1.Clinical application of evidence- based nursing in elderly patients with cardiovascular interventional surgery during perioperative period

ObjectiveTo evaluate clinical value of evidence-based nursing intervention in treatment of elderly patients with cardiovascular interventional surgery during perioperative period. Methods 150 cases of elderly patients with cardiovascular diseases who underwent interventional surgical treatment were selected.They were randomly divided into the observation group(80 cases)and the control group(70 patients). ResultsThe occurrence and degree of discomfort of bedridden patients in the observation group were milder,and the complications due to use of anticoagulants and pressing dressing were significantly less than the control group.The satisfaction degree and mastering of disease related knowledge in the observation group were significantly better than that of the control group. ConclusionsApplication of evidence-based nursing intervention during perioperative period for elderly patients with cardiovascular interventional surgery can reduce discomfort and complications of patients,as well as improve satisfaction degree and mastering of disease-related knowledge of patients and their families,which is worthy of promotion.
2.Observation on Therapeutic Effect in Acupuncture Treatment of Soft Tissue Injuries by Differentiation of Deficient and Excessive Patterns

Objective: To compare the therapeutic effects between the needling method by identification of deficient and excessive patterns and the needling method by routine meridian differentiation in the treatment of soft tissue injuries. Methods: 380 cases of the patients with soft tissue injuries were randomly divided into pattern identification group (200 cases, with the needling method by identification of deficient and excessive patterns) and meridian identification group (180cases, with the needling method by routine meridian differentiation), to observe and compare the therapeutic effects in the two groups. Results: The curative effect was 86% in the pattern identification group and 31.11% in the meridian identification group, with a significant difference (P< 0.01). In comparison with the total average treatment times in the cured and improved cases,the pattern identification group was 8.8 days and the meridian identification group was 15.6 days,with a remarkable significant difference (P< 0.01). The pattern identification group was better than the meridian identification group. Conclusion: In the treatment of soft tissue injuries, the needling method by identification of deficient and excessive patterns was better than the needling method by routine meridian differentiation.
3.Influence of Acupuncture plus LONG's Manual Manipulations on Functional Improvement in Lumbar Intervertebral Disc Protrusion

Objective: To observe the effect of acupuncture plus LONG's manual manipulations on the functional improvement of the patients with lumbar intervertebral disc protrusion. Methods: 60 cases of the patients were randomly divided into a observation group and a control group, 30 cases in each group. The observation group was treated with acupuncture plus LONG's manual manipulations, and the control group was treated with single acupuncture. The functions of the lumber vertebrae were processed in accordance with the assessment system of Japanese Orthopedic Association (JOA), and the therapeutic effects were assessed by Visual Analog Scale (VAS). Results: After the treatments, the differences in JOA and improvement indexes were statistically significant (P<0.05) between the two groups. VAS was decreased much remarkably in the observation group (P<0.01) than before the treatments and decreased remarkably in the control group (P<0.05) than before the treatments. There was a remarkable difference in VAS between the two groups after the treatments (P<0.05).Conclusion: Acupuncture plus LONG's manual manipulations are better than single acupuncture in improving the functions of the patients with lumbar intervertebral disc protrusion.
4.Acupuncture Treatment of 106 Cases of Peripheral Facial Paralysis in Different Stages

In the treatment of facial paralysis in the acute stage and restoration stage by shallow needling technique and penetrating needling technique respectively, totally 106 cases, the effective rate is respectively 98.6% and 83.9%, indicating that the curative rate is high if early treatment is given.

In the treatment of 32 cases of bile reflux gastritis by embedding catgut in the acupoints of Danshu (BL 19), Weishu (BL 21), Liangmen (ST 21), Zhongwan (CV 12),Shangwan (CV 13), Zusanli (ST 36), Neiguan (PC 6), Taichong (LR 3) and Yanglingquan (GB34), taken off one day later, embedded once every 15-20 days, the results showed cure in 20cases and improvement in 12 cases after four treatments.
6.Protective effect of bactericidal/permeability-increasing protein on sepsis induced by intra-abdominal infection in rats

AIM:To investigate the protective effect of bactericidal/permeability-increasing protein (BPI) on sepsis induced by intra-abdominal infection in rats and its mechanism.METHODS:Intra-abdominal infection induced sepsis was reproduced by cecal ligation and puncture (CLP). BPI or equal volume of physiological saline was intra-abdominally given immediately after CLP and 12 hours after CLP respectively (2.5 mg/kg of BPI each time). Plasma endotoxin levels were determined with limulus amebocyte chromogenic assay.RESULTS:(1)The survival time in BPI group was significantly higher than in physiological saline (PS) group. (2)The values of MAP, LVSP, IP, d p /d t max and -d p /d t max in BPI group, although decreasing ,were markedly higher than those in PS group. (3) Plasma glutamate-pyruvate transaminase and urea nitrogen levels in BPI group, though increasing, were significantly lower than those in PS group.(4) There was no significant change of plasma endotoxin levels in BPI group, while plasma endotoxin levels were markedly increased in PS group. There was significantly different between two groups. CONCLUSIONS:BPI has an obvious protective effect on intra-abdominal infection induced sepsis, which might be related to its antagonism against endotoxin.

