Neoadjuvant chemotherapy is widely used in the therapy of locally advanced breast cancer.In the recent studies,the expressions of ER,PR and C-erbB-2 in tumor may change in different scheme preand post-neoadjuvant chemotherapies for breast cancer.It is obviously changed in the neoadjuvant chemotherapy scheme of anthracyclines and paclitaxel/docetaxel,but hardly changed in anthracycline-based neoadjuvant chemotherapy scheme.
2.Sesquiterpenoids from gorgonian Muriceides collaris.

Seven guaiane-type sesquiterpenoids, a new compound 6-formyl-5-isopropyl-3-hydroxymethyl-7-methyl-1H-indene (1), a new natural product 5-isopropyl-3, 7-dimethyl-1H-indene-1-one (2), along with five known compounds: guaiazulene (3), 4-formyl-7-isopropyl-10-methylazulene (4), sesquiterpene ketolactone (5), alismoxide (6) and guaia-1 (5), 6-diene (7), were isolated from gorgonian Muriceides collaris collected in South China Sea. Their structures were elucidated on the basis of extensive spectroscopic analysis [MS, IR, 1H NMR, 13C NMR (DEPT), HMQC, HMBC, NOESY] and by comparison of the spectral data with those of the literatures.
3.Clinical analysis of insulin pump therapy in elderly patients with severe type 2 diabetes

Objective To compare the clinical efficacy and safety of continuous subcutaneous insulin infusion (CSII) and multiple subcutaneous insulin injection (MSII) in elderly patients with severe type 2 diabetes.Methods Thirty-seven elderly patients with severe type 2 diabetes in the hospital intensive care unit (ICU) were divided into observation group (19 cases) and control group (18 cases) by random number table method.The observation group was treated with continuous subcutaneous insulin recombinant human insulin lispro injection,and control group was treated with recombinant human insulin lispro injection before meals and at bedtime routine insulin glargine subcutaneous injection.Two groups of patients and their families were given diabetes education.Changes in blood glucose,insulin dosage,blood glucose control standard time,rate of hypoglycemia and hospitalization time between 2 groups before and after treatment were compared.Results After treatment,fasting blood glucose in observation group was (7.2 ± 1.2) mmol/L,2 hour postprandial blood glucose was (9.4 ± 1.2) mmol/L,bedtime blood glucose was (9.4 ± 1.3) mmol/L; which in control group was (8.5 ± 3.0),(10.0 ± 2.4),(10.2 ± 2.4)mmol/L respectively.Fasting blood glucose,2 hour postprandial blood glucose and bedtime blood glucose significantly decreased after treatment in two groups,but there was significant difference between 2 groups (P < 0.05).Blood glucose control standard time in observation group was (5.4 ± 2.5) d,insulin dosage was (43 ± 9) U/d,in control group was (12.8 ± 3.8) d,(55 ± 10) U/d respectively,and there was significant difference (P < 0.05).Observation group had no hypoglycemia.Conclusion CSII for elderly patients with severe type 2 diabetes has better curative effect and security than MSII,and is more conducive to the control of blood glucose and the improvement of the condition.
4.Expression of TGF?_1 and CTGF mRNA in Liver of Rats with Nonalcoholic Fatty Hepatic Fibrosis Induced by Highly Fat-enriched Diet

Objective To reveal the expression and potential mechanisms of TGF?_1 and CTGF mRNA in liver of rats with nonalcoholic fatty hepatic fibrosis induced by highly fat-enriched diet.Methods The model group(10 rats) was fed with highly fat-enriched diet while the normal control group(10 rats) was raised by standard animal feeds for 24 weeks.Hepatic histopathological changes were evaluated,and RT-PCR method was used to assay TGF?_1 and CTGF mRNA expression levels in the liver.Results The analysis indicated that liver inflammation and fibrosis were apparently present in model rats.RT-PCR analysis revealed that the expression of TGF?_1 and CTGF in hepatic tissue were inreased in the model group as compared with the normal control group.Conclusions The rats model for hepatic fibrosis can be established by feeding with highly fat-enriched diet for 24 weeks.The expression of TGF?_1 and CTGF were enhanced in hepatic liver tissue of fatty hepatic fibrosis rats,and these changes might stimulate the fatty fibrosis.
5.The Effect of Dehydration after a Sauna and Intermittent Hypoxia Stimuli on Heat Stress Protein 70 in Human Leucocytes and Aerobic Capacity during Incremental Exercise

