1.Study of the correlated risk factors for the sustained attentional function damage in patients with mild Alzheimer's disease

Objective To explore the condition and the related risk factors for attention function damage in patients with mild Alzheimer'S disease(AD). Methods Continuous performance test (CPT)was performed in 100 healthy elder people and in 68 patients with mild AD before treatment and after 8 weeks'treatment.Mini-mental state examination(MMSE)and behavioral pathology in Alzheimer's disease rating scale(BEHAVE-AD)were used to assess the disease.Regression analysis was also used in this study. Resuits The omission rate,mistake rate,reaction time and coefficient of variation of most iterns(visual one objective continuous ear continuous performance test,VOT;visual successive objective objective continuous performance test,VST;ear continuous performance test,ET)of CPT inpatients with AD were abnormal,even after 8 weeks'treatment.These parameters were improved after 8 weeks'treatment although they were higher than those in control group.The parameters before treatment had correlation with the course of disease and the behavior and psychological symptoms of dementia(BPSD). Conclusions Attention function damage in patients with AD is wide and severe,and the parameters of CPT before treatment are correlated with the course'of disease and BPSD.
2.Application of continue performance test in screening of attention deficit hyperactivity disorder

Objective To explore the effect of application of continue performance test (CPT) in screening of attention deficit hyperactivity disorder( ADHD ). Methods Thirty-five children with ADHD and65 normal children had been applied with CPT and Conner questionnaire test. Used rank sum test to examine the outcome, established receiver operating characteristic (ROC) curve, analyzed the most valuable index among various data, formulated abnormal critical value, and carried on a consistency check between CPT screening outcome and clinical diagnosis outcome. Results Each CPT index of 35 children with ADHD was obviously higher than those of normal children(P< 0.05 ). Most indexes' area under the ROC curve exceeded 0.7, reactive time of VST was the highest among each index which achieved 0.915. It was high conformity between CPT screening outcome and clinical diagnosis outcome (Kappa = 0.935 ). Conclusion CPT can be applied in ADHD screening.
3.Clinical Curative Effect Observation of Bumetanide on Elderly Type 2 Diabetic Patients with Middle and Advanced Stages Nephropathy

Objective The purpose of this study was to observe the effects of bumetanide on elderly type 2 diabetic patients with middle and advanced stages nephropathy. Methods Forty cases with diabetic nephropathy (DN) were divided into two groups:control group (20 cases) and treatment group (20 cases) . The control group accepted furosemide (20 mg, once a day) and the bumetanide was orally administrated at the dose of 1.0 mg, twice a day to the treatment group for 3 months. The assessment of transferrin (TRF) and urine microalbumin (UALB) was performed at the time points at the end of 1, 4, 8, 12 weeks after treatment. Results (1) The levels of BUN, SCr and UA after treatment were lower than before treatment, but there were no significant differences. (2) In the treatment group, the levels of TRF and UALB have decreased after treatment for 1 week, with no statistically significant. But at the end of 4, 8 and 12 weeks,the differences were statistically significant ( <0.05) . Especially, during the three months follow-up,the levels of TRF and UALB at 12 weeks have decreased obviously. Conclusion The therapeutic effect of bumetanide on DN might be better than that of furosemide.

Objective To evaluate the imaging characteristic and the value of application of breath hold two dimensional fast imaging employing steady state acquisition(FIFSTA) sequence, for aortic dissection(AD) magnetic resonance many time phases imaging. Methods To analyze retrospectively the manifestation of 5 cases with aortie dissection diagnosed, Bakey type Ⅰ,Ⅱ, Ⅲ, in 1, 1 and 3, all were examined using ECG triggered, two dimensional FIESTA sequence during end-expiratory breath hold. Scan with the long axes of aortic position(oblique position parallel with aortic arch) and transitional position. The results were analyzed by the soft of workstation AW4.2. Results The display rate of the true lumen and the false lumen of AD were 100% ,the display rate of membrane in aorta was 100% ,4 cases showed the hole of intimal tear,2 cases with thrombus,2 cases with aortic regurgitation,2 cases with ejecting in the hole of intimal tear. Conclusion Need not any contrast media, quickly acquired imaging, high contrast to noise ratio, least artifact,and especially show membrane in aorta and for the hole of intimal tear of aortic dissection(AD) magnetic resonance many time phases imaging with two dimensional FIESTA sequence.
5.The Clinical Relationship Beteen the Endothelial Dysfunction and Mellitus Retinopathy in the Old Type 2 Diabetes

Objective The changes of CECs,EPCs and CPCs in the old type 2 diabetes mellitus retinopathy(DR)patients and the clinical purpose were discussed.Methods 30 cases of T2DM(including 10 DM cases without complications and 20 DM cases with retinopathy)and 10 cases of health controls were enrolled in this study.Results(1)The changes of CECs in DR Patients were significantly higher while the levels of EPCs and CPCs were significantly lower than those in the patients of the control group.(2)There were some corrlation beteen the level of CECs,EPCs and CPCs and the degree of diabetic retinopathy.(3)The serum EPCs levels in type 2 diabetics were negativey associated with body mass index,waist-hip ratio,abdominal circumference,FPG,triglyceride,UAER,creatinine.Conclusion The levels of EPCs and CPCs in T2DM-DR patients may be the important detected targets for early discovery and monitoring of DR.
6.Early life stress and schizophrenia:a retrospective case-control study

A total of 105 patients with first-episode schizophrenia (male =51,female =54) and 99 normal controls (male =51,female =48) were included into this retrospective case-control study.Childhood trauma questionnaire-28 item short form (CTQ-SF) was used to assess the experience of childhood abuse.The result of binary logistic regression showed that emotional abuse (β =0.630,P < 0.05) and emotional neglect(β =0.270,P < 0.05) were included into the final model of predicting schizophrenia.It indicates that patients with first-episode schizophrenia experienced more early life stress than controls.Particularly emotional abuse and emotional neglect may play important roles in the onset of schizophrenia.
7.Study of multiple organ failure induced by paraquat in rats