Objective:To explore the functional mechanism of moxibustion and its enlightenment to the design and manufacture of moxibustion apparatus.Methods:The functional mechanism of moxibustion is explored from its pharmaceutical properties,warming stimulation,spectrum,radiation features,and penetrating effects of moxa products.Result:The mechanism of moxibustion is a combined action of moxa'medicinal properties,warming effect and non-thermal effect.Conclusion:It is necessary to combine with the functional mechanism to design and manufacture moxibustion apparatus.

Objective Exploring clinical prediction value of the ischemia modified albumin (IMA) for acute coronary syndrome (ACS) patients after the percutaneous coronary intervention (PCI).Methods A total of 109 cases of the PCI postoperative patients of ACS at the Panggang General Hospital from January 2010 to July 2012 were included.IMA was determined within 6 hours after admission,and standardized treatment after PCI.After a 6-month follow-up,they were divided into the event group and non-event group according to cardiovascular events occurrence.Results 101 cases were followed-up,including 26 cases of cardiovascular events (25.74%) and 75 cases of no cardiovascular events (74.26%).The left ventricular ejection fraction of the event group [(45 ± 7)%] was significantly lower than the non-event group [(52 ± 10) %] (t =1.894,P < 0.05).Serum IMA of the event group[(105.51 ± 13.26) U/ml]was significantly higher than the non-event group [(85.18 ± 11.36) U/ml] (t =7.3518,P <0.01).After controlling other cardiovascular risk factors,IMA was still independent risk factors for cardiovascular events (OR =1.69,95% CI: 1.18 ~ 2.13,P =0.01).Conclusions IMA have very good clinical prediction value of cardiovascular events occurrence for ACS after PCI.

OBJECTIVE: To investigate the effect and mechanism of bactericidal/permeability-increasing protein (BPI) on sepsis induced by intra-abdominal infection in rats. METHODS: Cecal ligation and puncture (CLP) was made on 20 rats with sepsis induced by intra-abdominal infection. BPI or equal volume of physiological saline (PS) was intra-abdominally given immediately and 12 h after CLP, respectively (2.5 mg/kg of BPI each time). Plasma endotoxin levels were determined with limulus amebocyte chromogenic assay. RESULTS: (1) The survival time in BPI group was significantly higher than that in PS group. (2) The values of the mean arterial pressure (MAP), the left ventricular systolic pressure (LVSP), the isovolumic ventricular pressure (IP), and the maximal change of left intraventricular pressure (+/-dp/dtmax) in BPI group, although decreasing, were markedly higher than those in PS group. (3) Plasma glutamic-pyruvic transaminase (GPT) and urea nitrogen levels in BPI group, though increasing, were obviously lower than those in PS group. (4) There was no significant change of plasma endotoxin levels in BPI group, while plasma endotoxin levels markedly increased in PS group. CONCLUSIONS: BPI has obvious protective effect on sepsis induced by intra-abdominal infection, which might be related to its neutralization of endotoxin.
10.Low standard oral anticoagulation therapy for Chinese patients with St. Jude mechanical heart valves.

OBJECTIVETo study the efficacy of the low standard oral anticoagulation therapy following St Jude Medical (SJM) valve implantation for Chinese patients.
METHODSTotally 805 patients with a mean age of 42.70 +/- 11.09 years, enrolled into this study. Among them, 230 underwent aortic valve replacements (AVR), 381 mitral valve replacements (MVR), 189 double valve replacements (DVR) and 5 tricuspid valve replacements (TVR). All patients received postoperative oral anticoagulation therapy based on a low standard of international normalized ratio (INR, 2.0 - 2.5). Of the 805 patients, 710 were followed up for 0.25 - 13 years (a median, 4.15 years).
RESULTSPostoperatively, 17 adverse events occurred. Operative mortality was 2.11%. The most frequent cause of operative mortality was a low cardiac output. During follow-up, there were 47 anticoagulant-induced hemorrhages [1.59%/patient-year (pt-yr)], 10 cases of thromboembolism (0.34%/pt-yr), and 3 mechanical valve thromboses (0.19%/pt-yr). There were 44 late deaths and the linearized late mortality rates were 0.51%pt-yr. Estimates of actuarial survival for all patients at 5 and 10 years was 97.45% (0.70%) and 77.96% (17.44%), respectively.
CONCLUSIONSA low target INR range of 2.0 - 2.5 is preferable for Chinese patients so as to reduce the severe bleeding complications in those with conventionally higher levels of INR. The long-term results were satisfactory in terms of the numbers of those who suffered thrombosis, embolism and bleeding.