The purpose of this study was to compare Heat Stress Protein70 (HSP70) in human leucocytes, VO2max and Blood Lactate (BLa) during incremental exercise under a thermoneutral condition without taking a sauna with those during the same workload and under same thermoneutral condition after a sauna and after intermittent hypoxia stimuli. Ten unacclimated men performed an incremental test to exhaustion on a treadmill under a thermoneutral condition without taking a sauna (N25℃, relative humidity 65%) and during the same workload and under same thermoneutral condition after a sauna (D25℃, relative humidity 65%), and after intermittent hypoxia stimuli (LN25℃, relative humidity 65%) . 6.Studies on chemical constituents of gorgonian Muriceides collaris from the South China Sea

Objective To investigate the chemical constituents of gorgonian Muriceides collaris. Methods The compounds were isolated and purified by silica gel column,Sephadex LH-20 column chromatography and HPLC. Their chemical structures were identified by physicochemical analysis,spectroscopic analysis,and comparison with the data of literatures. Results From the CH3OH soluble extract of Muriceides collaris,nine compounds were isolated,and their structures were determined as cholesterol (1),batyl alcohol (2),benzoic acid (3),uracil (4),thymine (5),2'-deoxyuridine (6),2'-deoxythymidine (7),thymidine (8) and 2'-deoxyadenosine (9),respectively. Conclusion All compounds were obtained from the species Muriceides collaris for the first time.
7.Clinical observation of Chansu injection combined with EOAP regimen in patients with advanced non-Hodgkin lymphoma

Objective To evaluate the efficacy and safety of Chansu injection (CHS) combined with EOAP regimen in patients with advanced non-Hodgkin lymphoma (NHL).Methods 65 patients with advanced NHL were divided into two groups according to random number table.All were given EOAP regimen (VP16 60 mg/m2 on d 1-5;VCR 1.4 mg/m2 on d1;Ara-C 60 mg/m2,Q12 h,on d 1-5;Pred 60 mg/m2 on d 1-5),and the observation group received EOAP combined with CHS (20 ml/d,intravenous drip,on d 1-14).The regimen was repeated every 21 days.The efficacy and toxicity were evaluated after two cycles.Results The effective rate of the observation group was higher than that of the control group,but there was no significant difference between two groups [87.9 % (29/33) vs 81.3 % (26/32),P > 0.05].The quality of life of the observation group was superior to that of the control group,and there was significant difference (P < 0.05).Hematological toxicities and gastrointestinal tract reaction were the main side effects in both groups.The incidence of myelosuppression of the observation group was significantly lower than that of the control group (P < 0.05).Conclusion Chansu injection combined with EOAP regimen can enhance the efficacy,reduce toxicity and improve the quality of life in the treatment of advanced NHL.
8.Accuracy of systolic pressure variation in monitoring blood volume