Objective To observe the injury of some chief organ induced by paraquat (PQ) poisoning in rats, and to explore the mechanism. Method A total of 60 inbred line SD rats were randomly divided into experimental group (n = 30) and control group (n = 30), and each group was further divided into 6 subgroups (n =5) as per the sacrifice of rats at different intervals. The rats of experimental group received the intra-abdominal injection of paraquat (1 mg/mL, 18 mg/kg), and the rats of control group were treated with the same amount of saline solution instead. The rats of each subgroup were sacrificed separately 2 h,6 h, 12 h,24 h,72 h and 120 h after administration of PQ or saline. Lungs, livers and kidneys were taken for histopathological study. Results There was noticeable exudate in lung tissue of rats in experimental group in the early stage. And then the cystic changes in the liver of rats in experimental group were found. A noticeable hemoglobin was found in the renal tubules 24 h after modeling. But the exudation in lung decreased 24 h later, and in the mean time, the disorganization of pulmonary alveoli was obvious and some remarkable collagen appeared in the interstitial tissue of lung, and it was significantly obvious 72 h after modeling. In the liver of rats in experimental group, the injured tissue had some extent of repair in 72 h after modeling, and recovered gradually. But the injury of kidney was exacerbated 72 h after modeling. In the control group, the lung, liver and kidney were not changed in all stages after modeling.Conclusions The paraquat could induce failure of some chief organs in SD rats. The injury was most remarkable in the lung in a progressive way. The kidney injury was not more severe than that of lung tissue, but the pathological changes of the kidney became worse and worse as time taken. The injury of liver induced by paraquat was slight, and the injury could heal up gradually.
8.Effects of 2-methoxyestradiol on the proliferation and apoptosis of B16 malignant melanoma cells

Objective To investigate the effects of 2-methoxyestradiol (2-ME) on the proliferation and apoptosis of a mouse malignant melanoma cell line B16,and to explore their mechanism.Methods B16 cells were cultured in vitro,and divided into a negative control group receiving no treatment and several intervention groups treated with 2-ME at final concentrations of 5,10,20,40 mmol/L,respectively.After different durations of treatment,inverted phase-contrast microscopy was conducted to observe the morphologic change of B16 cells,sulforhodamine B (SRB) assay to evaluate proliferative activity and to draw growth curve of B16 cells according to the absorbance value at 490 nm,flow cytometry to detect cell cycle and apoptosis,and reverse transcription PCR and real-time PCR were performed to measure the expressions of the apoptosis-inducing gene gadd45b and proto-oncogene c-myc.Results As repeated measures analysis of variance showed,there were significant differences in the inhibitory effect on B16 cell proliferation among different concentrations (5,10,20,40 mmol/L) and different treatment durations (24,48,72 hours) of 2-ME (F =1170.94,1843.04,respectively,both P < 0.01),and there was a significant interaction effect between these concentrations and treatment durations (F =272.79,P < 0.01).After 48-hour treatment with 2-ME at 10,20 and 40 mmol/L,the apoptosis rate of B16 cells was increased to (4.13 ± 1.12)%,(11.25 ± 2.380)% and (19.46 ± 2.9)% respectively,compared to (0.23 ± 0.5)% in the negative control group (all P< 0.01); the proportion of B16 cells in G0/G1 phase was increased to (59.5 ± 5.6)%,(63.4 ± 8.2)% and (70.8 ± 4.4)% respectively,compared to (44.1 ± 3.4)% in the negative control group.There was a significant difference in the proportion of B16 cells in G0/G1 phase among the negative control group and intervention groups (F =13.56,P < 0.05).Moreover,the mRNA expression of gadd45b was significantly enhanced after 24-hour treatment with 2-ME at concentrations of 20 and 40 mmol/L (both P< 0.01),while that of c-myc was significantly weakened after treatment with 2-ME at 10,20 and 40 mmol/L (all < 0.05) compared with the negative control group.Conclusion 2-ME can inhibit the proliferation of B16 cells in vitro,upregulate the expression of gadd45b gene and downregulate the expression of C-myc gene.
9.A descriptive analysis of triage,surge,and medical resource use in a university affiliated hospital ;after 8·12 explosion and burn at Tianjin Port

Objective To analyze the massive explosions and burn at Tianjin Port in 2015 resulted in a mass casualty event,and the entire course of response of a hospital to deal with such major sudden accident in order to find a rational strategy for optimal use of medical resources and reduce the critical mortality.Methods This study was done by a retrospective analysis of data from one trauma center at an academic hospital.Data including outcome,triage,severity and pattern of injuries,patient flow,and medical resources used were obtained by the review of hospital records.Results This disaster caused 165 deaths,8 missing contact,and 797 non-fatal casualties.The Pingjin Hospital admitted 298 casualties,and 29 of them were seriously injured referred to by Tianjin Emergency Medical Center.Excessive triage rate made after transfer to another hospital was 62.07% with 11 of the 29 severely injured patients.Maximum (also the first)surge had 147 injured patients arrived around one hour after incident,the second surge had 31 seriously injured patients occurred around 4 hours after incident.Of them,17 patients needed surgery and 17 patients were admitted to the intensive care unit.Conclusions These data showed that the number of casualties in the first surge was substantially larger than predicted and those casualties had less severe trauma,whereas the number of the injured in the second surge was less but the trauma was more severe.In order to maintain the hospital surge capacity,an effective re-triage and a hospital-wide damage control principle can be used to deal with.
