripsy

Objective To study the the causes, management and preventive measures for complications of holmium laser ureterolithotripsy. Methods Three hundred and seven cases of uteteral calculi from Jan. 2006 to Jan. 2010 were reported (203 men and 104 women, age ranged from 18-83 years,mean age 51.6 years). One hundred thirty-three patients had calculi in the lower ureter,94 in the middle part and 80in the upper ureter. Results The overall comminution rate was 89.9% (276/307 cases). sixteen cases (5.2%) were followed by ESWL due to the shift of the stones to the renal pelvis. The operation failed in 9 cases(2.9%), 3 cases(1%) had perforation,and recoveried after open operation. Deep infection was found in 3(1%)cases,which recoveried after anti-infective therapy. Conclusions Holmium laser ureterolithotripsy is an effective method for treating ureteral calculi, with advantage of high efficiacy,safety and minimal invasion. To master the operating induction, to raise operating technique and to take correct measure could reduce complications effectually.
6.Distribution characteristics of microorganisms on the skin of submariners during closed environment voyages

Objective To investigate the distribution and changes of microorganisms on the skin of submariners under a chronically closed environment , and provide reference for targeted medical support .Methods One hundred and twenty-two samples were collected using swab.After culture and isolation, the microbes were identified by matrix-assisted laser desorption/ionization time of flight mass spectrometry ( MALDI-TOF-MS) .Results A total of 52 types of 229 bacteria and 2 types of fungi were isolated . Major opportunistic pathogens included Acinetobacter baumannii, Pseudomonas aeruginosa and Enterobacter cloacae, while major dwelling bacteria included Micrococcus luteus, Oslo Mora bacteria, Acinetobacter, Staphylococcus epidermidis, and Serratia marcescens.Compared with the early period of the task, major opportunistic pathogens and dwelling bacteria were significantly increased in the middle and late period of the task .Conclusion The skin microbes of the submariners are investigated , targeted drugs need to be prepared for daily medical support and war trauma .
7.The effect of assertive case management on relapse and health economic evaluation in schizophrenics living in communities

Objective To evaluate the effect of assertive case management on relapse and health economic evalua?tion in schizophrenics living in communities. Methods Two hundred outpatients were randomly divided into the study group (107 enrolled, 107 completed) which received assertive case management and the control group (93 enrolled, 91 completed) which received normal management treatment for 12 months. Clinical global impression scale (CGI) and the cost of treatment were assessed every three months. Medication adherence and family burden were evaluated before treat?ment and 3, 6 and 12 months after the treatment using Medication Adherence Rating Scale (MARS) and Family Burden Instructing, respectively. Results The study group was less likely to relapse compared with the control group over the 12-month follow-up and the relapse rates were 1.9%and 11.0%in study and control groups, respectively (P<0.01). The repeated-measures analysis of variance indicated that time main effect was significant in severity of illness factor score of CGI (P<0.01). The time main effect and group main effect in factor 1 and factor 3 scores of MARS were significant (all P<0.05) and there was an interaction effect in factor1 score of MARS (P<0.01). In the study group, time effect were signifi?cant in factor 1 score of MARS (P<0.01). The time main effects in indirect cost and total cost were significant and so were interaction effects in direct cost and total cost (P<0.05). In the study group, time effects were significant in direct cost and total cost (P<0.01). Comparison of FBI dimensions before and after the intervention showed that family relation?ship was much more decreased in the study group than in the control group (P<0.01). Conclusions Assertive Case Man?agement can reduce the recurrence of schizophrenia living in communities, improve compliance medication and family re?lationship as well as reduce the cost of treatment.
8.Application of meticulous management in the clinical nursing pathway

Objective To apply meticulous management in clinical nursing pathway so as to make it scale and effectively advance .Methods The management of clinical nursing pathway focusing on hierarchical management , process system , training implementation and performance appraisal in meticulous management were carried out through the daily , mid-term, terminal evaluation .The patients with pilot disease and ward nurses who did not receive or received the meticulous management were divided into the control group and the intervention group .The patient satisfaction , the nursing quality and the comprehensive ability of clinical nursing were respectively evaluated by the questionnaire about patient satisfaction , the nursing quality assessment and the clinical skills assessment .Results The average hospital days and costs after the application of meticulous management were respectively (8.62 ±0.41) d, (12 920.54 ±415.19)yuan, and were lower than (10.46 ± 0.54) d, (14 061.74 ±423.17) yuan before the application of meticulous management , and the differences were statistically significant (t =37.41, 24.35, respectively;P <0.01).The patients satisfaction after the application of meticulous management was increased from (86.18 ±3.41) % to (95.27 ±3.62) %, and the score of the comprehensive ability of clinical nursing was increased from (73.21 ±4.21) to (81.24 ±4.37), and the differences were statistically significant (t=-23.12, -13.23, respectively;P<0.01).Conclusions The application of meticulous management in the process of clinical nursing pathway is practical .
9.Effects of Orem support-education system on negative emotions and self-management capabilities of lung cancer patients with chemotherapy

Objective To analyze the effects of Orem support-education system on negative emotions and self-management capabilities of lung cancer patients with chemotherapy.Methods A total of 85 cases of lung cancer patients treated with chemotherapy were selected and divided into,according to random number table method,observation group with 44 cases intervened by Orem support-education system,and control group with 41 cases treated by conventional health education.Changes of anxiety,depression and self-management capabilities before and after the intervention in the two groups were compared.Results After the intervention, anxiety and depression scores in the observation group were (41 .55 ±4.81 )points and (43.36 ±5.58)points, significantly lower than (46.73 ±3.91 )points and (48.22 ±6.1 0)points in the control group;capabilities of stress reduction,positive attitude,decision-making and self-management in the observation group were all higher than that in the control group (P <0.05).Conclusions Orem support-education system is significant for lung cancer patients with chemotherapy in alleviating their emotions of anxiety and depression,and enhancing their self-management capabilities.

With the rapid development of early diagnosis and cardiac surgery technology, the survival rate of children with congenital heart disease ( CHD ) was improved; survival time was prolonged, and the quality of life drew more and more attention. This article summarizes the internal and external influencing factors that affect the quality of life of children with CHD, in order to know better about the influencing factors of quality of life in children with CHD, and provides the intervention basis for the children with CHD.