Objective To evaluate the accuracy of systolic pressure variation (SPV) in monitoring blood volume in patients.Methods Twenty-two ASA Ⅱ or Ⅲ patients (17 male,5 female),aged 49-79 yr,with body height 153-173 cm,weighing 55-89 kg,scheduled for elective coronary artery bypass grafting under cardiopulmonary bypass,were studied.Stroke volume variation (SVV) was monitored based on the arterial pressure wave and SPV measured based on the invasive arterial pressure wave after changing the title.After the chest was closed,the volume load test was immediately carried out.6% hydroxyethyl starch 130/0.4 50-80 ml/min was infused via the jugular vein until cardiac index (CI) increased by 10%.HR,mean arterial pressure (MAP),SPV,CI,SVV,stroke volume index (SVI),systemic vascular resistance index (SVRI),central venous pressure (CVP),and pulmonary capillary wedge pressure (PCWP) were recorded in supine position (T1),at 30° head-down tilt before skin incision (T1'),before (T2) and after opening the chest in supine position (T2'),before (T3) and after the volume load test in supine position after closing the chest (T3'),in supine position after skin closure (T4),and at 30° head-down tilt after skin closure (T4').The difference in SPV and SVV (△SPV and △SVV) was calculated.The receiver operator characteristic curve for △SVV and △ SPV in determining blood volume changes was plotted.Results MAP,CVP,PCWP,CI,and SVRI were significantly increased,while SVV and SPV were decreased at T1' compared with those at T1 (P < 0.05).HR was significantly increased,while CVP was decreased at T2' compared with those at T2 (P < 0.05).MAP,CVP,PCWP,and CI were significantly increased,while SVV and SPV were decreased at T3' compared with those at T3 (P < 0.05).MAP,CVP,and PCWP were significantly increased,while SVV and SPV were decreased at T4' compared with those at T4 (P < 0.05).The area under receive operator characteristic curve for △SVV and △SPV in determining blood volume changes was 0.603 and 0.616 respectively,and there was no significant difference (P > 0.05).Conclusion SPV can accurately monitor the blood volume in patients.
9.Expression of GPX3 in non-small cell lung cancer and its clinical significance

Objective To investigate the mRNA and protein expression of GPX3 gene in non-small cell lung cancer (NSCLC) patients, and then discuss the clinical significance of GPX3 in NSCLC patients. Methods Reverse transcription-polymerase chain reaction (RT-PCR) and Western blot were used to detect the GPX3 mRNA and protein levels in NSCLC tissues and adjacent non-cancerous normal lung tissues from 60 patients undergoing surgical treatment. The correlation between the expression levels of GPX3 and clinicopathological features was analyzed. Results The relative expression value of GPX3 mRNA was higher in adjacent non-cancerous tissues than in non-small cell lung cancer tissue (P < 0.05). GPX3 mRNA expression was significantly correlated with TNM stage、differentiation and lymph node metastasis. The integral optical density value of GPX3 protein confirmed was lower in adjacent non-cancerous tissues than in non-small cell lung cancer tissues (P < 0.05). In additional, the expression of GPX3 was related to TNM stage and lymph node metastasis. Conclusion The expression of GPX3 gene may play an important role in the carcinogenesis and progression of NSCLC.
10.Liver fibrosis in 102 chronic hepatitis B patients with hepatic steatosis

Objective To evaluate the prevalence of hepatic steatosis in patients with chronic hepatitis B (CHB) and the impact of hepatic steatosis on the progress of fibrosis. Methods Five hundred and sixty two untreated CHB patients (405 males and 157 females) with an average age of 31.3 underwent liver biopsy from January to August 2007. On the day of liver biopsy, a questionnaire was completed and a blood sample was obtained for laboratory analysis. The degree of liver steatosis, necroinflammation and fibrosis was assessed; demographic information and clinical data including age, gender, body mass index (BMI), history of diabetes mellitus, hypertension, dyslipidemia, and HBeAg status, HBV DNA viral load were documented. Results In 562 patients, 102 (18. 2% ) had steatosis. Multivariate analysis demonstrated that liver steatosis was associated with the levels of TG, APO-B, UA, FSG, and higher BMI; and the progress of fibrosis was associated with high degree of hepatic steatosis and necroinflammation, age over 35 years, HBV DNA > 103 copies/L, high BMI and GGT. Conclusions The results show that obesity and dyslipidemia in CHB patients are associated with the hepatic steatosis, and the latter seems to be an important determinant for fibrosis.
